Avulsion injuries result from the application of a tensile force to a musculoskeletal unit or ligament. Although injuries tend to occur more commonly in skeletally immature populations due to the weakness of their apophysis, adults may also be subject to avulsion fractures, particularly those with osteoporotic bones. The most common sites of avulsion injuries in adolescents and children are apophyses of the pelvis and knee. In adults, avulsion injuries commonly occur within the tendon due to underlying degeneration or tendinosis. However, any location can be involved in avulsion injuries. Radiography is the first imaging modality to diagnose avulsion injury, although advanced imaging modalities are occasionally required to identify subtle lesions or to fully delineate the extent of the injury. Ultrasonography has a high spatial resolution with a dynamic assessment potential and allows the comparison of a bone avulsion with the opposite side. Computed tomography is more sensitive for depicting a tiny osseous fragment located adjacent to the expected attachment site of a ligament, tendon, or capsule. Moreover, magnetic resonance imaging is the best imaging modality for the evaluation of soft tissue abnormalities, especially the affected muscles, tendons, and ligaments. Acute avulsion injuries usually manifest as avulsed bone fragments. In contrast, chronic injuries can easily mimic other disease processes, such as infections or neoplasms. Therefore, recognizing the vulnerable sites and characteristic imaging features of avulsion fractures would be helpful in ensuring accurate diagnosis and appropriate patient management. To this end, familiarity with musculoskeletal anatomy and mechanism of injury is necessary.
Objectives : This study aimed to understand the characteristics of the cases covered in the case studies on traditional Korean medicine (TKM) and furthermore, to provide basic information that can lead the discussion on 'what cases are worth reporting' in future case reports. Methods : Case reports on TKM were searched using the OASIS. The searched researches were analyzed according to the type of case, including information on disease/symptoms and intervention. Results : A total of 940 researches were searched. The most frequently reported type of case study was the report on the effectiveness of intervention. Case reports, which were only two cases in the 1970s, increased rapidly in the 2000s, and in particular, 314 cases within the last five years accounted for about 33% of the total literature. As for the number of studies by disease, the cases dealing with musculoskeletal diseases such as spine, shoulder and knee joint disorders were the most prevalent. Besides, there were many case reports related to cardiovascular, gynecological, cancer, psychiatric, and dermatological diseases. In a total of 51.9% of the included case reports, a combination of two or more Korean medical treatments such as acupuncture and herbal medicine was used at once, and western treatment was used with Korean medical treatment in 28.2% of the studies. The types of Korean medical treatments were varied, such as acupuncture, moxibustion, pharmacopuncture, electroacupuncture, Chuna, acupotomy, herbal medicine, external preparation, and psychotherapy. The main purpose of the publication of the included case reports was analyzed as a report of TKM treatment for rare diseases, or the application of TKM treatment to diseases or symptoms that are "uncommon in TKM treatment" even if it is not a rare disease. Conclusions : Case reports have the strength of generating new scientific hypotheses by detecting the basic needs and novelty of medicine. The current case studies of TKM do not seem to be sufficient to highlight these strengths. It is necessary to discuss which cases are reported as cases of patients worth publishing, and based on this, it is necessary to activate case studies of TKM by utilizing diagnostic tools and science technology.

Survival rate of osteosarcoma has been improved recently due to the neoadjuvant and adjuvant chemotherapy. Limb-salvaging operation(LSO) has replaced the amputation technique without' lowering the survival rate. And there occurred a lot of patients who are suffering from the high cost of artificial implants and forced to choose amputation due to economic problem. In LSO, usually relatively high cost artifical implant is needed. When a patient and not afford such an expensive implant he had to choose an inexpensive way, amputation. Authors tried bone lengthening by adopting Ilizarov technique after wide resection of tumor in two patients. Bone transportation was successful in one patient and less successful in the other. One case in CDF(continuosly disease free since the surgical procedure) state at follow-up 3 year 4 months after knee joint fusion. And the other was given lobectomy for lung metastasis at postop. 1 year and 9 months, and given osteosynthesis for infected nonunion at the docking site. Bone transportation was thought to be a good method for the bony coverage of dead space caused by wide resection. Bone transportation technique was economical as well as biological We present two osteosarcoma patient who treated with Ilizarov bone transportation.
