목적: 임플란트 보철 수복에서 심미적 요구도가 증진되면서 최근에는 고강도, 고인성을 지니면서 생체적합성 우수하고 치아의 색과 조화로운 지르코니아 지대주의 사용이 증가하는 추세이다. 몇몇 임상보고를 통해 지르코니아 지대주의 우수한 주위조직 반응과 단기간의 성공적인 결과가 보고 되었으나, 장기간의 안정성을 평가하는 기계적 강도에 관한 연구는 미흡한 실정이다. 이에 본 연구에서는 직경, 각도, 연결방식이 상이한 임플란트 고정체 - 지르코니아 지대주의 단순 파절강도와 반복하중에 의한 피로한계 측정을 통해 지르코니아 지대주의 기계적 안정성을 평가해 보고자 하였다. 재료와 방법: ISO규정에 따라 지르코니아 지대주-고정체를 변연골이 3mm 흡수된 조건에서 $30^{\circ}$경사하중으로 압축굽힘강도를 측정하고, 그 값의 80%를 최대하중으로 10%를 최소하중으로 하여 10Hz의 sine형 반복하중을 $5{\times}10^6$ 주기를 초과하는 조건의 내구성한계를 측정하였다. 지르코니아 지대주로는 regular diameter를 가지면서 external butt joint의 straight 지대주와 $17^{\circ}$ angled 지대주, 그리고 narrow diameter 이면서 external butt joint의 straight 지대주와 internal conical joint의 straight 지대주를 사용하였다. 압축굽힘강도 값을 일원분산분석과 사후검정으로 검증하였으며, 피로파절면을 전자주사현미경을 통해 관찰했다. 결과: 압축굽힘강도는 최소 927N 이상으로, 직경의 차이에 따른 유의한 차이를 보였으며( P<.05), 내구성 한계는 503N에서 868N까지의 번위를 보였다. 결론: 지르코니아 지대주는 구강내 기능하중을 견딜수 있는 적절한 기계적 강도를 가지는 것으로 판단된다.
The modified ridge splitting/expansion technique combined with guided bone regeneration (GBR) for implant surgery is used to expand the narrow and atrophied edentulous alveolar ridge. Also, the simultaneous implant placement after ridge splitting/expansion technique can reduce the treatment and healing time. This case report includes three patients with a narrow edentulous alveolar ridge of the 2 to 4mm. All three patients underwent a fracture of thin buccal cortical bone plate, and these defects were corrected by the use of the guided bone regeneration (GBR). After 7 to 18 months, all surgical area was stable, and all implant showed a good healing state on the clinical and radiographic examination. In conclusion, though this surgical method is technique sensitive, the modified ridge splitting/expansion technique combined with GBR for implant surgery is recommended for a horizontal augmentation in the narrow edentulous alveolar ridge.
본 증례는 치조골의 흡수가 심한 두 명의 완전무치악 환자에서 서로 다른 임플란트 시스템을 이용한 하악의 미니-임플란트 지지 피개의치에 대한 증례보고이다. 두 환자는 하악 의치의 유지력 부족으로 인한 의치 사용시 불편감 및 동통을 주소로 호소하였다. 각각의 환자는 4개의 미니 임플란트를 이공사이에 판막의 거상없이 식립하였다. 첫번째 증례는 미니볼 어태치먼트와 일체형으로 된 미니-임플란트(Slimline, Dentium, Seoul, Korea)를 식립하고 구강내 직접법으로 어태치먼트를 연결하여 즉시하중을 시행하였다. 두번째 증례는 분리형으로 로케이터 어태치먼트와 연결되는 미니-임플란트(LODI, Zest Anchors, Escondido, CA, USA)를 이용하였으며, 임플란트 식립 8주 후 간접법으로 어태치먼트를 연결하였다. 최종 의치 장착 후 6개월 정기검진까지 미니볼과 로케이터 어태치먼트를 이용한 하악의 미니-임플란트 지지 피개의치 증례에서 환자의 전반적인 만족도가 개선되었으며 주목할 만한 합병증 없이 만족할 만한 임상적 결과를 얻었다. 단 추후 지속적인 유지관리 및 장기적인 평가가 더 필요하리라 사료된다.
