A new perspective on cholesterol in pediatric health: association of vitamin D metabolism, respiratory diseases, and mental health problems

  • Cholesterol, the main structural molecule of cell membranes, is involved in essential functions of the human body. Dyslipidemia is an established risk factor for cardiovascular diseases (CVDs) that is observed even in childhood. To reduce the risk of CVDs in children, several clinical guidelines have been published for the management of pediatric dyslipidemia. However, pediatric dyslipidemia is also associated with several health problems other than CVDs. This article reviews the current data on dyslipidemia-related pediatric health issues. There is strong evidence that low serum vitamin D levels, asthma, and mental health problems may be associated with dyslipidemia in the pediatric population regardless of body mass index. This review also highlights the need for further large-scale population-based studies in the Korean pediatric population to establish effective strategies for promoting children's health.

중학생의 정신보건교육 요구도 조사 (Needs for mental health education among middle school students)

  • Objectives: The middle school students face a turbulent period between the stages of childhood and adolescence. If effective mental health education can be provided over this period, it can help prevent behavioral and emotional difficulties of students. This study aims to understand the priority of mental health education as demanded by middle school students. Methods: The subjects of this study were 612 middle school students from five schools in the P city. A questionnaire arranged mental health education into 39 items for use as a tool in the study. For data analysis, followed by descriptive statistics, t-test, ANOVA and multiple comparison analysis of Scheffe. Results: For general students, demand was highest for education on "Communication and Healthy Life," followed by "Self-esteem," "Stress," "Interpersonal Relationships and School Life". Conclusions: If education on mental health shall be offered at middle schools based on this study, education on how to communicate and lead a healthy life and improve self-esteem should go to general students to prevent behavioral and emotional difficulties. On the other hand, education and group programs for "Stress," "Mental Problems in the Adolescent Period" and "Addiction" should be offered to students with mental health problems to help prevent behavioral and emotional difficulties.

정신보건심판위원회 계속입원치료 심사제도 운영현황 및 개선방안 (Operation Status and Improvement Plan of Continuing Hospitalization Judgement System in Mental Health Review Board)

  • Background: The recent revision of South Korea's Mental Health Law emphasizes the role of the Mental Health Review Board. For this study, we examined the current status of continuing hospitalization judgement in Mental Health Review Board at the national level and aimed to determine the directions of improvement. Methods: Using a qualitative case study as the research method, we interviewed 30 Mental Health Review Board members and analyzed the results. Results: Each municipality had very different continuing hospitalization judgement methods. In our systematic review, which consisted of document inspection, we identified reliability problems due to limitations in Mental Health Review Board's operating systems, discharge orders, etc. Additionally, continuing hospitalization judgement needs to improve the objectivity, fairness, and effectiveness of their screening examinations. Conclusion: Based on the results of this study, we suggest policy proposals to improve these systems, such as standardizing examination processes, strengthening on-site inspections, increasing the independence and neutrality of judgment in Mental Health Review Board, building community mental health infrastructures, and establishing integrated management systems.

Roles for Public Education in Mental Health Promotion, Prevention of Mental Illness and Treatment of Mental Disorders in Canada

  • Objectives: The purpose of this paper is to describe the role for public education in mental health promotion, prevention of mental illness and treatment of mental disorders in Canada. Results: A brief history of the changing nature of the way we treat mental disorders in Canada argues for an increasingly critical role for public education. Furthermore, the new understanding of the role of social determinants in prevention of mental illness again necessitates a vital role for public education as the shift in focus moves from exclusively individual-focused health care treatments to creating supportive environments. The roles for mental health public education then become: advocacy for the social determinants of health approach, better understanding of the nature of mental health problems, useful information for self-management, and more knowledge about appropriate treatments. A brief discussion of the structure of the response in Canada provides an overview of national and provincial responsibilities in mental health care with a case study highlighting one community's response to addressing mental disorders. Conclusion : Finally, challenges regarding the most effective educators and the issue of professional expertise embedded in the status quo are discussed.

