A Review Study on Movement System Impairment Syndromes (동태손상(動態損傷) 증후군(症候群)에 관한 연구 현황 고찰)

  • Objectives: Sahrmann, an American physical therapist, has developed a set of movement-related diagnoses on musculoskeletal pain syndromes (MPS). We reviewed articles for studies of objectification and quantification of Oriental medicine diagnosis system about MPS. Methods: The authors reviewed a total of 12 studies found on PubMed to obtain movement system impairment (MSI)-based classification categories. Results: The MSI system has been developed for objectification and quantification of physical therapists' MPS diagnoses. The MSI system of classification is based on the basic premise that loss of precise movement is the result of specific repetitive movements and positions in everyday life. The MSI system defines 8 categories in the shoulder region, 5 in the lumbar region, and 7 in the knee region. Treatment involves (1) educating the person about the specific directions of alignment and movement that appear to be contributing to the musculoskeletal problem, (2) modifying the direction - specific alignment and movement patterns during daily activities, and (3) exercises to address the impairments. Conclusions: The authors propose that MSI studies could help to advance an Oriental medical diagnosis system on musculoskeletal pain syndromes, because MSI shares similarity with Oriental medicine in terms of holism, views of constant motion, and mi-byung (subhealth) treatment.

  • Wheat quality score index was proposed as a new criterion of wheat_quality for evaluation of the laboratory testing value such as milling rate (Xm), flour ash content (Xa), flour protein content (Xp) and sedimentation value (Xs). 1) Wheat quality score index was expressed as follows; Milling score index (MSI) $=2.3X_m-72.4X_a$ Protein score index (PSI) =$5.2X_p+0.9X_{psi}$ Composite quality score index (QSI) =$0.63X_{msi}+0.37X_{psi}$ 2) The samples with the more than 100 of MSI would be the good milliability, and the samples with more than 100 of PSI would be the good bread making properties, while that with less than 100 of PSI be good pastry type properties. Whereas most of the samples with higher value of QSI would be related to the good milliability and good bread making properties. 3) MSI, PSI and QSI were significantly correlated to the each related quality components.

Msi1-Like (MSIL) Proteins in Fungi

  • Msi1-like (MSIL) proteins, which are eukaryote-specific and contain a series of WD40 repeats, have pleiotropic roles in chromatin assembly, DNA damage repair, and regulation of nutrient/stress-sensing signaling pathways. In the fungal kingdom, the functions of MSIL proteins have been studied most intensively in the budding yeast model Saccharomyces cerevisiae, an ascomycete. Yet their functions are largely unknown in other fungi. Recently, an MSIL protein, Msl1, was discovered and functionally characterized in the pathogenic yeast Cryptococcus neoformans, a basidiomycete. Interestingly, MSIL proteins appear to have redundant and unique roles in both fungi, suggesting that MSIL proteins may have evolutionarily divergent roles in different parts of the fungal kingdom. In this review, we will describe the current findings regarding the role of MSIL proteins in fungi and discuss future directions for research on this topic.

Screening for Lynch Syndrome in Young Colorectal Cancer Patients from Saudi Arabia Using Microsatellite Instability as the Initial Test

  • Background: Lynch Syndrome (LS) is a familial cancer condition caused by germline mutations in DNA mismatch repair genes. Individuals with LS have a greatly increased risk of developing colorectal cancer (CRC) and it is therefore important to identify mutation carriers so they can undergo regular surveillance. Tumor DNA from LS patients characteristically shows microsatellite instability (MSI). Our aim here was to screen young CRC patients for MSI as a first step in the identification of unrecognized cases of LS in the Saudi population. Materials and Methods: Archival tumor tissue was obtained from 284 CRC patients treated at 4 institutes in Dammam and Riyadh between 2006 and 2015 and aged less than 60 years at diagnosis. MSI screening was performed using the BAT-26 microsatellite marker and positive cases confirmed using the pentaplex MSI analysis system. Positive cases were screened for BRAF mutations to exclude sporadic CRC and were evaluated for loss of expression of 4 DNA mismatch repair proteins using immunohistochemistry. Results: MSI was found in 33/284 (11.6%) cases, of which only one showed a BRAF mutation. Saudi MSI cases showed similar instability in the BAT-26 and BAT-25 markers to Australian MSI cases, but significantly lower frequencies of instability in 3 other microsatellite markers. Conclusions: MSI screening of young Saudi CRC patients reveals that approximately 1 in 9 are candidates for LS. Patients with MSI are strongly recommended to undergo genetic counselling and germline mutation testing for LS. Other affected family members can then be identified and offered regular surveillance for early detection of LS-associated cancers.

