Overdenture 하에서 하악응력 및 의치의 변위에 관한 유한요소법적 분석 (FINITE ELEMENT ANALYSIS OF MANDIBULAR STRESSES AND DENTURE MOVEMENTS INDUCED BY OVERDENTURES)

  • The purpose of this study was to analyze the displacement and the magnitude and the mode of distribution of the stresses in the lower overdenture, the mucous membrane, the abutment tooth and the mandibular supporting bone when various denture base materials, such as acrylic resin and 0.5mm metal base, and various denture base designs were subjected to different loading schemes. For this study, the two-dimensional finite element method was used. Mandibular arch models, with only canine remaining, were fabricated. In the first denture base design, a space, approximately 1mm thick, was prepared between the denture and the dome abutment. In the second denture base design, contact between the denture and the dome abutment was eliminated except the contact of the occlusal third of the abutment. In order to represent the same physiological condition as the fixed areas of the mandible under loading schemes, the eight nodes which lie at the mandibular angle region, the coronoid process and the mandibular condyle were assumed to be fixed. Each model was loaded with a magnitude of 10 kgs on the first molar region(P1) and 7 kgs on the central incisal region (P2) in a vertical direction. Then the force of 10 kgs was applied distributively from the first premolar to the second molar of each model in a vertical direction(P3). The results were as follows. : 1. When the testing vertical loads were given to the selected points of the overdenture, the overdenture showed the rotatory phenomenon, as well as sinking and the displacements of alveolar ridge, abutment and lower border of mandible under the metal base overdenture were less than those under the acrylic resin overdenture. 2. The maximum principal stresses(the maximum tensile stresses) being considered, high tensile stresses occured at the buccal shelf area, the posterior region of the ridge crest and the anterior border region of the mandibular ramus. 3. The minimum principal stresses(the maximum compressive stresses) being considered, high compressive stresses occured at the inferior and posterior border region of the mandible, the mandibular angle and the posterior border region of the mandibular ramus. 4. The vertical load on the central incisal region(P2) produced higher equivalent stress in the mandible than that on any other region(P1, P3) because of the long lever arm distance from the fixed points to the loading point. 5. Higher equivalent stresses were distributed throughout the metal base overdenture than the resin base overdenture under the same loading condition. 6. The case of occlusal third contact of the abutment to the denture produced higher equivalent stresses in the abutment, the mandibular area around the abutment and the overdenture than the case of a 1mm space between the denture and the abutment. 7. Without regard to overdenture base materials and designs, the amounts and distribution patterns of equivalent stresses under the same loading condition were similar in the mucous membrane.

Preliminary Studies on Double-Diffusive Natural Convection During Physical Vapor Transport Crystal Growth of Hg2Br2 for the Spaceflight Experiments

  • We have conducted a preliminary numerical analysis to understand the effects of double-diffusive convection on the molar flux at the crystal region during the growth of mercurous bromide ($Hg_2Br_2$) crystals in 1 g and microgravity (${\mu}g$) conditions. It was found that the total molar fluxes decay first-order exponentially with the aspect ratio (AR, transport length-to-width), $1{\leq}AR{\leq}10$. With increasing the aspect ratio of the horizontal enclosure from AR = 1 up to Ar = 10, the convection flow field shifts to the advective-diffusion mode and the flow structures become stable. Therefore, altering the aspect ratio of the enclosure allows one to control the effect of the double diffusive natural convection. Moreover, microgravity environments less than $10^{-2}g$ make the effect of double-diffusive natural convection much reduced so that the convection mode could be switched over the advective-diffusion mode.

부적절하게 수복된 stainless steel crown에 의해 야기된 하악 제1대구치 이소맹출의 치험례 (TREATMENT OF ECTOPIC ERUPTING MANDIBULAR FIRST PERMANENT MOLAR CAUSED BY IMPROPERLY RESTORED STAINLESS STEEL CROWN : CASE REPORT)

  • 이소맹출이란 영구치열의 발육 과정중에 국소적 맹출장애로 인하여 정상적 위치에서 벗어난 치아의 맹출을 말한다. 일반적으로 이소맹출은 $2\sim6%$정도의 이환율을 보이고, 제1대구치에서 호발하며 주로 상악에서 많이 나타나고 하악에서는 드물다. 이소맹출의 원인은 국소적 원인으로 부적절한 치열궁 길이, 상악 후방부의 성장 부족, 제1대구치의 근심 맹출, 비정상적으로 큰 제 1대구치 등이 있고, 유전적인 원민으로 나타날 수도 있으며, 부적절하게 형성된 stainless steel crown에 의해서도 유발될 수 있다. 이소맹출된 치아중 66%는 가역성이므로 $3\sim6$개월 정도의 관찰 기간이 요구된다고 하였으며, 비가역성 이소맹출인 경우에는 brass wire나 elastic separator를 이용한 비교적 간단한 치료 방법, 제2유구치의 distal discing, Humphrey appliance 등과 같이 제2유구치를 유지하면서 치료하는 방법 및 제2유구치 발거후 가철식 장치나 cervical traction headgear를 이용하는 방법 등이 소개되고 있다. 본 증례는 연세대학교 치과대학병원 소아치과에 내원한 7세된 남환아로 구강 검사 및 방사선 사진 검사 결과, 4년전 개인 치과의원에서 수복된 우측 하악 제2유구치의 stainless steel crown에 우측 하악 제 1대구치가 고착되어 근심으로 심하게 경사된 양상을 보였다 원인이 stainless steel crown을 제거하고, Humphrey appliance를 제작하여 장착하였으며, 약 10주 후에 고착 상태가 해소되었다. 본 증례에서와 같이 이소맹출은 부적절하게 형성된 stainless steel crown에 의해 일어날 수 있으므로, stainless steel crown의 수복 치료에 있어서, 적절한 크기의 선택, trimming 및 contouring 등이 매우 중요하다.

