하악 무치악 부위의 임플란트 이식을 위한 전산화단층촬영 영상의 비교 평가 (Comparative evaluation of computed tomography for dental implants on the mandibular edentulous area)

  • Purpose: The purpose of this study was to evaluate the clinical usefulness of the recently developed multi-detector computed tomography and cone beam computed tomography in pre-operative implant evaluation, by comparing them with the single detector computed tomography, already confirmed for accuracy in this area. Materials and Methods: Five partially edentulous dry human mandibles, with $1{\times}1mm$ gutta percha cones, placed in 5mm intervals posterior to the mental foramen on each side of the buccal part of the mandible, were used in this study. They were scanned as follows: 1) Single detector computed tomography: slice thickness 1mm, 200mA, 120kV 2) Multi-detector computed tomography: slice thickness 0.75mm, 250mA, 120kV 3) Cone beam computed tomography: 15mAs, 120kV Axial images acquired from three computed tomographies were transferred to personal computer, and then reformatted cross-sectional images were generated using V-Implant $2.0^{(R)}$ (CyberMed Inc., Seoul, Korea) software. Among the cross-sectional images of the gutta perch a cone, placed in the buccal body of the mandible, the most precise cross section was selected as the measuring point and the distance from the most superior border of the mandibular canal to the alveolar crest was measured and analyzed 10 times by a dentist. Results: There were no significant intraobserver differences in the distance from the most superior border of the mandibular canal to the alveolar crest (p>0.05). There were no significant differences among single detector computed tomography, multi-detector computed tomography and cone beam computed tomography in the distance from the most superior border of the mandibular canal to the alveolar crest (p>0.05). Conclusion: Multi-detector computed tomography and cone beam computed tomography are clinically useful in the evaluation of pre-operative site for mandibular dental implants, with consideration for radiation exposure dose and scanning time.

제조사별 다중 검출기 컴퓨터단층촬영 장비의 관전류 자동노출조절 기법의 방사선량 감소 평가 (Evaluation of Radiation Dose Reduction from the Automatic Exposure Control Technique in Different Manufactures Multi-Detector Computed Tomography)

  • The purpose of the study was to evaluation of the radiation dose reduction using various automatic exposure control (AEC) systems in different manufactures multi-detector computed tomography (MDCT). We used three different manufacturers for the study: General Electric Healthcare, Philips Medical systems and Siemens Medical Solutions. The general scanning protocol was created for the each examination with the same scanning parameters as many as possible. In the various AEC systems, the evaluation of reduced-dose was evaluated by comparing to fixed mAs with using body phantom. Finally, when we applied to AEC for three manufacturers, the radiation dose reduction decreased each 35.3% in the GE, 58.2% in the Philips, and 48.6% in the Siemens. This applies to variety of the AEC systems which will be very useful to reduce the dose and to maintain the high quality.

  • Multi-detector row CT (MDCT) provides faster speed, longer coverage in conjunction with thin slices, improved spatial resolution, and ability to produce high quality muliplanar and three-dimensional (3D) images. MDCT has revolutionized the non-invasive evaluation of the central airways. Simultaneous display of axial, multiplanar, and 3D images raises precision and accuracy of the radiologic diagnosis of central airway disease. This article introduces central airway imaging with MDCT emphasizing on the emerging role of multiplanar and 3D reconstruction.

Photon-Counting Computed Tomography: Experience in Musculoskeletal Imaging

  • Since the emergence of the first photon-counting computed tomography (PCCT) system in late 2021, its advantages and a wide range of applications in all fields of radiology have been demonstrated. Compared to standard energy-integrating detector-CT, PCCT allows for superior geometric dose efficiency in every examination. While this aspect by itself is groundbreaking, the advantages do not stop there. PCCT facilitates an unprecedented combination of ultra-high-resolution imaging without dose penalty or field-of-view restrictions, detector-based elimination of electronic noise, and ubiquitous multi-energy spectral information. Considering the high demands of orthopedic imaging for the visualization of minuscule details while simultaneously covering large portions of skeletal and soft tissue anatomy, no subspecialty may benefit more from this novel detector technology than musculoskeletal radiology. Deeply rooted in experimental and clinical research, this review article aims to provide an introduction to the cosmos of PCCT, explain its technical basics, and highlight the most promising applications for patient care, while also mentioning current limitations that need to be overcome.