Clinical Implications of Microsatellite Instability in Early Gastric Cancer

  • Purpose: We aimed to evaluate the clinical characteristics of microsatellite instability in early gastric cancer. Materials and Methods: The microsatellite instability status of resected early gastric tumors was evaluated using two mononucleotide repeat markers (BAT25 and BAT26) and three dinucleotide repeat markers (D5S346, D2S123, and D17S250). Tumors with instability in two or more markers were defined as microsatellite instability-high (MSI-H) and others were classified as microsatellite stable (MSS). Results: Overall, 1,156 tumors were included in the analysis, with 85 (7.4%) classified as MSI-H compared with MSS tumors. For MSI-H tumors, there was a significant correlation with the female sex, older age, tumor location in the lower gastric body, intestinal histology, lymphovascular invasion (LVI), and submucosal invasion (P<0.05). There was also a trend toward an association with lymph node (LN) metastasis (P=0.056). In mucosal gastric cancer, there was no significant difference in MSI status in tumors with LN metastasis or tumors with LVI. In submucosal gastric cancer, LVI was more frequently observed in MSI-H than in MSS tumors (38.9% vs. 25.0%, P=0.027), but there was no difference in the presence of LN metastases. The prognosis of MSI-H tumors was similar to that of MSS tumors (log-rank test, P=0.797, the hazard ratio for MSI-H was adjusted by age, sex, pT stage, and the number of metastatic LNs, 0.932; 95% confidence interval, 0.423-2.054; P=0.861). Conclusions: MSI status was not useful in predicting prognosis in early gastric cancer. However, the frequent presence of LVI in early MSI-H gastric cancer may help guide the appropriate treatment for patients, such as endoscopic treatment or limited LN surgical dissection.

A Study on Decrease of Vertical Accelerations due to Changes in Location of the Habitation Division for Training Ship (실습선의 거주위치 변경에 따른 수직가속도 저감에 관한 연구)

  • Research on ship motion and seasickness is recognized as the important research area to ensure the pleasant operative environment in addition to the research of operation safety of ship. In this paper, the motion performance in waves for the training ship Kaya of Pukyong National University is obtained by using the computer program based on Strip Method. To guarantee the pleasant seafaring in ocean, the vertical acceleration of ship motion is calculated according to the habitation division location in the ship. The results of calculation by changes of location of habitation division are compared with the guideline of MSI(Motion Sickness Incidence). The degree of motion sickness is shown and discussed through the comparison between calculated vertical acceleration spectrum and MSI guideline. To improve the safety of ship in motion and the pleasant seafaring in waves, the downtrend of seasickness ratio is needed by the decrease on vertical acceleration of the ship. Through the results in this paper, the relocation of both bridge and accommodation toward the aftship reduced the vertical acceleration and MSI.

  • For boarding comfort and pleasant boarding sensitivity of passenger in passenger ship, there are a few methods such as motion sickness rate or MSI(Motion Sickness Incidence) to assess safety of passenger ship. To find out MSI or motion sickness rate of the training vessel Kaya of Pukyong National University, we use lots of various factors: sea condition, incident angle in main sail way, economic speed, calculation position of motion sickness, sea state, work place and reduce zone of ship. In this paper, we adopted Maxsurf Seakeeper program based on Strip Theory and carefully compared theoretical results with experimental results.

