Aesthetic Rhinoplasty for the Improvement of the Lateral Facial Profile; Image-up Rhinoplasty (얼굴의 측면 윤곽선 개선을 위한 미용 코성형술)

  • There are several characteristics of the nose of orientals. The dorsum of nose is flat and low, the skin is thick with severe tension, the nasal tip is bulbous, the nostril is wide, and the projection of the nose is limited due to a poorly developed alar cartilage with a short columella. In order to improve these untoward characteristics of the nose of Orientals aesthetically, plain augmentation of the dorsum and tip-plasty with conventional methods has been performed by many plastic surgeons. However these conventional rhinoplasty is not enough to obtain satisfactory results when transforming into a more beautiful and aesthetically charming appearance. In order to produce the optimal nasal shape and profile, it is extremely important to consider the aesthetic surgical factors, which are; the position of the nasion, the optimal nasolabial angle(95-100 degree in Orientals), the natural exposure of infra-tip lobule with and columella, the position of the tip defining point in harmony with the dorsal profile and the smooth and natural silhouette of the lateral nasal profile as it descends into the inferior portion of the nose. From April, 2003 to August, 2004, a total of 52 patients underwent open rhinoplasty, adhering to the strict aesthetic principles considered and described priorly. Surgical approach was done through a transcolumella incision and an alar rim incision. The nasal dorsum was augmented with a silicone implant and the shape of the columella and the nasolabial angle were finessed with a silicone strut implant which was placed in between the medial crurae in a manner of a non-visible graft. The nasal tip was corrected by alar cartilage suture technique and onlay graft of shield shaped Alloderm and Gore-Tex. Author obtained the optimal nasal shape and profile aesthetically, and the results, considered satisfactory in all patients without any complications, are as follows; 1. the average increase in nasal length was 2.5 mm, 2. the average decrease in nasal width was 2.1 mm, 3. the average increase in nasal tip projection was 3.2 mm, 4. the changes of nasolabial angles were from 85.5 degree to 94.7 degree, 5. the changes of the angle between the long axis of the external naris was from 101.3 degree to 89.5 degree. In conclusion, this surgical procedure is an effective, reliable and a valuable method in improving the nasal shape, tip projection, nasolabial angle and especially, the lateral facial profile of Orientals aesthetically.

Versatile midfacial degloving approach in oral and maxillofacial surgery

  • Objectives: Oral and maxillofacial surgeons must gain mastery of various approaches to the midface due to the increasing incidence, complexity, and severity of presenting midfacial fractures. Unlike in the case of other body parts, the need to preserve facial aesthetics makes it more difficult for the surgeon to select an approach for managing the facial injuries. The midfacial degloving (MFD) approach is a combination of intraoral and intranasal incisions made to access the midface without any external incision. The aim of the present study was to evaluate the efficacy of MFD in maxillofacial surgery and to assess its advantages and complications. Materials and Methods: The MFD approach was used in five cases, with three cases treated with open reduction and internal fixation and two cases operated on for posttraumatic deformity. Nasal dorsum augmentation was completed in three cases and nasal osteotomy was performed in one case. The bicoronal flap technique was combined with MFD for frontal bone augmentation in one case. The intraoperative time required for flap completion and the ease of performing the planned procedures were noted. Postoperative evaluation was done for reduction, aesthetics, function, and complications. Results: Access was excellent for performing all planned procedures. Average time spent for flap elevation and exposure of the midface was 63 minutes. Complications like postoperative swelling, infraorbital nerve paresthesia, and intranasal crusting were all transient. No long-term complications like stenosis of the nose, sneer deformity, or weakness of the facial muscles were noticed. Additionally, no complications were noted when MFD was combined with bicoronal flap. Conclusion: Though the MFD approach is technically demanding and takes more time than other facial approaches, it should be learned and applied by maxillofacial surgeons in selective cases, as it provides complete exposure of the midface without facial scarring.

Nasal Anthropometry on Facial Computed Tomography Scans for Rhinoplasty in Koreans

  • Background Cephalometric analysis is essential for planning treatment in maxillofacial and aesthetic facial surgery. Although photometric analysis of the Korean nose has been attempted in the past, anthropometry of the deeper nasal structures in the same population based on computerized tomography (CT) has not been published. We therefore measured three anthropometric parameters of the nose on CT scans in our clinical series of patients. Methods We conducted the current retrospective study of a total of 100 patients (n=100) who underwent a CT-guided radiological measurement at our institution during a period ranging from January of 2008 to August of 2010. In these patients, we took three anthropometric measurements: the nasofrontal angle, the pyramidal angle, and the linear distance between the nasion and the tip of the nasal bone. Results The mean nasofrontal angle was $131.14^{\circ}$ in the male patients and $140.70^{\circ}$ in the female patients. The mean linear distance between the nasion and the tip of the nasal bone was 21.28 mm and 18.02 mm, respectively. The mean nasal pyramidal angle was $112.89^{\circ}$ and $103.25^{\circ}$ at the level of the nasal root, $117.49^{\circ}$ and $115.60^{\circ}$ at the middle level of the nasal bone, and $127.99^{\circ}$ and $125.04^{\circ}$ at the level of the tip of the nasal bone, respectively. Conclusions In conclusion, our data will be helpful in the preparation of silicone implants for augmentation and/or corrective rhinoplasty in ethnic Korean people.

