Renal Cell Carcinoma is More Aggressive in Turkish Patients with the Metabolic Syndrome

  • Background: Metabolic syndrome (MetS) is a multifactorial disease characterized by impaired glucose tolerance/diabetes, obesity, high triglyceride levels, low HDL levels, and hypertension. In this study we evaluate the relationship between tumor size and grade, and presence of the metabolic syndrome in patients with renal cell carcinoma. Materials and Methods: Between 2007-2013, radical nephrectomy was performed for 310 patients with renal tumors in our clinic and those with pathology reported renal cell carcinoma were enrolled and divided into two groups, with and without metabolic syndrome diagnosed on the basis of the National Cholesterol Education Program (NCEP) Adult Treatment Panel III (ATP III) criteria. The relationship between tumor size and grade of the two groups (Fuhrman nuclear degree) was evaluated statistically. Results: The metabolic syndrome was found in 70 patients, with a mean age of 65.5 (40-87), as compared to 58.8 (31-84) years in the non-metabolic syndrome group. Tumor size over 7 cm was found in 54% and 33%, respectively, and tumor grade over Fuhrman 3 in 56% and 32% of patients. Patients with metabolic syndrome had significantly higher tumor size and grade (p<0.05). In the presence of hypertension, diabetes and high triglyceride levels, significant assocations were again observed (p<0.05). Tumor size and degree also increased with increasing body mass index but this was not statistically significant (p>0.05). Conclusions: Renal cancer is more aggressive in patients with metabolic syndrome. Lifestyle and risk factors were revealed to be significant influences in renal cancer patients.

Comparison of LDL-Cholesterol direct measurement with the estimate using various formula

  • Low-Density Lipoprotein cholesterol (LDLC) is the most important marker for the treatment of hyperlipidemia in NCEP-ATP III(National Cholesterol Education Program-Adult Treatment Panel III) guideline. Therefore, LDL cholesterol is pathologically meaningful, accurate measurement should be a top priority. Currently, LDLC is directly measured in most cases, but, the estimate is still used in mass health examination or screening test. This study is about the comparison of LDL-Cholesterol direct measurement with the estimate using various formula (Friedewald: [LDL-F=TC-HDL-TG/5], Nakajima: [LDL-N=TC-HDL-TG/4], Hattori: [LDL-H =0.94TC-0.94HDL-0.19TG], Puavilai: [LDL-P=TC-HDL-TG/6], Carvalho: [LDL-C=3(TC-HDL)/4]) for calculating more accurate value. We analyzed total cholesterol (TC), try-glyceride (TG), high-density lipoprotein cholesterol (HDLC), and LDLC levels of 210 subjects between June and November in 2011. Until now, the Friedewald formula is the most commonly used estimate for the LDLC. When Friedewald formula was applied, the correlation coefficient (r) was 0.940, showing high correlation. But, the result of the direct method was significantly different, compared with those of the Friedewald formula in triglyceride levels ${\geq}400mg/dL$(p<0.05). There was the highest correlation when we used LDL-P formula(r=0.947) in triglyceride levels <400 mg/dl. Also there was the lowest mean difference regardless of triglyceride level. Therefore, the study showed that TG/6 is more precise means of calculation than TG/5. On the other hand, the calculation of LDL-Cholesterol was underestimated, compared with direct measurement. It is necessary to have more data and modified Friedewald formula should be used for the accurate calculation.