Qualitative and quantitative evaluation of root injury risk potentially burdening insertion of miniscrew implants

  • Objective: Microscrew implants (MSls) offer many advantages, but some complications are known to occur during their insertion. One of the most commonly reported complications is root injury. Our aim was to identify factors associated with root injury and to evaluate their qualitative and quantitative values. Methods: Thirty-five orthodontists placed MSls (AbsoAnchor$AbsoAnchor^{(R)}$, Dentos Co. Ltd, Daegu, Korea) in the upper jaw of typodonts, labially between the second premolar and the first molar, in low and high vertical positions. Root contacts were counted, and distances between MSI apices and roots were measured. Fear level of the orthodontists was surveyed before and after the experiment. Wilcoxon's test, chi-square test, and Mann-Whitney test were used for statistical analysis. Results: Overall root contact rate of MSI insertion was 23.57%. The root contact rate was significantly higher in MSls inserted at $90^{\circ}$ (45.71%) than at $30^{\circ}$ (1.43%). The distance between the dental root and MSI also increased significantly in MSls inserted at $30^{\circ}$. Mean fear level before MSI insertion (4.6) significantly decreased after insertion (3.2); the causative factors were risk of injury to dental root and maxillary sinus or mandibular canal. Conclusions: Root injury is relatively rare, and oblique angulation reduces the risk of root and MSI contact.

Comparison of Bone Volume Measurements Using Conventional Single and Dual Energy Computed Tomography (전산화단층영상검사에서 단일에너지와 이중에너지를 이용한 뼈 부피측정의 비교)

  • The study examines changes in calcium volume on born by comparing two figures; one is measured by dual energy computed tomography(DECT) followed by applying variation in monochromatic energy selection(keV), material decomposition(MD), and material suppressed iodine(MSI) analysis, and the other is measured by conventional single source computed tomography(CSCT). For this study, based on CSCT images taken by using human mimicked phantom, 70, 100, 140 keV and MSI, MD material calcium weighting(MCW) and MD material iodine weighting(MIW) of DECT were applied respectively. Then calculated calcium volume was converted to Agatston score for comparison. Volume of human mimicked phantom was in inverse proportion to keV. The volume decreased while keV increased(p<0.05). The most similar DECT volumes were reconstructed at 70 keV, the difference was showed $35.8{\pm}12.2$ for rib, femur ($16.1{\pm}24.1$), pelvis($13.7{\pm}18.8$), and spine($179.0{\pm}61.8$). However, the volume of MSI was down for each organ; the volume of rib was 5.55%, femur(76.34%), pelvis(55.16%) and spine(87.58%). The volume of MSI decreased 55.9% for rib, femur(80.7%), pelvis(69.6%) and spine(54.2%) while MD MIW reduced for rib(83.51%), femur(87.68%), pelvis(86.64%), and spine(82.62%). With the results, the study found that outcomes were affected by the method which examiners employed. When using DECT, calcium volume of born dropped with keV increased. It also found that the most similar DECT images were reconstructed at 70 keV. The results of experiments implied that the users of MSI and MD should be cautious of errors as there are big differences in scores between those two methods.

Evaluation method of motion seasickness by ship motions during underway in irregular waves (선박운항 중 선체동요에 의한 뱃멀미 평가방법)

  • In order to deduce an objective evaluation method of motion seasickness incidence (MSI) by ship motions during underway in irregular waves and to present the fundamental data of passenger comfort on the yacht and the passenger ship according to the result, the MSI of the trainees by the questionnaires was analysed and compared with the rate of variation of salivary ${\alpha}$-amylase activity (VSAA) on the training ship "A-ra ho" of Jeju national university. Relationship between rate of variation (x) by salivary ${\alpha}$-amylase activity and motion seasickness incidence (y) was described by the equation, MSI(%) = 0.6073 x + 12.189 including the correlation coefficient ($R^2=0.9853$). The result obtained through the rate of variation of salivary ${\alpha}$-amylase activity which was the quantitative evaluation method for ship motions causing seasickness was most affected by z-vertical acceleration and occurred within the frequency range 0.1 to 0.3Hz centered on 0.2Hz, and the simulation result based on this finding showed the motion seasickness rate at approximately 4% lower than the rate obtained through the survey.