악골의 섬유성골병소에 관한 X선학적 연구 (A RADIOGRAPHIC STUDY OF FIBRO-OSSEOUS LESIONS IN THE JAWS)

  • This study was undertaken to document and better defined this condition to help clarify this clinical and radiographical appearances by the analysis of clinical and radiographical features of fibro-osseous lesions in the jaws. A study was made of a series of 128 cases with fibro-osseous lesions. The obtained results were as follows. 1. Fibrous dysplasia of the jaws occurred with equal predilection for males and females. But the females occurred in 68% of cemento-ossifying fibroma and 75% of periapical cemental dysplasia. 2. 43% of fibrous dysplasia and 32% of cemento-ossifying fibroma occurred in the 2nd decades and 33% of periapical cemental dysplasia in 5th decades. 3. 62% of fibrous dysplasia occurred in the maxilla, 73% of cemento-ossifying fibroma in mandible, 90% of periapical cemental dysplasia in mandible. 4. 98% of fibrous dysplasia occurred in premolar-molar region, 77% of cemento-ossifying fibroma in molar region, 68% of periapical cemental dysplasia in incisor region. 5. In serial radiographic features, mature stage were 55% of fibrous dysplasia, 45% of cemento-ossifying fibroma, 59% of periapical cemental dysplasia. 6. 87% of fibrous dysplasia had monostotic lesion, 67% of periapical cemental dysplasia had multiple lesions. 7. In fibrous dysplasia and cemento-ossifying fibroma, migration of tooth occurred in 61.7% and 36.4%, retention of tooth occurred in 4.3% and 9.1%, loss of lamina dura occurred in 6.4% and 9.1%, and root resorption had not occurred in fibrous dysplasia, but occurred in 18% of cemento-ossifying fibroma, displacement of mandibular canal occurred in 14.9% and 31.8%.

Proteome analysis of developing mice diastema region

  • Different from humans, who have a continuous dentition of teeth, mice have only three molars and one incisor separated by a toothless region called the diastema in the hemi mandibular arch. Although tooth buds form in the embryonic diastema, they regress and do not develop into teeth. In this study, we evaluated the proteins that modulate the diastema formation through comparative analysis with molar-forming tissue by liquid chromatography-tandem mass spectroscopy (LC-MS/MS) proteome analysis. From the comparative and semi-quantitative proteome analysis, we identified 147 up- and 173 down-regulated proteins in the diastema compared to the molar forming proteins. Based on this proteome analysis, we selected and evaluated two candidate proteins, EMERIN and RAB7A, as diastema tissue specific markers. This study provides the first list of proteins that were detected in the mouse embryonic diastema region, which will be useful to understand the mechanisms of tooth development.

X-선이 치배조직에 미치는 영향에 관한 연구 (EXPERIMENTAL STUDY ON THE EFFECT OF X-RAY IRRADIATION ON THE TOOTH GERM OF THE RAT)

  • The author observed the effect of x-ray irradiation on the tooth germ development of the rat fetuses. The lower right abdomen of the pregnant rats were exposed to x-ray irradiation (400 rads) on 9½th day of qestation. At 18½th day of qestation, the fetuses were removed from their mothers and histological sections of molar region were prepared. The results were as folows: 1. In the experimental fetuses, no significant changes appeared in the histological aspects of the enamel pulp, except the poor development of the innerenamel epithelium in the cusp region. 2. Pulp cells of cusp region in the irradiated fetuses were not differentiated to odontoblasts, The arrangement and population of pulp cells showed marked regional differences in the dental papilla. 3. Developmental features of dental follicle of irradiated fetuses were similar with controls.

