Fuzzy sliding-mode control of a human arm in the sagittal plane with optimal trajectory

  • Patients with spinal cord injuries cannot move their limbs using their intact muscles. A suitable controller can be used to move their arms by employing the functional electrical stimulation method. In this article, a fuzzy exponential sliding-mode controller is designed to move a musculoskeletal human arm model to track an optimal trajectory in the sagittal plane. This optimal arm trajectory is obtained by developing a policy for the central nervous system. In order to specify the optimal trajectory between two points, two dynamic and static optimal criteria are applied simultaneously. The first dynamic objective function is defined to minimize the joint torques, and the second static optimization is offered to minimize the muscle forces at each moment. In addition, fuzzy logic is used to tune the sliding-surface parameter to enable an appropriate tracking performance. Simulation results are evaluated and compared with experimental data for upward and downward movements of the human arm.

  • This research was designed to investigate biomechanical aspects of the evolution based on the hypothesis of dynamic cooperative interactions between the locomotion pattern and the body shape in the evolution of human bipedal walking The musculoskeletal model used in the computer simulation consisted of 12 rigid segments and 26 muscles. The nervous system was represented by 18 rhythmic pattern generators. The genetic algorithm was employed based on the natural selection theory to represent the evolutionary mechanism. Evolutionary strategy was assumed to minimize the cost function that is weighted sum of the energy consumption, the muscular fatigue and the load on the skeletal system. The simulation results showed that repeated manipulations of the genetic algorithm resulted in the change of body shape and locomotion pattern from those of chimpanzee to those of human. It was suggested that improving locomotive efficiency and the load on the musculoskeletal system are feasible factors driving the evolution of the human body shape and the bipedal locomotion pattern. The hypothetical evolution method employed in this study can be a new powerful tool for investigation of the evolution process.

무작위대조군연구(RCT)를 중심으로 한 근.골격계 질환의 약침 치료에 관한 고찰 (Review of Randomized Controlled Trials on Pharmacopuneture Treatment for Musculoskeletal Diseases)

  • Objectives : The purpose of this study is to review RCTs on pharmacopuncture treatment for musculoskeletal diseases and to establish standards of pharmacopuncture treatment model. Methods : We searched articles up to date of March 2009 via computerized databases of Pubmed, The Journal of Korean Acupuncture & Moxibustion, Journal of Korean institute of Herbal Acupuncture, Journal of Oriental Rehabilitation and Journal of Korean Oriental Medicine. Only Randomized Controlled Trials (RCT) concerning the effects of pharmacopuncture on musculoskeletal diseases. The pharmacopuncture treatment methods were assessed based on STRICTA and items considering the feature of pharmacopuncture. And the methodological quality of the trials was assessed by FEAS and modified Jadad score. Results : Eighteen trials of pharmacopuncture on musculoskeletal diseases were analyzed. Except for 4 trials comparing the effect of SBV and BV, positive outcome was reported in ten trials. Among eighteen trials; most of the trials were about Bee Venom acupuncture, and most of the trials used about five acupuncture points, mainly local acupuncture points. But, the amount of injection to each point and total injection were various. And most of trials were lack in the information about method of stimulation. The adjusted FEAS score ranged from 0 to 12, and modified Jadad scoreranged from 1 to 5. Conclusions: To standardize pharmacopuncture treatment, we need more well-designed, high quality clinical trials. And methodological assessment tools designed for pharmacopuncture treatment are also needed.

종합병원내 조리실 종사자들의 근골격계질환 예방을 위한 프로그램 개발과 평가: 실행연구 (A Program Development and Evaluation to Prevent Musculoskeletal Disorders among Kitchen Workers in General Hospitals: An action research)

  • 본 연구는 종합병원 내 조리실 종사자들의 근골격계 질환 예방을 위한 프로그램을 개발하고자 수행한 참여적 실행연구이다. 이 연구는 G시 소재 C종합병원에 근무하는 조리실 근로자 15명, 영양사 3명, 간호사 2명, 물리치료사 1명이 참여하였으며 2018년 3월부터 12월까지 이루어졌다. 참여적 실행연구에 대한 개념적 모형을 바탕으로 양적 자료와 질적 자료를 통합적으로 수집하고 분석하였다. 근골격계 질환 예방을 위한 프로그램을 개발하여 총 13회를 적용한 결과, 프로그램의 구성과 운영시간, 전문성 부분에 있어서 만족도가 높게 나타났으며 향후 지속적인 프로그램의 운영을 원하는 것으로 나타났다. 따라서 본 연구결과를 바탕으로 향후 다른 근로자들을 대상으로 하여 근무부서와 작업의 특성을 고려한 근골격계 질환 예방 프로그램을 참여적 실행연구를 통해 개발하고 적용할 필요가 있다.