하악 완전 무치악 환자의 보철수복에 있어 임플란트 식립 후 피개의치의 제작은 첫 번째 치료 옵션으로 고려되어질 수 있다. 임플란트 지지 텔레스코프 피개의치를 제작하는 경우 인공치 및 유지 장치의 배열을 위한 악간 공간이 필요하다. 임플란트 상부 보철물의 passive한 fit은 보철물의 기계적 실패를 막는 중요한 요소이다. Gold electroforming system을 이용하여 제작한 telescopic attachment는 우수한 마진 적합도 및 passive한 fit을 보이며 얇은 코핑 두께로 인하여 좁은 악간 공간에서도 사용이 가능한 장점이 있다. 본 증례에서는 4개의 임플란트 식립 후 Gold electroforming system을 이용하여 telescopic overdenture를 제작하여 기능 및 심미적으로 좋은 결과를 나타내었다.
Objective: The objective of this study was to evaluate the osseointegration of narrow-diameter implant with trapezoid-shape and to evaluate the effect of coronal microthreads on trapezoid-shape narrow-diameter implant. Materials and Methods: The experimental narrow-diameter implants were classified into two groups according to absence or presence of coronal microthreads: trapezoid-shape narrow diameter implant (TN group) and trapezoid-shape narrow-diameter implant with microthreads (TNM group). They were installed alternately in bilateral mandible in three dogs. After 8 weeks, the animals were sacrificed. Resonance frequency analysis, removal torque test, and histometric analysis were performed. Results: Statistically higher implant stability quotient (ISQ) values were observed in TNM group than in TN group at the time of implant installation. However, significant ISQ values difference was not observed between groups at 8 weeks. Both groups showed significantly increased ISQ values at 8 weeks, compared to the time of implant installation. There was no significant difference between groups in removal torque test. Bone-implant contact ratio also showed no significant difference between groups in total and coronal part. Conclusion: Within the limitation of this study, it could be concluded that the trapezoid-shape design on narrow-diameter implant showed successful ossointegration, and the microthreads on coronal part did not result in significant bone-implant contact and biomechanical stability at 8 weeks.
완전, 혹은 부분 무치악 환자의 가철성 보철물 수복치료에서 치아 상실에 따른 치조골 감소와 별형은 의치 사용에 어려움을 준다. 이런 문제를 극복하기 위해 임플란트를 이용하여 가철성 보철물에 지지, 유지, 및 안정을 제공할 수 있다. 임플란트를 이용한 피개의치에 사용되는 어태치먼트는 여러 종류가 있는데, 환자의 여러 조건을 고려하여 어태치먼트를 선택해야 한다. 그 중 milled bar의 경우 임플란트 간 거리가 좁아서 기성 bar를 사용할 수 없는 경우, 또는 기성 bar보다 상부 구조물의 확고한 안정성과 지지를 얻고자 할 때 선택 된다. Milled bar는 cross arch stabilization을 이룰 수 있고, 저작 시 의치의 움직임이 없어 환자에게 편안함을 제공하지만, 숙련된 기공과정을 요한다. 그러나 최근 CAD/CAM의 발전을 통해 milled bar의 제작이 간결해지고 적합성을 증진시킬 수 있었다. 본 증례는 8개의 임플란트가 식립된 하악 완전 무치악의 알츠하이머 환자로 milled bar와 magnet을 이용한 피개의치를 제작하였다. 전신병력이 양호해진 기간 동안 빠른 치료종결이 필요했기 때문에, 통법의 기공 작업을 CAD를 통해 시뮬레이션하여 복잡한 기공과정을 좀더 단순하게 하였고 이로써 치료 기간을 줄일 수 있었다. 이상과 같이 milled bar를 이용한 임플란트 지지 피개의치를 제작하는 과정 중 CAD/CAM system을 적절히 사용하여 심미적, 기능적으로 만족할 만한 결과를 얻었기에 보고하고자 한다.
