대사증후군 환자에서 영양소 섭취 상태와 우울증과의 연관성: 2014년 국민건강영양조사 자료를 이용하여 (Relationship between Dietary Intake and Depression in Metabolic Syndroem among Korean Adults: Korea National Health and Nutrition Examination Survey 2014)

  • 우울증과 대사증후군은 상호 연관성이 있으며, 영양소 섭취 상태가 두 질환의 발생과 예방에 영향을 미침에도 불구하고 대사증후군 환자를 대상으로 한 우울증 환자의 영양소 섭취 상태에 대한 연구가 국내는 물론 국외에서도 적은 편이다. 이에 본 연구는 국민건강영양조사에 참여한 대사증후군 환자를 대상으로 우울증과 영양소 섭취 상태의 연관성을 알아보고자 하였다. 본 연구는 국민건강영양조사 자료 중 제 6기 2차 (2014년) 자료를 분석하였으며, 만 20세에서 60세 사이의 대사증후군 환자 1.471명 중 의사에 의한 우울증 진단여부가 확인이 되는 1,334명을 최종 대상자로 하여 이들의 영양소 섭취 상태와 우울증과의 연관성을 분석하였다. 대사증후군 환자를 남녀로 구분하여 우울증 군과 정상 군의 영양소 섭취 상태를 비교하였을 때, 니아신 섭취량이 남성에서 각각 $14.5{\pm}6.7mg$$17.8{\pm}9.7mg$으로 우울증 군이 유의하게(P=0.047) 낮았고, 여성에서도 각각 $11.4{\pm}5.4mg$$13.1{\pm}8.0mg$으로 우울증 군이 유의하게(P=0.025) 낮았다. 결론적으로 본 연구에서는 대사증후군이 있는 성인에서 우울증이 니아신 섭취량과 관련이 있음을 확인할 수 있었다. 이는 대사증후군 환자에서 우울증에 대한 영양소 섭취 상태 평가가 의미가 있음을 시사하며, 또한 영양학적 중재가 대사증후군 환자의 우울증 유병을 감소시킬 수 있을 것으로 생각된다.

골관절염과 대사증후군과의 상관관계: 2009-2010년 국민건강영양조사 이용 (Association of Osteoarthritis with prevalence of Metabolic syndrome: based on 2009-2010 Korean National Health & Nutrition Survey)

  • 본 연구에서는 한국인의 대표성이 있는 자료인 국민건강영양조사 자료를 이용하여 대사증후군 및 그 구성요소들과 골 관절염의 상관관계에 관하여 조사하였다. 여성의 경우, 골 관절염군에서 대사증후군의 유병률이 증가하였으며, 여성에서 대사증후군의 구성요소들, 예를 들어 허리둘레, 중성지질, 수축기 혈압, HDL-콜레스테롤과 골 관절염의 유병률은 통계적으로 유의한 상관관계가 있음이 증명되었다. 또한 생활습관변수 및 사회경제적 변수로 보정하였을 때 대사증후군의 위험요소의 수가 증가할수록 골 관절염의 위험도는 더욱 커지는 경향을 보였다 (p<0.0001). 따라서 이번 연구에서 제시되어진 바와 같이 대사증후군은 골 관절염의 새로운 위험인자로서 인식되어야 하며 골 관절염의 예방과 치료 시 대사증후군 역시 고려되어야 할 요소의 하나로 사료된다.

Daily walnut intake improves metabolic syndrome status and increases circulating adiponectin levels: randomized controlled crossover trial

  • BACKGROUND/OBJECTIVES: Several previous studies have investigated whether regular walnut consumption positively changes heart-health-related parameters. The aim of this study was to investigate the effects of daily walnut intake on metabolic syndrome (MetS) status and other metabolic parameters among subjects with MetS. SUBJECTS/METHODS: This study was a two-arm, randomized, controlled crossover study with 16 weeks of each intervention (45 g of walnuts or iso-caloric white bread) with a 6 week washout period between interventions. Korean adults with MetS (n = 119) were randomly assigned to one of two sequences; 84 subjects completed the trial. At each clinic visit (at 0, 16, 22, and 38 weeks), MetS components, metabolic parameters including lipid profile, hemoglobin A1c (HbA1c), adiponectin, leptin, and apolipoprotein B, as well as anthropometric and bioimpedance data were obtained. RESULTS: Daily walnut consumption for 16 weeks improved MetS status, resulting in 28.6%-52.8% reversion rates for individual MetS components and 51.2% of participants with MetS at baseline reverted to a normal status after the walnut intervention. Significant improvements after walnut intake, compared to control intervention, in high-density lipoprotein cholesterol (HDL-C) (P = 0.028), fasting glucose (P = 0.013), HbA1c (P = 0.021), and adiponectin (P = 0.019) were observed after adjustment for gender, age, body mass index, and sequence using a linear mixed model. CONCLUSION: A dietary supplement of 45 g of walnuts for 16 weeks favorably changed MetS status by increasing the concentration of HDL-C and decreasing fasting glucose level. Furthermore, consuming walnuts on a daily basis changed HbA1c and circulating adiponectin levels among the subjects with MetS. This trial is registered at ClinicalTrials.gov as NCT03267901.

