The Development of Wholeness Program for Effects Dementia-Buffering Testing of the Demented Elderly (노인의 치매행동 관련 홀니스 프로그램 개발과 완충효과 검증)

  • This study of the Wholeness Program relevant to dementia behavior and verification of buffering effects. For the purpose of this, among those who were attending Day Care Centers belonging to Seniors Welfare Center of M1 and M2 located in Seoul. Among those who met all these requirements, 40 senior citizens were finally selected. After carrying out a preliminary test to the selected subjects, 20 elderly of Seniors Welfare Center of M1 were randomly placed as an experimental group, while other 20 people of Seniors Welfare Center of M2, as a control group. The Wholeness Program relevant to dementia behavior was carried out from March 14 to May 30 of 2016, for 12 weeks, twice a week. As for the measurement tool, this study depended on the Korean version of Mini Mental State Examination (MMSE-K), the Korean version of Modified Barthel Index (K-MBI) and the Korean version of Geriatric Depression Scale (GDS-K). While the elderly were participating in the Wholeness Program relevant to dementia behavior, they were measured two times in total, before and after the participation in dementia behavior. Based on SPSS-PC (version 21.0), firstly, independent t-test was carried out to understand the homogeneity between the experimental group and the control group. Secondly, matched-pair t-test was carried out between preliminary test and post-test to inquire into the effects of MMSE-K, K-MBI and GDS-K. Thirdly, ANOVA was conducted to confirm the variation (change amount) between preliminary test and post-test. According to the survey results, the Wholeness Program relevant to dementia behavior of the elderly shows overall effects in all spheres of changes of cognitive ability (MMSE-K), changes of physical capacity (K-MBI) and changes of depression level (GDS-K).

A Qualitative Study about Volunteer Work Experiences of Senior Citizens: On the Subject of Senior Citizen Volunteers in the Welfare Center of K City (노인의 자원봉사 경험에 대한 질적 연구: K시 노인종합사회복지관 노인자원봉사자를 대상으로)

  • The purpose of this study is to understand volunteer work experiences of senior citizen volunteers serving in the welfare center in K district in the aspect of an insider and to apprehend the meaning. The objects of this study are fifteen senior citizen volunteers and two social workers who do volunteer works in the welfare center, and this study utilized the qualitative study method through observations and in-depth interviews from 2009.09.02 to 2009.10.21. The data analysis is processed by the qualitative analysis program, Nvivo 2.0, and the processed data are analyzed in accordance with the grounded theories of Strauss and Corbin. As a result of reviewing data, the meanings about volunteer work experiences of senior citizens are induced as eight factors; senior citizens' identity in an old aging society, desire fulfillment through volunteer works by senior citizens, various leisure activities to help their health in mind and body, acceptance attitude about aging, mental supports from their families, finding activities suitable for each person's personality and capability, changes of perception about the handicapped, and endeavors to spend meaningful old age. Through results and discussion obtained by this study, the conclusion is as below: First, participation induction factors are identities of elderly people in aging society, acceptance attitudes toward aging and emotional support from families in the matter of senior citizens' volunteer work. Second, programs suitable for individual aptitude and capability should be implemented for activating senior citizens' volunteer work. Third, participation in senior citizens' volunteer work indicates desire satisfaction through senior citizens' volunteer work, leisure activities helping health for body and mind, and life changes such as changes of perception about handicapped people etc.

  • Korea is undergoing a change in its population structure, due to economic development, falling birth rates and the development of health care. As the population ages, the number of elderly people who have difficulty in daily life such as dementia and paralysis increases every day. The growth of nuclear families and the increased participation of women in society make it difficult to take care of elderly people who need long-term care at home. As the social problems resulting from this have emerged as serious problems, the government enacted and implemented the Long Term Care Insurance Act to improve the quality of life for the elderly subject to long-term care and to ease the burden of family support in order to solve such problems. Therefore, the present study explores ways to improve legal and institutional aspects, and seek mental and psychological measures for the stability of old life as well as the physical health of welfare.

A Horticultural Therapy Program Focusing on Gardening Activities to Promote Psychological, Emotional and Social Health of the Elderly Living in a Homeless Living Facility for a Long Time: A Pilot Study

  • Background and objective: The elderly living in homeless living facilities for a long time suffer from various mental health problems. This study aims to determine the psychological, emotional, and social effects of a horticultural therapy program composed of gardening activities, which was designed based on the semantic structures of life for the homeless elderly living in the facilities for a long time. Methods: A total of 12 subjects (6 in the control group and 6 in the experimental group) participated in the study. The horticultural therapy program consisted mainly of gardening activities, and a total of 16 sessions were conducted once a week for 16 weeks, 60-90 minutes per session. The subjects were tested to evaluate their self-esteem, depression, and horticultural activities. The data were analyzed using the Mann-Whitney U test, Wilcoxon rank test, and Friedman test, which were nonparametric tests, conducted at a 95% significance level. Results: First, in the case of self-esteem, a significant difference was found between the groups, 20.00 points (SD = 5.69) in the control group, and 25.50 points (SD = 3.73) in the experimental group (p = .034). Second, in the case of depression, no statistically significant difference was found in the posttest. Finally, in the case of the horticultural activity evaluation, the scores of most variables gradually and significantly increased during the program [Verbal interaction during activity (p = .006), Self-concept and identity (p = .006), Need-drive adaptation (p < .001), Interpersonal and social relations (p < .001)]. Conclusion: These results support that the horticultural therapy program could help the elderly improve psychological relaxation, emotional stability, and social relationships. In order to generalize the results, it is suggested to increase the number of subjects or conduct additional repetitive experiments in further research.