미충족의료 경험자의 한방의료이용에 미치는 영향 요인 분석 - 한국의료패널자료를 중심으로 - (Analysis of Influencing Factors of Korean Medical Utilization among Persons with Unmet Healthcare Needs - Based on Korea Health Panel -)

  • Objectives : This study aimed to analyze the factors influencing Korean medical utilization among persons with unmet healthcare needs. Methods : This study utilized Korea Health Panel data in 2019, with 10,771. We performed a t-test and ANOVA on the difference in general characteristics between Korean medical utilization and unmet healthcare needs. Logistic regression analysis and generalized linear model analysis were conducted to analyze on factors affecting the Korean medical utilization by people with unmet healthcare needs. Results : Among people with unmet healthcare needs, the variables influencing Korean medical utilization were western medical utilization, gender, education level, musculoskeletal disease, and other chronic diseases. The more people with unmet healthcare needs, the more likely they were to use Korean medicine. As a result of logistic regression analysis, the influencing factors on Korean medical utilization were analyzed for people with unmet healthcare needs, and the higher the household income, the more musculoskeletal diseases, and the higher the probability of using Korean medicine. Conclusions : Korean medicine has a large proportion of musculoskeletal disease, so it was found that musculoskeletal diseases have an impact. In addition, considering that household income is an important factor in the influencing factor of unmet healthcare needs, it is necessary to increase the use of Korean medicine by those with low household incomes.

최적화 기법에 의한 인체 하지 근골격 시스템의 최적제어 모델 개발 (An optimization approach for the optimal control model of human lower extremity musculoskeletal system)

  • 인체 하지 근골격 시스템의 수학적 모델에 대해 최적제어 기법을 이용하여 최대 높이뛰기 운동을 재현하였다. 근육의 비선형 동적특성에 의해 순동역학 접근방법을 사용하였으며 최적제어는 최적화 프로그램인 마이크로 유전알고리즘과 VF02 비선형 최적화 프로그램을 적용하였다. 최대 높이뛰기 운동을 위한 근골격 모델에서 유전알고리즘만으로는 최적해를 얻을 수가 없었다. 유전알고리즘의 해를 비선형 최적화 프로그램의 초기 예측값으로 하여 도약시간에 따른 최적의 운동 신경자극도를 결정하였다. 이러한 접근방법은 초기의 인위적 예측값 없이 최대높이뛰기 운동에 대한 전역해를 제공하였다.

Evaluation of Related Risk Factors in Number of Musculoskeletal Disorders Among Carpet Weavers in Iran

  • Background: Musculoskeletal disorders (MSDs) are a common problem among carpet weavers. This study was undertaken to introduce affecting personal and occupational factors in developing the number of MSDs among carpet weavers. Methods: A cross-sectional study was performed among 862 weavers in seven towns with regard to workhouse location in urban or rural regions. Data were collected by using questionnaires that contain personal, workplace, and information tools and the modified Nordic MSDs questionnaire. Statistical analysis was performed by applying Poisson and negative binomial mixed models using a full Bayesian hierarchical approach. The deviance information criterion was used for comparison between models and model selection. Results: The majority of weavers (72%) were female and carpet weaving was the main job of 85.2% of workers. The negative binomial mixed model with lowest deviance information criterion was selected as the best model. The criteria showed the convergence of chains. Based on 95% Bayesian credible interval, the main job and weaving type variables statistically affected the number of MSDs, but variables age, sex, weaving comb, work experience, and carpet weaving looms were not significant. Conclusion: According to the results of this study, it can be concluded that occupational factors are associated with the number of MSDs developing among carpet weavers. Thus, using standard tools and decreasing hours of work per day can reduce frequency of MSDs among carpet weavers.

