본 종설의 목적은 호스피스 음악치료의 필요성과 한국 호스피스 기관 내 적용 현황에 대해 살펴보고, 이를 올바르게 정착시키고 활성화하기 위한 방안을 제시하는 것이다. 호스피스 음악치료는 환자와 가족이 겪는 총체적 고통의 완화를 위해 전문가에 의해 수행되는 임상적 근거 기반 음악 중재이다. 다양한 연구를 통해 호스피스 음악치료의 효과가 보고되고 있으나 아직 국내에서는 호스피스 음악치료에 대한 인식이 부족하며 관련 제도 역시 미비한 실정이다. 최근 호스피스 활성화를 위한 여러 정책 및 법률 시행 중이며, 이에 따라 호스피스 음악치료의 수요도 함께 늘어날 것으로 예상되고 있다. 따라서 국내에서 호스피스 음악치료가 환자와 가족에게 유용한 전문 요법으로 올바르게 인식되고 활용될 수 있도록 주요 기관 및 학회를 중심으로 호스피스 음악치료의 의미와 역할을 명확히 정의하고 다음과 같이 제도적, 학술적인 노력을 함께 기울여야 한다. 첫째, 환자와 가족에게 검증된 음악치료 중재가 될 수 있도록 호스피스 전문 음악치료사의 자격기준을 수립해야 한다. 둘째, 체계적인 이론, 임상 교육 프로그램을 통해 전문 음악치료사를 양성하고 필요한 기관에 고용될 수 있도록 서비스 전달체계를 구축해야 한다. 셋째, 연구 활성화를 위해 연구 로드맵을 수립하고 표준 프로토콜을 개발 및 보급하여 임상과 연구가 원활히 연계될 수 있는 시스템을 구축해야 한다.
본 연구는 음악치료가 119구급대원(이하 구급대원)의 외상 후 스트레스(post-traumatic stress; PTS) 감소에 미치는 영향과 음악치료 효과의 지속성을 알아보고자 수행되었다. 연구대상은 C지역의 구급대원 42명으로 2019년 11월 25일부터 2020년 3월 1일까지 연구를 진행하였다. 실험군 21명은 매일 2회씩 5일간 총 10회의 음악치료 프로그램을 진행하였다. 프로그램은 음악치료 전문가 2인에 의해 진행되었으며, 각 단계는 즉흥적으로 연주하기, 리듬 창작 활동, 난타, 신체 활동 등 목표에 필요한 중재기법이 적용되었다. 대조군은 실험군의 프로그램 진행과 동일한 시간에 자유로운 휴식(TV시청, 휴대폰게임, 수면, 운동 등)을 취하도록 하였다. 연구 결과 대조군은 음악치료 적용 전과 후의 외상 후 스트레스의 차이가 없었으나, 실험군은 음악치료 적용 전에 비해 직후의 PTS가 크게 감소하였고, 4주 후와 12주 후에는 PTS가 점차적으로 증가하였다(p<.05). 따라서 정기적이고 반복적인 음악치료는 구급대원의 외상 후 스트레스 수준을 경감시킬 수 있는 효과적인 방법으로 판단된다.
Purpose: The purpose of this study was to determine the effects of 5 weeks self-help management program developed by Kim et al. (2000-b) and reinforced with music therapy for post stroke patients. Method: This was pre-experimental study and the subjects were 14 post stroke hemiplegic patients at home who were registered at the Gunsan Health Center. The program was applied for five weeks and two hours for each session composed of the preparation step, the main step and the finishing step. The contents of the program were ROM exercise, daily activity training, risk factors and aftereffects, nutrition management and stress management. To collect data, all subjects were questioned before and after the application of the program. Collected data were analyzed through frequencies, percentages, Wilcoxen signed rank test and Cronbach's alpha using SPSS-WIN program. Result: After the application of the self-help management program, a statistically significant increase was observed in the subjects' ability to perform activities of daily living (p=.039), ability to perform instrumental activities of daily living (p=.005), self-efficacy (p=.001), self-care behavior (p=.001) and quality of life (p=.001), and a statistically significant decrease was observed in depression (p=.012). Conclusion: The present self-help management program was found to be helpful in improving the subjects' physical and psychological functions after they were attacked by stroke, so to be an effective nursing intervention strategy for post stoke patients. Future researches need to reinforce and materialize music therapy and to develop and apply a self-help management program that includes not only post stroke patients but also their families. In addition, it is necessary to expand the scope of subjects and apply follow-up management in order to continue self-help meetings.