  • Implants for rhinoplasty should ideally be biocompatible and possess long-term stability after implantation. Silicone implants are most widely used for rhinoplasty. However, these implants suffer from problems related to high extrusion and infection rates. To minimize these complications, we propose a novel augmentation rhinoplasty technique using tissue engineering. To demonstrate its feasibility, a nasal-implant-shaped scaffold was designed using commercialized CAD software and fabricated using a Multi-head Deposition System, which is a solid freeform fabrication system that dispenses material. In vitro cell proliferation and chondrogenic differentiation tests were carried out using nasal septal chondrocytes.

Epicanthoplasty Using Y-V Advancement Flap Method (Y-V 전진피판술을 이용한 내안각췌피 성형술)

  • Purpose: The epicanthal fold is a unique finding in the medial canthal area of many Asians. Various methods have been developed to eliminate this fold. However, excessive and prominent scarring in the medial canthal and nasal area and recurrence restricted application of epicanthoplasty. The authors performed a epicanthoplasty using Y-V advancement flap method in order to obliterate the epicanthal fold without making incisions in the nasal area and as a result, to avoid postoperative scarring. Methods: Sixty one patients underwent epicanthal fold correction using Y-V advancement flap method from July 1999 to February 2005. There were 4 males and 57 females with ages ranging from 9 to 60 years. The epicanthoplasty was performed combined with double eyelid operation, ptosis correction, augmentation rhinoplasty, nasal alar reduction, and nasal tip-plasty. Results: There were few complications in our studies, and most of the patients were satisfied with the results. Conclusion: Remarkable advantages of our Y-V advancement flap epicanthoplasty are as follows: 1) minimal postoperative scarring in the medial canthal area, 2) application of modified double eyelid operation, 3) wider opening of the medial palpebral fissure, 4 the correction of entropion or epiblepharon, 5) no recurrence.

The Effect of Bangpungtongsungsan on Allergic Rhinitis Of Obese Mouse Fed High Fat Diet. (고지방식이로 비만을 유발한 마우스의 알레르기 비염에 대한 방풍통성산(防風通聖散)의 효과)

  • Objective : For various reasons, Obesity & Rhinitis are constantly rising. So Interest of treatment has been expanding. We want to verify The Chungyeol (fire extinguishing) Lisup (Draining) effect of Bangpungtongsungsan on obese model of allergy rhinitis.Material and Methods : BALB/c mouse were divided four groups: control(CON), allergic induction(ARE), Bangpungtongsungsan extract administration(BTT), Bangpungtongsungsan double concentration extract administration (BT2T). Every group except control group were caused allergic rhinitis by Ovalbumin. BTT & BT2T were orally administered the Bangpungtongsungsan for 21days. Since then we observed the liver tissue cell and the nasal mucous membrane.Results : In comparison with ARE, experimental groups show relief of the nasal mucous membrane damage(secretion of mucus decrease, Itching decrease), Th2 eruption control(IL-4 decline), effect of anti-inflamatory(reducing TNF-α creation, decreasing of iNOS through NF-κB activation-inhibition). In addition, experimental groups show a loss in weight, diminished accumulation of fat. (decreasing within liver tissue, reducing TNF-α creation) BT2T is more effective to BTT.Conclusion : Bangpungtongsungsan treat obese model on allergy rhinitis thereby control fat augmentation, relieving inflammation.

A HISTOLOGICAL STUDY ON SEVERAL IMPLANTS FOR AUGMENTATION RHINOPLASTY IN MOUSE CALVARIUM (쥐의 두개골에서 융비술에 사용되는 수종의 이식재에 대한 조직학적 연구)

  • Purpose: This study was aimed to histologically evaluate $Silicone^{(R)}$, $Gore-tex^{(R)}$, $AlloDerm^{(R)}$, and $Medpor^{(R)}$ implants for augmentation rhinoplasty after graft in the subperiosteum of mural calvarium respectively. Materials and method: Twenty four male ICR mice were used. $Silicone^{(R)}$, $Gore-tex^{(R)}$, $AlloDerm^{(R)}$, and $Medpor^{(R)}$ were grafted respectively in the subperiosteum of frontal bone. Animals were sacrificed at 1 week, 4 week and 8 week after graft. Histological observation was performed after H&E staining. Results: All groups were healed without any extrusion of implant materials and inflammatory cell infiltration. In Silicone group, $Silicone^{(R)}$ was well enclosed by thin fibrous tissue at 1 week, which became thicker and stable at 4 weeks and 8 weeks. And there was no destruction or resorption of $Silicone^{(R)}$ In Gore-tex group, there was no destruction or resorption of $Gore-tex^{(R)}$. Thin fibrous tissue and cell infiltration from peripheral tissue were observed at 1 week, 4 weeks and 8 weeks. In AlloDerm group, $AlloDerm^{(R)}$ was enclosed by fibrous tissue. Cell infiltration was observed at 1 week, 4 weeks and 8 weeks. In Medpor group, there was no inflammation, destruction or resorption of $Medpor^{(R)}$ and it was contacted directly to the bone without interposition of fibrous tissue. Porous area was filled by bone or soft tissue. Conclusion: These results suggest that $Gore-tex^{(R)}$, $AlloDerm^{(R)}$, and $Medpor^{(R)}$ graft are more stable than $Silicone^{(R)}$ graft and that $Silicone^{(R)}$, $Gore-tex^{(R)}$, $AlloDerm^{(R)}$ are appropriate for graft on nasal tip and $Medpor^{(R)}$ is appropriate for graft on nasal dorsum.