Patient radiation dose and protection from cone-beam computed tomography

  • After over one decade development, cone beam computed tomography (CBCT) has been widely accepted for clinical application in almost every field of dentistry. Meanwhile, the radiation dose of CBCT to patient has also caused broad concern. According to the literature, the effective radiation doses of CBCTs in nowadays market fall into a considerably wide range that is from $19{\mu}Sv$ to $1073{\mu}Sv$ and closely related to the imaging detector, field of view, and voxel sizes used for scanning. To deeply understand the potential risk from CBCT, this report also reviewed the effective doses from literatures on intra-oral radiograph, panoramic radiograph, lateral and posteroanterior cephalometric radiograph, multi-slice CT, and so on. The protection effect of thyroid collar and leaded glasses were also reviewed.

Discordant findings of dimercaptosuccinic acid scintigraphy in children with multi-detector row computed tomography-proven acute pyelonephritis

  • Purpose: The diagnosis of acute pyelonephritis (APN) is often difficult, as its clinical and biological manifestations are non-specific in children. If not treated quickly and adequately, however, APN may cause irreversible renal damage, possibly leading to hypertension and chronic renal failure. We were suspecting the diagnostic value of $^{99m}Tc$-dimercaptosuccinic acid (DMSA) scan by experiences and so compared the results of DMSA scan to those of multi-detector row computed tomography (MDCT). Methods: We retrospectively selected and analyzed 81 patients who were diagnosed as APN by MDCT during evaluation of their acute abdomen in emergency room and then received DMSA scan also for the diagnostic work-up of APN after admission. We evaluated the results of imaging studies and compared the diagnostic value of each method by age groups, <2 years (n=45) and ${\geq}$2 years (n=36). Results: Among total 81 patients with MDCT-proven APN, DMSA scan was diagnostic only in 55 children (68%), while the remaining 26 children (32%) showed false negative normal findings. These 26 patients were predominantly male and most of them, 19 (73.1%) were <2 years of age. Conclusion: DMSA scan holds obvious limitation compared to MDCT in depicting acute inflammatory lesions of kidney in children with APN, especially in early childhood less than 2 years of age. MDCT showed hidden lesions of APN, those were undetectable through DMSA scan in children.

256 다중 검출기 전산화단층촬영에서 두개부 전용 팬톰을 이용한 분해능 파라메터와 재구성 필터의 영상 평가 (Image Evaluation of Resolution Parameter and Reconstitution Filter in 256 Multi Detector Computed Tomography by Using Head Phantom)

  • 256 다중 검출기 전산화단층촬영 (multi detector computed tomography, MDCT)에서 두개부 전용 팬톰을 이용하여 분해능 파라메터와 재구성 필터의 영상 품질을 평가하고자 하였다. 사용한 장비는 256 MDCT 을 사용하였으며 philips system head phantom을 이용하여 Extended Brilliance Workspace에서 영상의 질 평가를 측정하였다. 장비에서 지원하고 있는 분해능 파라메터와 재구성 필터가 화질에 미치는 영향을 알아보기 위해서 검사조건을 $512{\times}512$ matrix, $128{\times}0.625$ mm beam collimation, 120 kVp, 250 mAs, 0.5 sec, 250 mm field of view (FOV), 절편 두께와 절편 간격은 5 mm, 1.0 pitch로 동일하게 적용하였다. 분해능 파라메터은 'Standard', 'High', 'Ultrahigh'로 구분하여 적용하였으며, 재구성 필터는 'A', 'B', 'C', 'D', 'UA', 'UB', 'UC' 등으로 바꿔가면서 영상을 재구성하여 노이즈, 균일도, 직선성, 변조전달함수 (modulation transfer function, MTF)의 50%와 10%를 측정하였다. 그 결과 'High' 분해능은 균일도, 직선성, MTF 50%와 10%에서 우수하였다. 'UA', 'UB' 재구성 필터는 균일도와 노이즈 평가에서 양호했으며 'C'재구성 필터는 직선성과 MTF 50%와 10%에서 양호하였다.