Purpose: This study has tried to find out the effects of music therapy on anxiety of surgery patients during operation under regional anesthesia. In order to find out the effects, this research design was used nonequivalent control group non-synchronized design. Method: The subjects were the sixty inpatients under regional anesthesia in K hospital. They were assigned to two groups, thirty to the experimental group and thirty to control group. The data were collected using the Visual Analogue Scale(VAS) for State Anxiety and vital sign. The Experimental group received favorite music of the 4 different genre. The control group were inserted ear plug during operation. Data were analyzed by $X^2$-test, T-test, ANOVA and Repeated Measured ANOVA of the SPSS WIN (12.0) version program. Results: 1) Hypothesis 1: The hypothesis "that the figure for state anxiety of the experimental group provided with music therapy would be lower than that of the control group" was confirmed(F=0.27, P=0.01). 2) Hypothesis 2 : Three subordinating hypotheses were established in order to verify the hypothesis "that the figures for vital sings of the experimental group provided with such as music therapy would be lower than those of the control group." were rejected. Conclusion: Music therapy can be regarded as an effective nursing intervention that relieves operative anxiety of surgical patients under regional anesthesia.
Purpose: The purpose of this study was to examine the effects of music therapy on anxiety of patients in emergency room. Methods: The study was designed using a noneqivalent control group nonsynchronized design. For 20 minutes, the experimental group(22 patients) had listened to music and the control group(23 patients) had bed rest. A six-item state anxiety scale developed by Marteau and Bekker, which was based on Spielberger's State-Trait Anxiety Inventory, blood pressure and pulse rate were measured to all study participants before and after the intervention. The data was analyzed by the SPSS 12.0 program. Results: 1) The experimental group's state anxiety level were significantly lower than the control group's(t=2.220, p=.032). 2) There were no significant differences in both group's diastolic(t=-.495, p=.623) and systolic blood pressure(t=831, p=.411). 3) The experimental group's pulse rate was significantly lower than the control group's(t=2.363, p=.023). Conclusion: Music therapy may be applied as a nursing intervention to decrease anxiety in emergency room.
Background and Objectives : Many students major in applied music singing have various difficulties in phonation when they sing or perform. But the studies about this cause are lack. The purpose of this study is whether singing voice therapy based on vocal music is effective or not to applied music singing students that have difficulties in phonation. Materials and Methods : Singing voice therapy program had been done to 8 students major in applied music singing during 7 weeks. We did laryngeal stroboscope, perceptual, acoustic and aerodynamic examination before and after singing voice therapy to this students. And we studied post examination results have differences or improvements compared to pre examination results. Results : Four male students don't have effective improvement after singing voice therapy but they shows slight improvement in aerodynamic examination values compared to pre examination values. Four female students shows improvement in aerodynamic examination values and pitch range increasing. Above all, one student's vocal nodules disappeared after singing voice therapy. Conclusion : Singing voice therapy based on music singing is very effective to the students major in applied music singing. Above all, their curriculum in applied music singing should include phonation training based on music singing before training perform and technic practices.
본 연구는 독거노인을 대상으로 성음 중심의 판소리 창작 음악치료 프로그램을 실시하고 자기효능감에 미치는 효과에 대해 알아보는 것을 목적으로 한다. 이에 서울에 위치한 S 노인복지관의 서비스를 제공받고 있는 독거노인 6명을 선정하고, 이들에게 5단계로 구성된 성음 중심의 판소리 창작 프로그램을 주 2회, 50분씩 총 6주 동안 제공하였다. 측정도구는 자기효능감척도(박경민, 1994)를 사용하였고, 하위영역별로 대상자들의 사전 사후 점수 결과와 평균에 대한 Wilcoxon 검정을 실시하였다. 세션이 종결된 후에는 인터뷰를 실시하여 만족도를 함께 살펴보았다. 연구 결과, 사전 사후에 측정한 자기효능감 척도의 점수변화가 통계적으로 유의한 것으로 분석되었다(p < .05). 이에 대해 비모수에 의한 Wilcoxon 검정 결과는 일반적 요인과 신체적 요인에 유의미한 차이가 있었고, 사회적 요인은 증가수준이 통계적으로 유의하지 않았다. 또한 대상자들의 사후 인터뷰를 통해서 높은 만족도를 확인할 수 있었다. 이러한 결과는 판소리 창작 프로그램이 독거노인의 자기효능감을 증진시키는 데 효과적인 중재도구임을 입증해주고, 판소리의 치료적 특성을 제시한다는데 의의를 가진다.
