생쥐 골수세포에서 아드리아마이신의 소핵생성에 미치는 N-마세틸시스테인의 억제효과 (Suppressive Effect of N-Acetylcysteine on the Adriamycin-Induced Micronuclei Formation in Mouse Bone-marrow Cells)

  • The anticlastogenic effect of N-acetylcysteine was tested in vivo in mouse bone-marrow micronucleus assay. The frequencies of micronuclei induced by adriamycin (5 mg/kg i.p.) in bonemarrow cells were decreased by the oral administration of N-acetylcysteine at 12 h before adriamycin injection. The observed suppressing effect was not a reflection of a delay in the formation of micronuclei by the cytotoxic effect of N-acetylcysteine. The anticlastogenic effects of SH compound including N-acetylcysteine, cysteine, cystine, S-carboxy methylcysteine and glutathione were also investigated by the multiple pretreatment. Each SH compound was administered orally every day for 5 days and adriamycin (5 mg/kg i.p.) was injected at 24h after the last dose of test compound. N-acetylcysteine and glutathione showed significantly the suppressive effect at dose of 10 and 25 mg/kg for N-acetylcysteine and at the dose of 25 mg/kg for glutathione. Our study suggests that N-acetylcysteine is capable of protecting the chromosomal damages in the normal cells during cancer chemotherapy by adriamycin, and may act as an anticlastogen against induction of micronuclei by superoxide generating agent such as adriamycin.

  • Radiation has stochastic aspects in its generation, its choice of interaction mode during traveling in media, and its impact on living bodies. In certain circumstances, like in high dose environments resulting from low-LET radiation, the variance in its impact on a target volume is negligible. On the contrary, in low dose environments, especially when they are attributed to high-LET radiation, the impact on the target carries with it a large variance. This variation is more significant for smaller target volumes. Microdosimetric techniques, which have been developed to estimate the distribution of radiation energy deposited to cellular and subcellular-sized targets, contrast with macrodosimetric techniques which count only the average value. Since cells and DNA compounds are the critical targets in human bodies, microdosimetry, or dose estimation by microscopic approach, helps one better analyze the biological effects of radiation on the human body. By utilizing microbeam systems designed for individual cell irradiation, scientists have discovered that human cells exhibit radiosensitive reactions without being hit themselves (bystander effect). During the past 10 or more years, a new therapeutic protocol using discontinuous multiple micro-slit beams has been investigated for its clinical application. It has been suggested that the beneficial bystander effect is the essence of this protocol.

ML-6M LINAC에 장착한 Radiosurgery Cone의 Beam 특성에 대한 고찰 (Dose Characteristics of Stereotatic Radiosurgery CONE used for ML-6M Linear Accelerator)

  • The radiosurgery treatment is one time, non surgical approach to the treatment of patients with intracranial disorders whose conditions would be difficult or dangerous to treat with conventional sugical procedures. The LINAC based radiosurgery is based on the combination of multiple isocentric arc irradiation with small fields centered in the stereotactic target. The absorption of the beam in a tissue equivalent medium, such as water, as well as the uniformity, or profile, of the beam must be precisely documented. The beam characteristics and dosimetric measurememts of the 6MV X-ray beam from a ML-6M linear accelerator are examined. The percent depth dose (PDD) and beam profile (including flatness, symmetry and penumbra) is calibrated with the radiosurgery cone in water phantom. The cone is made of lead which size is from $10{\times}10mm{\phi}$ to $30{\times}30mm{\phi}$ All of these dosimetric measurements sufficiently characterized the beam to permit safe clinical use.

다매체/다경로 노출을 고려한 benzo(a)pyrene의 총 인체 노출량 예측 (Estimating Human Exposure to Benzo(a)pyrene through Multimedia/Multiroute Exposure Scenario)

  • The objective of this study was to estimate human exposure to benzo (a)pyrene through multimedia/multi-pathway exposure scenario. The human exposure scenario for benzo(a)pyrene was consisted of 12 multiple exposure pathways, and the multipathway human exposure model based on this scenario constituted. In this study, the multipathway human exposure model was used to estimate the concentrations in the exposure contact media, human intake factors and lifetime average daily dose (LAD $D_{model}$) of benzo(a)pyrene in the environment. Sensitivity analysis was performed to identify the important parameters and Monte-Carlo simulation was undertaken to examine the uncertainty of the model. The total LAD $D_{model}$ was estimated to be 5.52${\times}$10$^{-7}$ mg/kg-day (2.06${\times}$10$^{-7}$ -8.65${\times}$10$^{-7}$ mg/kg-day) using the multipathway human exposure model. The inhalation dose accounted for 78% of the total LADD, whereas ingestion and dermal contact intake accounted for 20.2% and 1.8% of the total exposure, respectively. Based on the sensitivity analysis, the most significant contributing input parameter was benzo (a)pyrene concentration of ambient air. Consequently, exposure via inhalation in outdoor/indoor air was the highest compared with the exposure via other medium/pathways.

