치매와 구강 건강의 관계 (Association between dementia and oral health)

  • According to the burst of aged people, researchers have focused on aging-related diseases. Cognitive impairment including Alzheimer's disease (AD), one of the representative diseases related to aging, has no treatment option until now. Recently, it has been revealed that systemic inflammation plays a fundamental role in the pathogenesis of AD. Previous studies have suggested the association between poor oral health and cognitive impairment. Poor oral health can cause dental caries, chronic periodontitis, multiple tooth loss, and poor chewing ability, etc. Especially, periodontitis is a well-known chronic inflammatory disease and affects cognitive impairment directly and indirectly by inflammatory products mediators. Therefore, reduction of pathogenic microbial burden and inflammatory products by treating periodontitis can be a therapeutic modality to prevent cognitive impairment or to slow down the progression of it. Future studies are necessary to elucidate the causal relations and plausible mechanisms between poor oral health and cognitive impairment.

Optic neuritis and multiple cranial neuropathies in patient with chronic inflammatory demyelinating polyneuropathy

  • Chronic inflammatory demyelinating polyneuropathy (CIDP) is a chronic recurrent acquired immune-mediated disease of the peripheral nerves that presents with progressive sensory and motor deficits in all four limbs. Cranial nerve involvement is not as common as in Guillain-Barre syndrome, and central nervous system involvement including optic neuritis has rarely been reported in patients with CIDP. We recently experienced a case with classic CIDP involving bilateral facial and trigeminal nerves, right lower cranial nerves, and the right optic nerve.

만성질환자의 스트레스 지각, 기분상태, 스트레스 증상에 관한 연구 (Perceived Stress, Mood State, and Sympotms of Stress of the Patient with Chronic Illness)

  • Purpose: The purpose of this study was to identify the factors influencing Symptoms of Stress in patients with chronic illness. Method: Data were collected by questionnaires from 1,748 patients with chronic disease in General Hospital in Seoul. Chronic diseases of were cardiac disease including hypertension, peptic ulcer, pulmonary disease included COPD and asthma, DM, and chronic kidney disease. The data were analyzed using descriptive statistics, pearson correlation coefficients, and stepwise multiple regression. Result: 1. The level of symptoms of stress was moderate(M=2.17). 2. The score of symptoms of stress showed significantly positive correlation with the score of mood state(r=.58, p=.00), perceived stress(r=.57, p=.00), and ways of coping(r=.33, p=.00). The symptoms of stress showed significantly negative correlation with the score of social support(r=-.37, p=.00) and self-esteem(r=-.19, p=.00). 3. The most powerful predictor of symptoms of stress was mood state and the variance explained was 34%. A combination of mood state, ways of coping, perceived stress, social support, and duration of illness account for 45% of the variance in symptoms of stress of the patients with chronic illness. Conclusion: This study suggest that mood state, ways of coping, perceived stress, and social support are significantly influencing factors on symptoms of stress of the patients with chronic illness.

60세 이상 1인 가구 여성의 만성질환과 삶의 질이 주관적 건강인지에 미치는 영향에 대한 융합적 연구 (A Convergence Study on the Effect of Chronic Diseases and Quality of Life on Subjective Health Cognition in Single-Household Women Aged 60 or over)

  • 본 연구는 60세 이상 1인 가구 여성의 만성질환과 삶의 질이 주관적 건강인지에 미치는 영향을 실증적으로 분석하여, 보건의료와 사회복지서비스의 융합적인 제언을 위한 목적으로 수행되었다. 이를 위해 국민건강영양조사 2016-2019 자료를 활용하였으며, 연구대상은 이혼 혹은 사별한 60세 이상 1인 가구의 여성 1,111명을 선정하였다. 연구결과, 만성질환 유병에 따른 주관적 건강인지의 평균 차이가 가장 크게 나타난 것은 관절염이었다. 그리고 인구사회학적 특성들을 통제한 다중회귀분석에서, 주관적 건강인지에 가장 큰 부적(-) 영향을 미치는 요인은 만성질환 가운데 당뇨병(-0.084)이었으며, 정적(+) 요인은 삶의 질 가운데 운동능력(+0.173)이었다. 따라서 60세 이상 1인 가구 여성의 주관적 건강인지를 향상시키기 위해 만성질환에 대한 보건의료서비스를 강화하고 삶의 질을 개선하기 위한 사회복지정책이 융합적으로 함께 고려되어야 함을 시사한다.