Modified Septal Extension Graft for the Correction of Nasal Tip and Columella (코끝과 코기둥의 교정을 위한 변형된 코중격 연장 이식술)

  • Purpose: The nose of most Koreans is characterized as a low nasal dorsum, retracted columella, and an acute columella-labial angle. For the surgical correction of the tip and columella, a modified septal extension graft, along with augmentation rhinoplasty has been developed. Methods: With this technique, a septal extension graft is fixed on the entire caudal margin of the septum (the cephalic-caudal axis) and at the same time, it is placed above the anterior nasal spine, in the membranous portion of the septum and at the base of the columella(the anterior-posterior axis). The present report describes the results obtained in 13 patients and offers an analysis of the results as judged by the columella-labial angle and 4 proportional indices(nose height index, nasal bridge length index, nasal tip projection index, columella length index), measured by photogrammetry. Results: The postoperative values obtained in these 5 categories increased significantly compared to the preoperative ones, thus confirming that the projection of tip was augmented, the nose was lengthened, and the columella was advanced caudally and lengthened. Moreover, these positive outcomes were still maintained during the follow-up period, and no side effects, such as saddle nose deformity, were reported. Conclusion: A modified septal extension graft can be considered as an effective method for the surgical correction of the nasal tip and columella in Koreans.

Correlations between anatomic variations of maxillary sinus ostium and postoperative complication after sinus lifting

  • Objectives: The maxillary sinus mucosa is reported to recover to preoperative sterility after sinus floor elevation. However, when drainage of maxillary sinus is impaired, recovery can be delayed and maxillary sinusitis can occur. Therefore, in this study, we investigated the correlations between anatomic variants that can interrupt the ostium of the maxillary sinus and incidence of complication after sinus lifting. Materials and Methods: The subjects are 81 patients who underwent sinus lifting in Wonkwang University Dental Hospital (Iksan, Korea). Computed tomography (CT) images of the subjects were reviewed for presence of nasal septum deviation, anatomic variants of the middle turbinate, and Haller cells. Correlations between anatomic variations and occurrence of maxillary sinusitis were statistically analyzed. Results: Patients with anatomic variants of ostio-meatal units, such as deviated nasal septum, concha bullosa or paradoxical curvature of the middle turbinate, or Haller cells, showed a higher rate of complication. However, only presence of Haller cell showed statistically significant. Conclusion: Before sinus lifting, CT images are recommended to detect anatomic variants of the ostio-meatal complex. If disadvantageous anatomic variants are detected, the use of nasal decongestants should be considered to reduce the risk of postoperative sinusitis.

A COMPARATIVE STUDY ON THE CORRECTION METHODS OF NOSTRIL IN PATIENTS WITH CLEFT LIP NASAL DEFORMITY (구순열비변형 환자에서 비교정술에 대한 비교 연구)

  • The secondary correction of cleft lip nasal deformity (CLND) presents difficult surgical problems. Characteristically, nostrils are asymmetric. The present study was aimed to examine and compare the effect of Straith's alar web Z-plasty, Millard's alar web Z-plasty, alar web excision, and lateral V-Y advancement of the alar base for augmentation of the nostril with or without lengthening the columella in CLND. The subjects were 28 patients with unilateral cleft lip, who had secondary nostril correction. The nostril correction methods were Straith's alar web Z-plasty, Millard's alar web Z-plasty, alar web excision, and lateral V-Y advancement of the alar base. Facial photographs were taken before and 20 days after the operation. By using Adobe photoshop, the columella length and the nostril width were measured from the facial frontal photograph and Worm's eye view. The degree of improvement was calculated and statistically analyzed. The degree of improvement of the columella length using Straith's alar web Z-plasty was 70.20%. And then Millard's alar web Z-plasty was 55.01%, alar web excision was 39.93%, and lateral V-Y advancement of the alar base was 16.38% in order. The degree of improvement of the nostril size using lateral V-Y advancement of the alar base was 55.26%. And then alar web excision was 52.72%, Millard's alar web Z-plasty was 34.86%, and Straith's alar web Z-plasty was 16.06% in order. Straith's alar web Z-plasty and Millard's alar web Z-plasty resulted in elongation of the columella, equalization of asymmetrical nostril, and enlargement of small nostrils. Alar web excision enlarged nostrils and restored symmetry. Lateral VY advancement of the alar base increased nostril width and enlarged nostrils. These results indicate that the correction of nostrils improve the shape and the symmetry of the nostrils in CLND.