Objective: The purposes of this study are to investigate ergonomic problems and to propose their improving measures in office environment of big general hospitals. Background: Office tasks have varying risk factors of work-related musculoskeletal disorders (WMSDs). The first symptom resulted from the office work was recognized as an occupational disease in Korea in 1986. Although the symptoms have increased since its first recognition, there has been few study on the effect of office work environment settings. Method: First, the author took pictures of working scenes performed in three big university hospitals. Next, the pictures were analyzed in view point of ergonomics. Based on the analysis, their improving measures were proposed for reducing work stress. Results: The results showed that most physical office environment settings such as dimensions of tables/desks and chairs, leg room, thigh, knee and foot clearances, and chairs used did not satisfy the ergonomically recommended design guidelines. In addition, some clerks placed personal belongings under their desks, put monitors in high position and did not lean against the backrest of chairs in seated tasks, which resulted in poor working postures of leg, back, neck etc. It is recommended that the hospital management should provide their clerks with ergonomically designed office furniture and continuously perform ergonomics training program for raising clerks' recognition for office ergonomics. Conclusion: Most office environment settings investigated in this study were not in good condition in view point of ergonomic design for the settings. Application: It would be useful as basic data for establishing ergonomically good office environment in hospitals.
Objectives : This study is aimed to evaluate and objectify the therapeutic effect of moxibustion on muscle fatigue recovery. Methods : To evaluate the therapeutic effect on the muscle fatigue recovery, we compared the fatigue recovery of two groups (non-stimulation group and moxibustion group) by analyzing the EMG and peak torque after strenuous knee exercise of isokinetic contraction. Results : The median frequency (MF) of the moxibustion group was recovered faster than that of the non-stimulation group. However, the peak torques of both groups were not restored until after 20 minutes. Nevertheless, the moxibustion group's peak torque was regained higher than that of the non-stimulation group. Conclusions : We confirmed the therapeutic effect of moxibustion and found that the moxibustion can used as prevention method for musculoskeletal disease.

Tumoral calcinosis is a disease of unclear etiology which presents with periarticular and intramuscular calcification without the disorder of calcium and phosphorus metabolism. The incidence is very rare but the rate is higher among the blacks than whites. There has been no report on the recurrent occurrence on the asian race. We report a case that recurred several times with tumoral calcinosis of both knee and thigh. A 21-year-old woman visited to our department with masses in both right thigh and knee. She had a history of local excisions and biopsies(4 times at other hospital) and showed prompt recurrences. The complete marginal excision was performed for the treatment. The histological examination showed the findings that are compatible with tumoral calcinosis. There has been a free of recurrence over the past two years.
The purpose of this study is to establish the basic data for the management of the elderly patients aged more than 60 suffering from musculoskeletal disorders in geriatric hospitals. From January, 1 2012 to December 31, 2012, 2,500 patients who were taken the x-ray inspection and were analyzed x-ray order 5,042 cases above spines, upper lower extremities and joints. The elderly age was divided into 3 groups according to Brody elderly statistics. The majority of elderly patients who visited the hospital are exposed to spinal disorders and joint diseases. Shoulder, knee, L-spine examination cases had accounted for more than half the number of total. By age group, the number of the first quarter was most dominant commonly. Second and third quarter was maintaining slightly decrease. The fourth quarter had reduced compared to the first quarter. If the visiting purposes and the disease causes analysis and preventive measures are taken in a study, it is expected to utilize as basic data to make a plan of the education for preventing diseases and elderly behavior planning for geriatric hospitals and to redistribute health care resources of the rural areas efficiently.