The Analysis of Therapeutic Effects of Forest landscapes with different Water-scape types Using Hemodynamic measurement in Prefrontal cortex (전두엽 혈류 측정을 통한 산림녹지 내 수경관 유형별 치유 효과 분석)

  • When situated in green landscapes, water bodies play a crucial role in positively influencing mood and mental health, yet research on the cognitive mechanisms underlying these therapeutic effects is lacking. This study is intended to examine differences in brain function among adult males exposed to forest landscapes with or without water bodies. The wooded landscapes included views of a waterfall, a valley, and a forest without water. The control group was exposed to a local urban landscape. Twelve adult males participated in a field experiment in which prefrontal cortex (PFC) activity was measured using near-infrared spectroscopy (NIRS). In the experiment, participants engaged in low-intensity walking in three forested areas with similar vegetation and climatic conditions. Participants showed significant differences in left PFC activity depending on whether they were in the three forested landscapes or in the control landscape (P < 0.01). An analysis of variance (ANOVA) confirmed that significantly lower left PFC activity was recorded in the wooded landscape containing a water view . Notably, the lowest PFC values recorded in the landscape with a waterfall view suggest that landscapes with dynamic water flow may be associated with greater therapeutic benefits in terms of PFC activity than static landscapes. Our results underscore that water is a critical aspect of a landscape due to its therapeutic benefits and should be incorporated in the planning and design of green spaces for health promotion.

Effects of Diabetic Foot Care Education for the Aged with Low Health Literacy (건강정보 이해능력이 낮은 당뇨병 노인을 위한 발 관리 교육의 효과)

  • Purpose: The purpose of this study was to examine the effects of diabetic foot care education for the older adults with low health literacy. Methods: A quasi-experimental design with a non-equivalent control group pretest-posttest was used. The participants who were diagnosed with diabetes, were adults over 65 years old at the welfare center of Y and B city. They were divided into the experimental group (n=32) and the control group (n=31). Inclusion criteria were a score of 5 or under on the Short form of Korean Functional Health Literacy Test and 24 or more on the Korean version of Mini-Mental State Examination. Foot care education was conducted in a small group for 40 minutes, once a week, for three weeks. The education materials are composed of an easy term, picture and photographs to understand easily. Results: The scores of diabetic foot care knowledge (t=4.57, p<.001), foot care self-efficacy (t=6.07, p<.001), and foot self-care behavior (t=4.18, p<.001) were significantly increased in the experimental group compared to the control group. Foot health status was not significantly improved. Conclusion: The findings indicate that this education program can be used as a nursing intervention improving foot care knowledge, foot care self-efficacy, and foot self-care behavior in order to prevent the diabetic foot problems of elderly diabetic persons with low health literacy.

Factors Related to Perceived Life Satisfaction Among the Elderly in South Korea

  • Objectives: This study attempted to explore the aging phenomena that now characterize much of Korean society, and assessed issues associated with the life satisfaction experienced during the process of aging. Methods: By employing the National Survey on the State of Life and the Desire for Welfare of the Elderly, 2004 in South Korea this study attempted to identify the factors that determine subjective life satisfaction among the elderly. The data utilized herein consisted of 3278 elderly people aged 65 years or older, from 9308 households. Results: The results of analysis from the final model after the introduction of 19 variables in 8 factors showed statistically similar explanatory power in men (adj. $R^2=0.320$) and in women (adj. $R^2=0.346$). We found that economic condition was the most influential factor in both men (B = 0.278) and women (B = 0.336) except perceived health condition variables. The second most influential variable in life satisfaction was health checkups in men (B = 0.128) and degree of nutritional diet in women (B = 0.145). Those who had experience with chronic diseases also reported significantly lower perceived life satisfaction and this was particularly true of women. Conclusions: The aging society requires an understanding of the lives of elderly individuals. This study explored factors associated with life satisfaction in old age by using a life satisfaction model. The success of an aging society begins with an accurate understanding of the elderly, and thus political attention will need to be focused on this matter.