공공부문 보건의료조직의 만성질환 예방사업에서 요구하는 필요능력과 보건교육사의 직무 강화 방안 (Enhancement for competencies of health education specialists to prevent chronic diseases in public health organizations)

  • Objectives: This study performed to enhance the competency of health education specialist on population-based program to prevent chronic disease in public health organizations. In addition, it will provide the basic data to enhance specialized competency for health education specialist. Method: The current operating strategy and demanded competency related to chronic disease programs were analyzed by reviewing the Korean Health Plan 2020, the guideline of integrated health promotion programs, and the job description of health education specialist. Results: Preventive programs for chronic disease provide with healthy lifestyle programs in integrated health promotion service. First, health education specialist should learn to professional knowledge on health risk factor and chronic disease. Second, they should cultivate the integrated competency to manage operations on obesity, hypertension, diabetes, cardiovascular disease, and cancer prevention programs. Third, they get strengthened the-job training to implement health education, public relations, campaigns, media advocacy, and utilization of multi-media. Conclusion: It should implement the preventive programs for chronic disease in various health promotion services through coordination with multiple sectors. To identify of the job in preventive program for that, it would be required empowerment of health education specialists on disease prevention science and practical life skill.

우리 나라 치과기공사의 직업성 질병과 관련요인에 대한 조사연구 (A Study on the Occupational Diseases of the Dental Technicians and the Related Factors in Korea)

  • This study was performed to identify the occupational diseases in the dental technicians working in Korea, and to elucidate the main related affecting factors for the occurrence of the occupational diseases. The research was carried out using a mail questionnaire and data collection was done for two months from April 15, 2000. The questionnaires were delivered to 1000 dental technicians working at 250 dental laboratories which were chosen among the total 1330 dental laboratories in south Korea. And 763 questionnaires were collected, then 739 questionnaires were used for a logistic and a multiple regression analyses. The results obtained in this study are summarized as follows: 1. The occupational diseases in the dental technicians included pneumoconiosis, chronic bronchitis, asthma, pneumonia, hyposmia, allergic dermatitis, eczema, contagious dermatitis, central nervous disorder, dysfunction of finger, vertebral disorder, dysfunction of interphalangeal joint, ophthalmitis, auditory dysfunction, otitismedia, optic trauma, and bum. 2. The main related affecting factors for the occurrence of the occupational diseases in the dental technicians were elucidated as follows: On the ccurrence of chronic bronchitis, vibration of equipment and job career were affected positively. On the ccurrence of asthma, job career and career on current working part were affected positively, but exhaust ventilation, work hour were affected negatively. On the occurrence of eczema, career on current working part and vibration of equipment were affected positively, but personal protective equipment, local exhaust ventilation, isolation of gas source were affected negatively. And eczema was occurred more in methyl methacrylate work than in any other work part. On the occurrence of finger dysfunction, vibration of equipment and noise of work place were affected positively. But personal protective equipment, local exhaust ventilation, isolation of dust and gas source were affected negatively. But finger dysfunction was occurred more in metal polishing than in any other work part. On the occurrence of auditory dysfunction, noise of work place were affected negatively. The results obtained in this study can give a valuable information for a prevention of the occupational diseases in the dental technicians working in Korea, and for keeping them in healthy. This study also can be helpful for a researcher who is aiming at a identification of occupational diseases and a elucidation of related affecting factors in a similar occupation.

고령인구 비율이 높은 지역 장년, 노년층의 건강.영양상태 및 이에 영향을 미치는 인자에 관한 연구 - III. 농촌노인의 주관적 건강평가와 건강관련행동 및 식이섭취와의 관련성 - (Nutritional Status and Related Factors of the Elderly in Longevity Areas - III. Relation among Self-rated Health, Health-related Behaviors, and Nutrient Intake in Rural Elderly -)

  • The study aimed to identify major factors related to global self-rated health of the community-dwelling elderly people in rural areas. Interviews were conducted with 433 persons over 65 years of age. The socio-economic characteristics, chronic disease status, measures of functional and mental health, life satisfaction, health-related behaviors including smoking, drinking, exercise, social activity, dietary habit, and food intakes were analyzed to determine their influence on self-rated health index. Data on food intake were obtained through the 24-hour recall method. The self-rated health of rural elderly was poor or very poor as reported by 42.6% and 52.4% of the men and women, respectively. Poor self-rated health was found to be related to elderly aged $65{\sim}74$, female, absence of work, more chronic diseases, dependence on Instrumental Activity Daily Living (IADL), higher mental unhealthy days, lower current life satisfaction, less social activity, lower dietary habit scores, lower intake of total food, fruit, eggs, fishes & shell fishes, vitamin C (%RDA), and Nutrient Adequacy Ratio (NAR). The results of the multiple regression analysis showed that poor self-rated health index is significantly associated with more chronic diseases, mental unhealthy days, gastrointestinal disease, musculoskeletal disease, less social activity, and lower intake of fruits. The results also suggested that improving the nutritional status and functional ability, and reducing the burden of chronic diseases are beneficial to the self-rated health index of the elderly.