한국 전통시장 여성근로자 건강증진 프로그램 모형개발: 지역사회 참여형 연구틀 기반 (Intervention Model Development of Health Promotion for Women Workers in Traditional Marketplaces: Using Community based Participatory Action Research)

  • Purpose: The purpose of this study was to describe the intervention model development of health promotion for women workers working in the traditional marketplace, using community capacity building framework. Methods: Need assessment of the target population, work-related environment investigation, social network group building, and setting health management in the marketplace were performed. Then the interventions including cardiovascular health, musculoskeletal health, and psychological health were conducted. The results were evaluated using NCEP-ATP III(National Cholesterol Education Program's Adult Treatment Panel III), OWAS, and CES-D. Results: It was found that the intervention program for the vulnerable group in marketplace was appropriate, promoting the improvement of metabolic syndrome and the reduction of pain complaints. Therefore, the intervention framework for health promotion of women workers in the marketplace was developed. Work-related environment assessment also was included in the framework development. Several community capacity building strategies, including developing community resources and promoting partnership, making small social network group, and promoting program participation. Conclusion: It is suggested that occupational health nurses and professionals consider the appropriateness of intervention framework development after identifying the needs of women workers' work-related environmental problems.

사상체질과 대사증후군 유발에 관한 코호트 연구 (Metabolic Syndrome and Sasang Constitution in Cohort Study)

  • Objectives This study is to investigate the related factors to contribute the metabolic syndrome according to Sasang Constitution. Methods Nine hundred twenty six persons out of 1774 persons, over 40 years old, participated in community-based cohort in Wonju City of South Korea from June 2006 to August 2009. The diagnosis of metabolic syndrome was carried out by NCEP-ATP III(National Cholesterol Education Program in Adult Treatment Panel III) and Asian Pacific Criteria for abdominal obesity. The related factors were checked using questionnaire and blood samples. Sasang Constitution was verified by a Sasang Constitution specialist using the results of PSSC(Phonetic System for Sasang Constitution), facial pictures and simplified Sasang Constitutional questionnaires. Metabolic syndrome incidence rate according to Sasang Constitution and binary logistic regression analysis were performed with SPSS 19.0. Results Metabolic syndrome incidence rate was 30.3% and the majority of newly categorized as metabolic syndrome was Taeeumin(40.7%). There were significant risk factors like systolic blood pressure, fasting blood sugar, triglyceride and female and a significant defense factor like HDL-cholesterol. In terms of constitutional view, there were significant risk factors like waist circumference, systolic blood pressure, triglyceride in Soyangin, female, waist circumference, systolic blood pressure, fasting blood sugar, triglyceride in Taeeumin, female, waist circumference, systolic blood pressure, triglyceride in Soeumin. And there was a significant defense factor like HDL-cholesterol in only Taeeumin. Conclusions Regimens on metabolic syndrome were considered to be changed according to Sasang Constitution. Taeeumin female and Soeumin female should be cautious of body weight and metabolic syndrome when elderly. There are more cautious risk factors in each constitution; systolic blood pressure and triglyceride in Soyangin and fasting blood sugar and serum lipids levels in Soeumin and Taeeumin.

  • Purpose: This study was to examine the effects of 12-week Tai Chi exercise on the autonomic nervous system activity and risk factors of metabolic syndrome in patients with metabolic syndrome. Methods: A single group pre-posttest was used. Subjects were 16 participants aged 51~71 years with metabolic syndrome. All of the subjects were met the criteria of the National Cholesterol Education Program-Adult Treatment Panel III (NCEP-ATP III). The Tai Chi exercise consisted of 19 movements from Yang and Sun styles provided twice a week for 3 months. Metabolic syndrome risk factors including waist circumference, blood pressure, glucose, triglyceride, high density lipoprotein cholesterol (HDL-cholesterol) were measured. A heart rate variability device was used to measure the activity of autonomic nervous system. Results: After 12-week Tai Chi exercise, systolic pressure, waist circumference, and HDL-cholesterol improved significantly. Tai Chi exercise had no effect on the autonomic nervous system activity. Conclusion: The 12-week Tai Chi exercise was partially effective in managing risk factors of metabolic syndrome. A larger study with longer period to improve the autonomic nervous system activity in patients with metabolic syndrome is needed.