인체 모형을 이용한 다중 검출기 컴퓨터단층촬영기기의 다양한 자동노출제어 시스템에 대한 연구 (A Study on Various Automatic Exposure Control System in Multi-Detector Computed Tomography by Using Human Phantom)

  • 다중 검출기 컴퓨터단층촬영(multi-detector computed tomography; MDCT) 장비에서 이용되는 다양한 자동노출제어(automatic exposure control; AEC) 시스템에 대한 선량 감소 정도와 영상의 품질을 비교 평가하였다. 연구에 이용한 AEC 시스템은 General Electric(GE)사의 Auto-mA 3D, Philips사의 DoseRight, 그리고 Siemens사의 Care Dose 4D를 이용하였다. 모든 실험에서 X-선 노출 조건을 가능한 한 동일하게 하였다. 다양한 AEC 시스템에서 선량 감소 평가는 인체 모형을 이용하여 고정관류기법과 비교하여 평가하였다. 또한 영상 품질은 CT 계수(number)의 표준 편차를 이용하여 영상의 잡음을 측정하였다. 그 결과 AEC 시스템을 적용할 때 Auto-mA 3D는 35.3%, DoseRight는 58.2%, Care Dose 4D는 48.6%의 최대 선량감소가 있었으며, 영상품질에서는 Care Dose 4D의 강함/약함 (Strong/Weak)의 조합에서 유의한 차이가 없었다(P=0.269). 이런 다양한 AEC 시스템의 적용은 고품질의 영상을 유지하면서 선량을 감소하는데 매우 유용 할 것이라고 사료된다.

Comparison of effective dose for imaging of mandible between multi-detector CT and cone-beam CT

  • Purpose : The aim of this study was to compare the effective dose for imaging of mandible between multi-detector computed tomography (MDCT) and cone-beam computed tomography (CBCT). An MDCT with low dose technique was also compared with them. Materials and Methods : Thermoluminescent dosimeter (TLD) chips were placed at 25 organ sites of an anthropomorphic phantom. The mandible of the phantom was exposed using 2 different types of MDCT units (Somatom Sensation 10 for standard-dose MDCT, Somatom Emotion 6 for low-dose MDCT) and 3 different CBCT units (AZ3000CT, Implagraphy, and Kavo 3D eXaM). The radiation absorbed dose was measured and the effective dose was calculated according to the ICRP 2007 report. Results : The effective dose was the highest for Somatom Sensation 10 (425.84 ${\mu}Sv$), followed by AZ3000CT (332.4 ${\mu}Sv$), Somatom Emotion 6 (199.38 ${\mu}Sv$), and 3D eXaM (111.6 ${\mu}Sv$); it was the lowest for Implagraphy (83.09 ${\mu}Sv$). The CBCT showed significant variation in dose level with different device. Conclusion : The effective doses of MDCTs were not significantly different from those of CBCTs for imaging of mandible. The effective dose of MDCT could be markedly decreased by using the low-dose technique.

파노라마방사선사진과 다면상재구성 CT상의 치조골 높이 계측에 대한 비교 연구 (Comparative study on alveolar bone height of pantomography and multi planar reformatted computed tomography)

  • Purpose: To compare alveolar bony height of pantomograph with bony height of thin slice, multiplanar reformatted (MPR) Computed Tomograph. Materials and Methods : Panoramic radiograms of 12 young adult patients had been taken by one radologic technitian and the measurements were corrected by magnification ratio (1.20). The slice thickness of Multi-detector Computed Tomography (CT) was at least 1mm for the accuracy. The raw CT datas were imported into the V-works 4.0 (CyberMed Corp., Seoul, Korea) and transformed to MPR images. Pantomographic measurements of alveolar bone were compared to CT values by average mean bony height measurements for the accuracy. Inter-, and Intra-observer variability was evaluated. Results : There was no significant differences between height measurement of pantomography and that of CT (P>0.05). There were no significant differences in either inter-or intra-observer measurements (P>0.05). Conclusion : Pantomography showed relatively high accuracy and precision in measuring alveolar bony height.
