A Case of Schwannoma Arising from the Ethmoid Sinus (전사골동에 발생한 신경초종 1예)

  • Schwannomas are benign neurogenic tumors arising from the peripheral nerve sheath and rare in the nasal cavity and paranasal sinus. About 4% of schwannomas in the head and neck area arise in the sinonasal cavity. As unilateral nasal masses in the nasal cavity and paranasal sinus, schwannomas are likely to be mistaken for more common lesions such as nasal polyps or mucoceles. We recently experienced a case of schwannoma on the ethmoid sinus in a 33-year-old male which was surgically removed. Herein, we report this unusual case of schwannoma on the ethmoid sinus with a brief review of literature.

The Change of the Nasalance Score following Simulated Nasal Obstruction (인위적인 비폐색에 의한 비음치의 변화)

  • Nasal obstruction due to nasal deformities, rhinitis, sinusitis, or nasal polyps etc. induces hyponasality, which can influence the articulation and/or phonation of patients. However few studies were done on the subjects. Therefore, this study was performed to establish the guideline that can be used in diagnosing the hyponasality. We analyzed the nasalance scores of 6 different sentences for 26 normal adults. We measured the nasalance scores before simulated nasal obstruction, after one nostril obstruction, and after both nostrils obstruction, while the subjects are reading roe nasal sentences(so-called, 'Mama' sentence, 'Mimi' sentence, and 'Nana' sentence) and three non-nasal sentences('Papa' sentence, 'Bibi' sentence, and 'Tatda' sentence). The change of nasalance score of six sentences were compared in three conditions, that is, without obstruction, with one nostril obstruction, and with both nostrils obstruction. The nasalance scores of one nostril obstruction and both nostrils obstruction were significantly lower than those of no obstruction. The mean nasalance scores of one nostril obstruction and both nostrils obstruction were lower than those of no obstruction by 10.93% and 48.88% in 'Mama' sentence, 3.72% and 5.76% in 'Papa' sentence, 12.28% nd 38.12% in 'Mimi' sentence, 7.28% and 12.96% in 'Bibi' sentence, 13.32% and 34.62% in 'Nana' sentence, 4.27% and 717% in 'Tatda' sentence, respectively. In both nostrils obstruction, the nasalance score of nasal sentences were decreased more than those of non-nasal sentences significantly. The result suggests that nasal sentences can be used as efficient stimuli in assessing the patients suspected of hyponasality. Eventhough non-nasal sentences also induced significant discrimination, the dynamic range was too small.

A Case Report of a Patient Diagnosed with Nasal Polyp Treated with Korean Medicine (한약 치료를 통해 비용종(Nasal polyp)이 소실된 부비동염 환자 치험 증례 1례)

  • Objective: The purpose of this case study was to report the effect of Korean medicine treatment on a patient diagnosed with a nasal polyp. Methods: A 62-year-old female patient with a nasal polyp visited the clinic sixteen times from September 11th, 2018 to December 18th, 2018. The patient was treated with Eunhwayeongyo-tang, Haeyeol-tang, Hongsamkyungok-go extract, and acupuncture during the treatment period. We evaluated the improvement of symptoms by nasoendoscopy. Results: After the Korean medicine treatment, the nasal polyp was completely eradicated. Hyperemia of the nasal mucous membrane and rhinorrhea were also relieved. Conclusion: The study findings suggested that Korean medicine treatment (Eunhwayeongyo-tang, Haeyeol-tang, and Hongsamkyungok-go extract, and acupuncture) might be effective for treatment of patients diagnosed with nasal polyps.

Tobacco Smoking Could Accentuate Epithelial-Mesenchymal Transition and Th2-Type Response in Patients With Chronic Rhinosinusitis With Nasal Polyps

  • Tobacco smoking (TS) has been known as one of the most potent risk factors for airway inflammatory diseases. However, there has been a paucity of information regarding the immunologic alteration mediated by TS in patients with chronic rhinosinusitis with nasal polyps (CRSwNP). To identify the effect of TS, we harvested human tissue samples (never smoker: n=41, current smoker: n=22, quitter: n=23) and analyzed the expression of epithelial-derived cytokines (EDCs) such as IL-25, IL-33, and thymic stromal lymphopoietin. The expressions of Th2 cytokines and total serum IgE showed a type-2 inflammatory alteration by TS. In addition, the epithelial marker E-cadherin and epithelial-mesenchymal transition (EMT)-associated markers (N-cadherin, α-SMA, and vimentin) were evaluated. Histological analysis showed that EDC expressions were upregulated in the current smoker group and downregulated in the quitter group. These expression patterns were consistent with mRNA and protein expression levels. We also found that the local Th2 cytokine expression and IgE class switching, as well as serum IgE levels, were elevated in the current smoker group and showed normal levels in the quitter group. Furthermore, the expressions of E-cadherin decreased while those of N-cadherin, α-SMA, and vimentin increased in the current smoker group compared those in the never smoker group. Taken together, these results indicate that TS contributes to the deterioration of pathogenesis by releasing local EDCs and Th2 cytokines, resulting in EMT in patients with CRSwNP. We verified that alterations of immunological response by TS in sinonasal epithelium can play a vital role in leading to CRSwNP.

Congenital Diaphragmatic Eventration in Neonates -A Report of 3 cases- (신생아에서의 선천성 횡격막 내번증;치험 3례)

  • Three neonates with congenital diaphragmatic eventration underwent intrathoracic operation had marked improvements in symptoms postoperatively. Two were one day of ages, one was 1 month of age, and they were all female and had other congenital abnormalities of lung hypoplasia, cleft palate, nasal polyps and neonatal hepatitis. The right diaphragm was more affected than left as 2:1. The repair for diaphragmatic eventration was performed successfully by plication of remnant diaphragm, and there were no complications postoperatively.