치위생과 학생들의 건강-신념 모형에 의한 근골격계 질환 예방 행위 관련성 (Connections of Preventive Actions against Musculoskeletal Diseases by Dental Hygiene Students according to the Health Belief Model)

  • 본 연구는 치위생과 학생들이 건강-신념에 의한 근골격계 질환 지식과 예방행위를 알아보고 건강증진과 근골격계 질환 예방 프로그램의 필요성을 제안하고자 그 관련성을 조사하였다. 경기도 소재 치위생과 2학년 학생 83명, 3학년 학생 114명을 대상으로 설문조사를 하였다. 결과는 다음과 같다. 1. 건강-신념 중 민감성은 2학년 학생이 $22.50{\pm}2.37$점, 3학년 학생이 $22.29{\pm}3.01$점으로 유의한 차이가 있었으며(P < 0.01), 유익성은 2학년 학생이 $18.82{\pm}2.60$점, 3학년 학생이 $18.64{\pm}2.77$점으로 유의한 차이가 있었다.(P < 0.05) 2. 신체부위별 통증은 3학년 학생이 2학년 학생보다 통증 정도가 높았다. 통계학적으로 유의한 신체 부위는 목, 어깨, 허리, 무릎, 발, 발목이었다(P < 0.05). 3. 심각성은 목, 어깨, 팔꿈치, 등, 허리, 엉덩이, 무릎, 발목, 발이 양의 상관관계가 있었다(P < 0.01). 장애성은 무릎, 발목, 발이 음의 상관관계가 있다(P < 0.01). 건강-동기는 목, 어깨, 등, 허리, 엉덩이가 양의 상관관계가 있었으며(P < 0.01), 무릎이 음의 상관관계가 있었다(P < 0.01). 4. 건강-신념 중 심각성은 사용빈도가 높은 기구는 시술자 가까이 편리한 곳에 둔다, 가능한 한 같은 업무를 반복하지 않고 업무를 다양하게 한다와 유의한 양의 상관관계가 있었다(P < 0.01). 유익성은 진료 시 구부리기와 뻗치기 자세를 줄인다, 가급적 목, 등, 팔과 손목이 많이 기울어 지지 않도록 한다, 상체자세를 중립으로 유지하려고 한다와 유의한 상관관계가 있었다(P < 0.01).

  • Purpose: The purpose of this study is to identify the trends of studies on work-related musculoskeletal disorder and to suggest the direction of future research. Method: 144 research thesis for a degree on work-related musculoskeletal disorder were systematic reviewed and published from Jan. 1990 to Mar. 2005. The analyzed using descriptive statistics, an actual number, percentage. 기술사용이 근로환경과 근로자 건강에 미치는 영향력에 대한 연구 (Study on the Impact of use of Technology on Work Environment and the Health of Workers)

  • This study looks at the impact of psychological and physical factors of the working conditions on the health of workers depending on whether technology is used by such workers. The data used for the study is the third work environment survey. Out of 50,032 respondents, a total of 29,711 paid workers were used as analysis subjects. Although it was anticipated that the use of technology was a factor that hindered job autonomy and teamwork autonomy. However, the analysis results showed low levels of job autonomy and teamwork autonomy in the group that did not use technology. The study assumes a regression analysis model about work environment and work organizational practices of workspaces that have an impact on musculoskeletal complaints, stress symptoms and level of work satisfaction by controlling the social demographic variable that represents the level of individual sensitivity. As a result of the study, ergonomic risk had a significant effect on both groups that did or did not use technology with respect to stress symptoms, musculoskeletal complaints and level of work satisfaction. In particular, as workspace practices and work environment had an effect on the development of musculoskeletal complaints in the group that used machines, there is a need to improve such situation. The autonomous team work or level of job autonomy within the group that used technology may act as a risk factor to the health and welfare of workers. However, because it may also act as a buffer factor, there is a need for a change to reduce stress symptoms and increase the level of work satisfaction by improving autonomous team work and the level of job autonomy.
