흡연 및 비타민 D 수준과 한국 성인 남녀 대사증후군의 연관성 (Association between Smoking Status, Vitamin D Levels and the Korean Adult Male and Female Metabolic Syndrome)

  • Purpose : Several studies suggest that smoking and vitamin D level is a risk factor of metabolic syndrome. The objective of this study is to evaluate the association between smoking status, vitamin D levels and the Korean adult male and female metabolic syndrome. Methods : We assessed 3796 participants aged 19 years and older from the Korean National Health and Nutritional Examination Survey 2013, 2014. Smoking statuses were collected from self-reported questionnaires. Subjects were divided into three categories: non-smokers, former smokers, and current smokers. Vitamin D was tested by Radioimmunoassay method and the value of serum 25-hydroxyvitamin D, which is an index of vitamin D status in the body, was used. The diagnosis of metabolic syndrome was made using criteria modified NCEP-ATP III. Logistic regression analysis was used to calculate odds ratios between smoking status, vitamin D levels, and metabolic syndrome. Results : The overall prevalence of metabolic syndrome was 23.0 % in men and 15.4 % in women. After adjusting for smoking, the odds ratio for men's metabolic syndrome in current smokers was 1.77 (95 % CI, 1.30~2.41), while for former smokers OR was 1.63 (95 % CI, 1.15~2.31) compared with nonsmokers. After adjustment vitamin D, the odds ratio for women's metabolic syndrome in vitamin D deficiency was 1.44 (95 % CI, 1.11~1.87) compared with normal. Conclusion : Smoking status was associated with an increased risk of metabolic syndrome in Korean adult males and decreased vitamin D level was associated with an increased risk of metabolic syndrome in Korean adult females.

한국여성의 폐경에 따른 대사증후군과 영양섭취에 대한 단면 연구 (A Cross-sectional Study of Nutrient Intake for Korean Women with Menopause and Metabolic Syndrome)

  • Menopausal status and hormonal changes are important factors related to the prevalence of metabolic syndrome in women. Nutrient intake is also a risk factor for metabolic syndrome. Although, postmenopausal status and hormonal changes result in a 60% increased risk for metabolic syndrome, there has been no method to elucidate the effects of nutrient intake on metabolic syndrome following menopause. This study was conducted to evaluate the effects of nutrient intake by menopausal women on the prevalence of metabolic syndrome. All data were obtained from the Korean National Health and Nutrition Examination Survey 2007-2009. Menopausal women showed a 1.8-fold increase in the prevalence of metabolic syndrome. Metabolic syndrome group showed significantly lower values in calcium, iron, vitamin A, carotin, retinol and riboflavin intake than those of normal group in premenopausal women. In postmenopausal women, there are significant differences in crude fiber, calcium, iron, potassium, riboflavin and niacin. Indeed, different patterns of nutrient intake were observed by menopausal status and metabolic syndrome. As menopause cannot be controlled, a diet with adequate nutrient intake may be useful to control the rapid increase in the prevalence of metabolic syndrome due to menopause.

Age at Menarche and Brachial-ankle Pulse Wave Velocity in Women with Metabolic Syndrome

  • Early age at menarche, which is indicator of early biological maturity, has been shown to be associated with increased adult body mass index. Early menarche has also been associated with many cardiovascular disease risk factors and metabolic syndrome. To evaluate the impact of menarche to cardiovascular risk factor, we assessed by age at menarche, brachial-ankle pulse wave velocity (baPWV), which represents arterial stiffness, in women with or without metabolic syndrome. The subjects recruited for this study were three hundred one women. Relatively early menarche and relatively late menarche were classified according to less than $50^{th}$ percentile for relatively early menarche, and great than the $50^{th}$ percentile for relatively late menarche. Subject were divided four group, 1) women who had not adulthood metabolic syndrome and relatively early menarche, 2) women who had not adulthood metabolic syndrome and relatively late menarche, 3) women who had adulthood metabolic syndrome and relatively early menarche, 4) women who had adulthood metabolic syndrome and relatively late menarche. Women who had a relatively early menarche with adulthood metabolic syndrome had significantly high levels of blood pressure, triglyceride, fasting insulin and homeostatic model assessment of insulin resistance (HOMA-IR) levels than women with late menarche with adulthood metabolic syndrome, and had significantly lower HDL-cholesterol levels. And also, women who underwent a relatively early menarche with metabolic syndrome had highest level of baPWV in adult. In this study we found effect of age at menarche on adulthood metabolic risk factors for cardiovascular disease (e.g., baPWV, insulin resistance, hyperlipidemia) in Korean women.