A Study of Effect on Quality of Life of Cancer Patient's Caregiver : Focusing on the Mediating Effect of Feeling of Burden and Growth (사회적 지지와 암환자 가족의 삶의 질의 관계에서 돌봄부담감과 내적성장의 매개효과)

  • This study intends to investigate the main and mediating effects which caregiving appraisal and positive reappraisal exert on quality of life (QOL) of primary family caregivers of cancer patient considering the relationship with social support. The processes of this study areas follows. First, the variables which research model were chosen on the basis of stress-appraisal-coping theory through reviews of the previous studies. Second, a survey was conducted upon 295 primary caregiver of patient with cancer at National Cancer Center. Collected data were analyzed by SPSS 12.0 and SEM (Structural Equation Modeling) method using AMOS 5.0. The summary of the result is as follows. First, the entire model including measurement and structural model shows sufficient fit index of CFI(.951), TLI(.940) and RMSEA(.062). Second, the results of analysis of direct effects among variables are as follows. The 'Social support' has statistically significant direct effect on the 'feeling of burden' and 'growth'. The 'feeling of burden' has statistically significant direct effect on the 'growth' and 'QOL-mental and physical'. The 'growth' has statistically significant direct effect on the 'QOL-mental'. Third, the results of analysis of mediating effects of the 'social support and QOL' and 'feeling of burden and QOL' are as follows. The effects of 'social support' on 'QOL-mental' are significantly mediated by the 'feeling of burden' and 'growth'. The effects of 'social support' on 'QOL-physical' are significantly mediated by the 'feeling of burden'. The effects of 'feeling of burden' on 'QOL-mental' are significantly mediated by 'growth'. Through this research, these implications in social work study and practice are found: (1) this study extended the scope of study in the caregiver's health area from negative sides into positive ones by using growth variables as positive reappraisalof caregiving in research model, which has not been tried on the Korean family caregivers of the cancer patient. (2) The effects of positive reappraisal on QOL-mental can provide a foundational necessity for social workers to help family caregivers find positive meaning in their caregiving experience. This approach of social work practice will improve QOL of family caregivers. (3) This study present a framework including social support, negative appraisal, positive reappraisal, and QOL variables available to social work practice and explaining affective relationships among these variables in various aspects.

Effects of Meaning-Centered and Mindfulness-Based Stress Management Program with Using Marine Resources on the Improvement of Emotion, Sleep Quality, Quality of Life and Cognitive Function in Family Caregiver of Patients with Severe Physical Disability : Preliminary Study (해양자원을 활용한 의미중심, 마음챙김기반의 집단스트레스중재프로그램이 중증장애환자 간병가족의 정서, 수면, 삶의질 및 인지기능 향상에 미치는 효과 : 예비연구)

  • Objectives : The purpose of this study was to investigate the effects of meaning centered and mindfulness-based stress management program with using marine resources on positive changes in mood, sleep, quality of life and cognitive function. Methods : Nine family caregivers of patients with severe disability experienced meaning centered and mindfulness-based stress management program for four-days in marine areas near Uljin-gun, Gyeongsangbuk-do in South Korea. Subjective questionnaires and objective assessments were conducted and statistically analyzed to examine changes in mood, sleep, quality of life, and cognitive function before, after, and after 6 weeks of participation. Results : After participating in the program, moods including depression improved significantly. This effect lasted until 6 weeks. Participants' sleep quality, quality of life and cognitive function improved significantly after 6 weeks of program participation. Conclusions : This new specialized stress management program using marine resources for family caregivers of patients with severe disability is expected to be used effectively in terms of improving their overall quality of life, mental health status and cognitive efficiency.

Development of Prevention and Educational Programs for Battered Women (아내구타 예방 및 대처 교육프로그램 개발)

  • The purpose of this study was to develop prevention and educational programs for battered women. The first stage was to assess battered women's health status and health needs. The second stage was to develop prevention and educational programs to improve battered women's health. A survey research design was used to gather data from June-November 1999 by the counselor. Research sample was recruited from shelter and women's counseling centers in Seoul, Inchon, Chooncheon, Taejeon, and Jeoju. Trained counselors contacted 218 women, who agreed verbally to participate in the study. The researcher did a literature review and developed the tool used. We did ten pretests and three counselors checked the tool for content validity. The reliability and validity of the instrument were acceptable. The second stage of the research consisted of lay-professional groups discussing the abuse problems and possible solutions. Each group had five professionals (counsellor, nurse, minister, psychiatrist, social worker) and five women who have experienced battering from her husband in community. Result of the survey and two(lay persons and professionals) group discussions showed that victims need more assessment of physical, mental and spiritual needs. Victims were beaten more often during their pregnancies than at other times. Various kind of strategy were frequently utilized, but still the number of women abused and suicides committed increased. Both studies showed that the women experienced severe beating, that they felt powerless, that they need to understand that society abuser characteristics, that they lack a protection support system and that society lacks would an understanding of battered women. Based on the result of both studies, we developed model for prevention and education of battered women. This educational program will increase women s power to solve the abuse problems. The ultimate goal of the program is to develop a one-stop center to give wholistic approach care to women in abuse situations.