  • Chronic rhinosinusitis (CRS) is a multifactorial, heterogeneous disease characterized by persistent inflammation of the sinonasal mucosa and tissue remodeling, which can include basal/progenitor cell hyperplasia, goblet cell hyperplasia, squamous cell metaplasia, loss or dysfunction of ciliated cells, and increased matrix deposition. Repeated injuries can stimulate airway epithelial cells to produce inflammatory mediators that activate epithelial cells, immune cells, or the epithelial-mesenchymal trophic unit. This persistent inflammation can consequently induce aberrant tissue remodeling. However, the molecular mechanisms driving disease within the different molecular CRS subtypes remain inadequately characterized. Numerous secreted and cell surface proteins relevant to airway inflammation and remodeling are initially synthesized as inactive precursor proteins, including growth/differentiation factors and their associated receptors, enzymes, adhesion molecules, neuropeptides, and peptide hormones. Therefore, these precursor proteins require post-translational cleavage by proprotein convertases (PCs) to become fully functional. In this review, we summarize the roles of PCs in CRS-associated tissue remodeling and discuss the therapeutic potential of targeting PCs for CRS treatment.

Congenital Fibroepithelial Polyp Presenting as a Cutaneous Horn on the Nasal Tip: A Case Report (코에서 선천성 섬유상피폴립이 피부뿔로 발현한 환자 증례보고)

  • Cutaneous horn is the clinical entity, which is circumscribed, conical, markedly hyperkeratotic lesion in which the height of the keratotic mass amounts to at least half of its largest diameter. It may be associated with many different pathological lesions. It is a relatively rare and a kind of epidermal tumor that generally appears as a conical projection. Here, we report rare case of congenital cutaneous horn. A 39-month-old female Korean patient presented at our clinic with a mass at the tip of her nose present since birth. Under general anesthesia, cutaneous horn of nasal tip was completely excised without any complications. The operation site was small enough to perform a primary closure, without any nasal deformity. Histopathologically, it was reported as a fibroepithelial polyps. After operation, there is no evidence of recurrence at 16 months of follow-up.

  • Obstructive sleep apnea syndrome(OSAS) is a common disease in the field of otorhinolaryngology and is characterized by repeated upper airway occlusions occurring during sleep. OSAS can occur due to various etiologies of the nasal, oral, pharyngeal and laryngeal airway in adults. Nasal obstruction can be caused by septal deviation, nasal polyps, concha bullosa, choanal atresia, neoplasms, foreign body, postoperative/post-traumatic synechiae, various rhinitis and so on. There are various kinds of surgical treatment of OSAS including nasal surgery, LAUP, UPPP, surgery of tongue base, tracheostomy and so on, but the effect of nasal surgery on snoring and OSAS is controversial. The authors report the case of a patient who had experienced nasal obstruction, moderate snoring and OSAS and who improved after septoplasty and turbinoplasty.

Subepithelial neutrophil infiltration as a predictor of the surgical outcome of chronic rhinosinusitis with nasal polyps

  • Background: Neutrophils present as major inflammatory cells in refractory chronic rhinosinusitis with nasal polyps (CRSwNP), regardless of the endotype. However, their role in the pathophysiology of CRSwNP remains poorly understood. We investigated factors predicting the surgical outcomes of CRSwNP patients with focus on neutrophilic localization. Methods: We employed machine-learning methods such as the decision tree and random forest models to predict the surgical outcomes of CRSwNP. Immunofluorescence analysis was conducted to detect human neutrophil elastase (HNE), Bcl-2, and Ki-67 in NP tissues. We counted the immunofluorescence-positive cells and divided them into three groups based on the infiltrated area, namely, epithelial, subepithelial, and perivascular groups. Results: On machine learning, the decision tree algorithm demonstrated that the number of subepithelial HNE-positive cells, Lund-Mackay (LM) scores, and endotype (eosinophilic or non-eosinophilic) were the most important predictors of surgical outcomes in CRSwNP patients. Additionally, the random forest algorithm showed that, after ranking the mean decrease in the Gini index or the accuracy of each factor, the top three ranking factors associated with surgical outcomes were the LM score, age, and number of subepithelial HNE-positive cells. In terms of cellular proliferation, immunofluorescence analysis revealed that Ki-67/HNE-double positive and Bcl-2/HNE-double positive cells were significantly increased in the subepithelial area in refractory CRSwNP. Conclusion: Our machine-learning approach and immunofluorescence analysis demonstrated that subepithelial neutrophils in NP tissues had a high expression of Ki-67 and could serve as a cellular biomarker for predicting surgical outcomes in CRSwNP patients.

Primary sinonasal mucosal melanoma simulated as cystic lesions: a case report

  • Sinonasal mucosal melanoma (SNMM) in the maxillary sinus is a rare disease condition. Compared to oral mucosal melanoma, SNMM has a bulky, exophytic, and polypoid appearance, is weakly pigmented, and associated with unspecific symptoms. Due to these features, SNMM in the maxillary sinus has been misdiagnosed as nasal polyps and chronic sinusitis. In this case report, we described SNMM occurring in the right maxillary sinus simulated as a cystic or benign lesion. Cortical bone thinning and expansion were observed around the mass. The excised soft mass was encapsulated and weakly pigmented. The mass was clearly excised and covered with a pedicled buccal fat pad graft. Diagnosis using immunohistochemistry with S-100 and homatropine methylbromide-45 (HMB-45) is critical for proper treatment.
