A Case Report of One Patient with Paralytic Strabismus Resulting from Pontine Hematoma During Puerperium (산욕기에 발생한 뇌교출혈로 인한 마비성 사시 환자 치험 1례)

  • Pregnancy related stroke is significant because it has dire impact on both mother and the fetus. Pregnant or puerperal women are reported to have about 13-fold more risk of stroke compared to non-pregnant women. Pregnancy-related stroke must be differentiated from stroke in elderly patients, because it is different in pathophysiological etiologies and recurrence rates and also diagnostic and therapeutic methods may affect the fetal health status. Because the VI cranial nerve manages lateral rectus muscles, it' s palsy causes sudden onset of double vision. The eyes turn medially. In this article, one case of paralytic strabismus resulting from pontine hematoma during puerperium is reported. It was treated with herb medicine and acupuncture. Symptoms improved. This case is submitted with a brief overview of related lit.

Postpartum Depressive Score and Related Factors Pre- and Post-delivery (산후 우울 수준과 분만전후 관련 요인에 관한 연구)

  • Purpose: This study aimed to identify the scores of postpartum depression(PPD) on the first day, 1st week, and 6th week after the delivery and to explore their related factors before and after delivery in postpartum women. Methods: With a survey design, 293 postpartum women were recruited from a postpartum unit, Ilsin Christian hospital in Pusan via convenience sampling and were followed at 1st week and 6th week in the outpatient clinic. Results: Results showed that the scores of PPD(EPDS score) were low at postpartum 1st day, 1st week and 6th week but prevalence of PPD(EPDS ${\geq}13$)was 3.1%at 1st day, 8.2%at 1st week and 7.5%at 6th week, respectively. The pre-delivery factors were experience of depression, and the post-delivery factors were baby's sex(1st day), no caregiver for baby(1st week), and no help and concern for taking care of baby from husband and family(1st day and 6th week). The greater satisfaction with becoming a mother and her life, and greater maternal attachment were related to lower level of PPD at the three time points. Conclusion: Regular screening for postpartum depression and supportive and informative education is needed for postpartum women visiting the outpatient clinic for follow-up.

Factors Explaining Mothers' Breastfeeding Satisfaction (어머니의 모유수유 만족감에 미치는 영향요인)

  • Purpose: This study was done to investigate the level of breastfeeding satisfaction, the relationship among knowledge about breastfeeding, attitude toward breastfeeding, self-efficacy of breastfeeding, intention of breastfeeding, and perceived social encouragement of breastfeeding and the factors influencing breastfeeding satisfaction in mothers 3~6 months after birth. Methods: For this descriptive research, 209 mothers who had been breastfeeding their babies agreed to participate in this study. Data on mother's satisfaction' knowledge, attitude, self-efficacy, intention, and perceived social encouragement regarding breastfeeding were collected by closed-ended questionnaires on September 14, 2007 at a contest for healthy breast-fed babies. Results: The major findings of this study were as follows: First, there were significant differences in the breastfeeding satisfaction level according to planned length of breastfeeding (F=10.43, p<.001). Second, there was a significant relationship between breastfeeding satisfaction and knowledge about breastfeeding (r=.37, p<.001), attitude toward breastfeeding (r=.62, p<.001), self-efficacy of breastfeeding (r=.66, p<.001), intention of breast feeding (r=.40, p<.001), and perceived social encouragement of breastfeeding (r=.32, p<.001). Finally, the strongest factor affecting mothers' breastfeeding satisfaction was attitude toward breastfeeding ($\beta=.38$). Other factors included self-efficacy of breastfeeding ($\beta=.36$), knowledge about breastfeeding ($\beta=.14$), perceived social encouragement of breastfeeding ($\beta=.10$), and planned length of breastfeeding ($\beta=.10$). Conclusion: This study suggests that nurses should help mothers improve breastfeeding satisfaction through promotion of knowledge about, attitudes toward, and self-efficacy of breastfeeding.

