Postpartum Depression in Young Mothers in Urban and Rural Indonesia

  • Objectives: Young mothers are vulnerable to postpartum depression due to role transition-related stress. Understanding the causes underlying these stressors is essential for developing effective interventions. Methods: This study analyzed the 2018 Indonesian Basic Health Research data. The Mini International Neuropsychiatric Interview was used to assess postpartum depression symptoms in mothers aged 15-24 years with infants aged 0-6 months. In 1285 subjects, the risk factors for postpartum depression were evaluated using multivariate logistic regression. Results: The overall prevalence of depression in the 6 months postpartum was 4.0%, with a higher prevalence in urban areas (5.7%) than in rural areas (2.9%). Urban and rural young mothers showed distinct postpartum depression risk factors. In urban areas, living without a husband (odds ratio [OR], 3.82; 95% confidence interval [CI], 1.24 to 11.76), experiencing preterm birth (OR, 4.67; 95% CI, 1.50 to 14.50), having pregnancy complications (OR, 3.03; 95% CI, 1.20 to 7.66), and having postpartum complications (OR, 5.23; 95% CI, 1.98 to 13.80) were associated with a higher risk of postpartum depression. In rural areas, postpartum depression was significantly associated with a smaller household size (OR, 3.22; 95% CI, 1.00 to 10.38), unwanted pregnancy (OR, 4.40; 95% CI, 1.15 to 16.86), and pregnancy complications (OR, 3.41; 95% CI, 1.31 to 8.88). Conclusions: In both urban and rural contexts, postpartum depression relates to the availability of others to accompany young mothers throughout the postpartum period and offer support with reproductive issues. Support from the family and the healthcare system is essential to young mothers' mental health. The healthcare system needs to involve families to support young mothers' mental health from pregnancy until the postpartum period.

국외 요양시설 디자인가이드라인에 나타난 치유성능 연구 (Healing Performance Shown in International Design Guidelines for Nursing Homes)

  • Although nursing homes where physically weak seniors live must be planned as an healing environment for maintaining and improving holistic health conditions of the senior residents, many of the nursing homes in Korea are currently placed in poor environment due to lack of minimum criteria or design guideline for spatial planning. Therefore, this study aims to extract healing elements shown in nursing home design guidelines that have been developed and utilized in several developed welfare countries where aging has started before Korea. The subjects of this study were 4 international nursing homes design guidelines; and they were analyzed by the content analysis technique. As for analysis units, design guideline items were selected; and spatial composition, three dimensions of health, and healing elements for supporting the health conditions were used as analysis categories. As results, firstly, among total 259 guidelines, the guidelines that commonly applied to all the spaces turned out to be the most, then the guidelines to be applied in bathroom turned out to be the most. Secondly, in terms of holistic health, the proportion of design guideline for supporting physical health turned out to be highest followed by the one for mental health and social health. Thirdly, In terms of healing elements, items for safety, accessibility, comfort, and convenience that supported physical health and cognition that supported mental health turned out to be high. This study is expected to provide the framework for comprehensively understanding the existing design guidelines of nursing homes in the perspective of healing and to be utilized as fundamental resource for spatial planning to improve healing effects in the nursing homes in the future.

목회자 사모의 정신건강과 영적 안녕감의 관계 분석 (Relation Analysis on Mental Health and Spiritual Wellbeing of the Pastors' Wives)

  • 본 논문은 목회자 사모들의 정신건강과 영적 안녕감 간의 관계를 파악하기 위해 부정적 정신건강 요소인 우울과 긍정적 정신건강 요소인 심리적 안녕감이 영적 안녕감에 미치는 영향을 분석하였다. 목회자 사모 236명을 대상으로 심리적 안녕감, 우울, 영적 안녕감 척도를 실시하고 분석한 결과는 다음과 같다. 첫째, 인구통계학적 변인에 따른 우울, 심리적 안녕감, 영적 안녕감 차이를 검증한 결과 목회만족도만이 유의수준 5%에서 유의한 모습을 보였다. 둘째, 우울은 영적 안녕감과 부적 관계를, 심리적 안녕감과는 정적 관계가 있었고, 우울보다 심리적 안녕감이 영적 안녕감에 더 많은 영향을 주는 것을 알 수 있다. 셋째, 우울과 심리적 안녕감이 한 단위 증가할 때, 영적 안녕감에 대해 각각 -0.282, 0.668만큼 영향을 주는 것을 확인하였다. 마지막으로 목회 만족도에 따라 만족/만족하지 못하는 그룹을 나누고 우울과 심리적 안녕감의 하위요인들이 영적 안녕감의 하위요인에 미치는 영향을 분석하였다. 그 결과 두 그룹 모두에서 우울은 실존적 안녕에만 유의한 영향을 미쳤고, 심리적 안녕감은 종교적 안녕과 실존적 안녕 모두에 영향을 미쳤으며 부정적 정신건강인 우울보다는 긍정적 정신건강인 심리적 안녕감이 목회자 사모의 영적 안녕감에 더 큰 영향을 미치고 있었다. 또한 우울과 심리적 안녕감이 목회에 만족하는 그룹보다 만족하지 않는 그룹의 영적안녕감에 더 큰 영향을 준다는 결과를 얻었다.