복합운동 프로그램이 만성정신질환자의 비만 및 대사 증후군 인자에 미치는 효과 (The Effects of a Combined Exercise Program on Obesity and Metabolic Syndrome Factors for Chronic Psychiatric Inpatients)

  • Purpose: The purpose of this study was to examine the effects of a Combined Exercise Program on Obesity and Metabolic Syndrome Factors for Chronic Psychiatric Inpatients. Methods: Thirty two subjects participated in this program who were admitted into a national mental hospital of C city. All measurements were done at baseline, 4 week, 8 week, and 12 week during the program. The effectiveness of the program was evaluated according to the change of body mass index, body fat, waist circumference, total cholesterol, high density lipoprotein cholesterol, triglyceride, and fasting blood sugar. Collected data was analyzed by frequency, percentage, and repeated measures ANOVA with SPSS WIN 19.0. Results: There were statistically significant differences in the body mass index (p=.002), body fat (p<.001), waist circumference (p<.001), triglyceride (p=.020), and fasting blood sugar (p=.008). However, there were no statistically significant differences in the total cholesterol, or high density lipoprotein cholesterol. Conclusion: The results of this study showed that a combined exercise program has positive effect on obesity and metabolic syndrome factor. Thus, a Combined Exercise program is recommended as an intervention to improve obesity and metabolic syndrome factors for chronic psychiatric inpatients.

Association between Metabolic Syndrome and Physical Fitness in Postmenopausal Women

  • The research is performed in order to know the relationship between the metabolic syndrome and the physical fitness targeted for menopausal women (over 45 years). All subjects were divided into 4 groups; group without risk factors of metabolic syndrome (MS-0: n=74), group having one risk factor of metabolic syndrome (MS-1: n=68), group having two risk factors of metabolic syndrome (MS-2: n=44), and group having more than three risk factors of metabolic syndrome (MS ${\geq}$ 3: n=30). All groups' height, weight, body mass index and percent of body fat were measured. High density lipoprotein cholesterol (HDL), triglyceride, glucose and blood pressure (BP) levels were measured. Their cardiorespiratory ($VO_2max$) endurance, muscular strength, muscle endurance, and flexibility were measured. HDL, triglyceride, glucose and BP levels in MS-1, MS-2, and MS ${\geq}$ 3 group were significantly greater than those of MS-0 group. The endurance ($VO_2max$) in MS ${\geq}$ 3 group was higher than that of MS-0 group. Multiple regression with the risk factors of metabolic syndrome and the physical fitness showed a statistical significance in only $VO_2max$. We found that the risk factors of the metabolic syndrome adversely affect postmenopausal women's $VO_2max$ and that a decreased $VO_2max$ may have prognostic value for the prediction of metabolic syndrome.

  • Purpose: Metabolic syndrome is known as a factor that increases the incidence of chronic diseases, such as diabetes and cardiovascular disease. In particular, the metabolic syndrome among a middle-aged population is rapidly increasing from 15.6% to 31.9%. The purpose of this study was to investigate the effect of muscle strength exercise on the metabolic syndrome in middle aged. Methods: This study was a secondary data analysis using National Health and Nutrition Survey 8th, including 2,739 middle aged people (40~64 years old). We used multivariate logistic regression to identify risk factors associated with metabolic syndrome. Statistical analysis was performed using the SPSS 23.0 program. Results: There were 772 patients in the group with metabolic syndrome and 1,967 patients in the non-metabolic syndrome group. The risk of metabolic syndrome was 1.29 times higher in those who did not do muscle strength exercise than those who did exercise (OR=1.29, 95% CI=1.01~1.66). Conclusion: We have found that muscle strength exercise was effective in lowering the risk of developing metabolic syndrome in middle aged. Thus, it is necessary to develop practical muscle strength exercise and education programs.

청소년의 비타민 D 결핍과 대사증후군 관련요인 분석: 2010년 제5기 1차 국민건강영양조사를 기반으로 (Vitamin D deficiency and Metabolic Syndrome among Korean Adolescents: Based on Korea National Health and Nutrition Examination Survey V (KNHANES))