Comparison of transverse dental changes induced by the palatally applied Frog appliance and buccally applied Karad's integrated distalizing system

  • Objective: To compare the transverse dental changes induced by the palatally applied Frog appliance and buccally applied Karad's integrated distalizing system (KIDS). Methods: We evaluated the pre- and post distalization orthodontic models of 39 patients, including 19 treated using the Frog appliance, which is palatally positioned (Frog group), and 20 treated using KIDS, which is buccally positioned (KIDS group). Changes in intermolar and interpremolar distances and the amount of maxillary premolar and molar rotation were evaluated on model photocopies. Wilcoxon and Mann-Whitney U tests were used for statistical evaluations. A p-value of < 0.05 was considered statistically significant. Results: Significant distopalatal rotation of premolars and distobuccal rotation of molars were observed in Frog group (p < 0.05), while significant distopalatal rotation of molars (p < 0.05), with no significant changes in premolars, was observed in KIDS group. The amount of second premolar and first molar rotation was significantly different between the two groups (p < 0.05 and p < 0.001, respectively). Furthermore, expansion in the region of the first molars and second premolars was significantly greater in KIDS group than in Frog group (p < 0.001 for both). Conclusions: Our results suggest that the type and amount of first molar rotation and expansion vary with the design of the distalization appliance used.

Anatomical position of the mandibular canal in relation to the buccal cortical bone: relevance to sagittal split osteotomy

  • Objectives: Classification of the degree of postoperative nerve damage according to contact with the mandibular canal and buccal cortical bone has been studied, but there is a lack of research on the difference in postoperative courses according to contact with buccal cortical bone. In this study, we divided patients into groups according to contact between the mandibular canal and the buccal cortical bone, and we compared the position of the mandibular canal in the second and first molar areas. Materials and Methods: Class III patients who visited the Dankook University Dental Hospital were included in this study. The following measurements were made at the second and first molar positions: (1) length between the outer margin of the mandibular canal and the buccal cortical margin (a); (2) mandibular thickness at the same level (b); (3) Buccolingual $ratio=(a)/(b){\times}100$; and (4) length between the inferior margin of the mandibular canal and the inferior cortical margin. Results: The distances from the canal to the buccal bone and from the canal to the inferior bone and mandibular thickness were significantly larger in Group II than in Group I. The buccolingual ratio of the canal was larger in Group II in the second molar region. Conclusion: If mandibular canal is in contact with the buccal cortical bone, the canal will run closer to the buccal bone and the inferior border of the mandible in the second and first molar regions.

매복 정도에 따른 대구치 이소맹출의 치험례 (MANAGEMENT OF ECTOPICALLY ERUPTING PERMANENT MOLARS BY THE DEGREE OF IMPACTION)

  • 이소맹출은 치아가 비정상적인 위치나 방향으로 맹출하는 것으로 정의되며 구치부에서는 근심측으로 맹출하여 인접치아의 치근 흡수나 조기 탈락을 야기하여 공간 상실을 일으킬 수 있고, 부분 맹출된 경우 치은 하방의 치아우식 위험성이 증가하고 농양을 형성할 수도 있다. 구치부의 이소맹출 가운데 66% 정도는 가역적인 경우로 특별한 치료 없이 자연적으로 정상적인 맹출이 일어나며, 비가역적인 이소맹출의 경우에는 치아의 맹출 상태, 인접치에 의해 매복된 양, 인접치의 동요도, 통증이나 감염 여부 등의 여러 요인을 고려하여 본격적인 치료가 필요하다. 본 논문에서는 비가역적으로 이소맹출된 상악 제1대구치와 하악 제2대구치를 치간이개 고무줄, modified Halterman appliance 그리고 외과적 노출 후 modified Halterman appliance를 이용한 방법 등으로 정상적인 맹출을 유도한 증례를 보고 하는 바이다.

Histological analysis on tissues around orthodontically intruded maxillary molars using temporary anchorage devices: A case report

  • Before progress was recently made in the application of temporary anchorage devices (TADs) in bio-mechanical design, orthodontists were rarely able to intrude molars to reduce upper posterior dental height (UPDH). However, TADs are now widely used to intrude molars to flatten the occlusal plane or induce counterclockwise rotation of the mandible. Previous studies involving clinical or animal histological evaluation on changes in periodontal conditions after molar intrusion have been reported, however, studies involving human histology are scarce. This case was a Class I malocclusion with a high mandibular plane angle. Upper molar intrusion with TADs was performed to reduce UPDH, which led to counterclockwise rotation of the mandible. After 5 months of upper molar intrusion, shortened clinical crowns were noticed, which caused difficulties in oral hygiene and hindered orthodontic tooth movement. The mid-treatment cone-beam computed tomography revealed redundant bone physically interfering with buccal attachment and osseous resective surgeries were followed. During the surgeries, bilateral mini screws were removed and bulging alveolar bone and gingiva were harvested for biopsy. Histological examination revealed bacterial colonies at the bottom of the sulcus. Infiltration of chronic inflammatory cells underneath the non-keratinized sulcular epithelium was noted, with abundant capillaries being filled with red blood cells. Proximal alveolar bone facing the bottom of the gingival sulcus exhibited active bone remodeling and woven bone formation with plump osteocytes in the lacunae. On the other hand, buccal alveolar bone exhibited lamination, indicating slow bone turnover in the lateral region.