Objective : To compare the efficacy between SKI306X and Diclofenac by using generalized estimating equations (GEE) methodology in the analysis of correlated bivariate binary outcome data in Osteoarthritis (OA) diseases. Methods : A randomized, double-blind, active comparator-controlled, non-inferiority clinical trial was conducted at 5 institutions in Korea with the random assignment of 248 patients aged 35 to 75 years old with OA of the knee and clinical evidence of OA. Patients were enrolled in this study if they had at least moderate pain in the affected knee joint and a score larger than 35mm as assessed by VAS (Visual Analog Scale). The main exposure variable was treatment (SKI 306X vs. Diclofenac) and other covariates were age, sex, BMI, baseline VAS, center, operation history (Yes/No), NSAIDS (Y/N), acupuncture (Y/N), herbal medicine (Y/N), past history of musculoskeletal disease (Y/N), and previous therapy related with OA (Y/N). The main study outcome was the change of VAS pain scores from baseline to the 2nd and 4th weeks after treatment. Pain scores were obtained as baseline, 2nd and 4th weeks after treatment. We applied GEE approach with empirical covariance matrix and independent(or exchangeable) working correlation matrix to evaluate the relation of several risk factors to the change of VAS pain scores with correlated binary bivariate outcomes. Results : While baseline VAS, age, and acupuncture variables had protective effects for reducing the OA pain, its treatment (Joins/Diclofenac) was not statistically significant through GEE methodology (ITT:aOR=1.37, 95% CI=(0.8200, 2.26), PP:aOR=1.47, 95% CI=(0.73, 2.95)). The goodness-of-fit statistic for GEE (6.55, p=0.68) was computed to assess the adequacy of the fitted final model. Conclusions : Both ANCOVA and GEE methods yielded non statistical significance in the evaluation of non-inferiority of the efficacy between SKI306X and Diclofenac. While VAS outcome for each visit was applied in GEE, only VAS outcome for the fourth visit was applied in ANCOVA. So the GEE methodology is more accurate for the analysis of correlated outcomes.

Synovial sarcoma is a malignant soft tissue tumor which is the most prevalent in adolescents and young adults between 10 and 40 years of age. It occurs primarily in the para-articular regions, usually in close association with tendon sheath, bursae and joint capsules. Favorable clinical factors are young age of the patients, tumor size smaller than 5cm, and distal rather than proximal location in the extremities. We analysed clinical findings of 13 cases of synovial sarcoma that had been experienced from January 1983 to December 1992. There were 8 females and 5 males, whose age was averaged as 28 years and 10 months ranging from 6 years to 54 years. The mean follow-up was 3 years(range : 9 months- 9 years 1 month). Palpable mass was frequent clinical symptom and lower extremity especially around the knee was the most prevalent site. Treatment modalities were the combination of surgery, radiotherapy and chemotherapy. Distant metastasis occured in 5(38%) cases : 4 cases to lung and 1 case to neck, and 2 cases had local recurrences. At final follow-up 6 cases were continous disease free, 2 alive with disease and 5 died of disease. The Kaplan-Meier's estimated 5 year survival rate of total 13 cases was 66% and satisfactory results were obtained with mass size smaller than 5cm.

Purpose: We experienced 8 cases of osteoid osteoma arising around hip joint which had some characteristics that differ from those arising on long bone. we reports the characteristics of osteoid osteoma arising around the hip joint. Materials and Methods: 8 cases of osteoid osteoma were diagnosed during 1985 to 2004 at hanyang university hospital. all cases were comfirmed patholgically. 6 cases were male, 2 cases were female patients. The mean age was 17 years old (ranged from 8 to 29). They occurred in intertrochanteric area (4 cases), subtrochanteric area (2 cases), acetabulum (1 case) and femoral neck (1 case). We used radiologic tools including magnetic resonance image, computed tomography, bone scintigraphy. clinicopathologic test including erythrocyte sedimentation rate and Creactive protein. Results: The patients expressed various symptoms including thigh pain, knee pain, low back pain and radiating pain respectively. 2 patients had experienced operation on knee joint. 3 patients showed limping gait. Aspirin relieved the pain in 3 patients. The difference in circumference was 1cm between both thighs in 2 cases. Conclusion: Patients with osteoid osteoma arising around hip joint which have various symptoms such as severe knee pain and claudication, differ from infectious disease by clinicopathologic test including erythrocyte sedimentation rate and C- reactive protein and had better diagnostic result in computed tomography.