Quality of Life of Multiethnic Adolescents Living with a Parent with Cancer

  • Background: Research evidence suggests a debilitating impact of the diagnosis of cancer on the quality of life of the afflicted individuals, their spouses and their families. However, relatively few studies have been carried out on the impact on the QOL of adolescents living with parents diagnosed with cancer. This paper presents a sub-analysis on the impact of parental cancer (colorectal, breast and lung) on adolescents. Materials and Methods: This is a cross-sectional study on adolescents aged 13-18 years old. Upon ethical clearance obtained from UMMC Medical Ethics Committee, patients with colorectal, breast or lung cancer and their adolescent children were recruited from the Clinical Oncology Unit of University of Malaya Medical Centre. Respondents who gave consent completed a demographic questionnaire and the Pediatric Quality of Life Inventory, via the post, email, home visit or meetings at the clinics. Results: 95 adolescents from 50 families responded, giving a response rate of 88 percent. The adolescent's mean age was 16 years (ranging between 13-18 years). Adolescents with parental cancer had the lowest mean score in emotional functioning (p<0.05). Male adolescents had significantly higher quality of life overall and in physical functioning compared to female adolescents. Adolescents with a father with cancer had better school functioning compared to adolescents whose mothers had cancer. Families with household income of RM 5000 and above have significantly better quality of life compared to families with lower household income. Conclusions: Adolescent sons and daughters of parents with a cancer diagnosis show lowered QOL, particularly with reference to emotional functioning and school performance. Addressing the needs of this young group has been slow and warrants special attention. Revisiting the risk and resilience factors of adolescents might also inform tailored programs to address the needs of this neglected adolescent population.

  • This study, taking the children with a disorder as the subject, has an object of making an comparative analysis on the dental status according to a degree of recognition on dental hygiene and to the actional factors of home dental hygiene to search for its improving measure. As its analytical method, with the use of SPSS, I used a cross analysis and percentage to take statistics on them. 1.The higher educational level a child with a mental order has, the higher frequency of brushing teeth he(she) has. And then the higher monthly income resulted in the higher frequency of brushing teeth. 2. In a teeth inspection according to the times of snack-eating of a child with a mental disorder, there was a statistics that according to whether the child's mother has a job or not, the times of snack-eating is different. 3.In analyzing the importance of the education of dental hygiene according to the parents' social characteristics, he answered to participate in an educational program for dental hygiene.

Immunologic Aspects at the Feto-Maternal Interface (태아모체간 계면에서의 면역학적 측면)

  • Precise mechanism by which the fetus can escape from mother's immune rejection is not well understood yet over the last 50 years. The clarification of immune mechanism at the feto-maternal interface is very important, because this can be a common pathogenesis of various pathologic conditions including spontaneous abortion, habitual abortion fetal growth restriction preeclampsia, implantation failure after assisted reproductive techniques, and fetal death. In this review, current hypothetical contents were described with the priority of importance: 1) The center of this mechanism is cross-talk between the expression of HLA-C, E, G on the extravillous cytotrophoblasts and their receptors on decidual NK cell, 2) immunomodulation, 3) innate immunity is the main immunologic mechanism, 4) various mechanisms besides HLA system(eq. complement) may be associated. The overall balance of immunomodulation among these mechanisms should result in the outcome of each pregnancy. Further researches regarding the regulation of HLA system, roles of cytokines, complements should be followed in the future.

Relationship between perceived oral symptoms and smoking, drinking of high school students in metropolitan area (수도권지역 고등학생의 흡연 및 음주실태와 구강자각증상과의 관련성)

  • Objectives : The purpose of this study was to examine actual conditions of smoking and alcohol drinking behaviors among high school students, so that it could recognize possible harmful effects of smoking on oral health and could give positive motivations for students to quit smoking and drinking. Methods : This research was based on self-filling survey which 1,385 high school students in metropolitan area in Korea from September 5 to October 28, 2011. Surveyed data were analyzed by descriptive statistics, ${\chi}^2$-test and logistic analysis using SPSS WIN 12.0 program and its signification level was 0.05. Results : 1. As for smoking rate and drinking rate, boy students accounted for 11.6% and 25.4%, thereby having been higher than girl students(p<.001). It was the highest in over 180 cm(16.0%, 35.0%, p<.001) for height, in 60-under 70 kg for weight(13.4%, 23.5%, p<.001), and in a case of profession for mother's job(13.8%, 28.4%, p<.005). 2. The perceived oral symptoms had relationship with the less than 10 cigarettes of smoke (OR=2.41; 95% CI:1.31-4.41), more than 11 cigarettes of smoke (OR=3.16; 95% CI:1.42-7.00) and more than 1 bottle of alcohol (OR=1.75; 95% CI:1.00-3.06). Conclusions : This result implies that adolescents' smoking and drinking have correlation with oral health status, which makes uncomfortable sense felt given chewing along with the pain in teeth and gum. Based on the above findings, a school or community needs to reinforce education for preventing smoking and drinking and to offer environment available for practicing anti-smoking and anti-drinking plan, in order to reduce adolescents' smoking and drinking.