성인남성에서 현재 및 과거 흡연과 대사증후군의 관련성 (Association of Current and Past Smoking with Metabolic Syndrome in Men)

  • Objectives : The objective of this study was to determine the relationship between past smoking and the risk factors for metabolic syndrome. Methods : From January 2007 to December 2007, a total of 3,916 over thirty years old male health screen examinees were divided into the nonsmoking, smoking, ex-smoking groups. The diagnosis of metabolic syndrome was based on the criteria of the NCEP ATP III(Executive Summary of The Third Report of The National Cholesterol Education Program). Metabolic syndrome was defined as the presence of three or more of the following: a blood pressure $\geq$ 130/85 mmHg, a fasting glucose level $\geq$ 110 mg/dL, a HDL-C (High Density Lipoprotein Cholesterol) level < 40 mg/dL, a triglyceride level $\geq$ 150 mg/dL and, a waist circumference men $\geq$ 102 cm, but a waist to hip ratio > 0.90 was used as a surrogate for the waist circumference. Results : After adjustment for age, alcohol consumption and, exercise in the smokers, for the ex-smokers compared with the nonsmokers, the odds ratio (OR) of a lower HDL cholesterol level (<40 mg/dL) was 1.29 (95% CI=1.03-1.61) in the smokers, the ORs of a higher triglyceride level were 1.35 (95% CI=1.09-1.66) in the ex-smokers and, 2.12 (95% CI=1.75-2.57) in the smokers, and the OR of a waist to hip ratio was 1.25 (95% CI=1.03-1.52) in the ex-smokers. When there were over three components of metabolic syndrome in the ex-smokers and smokers as compared with the nonsmokers, the odds ratio against the risk of metabolic syndrome were 2.39 (95% CI=1.00-6.63) and 2.37 (95% CI=1.02-6.46), respectively. Conclusions : The present study suggested that there is an association of smoking with metabolic syndrome in men.

건강한 성인 남성의 대사증후군 위험 예측인자로서 혈청 요산/크레아티닌 비율 (Serum Uric Acid to Creatinine Ratio as a Predictor of Metabolic Syndrome in Healthy Adults Men)

  • 이 연구는 대사증후군 위험 예측인자로서 요산과 비교하여 혈청 요산/크레아티닌 비율의 유용성을 확인하고자 하였다. 2016년 11월부터 2018년 10월까지 수도권 소재의 일개 종합병원에서 종합건강검진을 받은 20세 이상 80세 미만의 성인 남성 14,190명을 대상으로 하였다. 대사증후군 진단기준은 American Heart Association/National Heart Lung and Blood Institute (AHA/NHLBI)의 기준에 따라 5가지 대사증후군 구성요소 중 3가지 이상 해당되는 경우 대사증후군을 평가하였으며, 이 중 복부비만은 WHO 서태평양지역의 기준에 따랐다. 이 연구결과 혈청 요산/크레아티닌 비율은 1사분위수보다 4사분위수의 대사증후군 및 대사증후군 구성요소의 높은 발생빈도를 보였다. 그러나 ROC 분석 결과 혈청 요산/크레아티닌 비율은 대사증후군 위험 예측에 있어 혈청 요산치보다 더 유용한 지표는 아니었으며(AUC, 0.554 vs 0.566), 요산치보다 낮은 민감도와 높은 특이도를 보였다. 결론적으로 혈청 요산/크레아티닌 비율의 유용성을 확인한 결과 대사증후군 위험을 예측하는 독립적인 지표로 사용하는데 제한이 있으며, 보조적인 표지자로 사용하는 것이 바람직하겠다.

