Relationships among snack habits, oral health practice, and oral health status in preschool children (일부 미취학 아동의 간식습관 및 구강건강실천행위와 구강건강상태와의 관련성)

  • Objectives: The purpose of the study was to investigate the relationships among snack habits, oral health practice, and oral health status in preschool children. Methods: The subjects were 209 preschool children and their mothers in Jeonbuk from March 24 to May 30, 2014. The study instruments comprised snack habits, oral health practice, and oral health status. Data were analyzed for independent t-test, one way ANOVA and hierarchical multiple regression using SPSS Win 21.0 version. Results: Higher dft index was shown in the older age (p=0.033), lower mother's education (p<0.001), lower mother's daily toothbrushing (p<0.001), check-up after tooth-brushing (p<0.001), tooth-brushing practice before sleeping (p<0.001), tooth-brushing practice after snack (p<0.001), regular dental check-up (p<0.001), fluoride and sealant for prevention caries (p<0.001), limitation of sugar snack intake (p<0.007), periodic replacement toothbrush (p=0.022). The cause of higher dft index included soda (p<0.001), yogurt (p<0.001), snack (p=0.002), bread and cake (p=0.002) and caramel and candy (p<0.001). Fruit (p<0.001), vegetable (p<0.001) and milk (p=0.004) decreased dft index. Factors affecting oral health status were tooth-brushing practice before sleeping, tooth-brushing practice after snack, regular dental check-up, fluoride and sealant for prevention caries, and intake of soda, yogurt, caramel, candy, and fruit. The explanation power of the final model was 67.6%. Conclusions: It is necessary to develop the oral health education program for the preschool children and mothers to enhance the best oral health condition.

A Study on the Health Status and Oral Health-Related Quality of Life in the Elderly Patients with Long-Term Care (노인장기요양 환자의 건강상태와 구강건강관련 삶의 질)

  • This study intended to examine the health status and quality of life among the elderly patients who stayed in geriatric hospitals for a long period of time. The study involved 222 elderly who were older than 65-year-old, an oral examination was conducted in 10 geriatric hospitals located in the metropolitan area and Chungcheong-do. Cross-tabulation, one-way ANOVA, t-test, and Stepwise multiple regression were performed for analysis. The average number of teeth among the subjects was 11.22 and that of dental caries was 1.17. Also, the average number of teeth mobility was 0.93 and halitosis was detected among 81.9%. Only 29.70% reported absence or a kind of systemic disease and 70.31% of the subjects was shown to have more than two kinds of complex disease. In addition, 87.8% of the subjects had depression and among them, 55.40% showed serious depression. The average score on quality of life was 2.62 points and as for the related factors, the number of remaining teeth and dental caries were counted along with care grade, hospitalization period, and age. According to such results, the oral health status among the elderly patients staying long in geriatric hospitals were not good in general; also they showed high level of systemic disease and depression and low quality of life.

  • A female patient (14 years-old) with mental retardation was scheduled for ambulatory general anesthesia to treat peri-apical abscess and multiple dental caries. She had got cleft palate plasty at 5 years, but there was no past history of difficulty airway during general anesthesia or airway obstruction. Following induction of anesthesia using an inhalational anesthetic technique, conventional naso-tracheal intubation was tried. However, with conventional intubation technique we could not insert tube. And following several trial of intubation with laryngoscope, she developed an upper airway obstruction. Her lungs could not be ventilated using a facial mask and oxygen saturation was decreased. A #3 laryngeal mask airway (LMA) was inserted immediately, which allowed us to ventilate her lungs and restore the oxygen saturation. And dental treatment was carried out under LMA insertion successfully.

Association between dementia and oral health (치매와 구강 건강의 관계)

  • According to the burst of aged people, researchers have focused on aging-related diseases. Cognitive impairment including Alzheimer's disease (AD), one of the representative diseases related to aging, has no treatment option until now. Recently, it has been revealed that systemic inflammation plays a fundamental role in the pathogenesis of AD. Previous studies have suggested the association between poor oral health and cognitive impairment. Poor oral health can cause dental caries, chronic periodontitis, multiple tooth loss, and poor chewing ability, etc. Especially, periodontitis is a well-known chronic inflammatory disease and affects cognitive impairment directly and indirectly by inflammatory products mediators. Therefore, reduction of pathogenic microbial burden and inflammatory products by treating periodontitis can be a therapeutic modality to prevent cognitive impairment or to slow down the progression of it. Future studies are necessary to elucidate the causal relations and plausible mechanisms between poor oral health and cognitive impairment.

Serum 25-hydroxyvitamin D levels are associated with dental caries experience in Korean adolescents: the 2010~ 2014 Korean National Health and Nutrition Examination Surveys (청소년에서 혈중 비타민 D 농도와 치아우식 경험 간의 연관성 : 2010~ 2014 국민 건강영양조사)

  • Purpose: This study was conducted to evaluate the association between serum 25-hydroxyvitamin D (25(OH)D) levels and dental caries experience in Korean adolescents based on the 2010 ~ 2014 Korean National Health and Nutrition Examination Surveys. Methods: The study subjects were 2,655 Korean adolescents aged 10 to 18 years. Subjects were classified into four groups according to their serum 25(OH)D levels. We used logistic regression to evaluate the relationship between vitamin D and for dental caries experience after adjusting for age, household income level, recipient of basic livelihood, tooth brushing and visiting dental clinics. Result: Multiple logistic regression analysis showed that serum 25(OH)D insufficiency (20 ng/mL ${\leq}25(OH)D$ < 30 ng/mL) was associated with increased odd ratios (ORs) for dental caries experience in boys (OR = 2.577, 95% CI = 1.013-6.557), compared with serum 25(OH)D sufficiency (25(OH)D ${\geq}30ng/mL$). Conclusion: The serum 25(OH)D levels were found to be related to risk of dental caries experience in Korean adolescent boys.