대학생의 도박성 게임 경험이 도박문제에 미치는 영향 (Effects of College Student Gambling Experience on the Gambling Problems)

  • Objectives: The purpose of this study was to identify the effects of college student gambling experience on the gambling problems. Methods: The study included 260 students from four-year universities in Seoul and Gangwon Province, and collected data from March 1, 2018 to March 31, 2018. The data were analyzed with the SPSS 23.0. Results: First, gambling experience showed high frequency among males (68.5%), 21 years of age and older (45.5%), boarding house (66.1%), and two or more siblings (40.5%). Second, the gambling problem based on the experience of gambling was statistically significant in academic problem, interpersonal relationship, and gambling control. Third, the mental health of the subjects was highly correlated by interpersonal relationship, gambling recognition and academic problem. Also, academic problem was highly correlated in order of gambling control and interpersonal relationship. Fourth, effects of gambling experience on the gambling problems were identified as gambling control, interpersonal relationships, gambling awareness and mental health. Conclusion: Recognize the seriousness of the gambling problem of college students in order to solve the gambling problem, the government, communities and universities should actively promote the harmful effects of gambling and provide regular mental health. education at universities.

학령기 아동 정서.행동문제에 대한 부모-교사 평가 일치도 (Agreement between Parents and Teachers on School Children's Emotional/Behavioral Problems)

  • Objectives: In assessing behavioral/emotional problems in school-aged children, the importance of multi-informant reporting has been well documented. However, in clinical settings obtaining multiple informants' opinions has proven difficult. For that reason, we researched the agreement and predictive validity of the Child Problem-Behavior Screening Questionnaire (CPSQ) in order to reveal how accurate parents' assessments reflected teachers'opinions. Methods: We conducted the first screening for second- and third-grade children from 3 elementary schools in Seoul from 2003 to 2007 using the CPSQ. There were 1178 children included in the analysis. We then administered the Korean version of the Child Behavior Checklist (K-CBCL) as a second screening tool and subsequently, the ADHD Diagnostic System (ADS) and the Korean Educational Development Institute version of the Wechsler Intelligence Scale for Children (KEDI-WISC) was administered by a psychiatrist. We examined each item on the CPSQ and the subscale's agreement between parent and teacher as well as the predictive validity of the CPSQ in children diagnosed with emotional/behavioral problems. Results: The agreement rates between parents and teachers appeared high for questions 18 (0.433), 1 (0.385), and 2 (0.325). Among the subscales, a relatively high correlation was found for externalizing problems, attention deficit hyperactivity disorder, and cognitive problems. For all diagnosed children, their parents revealed a higher sensitivity and lower specificity than teachers. Conclusion: From these results, we confirmed that the CPSQ can be useful for sorting out externalizing and cognitive problems. There is a need for further study, however, with a larger sample size.

학생들의 정신건강문제와 정신과 의뢰에 대한 교사의 인식도 조사 (TEACHERS' PERCEPTIONS OF MENTAL HEALTH PROBLEMS IN STUDENTS AND PSYCHIATRIC CONSULTATIONS)