A Study on the Historical Background and Deployment Characteristics of Mobile Hospital for Disaster - Focused on Military Mobile Hospitals (재난대응 이동형 병원의 역사적 배경 및 배치 특성 연구 - 군 이동병원을 중심으로)

  • Purpose: The purpose of this study is to examine the deployment types and characteristics of mobile hospitals with it's historical backgrounds. Methods: Since the disaster can not be classified by country, the scope of the study is to include both domestic and foreign mobile hospitals. In order to minimize the casualties from the field hospitals used at the time of the First World War, which is the mother of mobile hospitals, we analyzed the mobile hospitals which are more compact and changed to the target areas and analyzed the mobile hospitals. Results: Historically, mobile hospitals have been transformed in a way that they are close to the target area and rapidly inject essential elements, and the deployment of wards has evolved to be able to combine in any form with center corridor. In the case of hospitals that can deal with infectious diseases, each treatment room was installed separately to thoroughly separate the copper wire. Implications: As disaster damage increases and incidence increases, field response should be quick. However, research on mobile hospitals, which are indispensable for field response in Korea, has not been conducted in various ways. From the origins of mobile hospitals, the development process is reviewed, and research is carried out to clarify the grounds and backgrounds for the planning of mobile hospitals that are quick and appropriate to the situation in Korea and abroad.

Typology of Young Korean Adults' Relationships with their Parents from an Intergenerational Solidarity Lens (청년의 세대관계 유형화: 세대 간 결속의 하위차원을 중심으로)

  • The literature on parent-child relationships in young adulthood lacks a systemic approach that considers the multifaceted nature of intergenerational relationships. In this paper, we identify the latent profiles of young Korean adults' relationships with their parents based on the six dimensions of intergenerational solidarity (structural, associational, affectual, consensual, functional, and normative solidarity) as indicators. We considered solidarity for the mother and the father separately for structural, associational, affectual, and consensual dimensions. In terms of functional and normative solidarity, we measured both upstream and downstream intergenerational support. The sample included 1,015 young adults who were 19-34 years old, never married, not in secondary school, and had both parents living. The latent profile analysis revealed four profiles: (a) independent but intimate (22.7%), (b) coresident, outwardly intimate (32.4%), (c) detached from father (6.7%), and (d) coresident, intimate (38.2%). The factors that predicted each profile included the young adults' education, income, subjective socioeconomic status, and experiencing the features of emerging adulthood along with the parents' marital status, father's employment, and overparenting. The "coresident, intimate" group reported significantly higher levels of life satisfaction and fewer depressive symptoms compared to the other three groups. The lowest levels of psychological adjustment were found in the "coresident, outwardly intimate" group and "detached from father" group.

A study on Functional Status of Women after Childbirth (산후 여성의 기능적 상태에 관한 연구)

  • Recovery of functional status and affecting factors it were examined at 1 and 2 months after delivery in 111 women who defined as the resumption of household, self-care, social & community, and occupational activities, and assumption of infant care responsibilities. The data were collected by Tulman & Fawcett (1988)'s IFSAC questionnaire at 2 local hospitals & health center in Kwangju city & Chonnam province. The results were as follows : 1. Total mean scores of functional status were 2.3 points and increased in total functional status between 1 & 2 months after childbirth. The analysis revealed significant changes in 3 dimensions-household, social & community, and occupational activities-of IFSAC between 1 & 2 months after childbirth. 2. Mean scores of 5 dimensions of IFSAC were : self-care activities, 1.9 points : household activities, 2.7 points ; infant care responsibilities, 3.6 points ; occupational activities, 1.5 points ; and social & community activities, 1.6 points. 3. The job affected significantly the household, occupational activities and infant care responsibilities. And the complication of infant or mother affected significantly the infant care responsibilities in association with recovery functional status. Based on the findings and a review of the literature in regard to our understanding of recovery of functional status, the following recommendations were derived. 1. Future research needs to be policied concerning length of maternity leave after delivery on a firmer basis throught longitudinal study. 2. The data also nay be used to develop individual interventions to facilitate recovery from childbirth. 3. The IFSAC may be used to clinical assessments of functional status in the case of women cancer or obstetric and gynecologic operation.