집합주의 문화가 청년실업에 대한 불안 지각 및 정신건강에 미치는 영향 (The Effect of Collectivism on Anxiety Perception and Mental Health in Youth Unemployment)

  • 본 연구의 첫 번째 목적은 집합주의적 성향이 청년들이 지각하는 취업불안에 미치는 영향을 살펴보는데 있었고, 두 번째 목적은 이러한 집합주의적 성향이 취업불안 지각에 미치는 영향에 있어서 구직효능감, 자존감 그리고 상향·하향 사회비교의 매개효과를 검증하는데 있었다. 이를 위하여 연구 1에서는 서울특별시에 소재한 상위권 A 대학과 중위권 B 대학의 대학생을 대상으로 총 179명의 응답자료를 분석에 사용하였다. 연구 결과, 집합주의적 성향을 가진 사람일수록 취업불안이 높고 취업 정신건강 수준이 낮은 것으로 나타났다. 또한, 구직효능감과 자존감이 집합주의적 성향과 취업불안 간의 관계를 매개하는 것으로 드러났다. 연구 2에서는 서울특별시에 소재한 C 대학의 대학생을 대상으로 총 118명의 응답자료를 분석에 사용하였다. 연구 결과, 상향 사회비교는 집합주의적 성향과 취업불안 간의 관계를 매개한 것으로 나타났다. 하지만 하향 사회비교에서는 매개효과가 나타나지 않았다. 이러한 연구결과를 토대로 본 연구의 시사점과 제한점, 그리고 추후 연구과제에 대해 논의하였다.

  • 질병의 생물정신사회적 이해는 건강의 의미 있는 증진을 위해서 매우 중요한다. 건강의 유지와 증진은 신체적, 정신적, 사회적, 그리고 영적인 안녕감을 통하여 이루어진다. 건강은 단순한 상태와 삶의 목적이 아니며, 일상생활에서의 자원으로, 개인적, 사회적뿐만 아니라 신체적 능력의 긍정적 개념이다. 한편 의료 체계에서 생물정신사회적 모델의 이해 역시 매우 중요하다. Engel은 의사들의 교육 및 진료에서 생의학적 접근을 비판하고, 새로운 모델인 생물정신사회적 모델을 주장하였다. Engel은 인간을 특정한 방법으로 자신의 건강을 위해하거나 증진시킬 수 있는 생물학적, 정신적, 사회적 존재로 보았다. 질병의 생물정신사회적 모델의 중요함에도 불구하고, 우리나라의 의료는 질병의 생의학적 모델에만 중점을 두고 있다. 그 결과 한국의 의료가 기계화 되어가고 있고 비인간화 되어가고 있다. 저자는 한국 정신신체의학회 창립 20주년을 맞이함에 따라 우리나라 의료 체계 및 실제적인 임상 진료에서 질병의 생물정신사회적 모델의 중요성을 강조하고자 하였다.

유아기 신체 건강과 관련한 일상생활습관 실태 연구 (A Study on Daily Life Habits Related with the Physical Health of Young Children)

  • The purpose of this study was to provide basic understanding of healthy habits of young children by analyzing existing life patterns that affect the their physical health. 416 parents of young children from kindergartens and daycare centers have responded to a questionnaire. The collected data were analyzed by using mean, standard deviation frequency, was used by SPSS 16.0 program. The conclusion was that the young children present daily life habits, including sleeping, eating, defecating and playing habits were not positive and that the children mostly led an indoor lifestyle. From these results, observations were made that the physical activity for young children is imperative to develop the physical and mental health of the 21st century young children.

  • The purpose of this study was to understand the adult day care center as a place for the elders and adults and to develop an initial understanding of the services and activity programs for adult day care for the cognitively-impaired in the U.S.A. The data were collected from 12 Adult Day Care Centers(ADC) from 2001 to 2002 by personal interview and the documents about the programs. ADC programs provided primarily social, recreational, and health activities and services in a group setting. Centers offered participants to socialize, enjoy peer support, and receive health and social services in a stimulating and supportive environment that promotes better physical and mental health. For this purpose those centers offered not only professional health care, occupational.speech.physical therapies but also socio-recreation and therapeutic-recreation services.