  • Double outlet of right ventricle (DORV) refers to a congenital heart disease in which pulmonary and systemic circulation originates from the right ventricle. In the patient with DORV, it is important to maintain the balance between pulmonary and systemic circulation in anesthetic management. A 4-year-old boy with DORV, who underwent a Blalock-Taussig shunt operation, was transferred to the clinic with a chief complaint of multiple caries. Due to poor cooperability, it was impossible to treat the caries without sedation or general anesthesia. We planned to sedate him with consideration with detrimental effects associated with positive pressure ventilation for dental treatment. After a prophylactic administration of antibiotics, sevoflurane was administered through T-cannula site. Throughout the treatment, His blood remained stable around 80/40 mmHg, oxygen saturation remained around 91%. After 3 hour of sedation with sevoflurane (end-tidal sevoflurane con 1-1.8 vol%), he fully regained consciousness, and discharged from hospital without complications. In case of DORV patient, deep sedation with sevoflurane may be used as effective method of behavioral management during dental treatment.

Detection for demineralization of dental hard tissues using index-sequenced radiographs (방사선사진의 인덱스-순서 분석을 통한 치아경조직의 탈염화 검출)

  • Purpose : The purpose of this study was to develop the radiographic technique for detecting the demineralization which is known as indication of dental caries. Materials and Methods: This technique was based on the comparing of multiple radiographs which was irradiated by multiple X-ray spectra. For the meaningful comparing, the multiple radiographs were reconstructed to the dosimetrically consistent images using a standard material. The difference of resulting images of same target with multiple spectra represents the difference of response of material as regards the spectra. Results : We have found about 10% of demineralization of dental hard tissues particularly in the proximal region through the analyzing of differences. Conclusion: Most intriguing thing in this investigation was that the method to analyze difference shows us to an anatomic structure of dental hard tissues even if absolute values of optical density were excluded during the procedures.   • Metachromatic leukodystrophy (MLD) is a progressive and degenerative neurological disease caused by a deficiency of the catabolic enzyme arylsulfatase A. Deficiency of arylsulfatase A results in accumulation of sulfatide in the white matter of the peripheral and central nervous system and it occurs demyelination as a result. The patient gradually goes through mental and motor failure. General symptoms of MLD include gait disturbance, mental deterioration, muscle rigidity and impaired swallowing. Inheritance of the disease is autosomal recessive. We report a dental caries treatment of a 3-year old boy with MLD. The patient underwent hematopoietic stem cell transplantation (HSCT) to slow the progression of the disease. He was suffered from difficulties of mastication and swallowing from the degenerative neurological symptom. He was ingesting food by both oral feeding and tubal feeding after he took percutaneous endoscopic gastrostomy (PEG). The cause of multiple caries was mainly presumed as patient's prolonged time of meal. The treatment was performed under general anesthesia considering patient's incompliance. Severely affected lower primary molars were treated with pulp treatment and restored with stainless steel crown. Others were restored with composite resin. There were no postoperative complications. MLD is life threatening progressive disease and also has an impact on unfavorable condition for oral health. Routine home oral care and periodic professional dental care should be emphasized to the caregiver of patient considering the susceptibility of dental caries. Not only the medical care, but periodic dental office visit would benefit the quality of life of the patient.

  • Oculodentodigital syndrome(ODD) was first reported by Lohmann in 1920 and termed by Meyer Schwicketath, which they called "dysplasia oculo-dento-digitalis" in 1957. It is somewhat rare heritable disease. ODD is generally inherited in an autosomal dominant pattern with a complex phenotype. The characteristic features are : (1) unique facial features, (2) microphthalmos, (3) syndactyly and camptodactyly of 4th and 5th fingers, (4) osseous anomalies of the middle phalanges of 5th fingers and toes, (5) enamel hypoplasia, (6) dry lusterless hair. We found several occlusal wearing and yellow discoloration of succedaneous teeth, multiple caries lesions, premature loss and pulpal involvement of primary teeth with associated enamel abnormalities caused by generalized enamel hypoplasia in a fairly constant oral finding. Occasionally partial anodontia, microdontia and cleft lip and palate can be manifested. This case, a 9-year-old female with repaired bilateral syndactyly was referred to pediatric dental clinic, Pusan National University Hospital for evaluation of severe attrition of teeth and caries lesions. She had most of the above mentioned typical manifestations of the syndrome. Dental treatment including caries control, stainless steel crown were performed.

  • Vitamin D-resistant rickets(VDRR) is hereditary disease manifesting marked hypophosphatemia caused by renal tubular loss of phosphate into urine and an associated decrease in the calcium and phosphorous ion product. VDRR is identified by clinical symptoms, such as in the limbs, gait disturbance, dwarfism, familial occurrence, bowlegs, and knock-knees, as well as by laboratory findings. Dental findings are enlarged chambers and extension of the pulp horns into the cusp tips in both the primary and the permanent dentition in contrast to the vitamin D-deficient rickets. The major oral manifestations are multiple spontaneous abscesses in a caries-free dentition. We reported the clinical features and treatment of a 6-year-old boy with vitamin D-resistant rickets referred to the department of pediatric dentistry in Kyung Hee University for multiple spontaneous periapical abscesses and gingival fistula without severe dental caries.