Cardiometabolic Index, Triglyceride-glucose Index를 이용한 대사증후군 진단 예측지수에 대한 고찰: 부산지역 건강검진대상자 중심으로 (Consideration of Predictive Indices for Metabolic Syndrome Diagnosis Using Cardiometabolic Index and Triglyceride-glucose Index: Focusing on Those Subject to Health Checkups in the Busan Area)

  • This study investigates the utility of the Triglyceride-glucose(TyG) index and Cardiometabolic Index(CMI) as predictors for diagnosing metabolic syndrome. The study involved 1970 males, 1459 females, totaling 3429 participants who underwent health checkups at P Hospital in Busan between January 2023 and June 2023. Metabolic syndrome diagnosis was based on the presence of 3 or more risk factors out of the 5 criteria outlined by the American Heart Association/National Heart, Lung, and Blood Institute(AHA/NHLBI), and participants with 2 or fewer risk factors were categorized as normal. Statistical analyses included independent sample t-tests, chi-square tests, Pearson's correlation analysis, Receiver Operating Characteristic(ROC) curve analysis, and logistic regression analysis, using the Statistical Package for the Social Sciences(SPSS) program. Significance was established at p<0.05. The comparison revealed that the metabolic syndrome group exhibited attributes such as advanced age, male gender, elevated systolic and diastolic blood pressures, high blood sugar, elevated triglycerides, reduced LDL-C, elevated HDL-C, higher Cardiometabolic Index, Triglyceride-glucose index, and components linked to abdominal obesity. Pearson correlation analysis showed strong positive correlations between waist circumference/height ratio, waist circumference, Cardiometabolic Index, and triglycerides. Weak positive correlations were observed between LDL-C, body mass index, and Cardiometabolic index, while a strong negative correlation was found between Cardiometabolic Index and HDL-C. ROC analysis indicated that the Cardiometabolic Index(CMI), Triglyceride-glucose(TyG) index, and waist circumference demonstrated the highest Area Under the Curve(AUC) values, indicating their efficacy in diagnosing metabolic syndrome. Optimal cut-off values were determined as >1.34, >8.86, and >84.5 for the Cardiometabolic Index, Triglyceride-glucose index, and waist circumference, respectively. Logistic regression analysis revealed significant differences for age(p=0.037), waist circumference(p<0.001), systolic blood pressure(p<0.001), triglycerides(p<0.001), LDL-C(p=0.028), fasting blood sugar(p<0.001), Cardiometabolic Index(p<0.001), and Triglyceride-glucose index (p<0.001). The odds ratios for these variables were 1.015, 1.179, 1.090, 3.03, and 69.16, respectively. In conclusion, the Cardiometabolic Index and Triglyceride-glucose index are robust predictive indicators closely associated with metabolic syndrome diagnosis, and waist circumference is identified as an excellent predictor. Integrating these variables into clinical practice holds the potential for enhancing early diagnosis and prevention of metabolic syndrome.

성별에 따른 습관성 코골이와 대사증후군 요소의 관련성 (Sex Differences in the Association of Habitual Snoring with Metabolic Syndrome)

  • 본 연구는 한국인 성인을 대상으로 습관성 코골이와 대사증후군 구성 요소들과의 관련성에 대해 알아보고 또한 성별에 따른 차이점을 알아보고자 연구를 시행하였다. 2012년 3월부터 2012년 12월까지 일개 대학병원에 내원한 40세 이상 69세 이하까지 성인들 중 연구에 참여하기로 동의한 2,007명(남자 434명, 여자 1,573명)을 대상으로 하였다. 연구 대상자들에 대하여 구조화된 설문지를 작성하였고 신체 계측, 생활 습관 및 임상검사결과를 비교분석하였다. 연구 조사대상자들의 대사증후군의 유병률은 17.4% (남성 20.7%, 여성 16.5%)였으며, 연령이 증가할수록 대사증후군의 유병률도 같이 증가하였다. 연구 대상자에 대해 연령, 흡연, 음주, 수면시간, 규칙적 운동 변수를 보정한 후 다변량 로지스틱 회귀분석 결과 습관성 코골이에 의한 대사증후군의 위험이 여전히 유의하게 높았다(교차비: 1.96, 95% CI=1.45-2.66). 성별에 따라 연령, 음주, 흡연 요인을 보정한 후 다변량 로지스틱 회귀분석을 한 결과 여자에서 고밀도지질단백콜레스테롤, 혈당, 허리둘레의 발생 위험이 유의하게 높았으며(p<0.05), 대사증후군의 발생 위험도 유의하게 높았으나(교차비: 2.36, 95% CI= 1.64-3.40), 남자에서는 대사증후군 구성 요소 및 대사증후군 발생 위험이 높아지지 않았다.