초등학교 보건교육내용의 국제비교분석(한국, 미국, 일본을 중심으로) (A Study on International Comparison and Analysis of Health Education Contents in Elementary Schools(based on Korea, USA, and Japan))

  • The purpose of this study is to analyze hygiene education contents of elementary schools in Korea, USA, Japan and compare this and to provide with basic material present improvement method when hygiene education contents of elementary schools in Korea are organized. For this purpose the problems as following are selected centering on the hygiene education contents of the 7th education process in Korea and Japan and the hygiene text of public school in California, USA. First, this study tries to analyze the composition classified by 12 guidance contents each grade of hygiene education contents of elementary schools in Korea, USA, and Japan. Second, this study tries to analyze the composition classified by fields(physical, mental, social) of hygiene education contents of elementary schools in Korea, USA, and japan. As a result of the analysis and comparison on the matters stated above, the conclusion as following was gotten. First, as a result of searching the hygiene education contents system of elementary schools in Korea, USA, and Japan classified by 12 guidance contents each grade, the subjects which have good contents composition classified by class step by step were safe life and disease prevention. And the subjects such as growth development and health, physical fitness, drug use and health were weak and the parts such as consumer health and the careers concerned with are not dealt in the present education process of Korea at all. Second, as a result of analyzing the composition classified by fields of hygiene education contents of elementary schools in Korea, USA, Japan, all the three countries put great on physical part. And there are 57 units about mental part in USA, 27 units in Japan, only 17 units in Korea. And this fact reveals that there was lack of understanding about the need of mental part in Korea. This study tries to suggest improvement method as following through the study results stated above. First, as it is grasped in the analysis classified by guidance part of hygiene education contents, community and environment preservation, physical aptitude and stress management are to be emphasized more strongly and about the parts like consumer health and the persons concerned with hygiene new education programs as contents satisfying the need of age are to be inserted or complemented Second, because the hygiene education contents in Korea are still lack in comparing contents of mental parts compared with those of the two countries, the contents proper to the level and the requirement of learners to each grade are to be developed and complemented. Third, hygiene education units which were reduced in the 7th curriculum in Korea are lack compared with those in the two countries. Therefore, it is necessary that teachers should recognize the need of hygiene education so that the hygiene education can be separated and be taught out regularly.

초등학생의 건강지식과 건강증진행위에 관한 보건과 교육의 효과 (The Effects of Health Education on Health Knowledge and Health Promoting Behaviors in Elementary School Students)

  • Purpose: This study was to investigate the effects of health education on health knowledge and health promoting behaviors in elementary school students. Methods: 17 sessions of health education were provided to the fifth graders in two elementary schools in Gyunggi Province and data were collected from 268 students. The data of 250 students who had responded both pretest and posttest were analyzed using SPSS program. Results: The scores of health knowledge for mental health, social health, sex and health were lower than other domain. The scores of knowledge for drug abuse/smoking and disease prevention/management were higher than other domain. Overall health knowledge was improved significantly from 0.60 at pretest to 0.81 at posttest (t=15.98, p<.001). The score of health promoting behaviors at post test was higher than score at pre test but this change was not significant (t=-0.91, p<.365). Conclusion: Health education had significant effects on health knowledge and we need to maintain and activate health education in elementary schools. Health education did not have significant effects on health promoting behaviors, we need to do further research for understanding why and how we improve health promoting behaviors.

고위험 노인돌봄과 정신건강 개입의 만남: 「노인맞춤돌봄서비스」 내 「특화서비스」 (Integration of care services and mental health intervention for older adults at high risk: the Specialized Service in the Individualized Support Service for older adults)

  • 돌봄의 사회화가 급속히 진행되는 가운데 사회적 고립과 우울 등이 심한 노인을 위한 지역사회 차원의 정신건강 개입의필요성이 증가하고 있다. 2020년 「노인맞춤돌봄서비스」 개편에 맞추어, 2014~19년 시범사업으로 진행하였던 '독거노인 사회관계 활성화사업'이 「노인맞춤돌봄서비스」 내 별도의 특화 사업으로 확대 개편되었다. 고위험 노인 대상 정신건강 지원 및 사례관리 서비스를제공하는 것을 기본 골격으로 하는 「특화서비스」는 노인돌봄과 정신건강 서비스가 만난 고위험 노인돌봄의 새로운 실천모델이라고할 수 있다. 본 논문의 목적은 「특화서비스」에 대한 이해를 높이고 이것의 개념적 틀을 명확히 하는 것이다. 국내·외 관련 문헌, 그리고 「특화서비스」의 전신인 '독거노인 사회관계 활성화 시범사업'에 대한 분석을 포함한 「특화서비스」 제도화 과정 고찰을 통해 「특화서비스」의 특성 및 개입의 구조와 내용을 살펴보았다. 마지막으로 제도 초반 현장에서 발생하는 몇 가지 상황을 중심으로 향후 「특화서비스」가 의도한 대로 잘 진행되기 위해 고려할 것에 대해 논의하였다.