다양한 소재(금속, 세라믹, 고분자 소재 등)들이 3차원 형상기반 적층제조법에 적용되고 있는데, 금속 소재를 이용하여 3D 프린팅 법으로 치과용 수복물을 제조하는 연구가 많이 보고되고 있다. 하지만, 티타늄 또는 티타늄 합금 분말을 이용하여 3D 프린팅 법으로 제작한 치과용 보철물에 관한연구 보고는 많지 않다. Kanazawa 등 (2014)은 Ti-6Al-4V 합금분말을 이용하여 SLM법으로 총의치 용 framework를 제작하여 주조법으로 제작한 것과 비교 평가하였고, Mangano 등(2013)은 Ti-6Al-4V 합금분말로 지름이 작은 일체형 (1-piece narrow-diameter) 임플란트를 SLS법으로 제작하여 16명의 환자에게 식립한 다음, 2년간 관찰하였고, Mangano 등 (2014)은 cone-beam computed tomography (CBCT) data를 3D이미지로 변환시켜 DLMS법으로 치근 형상의 임플란트를 제작하여 15명의 환자에게 식립한 다음, 1년간 관찰하였다. 또한 서울대학교 및 연세대학교 치과생체재료과학교실 (2016)에서는 3D 프린팅 법으로 제작한 티타늄 시편과 기계 가공한 티타늄 시편의 물성을 비교하였다. 그러나 티타늄 합금 분말을 이용하여 3D 프린팅 법으로 제작한 치과용 보철물을 실제 임상에 적용하는 단계에서 기존 기계가공 방식으로 제작한 티타늄 보철물과 3D 프린팅 법으로 제작한 티타늄 보철물의 물성과 표면특성을 다양하게 비교 평가하는 것이 필요하여 본 연구에서는 3D 프린팅 법으로 제작한 티타늄 시편과 기계 가공한 티타늄 시편의 물성특성과 표면특성을 비교하여 조사하였다.
Long-term survival and prognosis of narrow-diameter implants have been reported to be adequate to consider them a safe method for treating a deficient alveolar ridge. The objective of this study was to perform case report of narrow-diameter implants with a trapezoid-shape in anterior teeth alveolar bone. A 50-year-old male patient presented with discomfort due to mobility of all of the maxillary teeth and mandibular incisors. Due to destruction of alveolar bone, four anterior mandibular teeth were extracted. Soft tissue healing was allowed for approximately 3 months after the extraction, and a new design of implant placement was planned for the mandibular incisor area, followed by clinical and radiological evaluation. Implant placement was determined using an R2GATE surgical stent. The stability of the implants was assessed by ISQ measurements at the first and second implant surgery and after prosthetic placement. At 1 and 3 months and 1 year after implantation of the prosthesis, clinical and radiological examinations were performed. Another 50-year-old male patient presented with discomfort due to mobility of the mandibular central incisors. For the same reason as in the first patient, implant placement was carried out in the same way after extraction. ISQ measurements and clinical and radiological examinations were performed as in the previous case. In these two clinical cases, 12 months of follow-up revealed that the implant remained stable without inflammation or additional bone loss, and there was no discomfort to the patient. In conclusion, computer-guided implant surgery was used to place an implant in an optimal position considering the upper prosthesis. A new design of a narrow-diameter implant with a trapezoid-shape into anterior mandibular alveolar bone is a less invasive treatment method and is based on the contour of the deficient alveolar ridge. Through all of these procedures, we were able to reduce the number of traumas during surgery, reduce the operation time and total treatment period, and provide patients with more comfortable treatment.

Introduction: Mini-implant system is applicable to areas of narrow space and area requiring temporary loading support. The purpose of this study was to evaluate the clinical outcome of a mini-implant system as well as the application of mini-implant system in the dental clinical field. Materials and Methods: The patients who had been operated from Jan 2007 to Dec 2007 in the four dental facility including Seoul National University Bundang Hospital were enrolled. To evaluate the factors associated with the clinical outcome, the patients were classified according to gender, age, area of surgery, type of implant, diameter and length of the implant, and the purpose of the mini-implant system application. Results: From 147 implants, only three implants failed, one of them was for temporary loading. There were no serious surgical or prosthetic complications in this study. Conclusion: An analysis of the preliminary data revealed a satisfactory clinical outcome. However, more long-term evaluation of narrow ridge type as well as the patient’s satisfaction on the use of a provisional type mini-implant system is needed.

For implant treatment there must be sufficient bone to house the implant body. At least 5mm wide residual bone is needed and usually a 6mm width is preferred by clinicians. However, surgeons sometimes find patients with a narrow ridge, which makes it difficult to place an implant. Therefore, many clinicians perform bone graft or a ridge splitting technique to overcome these poor conditions. The time and cost can be reduced using the ridge splitting technique with immediate implant placement. Recently, many studies reported reliable consequences of ridge splitting technique. This paper reports a successful of implant placement with a ridge splitting technique in a very thin alveolar ridge.