사상체질과 대사증후군 및 경동맥 내중막두께와의 상관관계 연구 (A Study on the Correlation of Metabolic Syndrome and Intima-media Thickness of Common Carotid Artery with Sasang Constitution)

  • 1. Objectives The purpose of this study was to investigate the correlation of Metabolic syndrome(MetS) and Intima-media thickness(IMT) of common carotid artery with Sasang constitution. 2. Methods 197 subjects who had taken health examinations and diagnosis of Sasang constitution from Jan 1, 2006 to Dec 31, 2006 at East-west health examination center of Kyung-Hee medical center were included and underwent B-mode ultrasonography for measurement of intima-media thickness of common carotid arteries. MetS was defined by the criteria of the criteria of the National Cholesterol Education Program Adult Treatment Panel III. 3. Results and Conclusions Mean intima-media thickness of common carotid artery was significantly higher in subjects with MetS compared with control subjects($0.070{\pm}0.017$ vs $0.063{\pm}0.014cm$) and a significant trend towards increased IMT was observed with increasing numbers of metabolic syndrome components(r=0.331, p=0.000). After adjustment for age and sex, IMT of common carotid arteries was significantly different among three constitutional groups (Soyangin:$0.065{\pm}0.015cm$, Taeumin:$0.068{\pm}0.016cm$, Soeumin:$0.056{\pm}0.010cm$), however, after adjustment for age, sex and existence of metabolic syndrome, there was no significant different. But, factor of Taeumin was the effective determinant of the increase of mean IMT of common carotid arteries.

걷기운동이 비만여중생의 대사증후군 위험인자와 신체구성에 미치는 효과 (Effects of Walking Exercise on Metabolic Syndrome Risk Factors and Body Composition in Obese Middle School Girls)

  • Purpose: This study was done to determine the effects of walking exercise training(WET) on metabolic syndrome risk factors and body composition in obese middle school girls. Method: A non-equivalent pretest-posttest experimental design was used. Twenty seven subjects participated in this study from one women's middle school in Busan. The participants were purposely allocated to an experimental group (n=14) and a control group (n=13). The experimental group participated in 30-60 minutes of WET with 55 to $75\%$ of a maximal heart rate six days a week for 12 weeks. Results: The prevalence of individual risk factors on metabolic syndrome were improved in the experimental group after the intervention. The high waist circumference(WC), high triglyceride(TG), low high density lipoprotein cholesterol(HDL-C), high blood pressure(BP) and high fasting glucose(FG) were 21.4, 21.4, 14.3, 28.6 and $7.2\%$, respectively. There were significant differences in WC (F=22.24, p<.001), TG (F=5.34, p=0.30), body weight(F=21.99, p<.001), fat mass(F=19.17, p<.001), and $\%$ body fat(F=17.93, p<.001) between the experimental and control group after the intervention. However, there were no significant differences in HDL-C, FG and BP between the experimental and control group after the intervention. Conclusion: These results indicate that WET is effective in decreasing risk factors of the metabolic syndrome and body composition components in obese middle school girls. These findings suggest that WET can be useful as a nursing intervention in the prevention of obesity-related disorders in obese adolescents.

  • With the recent rapid increase in obesity worldwide, metabolic syndrome (MetS) has gained significant importance. MetS is a cluster of obesity-related cardiovascular risk factors including abdominal obesity, atherogenic dyslipidemia, high blood pressure and impaired glucose tolerance. MetS is highly prevalent and strongly associated with an increased risk of developing diabetes and cardiovascular disease, putting a great burden on human society. Therefore, it is very important to reduce MetS risk, which can improve patients' cardiovascular prognosis. The primary and most effective strategy to control each component of MetS is lifestyle change such as losing body weight, keeping regular exercise, adopting a healthy diet, quitting smoking and alcohol drinking in moderation. Many studies have shown that lifestyle modification has improved all components of MetS, and reduces the incidence of diabetes and cardiovascular disease. Here, the Korean Society of CardioMetabolic Syndrome has summarized specific and practical methods of lifestyle modification in the management of MetS in the healthcare field.