  • Purpose: This study is intended to analyze factors of metabolic syndrome and vitamin D deficiency with resources obtained from the Korea National Health and Nutrition Examination Survey (KNHANES) V conducted in 2010. Methods: The subjects were 870 adolescents aged from 10 to 18 who participated in the first KNHANES V. Secondary data analysis was done by Chi-square, ANOVA, correlation analysis and simple linear regression. Results: The influencing factors of metabolic syndrome were vitamin D, systolic blood pressure and diastolic blood pressure, which were statistically significant. Simple linear regression was conducted to identify the influence of vitamin D on the factors related to metabolic syndrome, which showed that waist size (t=-1.97, p<.05), systolic blood pressure (t=-2.88, p<.01), and diastolic blood pressure (t=-5.72, p<.001) were influenced by vitamin D. In addition, each factor decreased by .131, .209, and .355, respectively, when vitamin D increased by one unit. Conclusion: Metabolic syndrome is a factor to be considered in order to improve adolescent health and form adolescent health behaviors. Schools, families, and local communities should cooperate with each other on the foundation of a regular health screening process and systematic health education programs. In addition, it is expected enhanced physical activities and sports club participation can prevent metabolic syndrome and increase vitamin D among adolescent.

Prevalence of Metabolic Syndrome according to Menopausal Status: The 5th Korea National Health & Nutrition Examination Survey

  • The purpose of this study was to investigate the prevalence and differences of risk factors of Metabolic syndrome according to menopausal status. From the database of the 5th Korea National Health and Nutrition Examination Survey (KNHANES V) conducted in 2012, data of 3,537 women who responded to health interview and health examination questionnaires. The presence of Metabolic syndrome was assessed using the International Diabetes Federation (IDF) criteria. The prevalence of Metabolic syndrome was 17.3% in premenopause and 54.3% in postmenopause. BMI, waist circumference, blood pressure, total cholesterol, LDL cholesterol, triglyceride, and Ferritin were higher in postmenopausal women compared to premenopausal women. However, postmenopausal women had lower HDL cholesterol and TIBC when compared to premenopausal women. Hypertension was the most prominent characteristic of postmenopausal women. Low HDL cholesterol showed up as a meaningful factor in premenopausal women. According to multiple regression analysis, waist circumference, fasting glucose, and HDL cholesterol were independent predictors of menopause. Therefore, obesity control and blood lipids management is recommended for postmenopausal women.

Effects of Sarcopenic Obesity on Metabolic Syndrome in Korean Elders: Using Data from the Korea National Health and Nutrition Examination Survey (2008-2011)

  • Purpose: This study was conducted to examine effects of sarcopenic obesity on metabolic syndrome in Korean elders. Methods: This study is based on the analysis of the Korea National Health and Nutrition Examination Survey (KNHANES) with 1,155 subjects (524 men, and 631 women) aged 60 or older, from 2008 to 2011. Sarcopenia was defined as an appendicular skeletal muscle (ASM), divided by weight (%) of <1 SD (standard deviation) below the sex-specific mean for young adults. Obesity was defined as a total body fat percent (men${\geq}$25%, women${\geq}$35%). Results: The prevalence of SO (sarcopenic obesity) was 13.3% among men and 22.5% among women. Both sexes showed a higher total body fat percent, and the SMI (skeletal muscle index) was the lowest in the SO group. Metabolic syndrome was highly prevalent in the SO group (52.5% men, 60.4% women). The SO group showed a higher risk for metabolic syndrome (odds ratio men 6.57 [95% CI 5.19~7.27], women 3.89 [95% CI 2.41~6.29]) than the obese group (men 3.14 [95% CI 1.76~4.14], women 2.54 [95% CI 1.38~4.65]). Conclusion: SO is a major risk factor for metabolic syndrome in Korean elders. Therefore, a nursing program should be given to the Korean elderly SO group to prevent metabolic syndrome.

피하지방 및 피하지방의 분획과 관련된 임상적 의의에 관한 고찰 (The Review on the Subdivision of Subcutaneous Adipose Tissue)

  • Objectives In clinical studies, the visceral fat obesity has been emphasized because of its correlation with the metabolic syndrome. But the subcutaneous adipose tissue also would correlate with the risk factor of metabolic syndrome. Especially deep tissue, which is a subdivision of the subcutaneous adipose tissue would be more related. This study is to investigate the relationship between subcutaneous adipose tissue and various diseases. Methods We searched for papers which had subcutaneous adipose tissue, deep subcutaneous adipose tissue and obesity for subjects in the Pubmed site. Results : 24 papers were found. Subcutaneous adipose tissue, deep subcutaneous adipose tissue especially, was related with the insulin resistance, metabolic syndrome, sex hormones and other diseases. Conclusions Subcutaneous adipose tissue is a risk factor of insulin resistance but not lipoprotein. But deep subcutaneous adipose tissue was related with lipoprotein. So deep tissue, which is a subdivision of the subcutaneous adipose tissue is a more important risk factor of the metabolic syndrome.