  • Background: Although airway obstruction in chronic obstructive pulmonary disease (COPD) is due to pathologic processes in both the airways and the lung parenchyma, the contribution of these processes, as well as other factors, have not yet been evaluated quantitatively. We therefore quantitatively evaluated the factors contributing to airflow limitation in patients with COPD. Methods: The 213 COPD patients were aged >45 years, had smoked >10 pack-years of cigarettes, and had a post-bronchodilator forced expiratory volume in one second ($FEV_1$)/forced vital capacity (FVC) <0.7. All patients were evaluated by medical interviews, physical examination, spirometry, bronchodilator reversibility tests, lung volume, and 6-minute walk tests. In addition, volumetric computed tomography (CT) was performed to evaluate airway wall thickness, emphysema severity, and mean lung density ratio at full expiration and inspiration. Multiple linear regression analysis was performed to identify the variables independently associated with $FEV_1$ - the index of the severity of airflow limitation. Results: Multiple linear regression analysis showed that CT measurements of mean lung density ratio (standardized coefficient ${\beta}$=-0.46; p<0.001), emphysema severity (volume fraction of the lung less than -950 HU at full inspiration; ${\beta}$=-0.24; p<0.001), and airway wall thickness (mean wall area %; ${\beta}$=-0.19, p=0.001), as well as current smoking status (${\beta}$=-0.14; p=0.009) were independent contributors to $FEV_1$. Conclusion: Mean lung density ratio, emphysema severity, and airway wall thickness evaluated by volumetric CT and smoking status could independently contribute to the severity of airflow limitation in patients with COPD.

다발성 골수종 환자에서 발생한 아밀로이드종의 세침흡인 세포학적 소견 (Fine Needle Aspiration Cytology of Amyloid Tumor Arising in Association with Multiple Myeloma)

  • Amyloid tumor is a tumorlike localized deposit of amyloid which is encountered occasionally in association with multiple myeloma and various chronic inflammatory diseases. This report describes a case of solitary amyloid tumor of the neck which was the presenting symptom arising in association with multiple myeloma. A 56-year-old woman complained of a palpable neck mass and fine needle aspiration was done. Multiple myeloma was diagnosed on the basis of the bone marrow biopsy and monoclonality of kappa light chain. The histologic and cytologic features of the amyloid appear to be characteristic and may allow a definitive diagnosis to be made on needle aspiration biopsy.

Stem Cells and Cell-Cell Communication in the Understanding of the Role of Diet and Nutrients in Human Diseases

  • The term, "food safety", has traditionally been viewed as a practical science aimed at assuring the prevention acute illnesses caused by biological microorganisms, and only to a minor extent, chronic diseases cause by chronic low level exposures to natural and synthetic chemicals or pollutants. "food safety" meant to prevent microbiological agents/toxins in/on foods, due to contamination any where from "farm to Fork", from causing acute health effects, especially to the young, immune-compromised, genetically-predisposed and elderly. However, today a broader view must also include the fact that diet, perse (nutrients, vitamins/minerals, calories), as well as low level toxins and pollutant or supplemented synthetic chemicals, can alter gene expressions of stem/progenitor/terminally-differentiated cells, leading to chronic inflammation and other mal-functions that could lead to diseases such as cancer, diabetes, atherogenesis and possibly reproductive and neurological disorders. Understanding of the mechanisms by which natural or synthetic chemical toxins/toxicants, in/on food, interact with the pathogenesis of acute and chronic diseases, should lead to a "systems" approach to "food safety". Clearly, the interactions of diet/food with the genetic background, gender, and developmental state of the individual, together with (a) interactions of other endogenous/exogenous chemicals/drugs; (b) the specific biology of the cells being affected; (c) the mechanisms by which the presence or absence of toxins/toxicants and nutrients work to cause toxicities; and (d) how those mechanisms affect the pathogenesis of acute and/or chronic diseases, must be integrated into a "system" approach. Mechanisms of how toxins/toxicants cause cellular toxicities, such as mutagenesis; cytotoxicity and altered gene expression, must take into account (a) irreversible or reversal changes caused by these toxins or toxicants; (b)concepts of thresholds or no-thresholds of action; and (c) concepts of differential effects on stem cells, progenitor cells and terminally differentiated cells in different organs. This brief Commentary tries to illustrate this complex interaction between what is on/in foods with one disease, namely cancer. Since the understanding of cancer, while still incomplete, can shed light on the multiple ways that toxins/toxicants, as well as dietary modulation of nutrients/vitamins/metals/ calories, can either enhance or reduce the risk to cancer. In particular, diets that alter the embryo-fetal micro-environment might dramatically alter disease formation later in life. In effect "food safety" can not be assessed without understanding how food could be 'toxic', or how that mechanism of toxicity interacts with the pathogenesis of any disease.