본 연구의 목적은 일반초등학교 통합학급에 배치된 장애 아동과 비장애 아동의 사회적 통합을 증진하기 위해 사회정서학습을 기반으로 하는 음악활동 프로그램을 개발하고 타당도를 검증하는 것이다. 이에 사회적 통합 프로그램과 사회정서학습 프로그램 관련 선행연구를 분석하여 프로그램의 구성요소를 추출하고, 음악활동 프로그램 관련 선행연구 고찰을 통해 음악활동을 선정하는 과정을 거쳤다. 이후 지적장애 아동이 포함된 초등 5-6학년 학급을 프로그램의 대상으로 설정하고, 창의적 체험활동시간에 실행 가능한 12회기의 프로그램을 지도안 형식으로 개발하였다. 개발된 프로그램은 교사전문가 집단에게 2회의 타당도 검증을 받았으며, 전문가들의 의견을 반영하여 프로그램을 수정, 보완하여 최종 프로그램을 완성하였다. 본 연구는 사회 정서적 역량 증진을 통해 사회적 통합을 촉진하는 음악활동 프로그램의 모형 및 구성과 다양한 활동자료를 제시함으로써 통합교육현장에서 음악활동의 적용가능성을 보여준다. 개발된 음악활동 프로그램은 통합학급 아동들의 사회적, 정서적 역량 증진을 촉진할 수 있을 것으로 기대되며, 장애-비장애 아동 간의 사회적 통합을 위해 활용될 수 있다.
This study was purported to examine the definition of tone deafness, various factors for the cause based on literature review of research findings, and to examine therapeutic application of music for treatment of tone deafness. With research, it was found that there can be three different kinds of tone deafness; amusia, agnosia, and asonia. Literature review showed that tone deafness has been frequently dealt in many research in order to verify the causal factors, such as gender, age, and environments. With time, the research trend on tone deafness has shifted towards neurological approach closely examining brain activity, presenting the statement that the brain's capacity to perceive modest pitch changes may be congenitally impaired. Also physiological factors contribute to tone deafness called diplacusis, which is a phenomenon wherein a given tone is heard as different pitches by the two ears, resulting in conflicting bilateral perception of pitch. Music can be used for treatment of various factors causing tone deafness. The most efficient intervention was singing program. Pitch-matching training can be effective training using operant conditioning procedure. Successive approximation or reinforcement of correct response alone was more efficient procedure in helping uncertain singers to sing on pitch. Also progressive breathing exercises helped the training the pitch-matching where one had to coordinate hearing and voice.
미국에서는 1975년 장애아동교육법(Education for All Handicapped Children Act)이 시행되면서 특수교육 프로그램에서 일하는 음악치료사의 역할이 확대되기 시작하였고, 학령기 아동을 위한 음악치료의 역사는 반세기가 넘었다. 우리나라의 경우 2005년에 개정된 특수교육계획에서 특수교육이 필요한 학생의 치료교육에 대한 지원을 확대하고 각 지역 교육청별로 특수교육 지원센터를 두어 치료교육 교사를 배치 시행하는 안을 발표하여 학교 현장에서의 음악치료 보급이 시작되는 단계라 할 수 있다. 이에 본 연구는 2005년 특수교육계획이 시행된 지 6개월 정도 지난 시점에서 초등학교 음악치료와 관련된 특수교사들과 음악치료사들의 의견과 학교 현장에서의 음악치료 현황 및 실태는 어떠한지를 알아보고 그에 따른 개선점이나 문제점을 제시하는 데에 그 목적을 두었다. 이를 위해 본 연구와 관련된 선행 연구들과 문헌들을 조사 검토하고, 2005년 10월 18일부터 11월 2일에 걸쳐 특수교사 65명과 음악치료사 30명을 대상으로 초등학교 특수학급에서의 음악치료 현황과 인식도, 기대도 등에 관한 설문 조사를 실시하여 그 결과를 기술통계 처리하였다. 분석된 결과에 의하면, 특수교사와 음악치료사는 초등학교의 음악치료 필요성에 대해 인식하고 있으며 음악치료의 효과에 대해서도 긍정적인 기대를 보이는 것으로 나타났다. 또한, 음악치료 프로그램의 질을 높이고 체계화하기 위해서는 악기 및 음악치료실의 완비가 무엇보다도 시급한 것으로 나타났다. 이를 위해 국가나 학교 차원에서의 적극적인 행 재정적 지원이 있어야 한다는 지적도 있었다. 특수학교나 특수학급을 비롯한 지역사회 복지관, 조기교육 현장 등 장애아동이 있는 모든 곳에서 치료교육에 대한 학부모들의 요구는 최근 들어 증가하고 있다. 본 연구는 초등학교 현장에서 치료와 교육을 접목시킨 음악치료교육에 대한 전반적인 조사를 통해 초등학교에서 음악치료교육의 필요성을 제시하였다는 데 그 의의가 있다고 하겠다.