일부 우리나라 성인의 대사성증후군의 유병률과 허리둘레 예측치 (Prevalence of Metabolic Syndrome and Waist Circumference Estimation in Some Korean Adults)

  • Objectives : The purpose of this study was to estimate the prevalence of metabolic syndrome, as defined by the ATP III report, in some Korean adults and use the Asian-Pacific proposed waist circumference to investigate waist circumference in some Korean adults using ROC curves. Methods : Study subjects were seventy-five thousands and ninety one persons(47,979 men and 27,111 women) who were selected among the patients who visited hospital for health evaluation from January 2000 to December 2001. All subjects were measured by height, weight, waist and hip circumferences, blood pressure and blood chemistry(lipid profile). Results : The mean age was $41.6{\pm}8.5$ years in men, $41.1{\pm}10.4$ years in women(p<0.05). Body mass index was in the normal range in 35.3% of men, and 55.9% of women. In both men and women, blood pressure, blood sugar, total cholesterol and triglyceride were positively correlated with BMI. waist circumference, and Broca's index(p<0.01). However HDL. choloesterol was correlated negatively (p< 0.01). Using ROC curve, the calculated waist circumferences were 84 cm in men(sensitivity 61.4% and specificity 64.1%) and 74 cm in women(sensitivity 65.0% and specificity 73.2%). The age adjusted prevalences of the metabolic syndrome as defined by NCEP ATP III were different for men(6.4%) and women(14.6%). The prevalence increased from 1.2% among participants aged 20 through 29years to 15.0% among participants aged over 60years in men(p<0.05) and from 1.6% to 27.4% respectively, in women. The age adjusted prevalences, as defined by using the waist circumference that was recommended by WHO's regional office for the western Pacific, were 10.6% in men and 18.5% in women. The age adjusted prevalences, as defined by using the waist circumference that was calculated by the ROC curves, were 17.1% in men and 22.4% in women. And All prevalences were increased following increased BMI and Broca's index. Conculsions : The prevalence of metabolic syndrome in some Korean adults was lower than that in western adults. Nevertheless because waist circumference was differed among race and region, application of the same criteria was not proper. Morcover, a higher awareness was required in women, because the prevalence of metabolic syndrome was rapidly increased with increment of age.

아동 및 청소년의 대사증후군 유병여부에 따른 식생활 평가 및 관련 요인: 2007~2013 국민건강영양조사 자료를 중심으로 (Dietary Assessment and Factors Related to Prevalence of Metabolic Syndrome in Korean Youth: Based on the Korea National Health and Nutrition Examination Survey 2007~2013)

  • The aim of this study was to assess a dietary status and to examine the factors related to the prevalence of metabolic syndrome in Korean children and adolescents. For this study, 5,576 subjects aged 10~18 years, who participated in the health and dietary intake survey (24h recall method) of the 2007~2013 Korea National Health and Nutrition Examination Survey (KNHANES), were sampled. The five components for the diagnosis of metabolic syndrome in the subjects were taken from the modified NCEP-ATP III. The total prevalence of metabolic syndrome among the subjects was 4.6%. The total prevalences of the metabolic syndrome components among the subjects were central obesity 8.4%, hypertriglyceridemia 18.8%, low HDL-cholesterol 13.4%, hypertension 22.4%, and hyperglycemia 5.2%. The gender, age, weight status, frequency of daily meals and eating-out frequency of subjects affected the prevalence of metabolic syndrome. Based on these results, There should be to improve the dietary guidelines and nutrition education to decrease the prevalence of metabolic syndrome for Korean children and adolescents.

대사성증후군 중재연구 논문 분석 (The Analysis of Intervention Studies for Patients with Metabolic Syndrome)