Linac 기반 VMAT 정위적 수술 뇌 병변 연구와 기존의 정위적 방사선 수술 비교 (Comparison of Linac-based VMAT Stereotatic Radiosurgery and Conventional Stereotatic Radiosurgery for Multiple Brain Lesions)

  • 기존 연구인 선량평가의 임상 적용 및 신뢰도 확보를 위하여 EPID를 이용하여 Portal Dosimetry를 검증하였다. 뇌정위방사선수술 2.5 cm cone을 장착하여 360° 회전조사에 의한 측정치와 Geant4의 선량분포를 비교하였다. 뇌전이 환자의 선량분포를 확인하기위해 두부인체 팬톰에 Gafchromic EBT필름을 삽입하여 조사한 선량분포와 VMAT을 이용한 두부인체 팬톰에서 얻은 선량분포를 비교하여 실제 환자에 적용하고자 한다. 분석결과 beam center와 couch의 center가 정확하게 일치하는가를 pin ball을 통해 QA한 결과 1 mm 이내의 오차로 정밀함을 확인할 수 있었다. 또한 EBT3 Film에 다양한 선량조사에 따라 0 ~ 10 Gy영역까지 우수한 선형성임을 확인할 수 있었다. 두경부 팬텀과 같은 설정에서 광자 빔을 사용한 Geant4에 기반한 선량 계산 도구의 구현과 시뮬레이션 결과 계산치는 치료계획용적(PTV)내에서 실험 데이터와 일치함을 확인하였다. 따라서 체적변조 아크치료(VMAT) 360° 회전 조사를 실시하여 회전조사에 의한 등선량분포 분석결과 가상 종양을 포함하기에 적절함을 확인하였다.

Multiple low-dose Streptozotocin 유도 Type I 당뇨병 상태의 면역 기능에 미치는 Brazilin의 영향

  • 다양하고 복잡한 기전에 의하여 정교히 조절되고 있는 면역 체계의 비정상적인 기능에 비롯된 자가 면역 질환을 개선, 치료하기 위하여 지금까지 많은 면역 조절제가 개발되어 온 바 있으며, 이들의 면역 조절 기전은 약물에 따라 그 효능에 있어 차이가 크고 독성등의 부작용등으로 인해 그 사용이 현저히 제한되고있는 경우가 많아 보다 안전하고 효능있는 새로운 면역 조절제의 개발은 아직도 매우 중요한 의의를 지니고 있다. 이러한 관점에서 본 연구실에서는 천연물로부터의 새로운 면역 조절제의 검색을 수행하여 온 바 있으며, 그중 Brazilin이 Low-dose Streptozotocin의 반복 투여에 의해 유도되는 당뇨병 상태의 면역 조절 기능에 미치는 영향에 대해서 검색하고자 하였다. 실험의 결과를 종합하여보면, Brazilin은 외부항원 (SRBC)의 유입시 정상 대조군에 비해 현저히 저하되어 있는 T cell의 기능을 정상 대조군 혹은 그 이상으로 증가시키는 효과를 나타내었으며, B cell의 기능 역시 활성화 시키는 것으로 관찰되었다. 이로부터 Brazilin은 Streptozotocin 유도 당뇨병 상태의 면역기능을 정상화시키는 면역 조절활성을 갖는 것으로 판단되었다.

Treatment of Drug Susceptible Pulmonary Tuberculosis

  • Tuberculosis (TB) remains a major global health problem, and the incidence of TB cases has not significantly decreased over the past decade in Korea. The standard short course regimen is highly effective against TB, but requires multiple TB-specific drugs and a long treatment duration. Recent studies using late-generation fluoroquinolones and/or high-dose rifapentine-containing regimens to shorten the duration of TB treatment showed negative results. Extending the treatment duration may be considered in patients with cavitation on the initial chest radiograph and positivity in sputum culture at 2 months of treatment for preventing TB relapse. Current evidence does not support the use of fixed-dose combinations compared to separate drugs for the purpose of improving treatment outcomes. All patients receiving TB treatment should be monitored regularly for response to therapy, facilitation of treatment completion, and management of adverse drug reactions. Mild adverse effects can be managed with symptomatic therapy and changing the timing of the drug administration, but severe adverse effects require a discontinuation of the offending drugs.

Practical Surface Sculpting Method for the Fabrication of Predefined Curved Structures using Focused Ion Beam

  • Surface erosion using focused ion beam irradiation is the most promising technology for the realization of micro/nanofabrication. However, accurate fabrication of predefined structures is still challenging. This article introduces a single step surface driving method to fabricated predefined curved structures. The previously reported multi step surface driving method (MSDM) has been modified so that a single ion dose profile can be used instead of multiple ion dose profiles. Experimental realization of the method is presented with the fabrication of predefined curved surfaces as well as reference to surface propagation theory. For the purpose of verification, simulations are performed on the basis of a sound mathematical model.

Nano-medicine effectiveness in pediatric patients: An artificial intelligence investigation

  • Emerge of nanotechnology has affected many aspects of our life and also triggers research studies in many fields. Nano-medicine are proven to be effective in encountering diseases. In the present study, aspects of the applications and effectiveness of nano-medicine in pediatrics patients are studied. In this regard, using experimental data of previous published researches, combination and dose of nano-medicines are optimized using response surface method and neural-fuzzy inference network. The input parameters of the selected multiple nano-medicines are dose and type and the output is the effectiveness of the combinations using IC50 parameter. A detailed parameter study is presented to observe effects of each inputs on the IC50. The results indicate that personalized scaling of nano-medicine is required in therapy of pediatric diseases such as cancers.

Validation of electromagnetic physics models and electron range in Geant4 Brachytherapy application

  • The mechanics underlying photon and electron interactions was validated using our developed Brachytherapy computer code for high Dose Rate (HDR). By comparing the photon cross-section utilizing multiple physics libraries in the developed code, the results were benchmarked against experimental and theoretical findings. Klein-Nishina and experimental cross-section results were in good agreement with the Livermore library results. For two therapeutically relevant materials, the first scattered electron range was measured within 1 mm and 2 mm, which has significant implications for the interpretation of the kernel dose spikes observed in previous research.