  • Purpose: This study was to analyze the trend of research on intervention for patients with metabolic syndrome. Method: Using Pubmed, Medline, and CINAHL search engines, a randomized controlled trial(RCT) researching titles such as 'metabolic syndrome', 'intervention', 'lifestyle modification', or 'community-based' were collected. A total of 16 researches were analyzed based on the guidelines of the National Cholesterol Education Program - Adult Treatment Panel(NCEP-ATP III). Results: 1) The total period of the intervention was from 12 to 24 weeks, the frequency was 3 to 5 times per week, and the duration of each session was from 45 to 60 minutes. The types of intervention included exercise, diet, and medication. Among these types, diet was performed most frequently. 2) The outcomes of the intervention was measured with physical aspects such as anthropometric measures, body composition, or biological markers. No studies have evaluated psychosocial outcomes such as quality of life. 3) In terms of effectiveness of the intervention, anthropometric indicators, body composition, or serological markers showed positive effects, whereas results on endothelial or urine indicators were inconsistent. Conclusion: Methodological research developing comprehensive therapeutic lifestyle modification programs and intervention studies are needed for patients with metabolic syndrome. In addition, effects should be evaluated with multidimensional perspectives.

한국 성인 여자에서 복부비만에 따른 만성질환 위험 비교 - 2013~2014년 국민건강영양조사 자료를 이용하여 - (Comparison of Chronic Disease Risk by Abdominal Obesity in Korean Adult Women - Using Data from the 2013~2014 Korea National Health and Nutrition Examination Survey -)

  • This study was conducted to investigate the risk of chronic disease as predicted by abdominal obesity in Korean adult females. Data on 2,738 adult females aged 40~64 yrs was obtained from the 2013~2014 Korean National Health Nutrition Examination Survey. The subjects were divided into normal (n=1,835), abdominal obesity (AO, n=73), and BMI-diagnosed abdominal obesity (BMI-AO, n=505) groups based on the NCEP-ATP III guidelines and by applying the KSSO definition regarding waist circumference. Triglyceride blood levels, fasting blood sugar levels, systolic blood pressure, and diastolic blood pressure were higher in the AO and BMI-AO groups than in the normal group. The HDL-cholesterol levels of subjects in the two abdominal groups were lower as compared to those of subjects in the normal group. The mean adequacy ratio was lower among subjects in the AO and BMI-AO groups than among those in the normal group. Moreover, the NAR and INQ scores of some micro-nutrients in both of the abdominal obesity groups were lower than those in the normal group. Both the AO and BMI-AO groups showed significantly higher risks of hypertriglycemia, hyperglycemia, hypertension, hypoHDL-cholesterolemia, and metabolic syndrome than were shown by the normal group. Notably, as compared to the normal group, the AO group showed higher risk of hypercholesterolemia, hyperLDL-cholesterolemia, hypertriglycemia, and metabolic syndrome than did the BMI-AO group. This finding suggests that it is essential to develop prevention programs including programs for those with abdominal obesity despite their having a BMI within the normal range.

한방병원 건강검진 수진자의 대사증후군 발생과 관련요인 연구 (Incidence and Related Factors of the Metabolic Syndrome in a Korean Medicine Hospital)

  • The purpose of this study was to investigate incidence and related factors of the metabolic syndrome in a Korean medicine hospital. The 716 subjects were analyzed using biochemical data and survey who took medical examination in Daejeon Korean Medicine Hospital for general health check-up. This investigation was conducted from February in 2008 to July in 2010. The metabolic syndrome was diagnosed according to the definition by the NCEP ATP III. The abdominal obesity guidelines for waist circumference applied by the WHO Western Pacific Region, IASO and IOTF: The Asia-Pacific Perspective in 2000. Incidence of metabolic syndrome was 12% (14.6% in men, 8.2% in women). The groups that have two metabolic risk factors were 21.9% in men and 7.5% in women. The incidence increased with ageing. The mean of metabolic syndrome`s triglyceride was in hypertriglyceridemia, and that of their BMI in men was in primary obese and that of their AST, ALT, ${\gamma}$-GTP means were in abnormal liver function. Smokers in men have metabolic syndrome 10 times more than non-smokers in men. Exercisers that do the exercise once or twice a week in women have metabolic syndrome 0.2 times more than non-exerciser in women. Women that have family history of stroke, were associated with metabolic syndrome by $x^2$-test. Men that have family history of hypertension, have metabolic syndrome 4 times more than otherwise men. Men that have family history of diabetes mellitus, have metabolic syndrome 3 times more than otherwise men.