  • 서울과 광주 및 전라남도의 초등학교, 중학교, 고등학교에 재직하는 일반교사 450명을 대상으로 하여 학생들이 보이는 정신건강문제의 빈도와 유형 및 원인에 대한 교사들의 인식과, 애로점 및 개선요구 사항, 의뢰나 도움을 청하는 대상, 정신과 의뢰나 권유 정도 및 의뢰 장애요인을 설문 조사하여 다음과 같은 결과를 얻었다. 1) 교사에 의해 정신건강문제가 있다고 판단되는 학생은 5.3%, 정신과 진찰이 필요하다고 생각되는 학생은 1.9%, 정신과 의뢰나 권유를 한 학생은 1.2%였다. 2) 교사에게 발견되는 학생들의 주된 정신건강문제의 유형은 초등학교의 경우에는 산만함과 주의력결핍, 학습능력결여, 언어능력결여의 순이며, 중학교는 산만함과 주의력결핍, 가출이나 비행문제, 학습능력결여의 순이었고, 고등학교는 가출이나 비행문제, 산만함과 주의력결핍, 신체증상의 호소, 담배나 본드의 상습적 사용의 순이었다. 3) 교사들은 학생들의 정신건강문제의 유발 원인에 대해 양육태도나 가정환경, 심리적 요인, 성적위주의 교육제도, 상담이나 정신건강관리 미흡 등의 순으로 중요하게 생각하였다. 4) 학생들의 정신건강문제를 접할 때 교사가 느끼는 애로사항은 담당 학생수의 과다, 시간적 여유 부족, 상담교육 기회 결여의 순이었고, 개선을 희망하는 사항으로는 담당 학생수의 감축, 성적위주 교육환경의 개선, 시간적 여유, 정규적인 정신건강교육, 문제학생을 위한 개별 프로그램, 학교와 연계된 상담전문가나 자문의사의 순이었다. 5) 도움을 받거나 의뢰할 수 있는 대상은 동료교사, 학부모, 상담교사, 양호교사, 상담기관, 신경정신과, 교장 ${\cdot}$ 교감의 순이었으며, 정신과 의뢰를 못하는 이유로는 정신과에 대한 학부모의 편견, 학생들의 문제에 대한 교사의 우위의식, 시간적 여유 부족, 정신과에 대한 교사의 편견, 과거 정신과 의뢰 결과에 대한 불만족의 순이었다. 6) 정신과 의뢰나 진찰 권유의 경험이 있는 교사는 20.4%였고, 서울시의 교사가 전남의 교사보다 정신과 의뢰나 권유의 경험이 많았다.

지역사회 만성정신질환자의 자살사고에 영향을 미치는 요인 (Factors Influencing Suicidal Ideation in Community-dwelling Patients with Chronic Mental Health Problems)

  • Purpose: The study was performed to identify factors influencing suicidal ideation in patients with chronic mental health problems and who live in the community. Methods: Data were collected using questionnaires and participants were 140 people with mental illness living in Y-si, Gyeonggi-do. The study was carried out from May 15 to September 30, 2014. Data were analyzed using descriptive statistics, Pearson Correlation Coefficients, and Logistic Regression with the SPSS/WIN 18.0 statistical program. Results: The number of persons who attempted suicide was 47(33.6%) and the number who had suicide ideation was 22(15.7%). The mean score for depression was $6.4{\pm}6.45$. The mean score for life satisfaction was $16.5{\pm}8.21$. Depression had a significant negative correlation with life satisfaction. Odds ratio of suicide ideation was 1.19. The significant factor influencing suicidal ideation was depression. Conclusion: The results of this study can be used in the development and practice of programs to decrease suicide in people with mental illness.

금융사무원의 심리사회적 작업환경과 정신건강 (Psychosocial Working Environment and Mental Health of Financial Clerks)

  • Purpose: The purpose of this study was to identify the psychosocial working environment and mental health of financial workers, and analyze the impact of the former on the latter. Methods: Data of 257 financial clerks were extracted from the 2017 5th Korean Working Conditions Survey. Psychosocial working environment was divided into five fields: demands at work, work organizations, interpersonal relations, workplace violence, and working hour quality. Mental health included sleeping problems, psychological well-being, and job stress. Results: A total of 6.1% subjects reported sleep problems, 28.2% experienced poor psychological well-being, and 39.6% had job stress. More than half the subjects were exposed to tight deadlines, complex tasks, hiding feelings at work, fair treatment, fair distribution of work, colleagues' support, and managers' support. Tight deadlines, workplace violence, long working hours, hiding feelings at work, and managers' support had a significant impact on the mental health of financial clerks. Conclusion: Based on the results of this study, we propose that employers, workers, and health managers in the financial industry should work together to establish a respectful organizational culture, prevent long working hours through recruitment, and conduct programs to protect emotional health.

일 도시 중학생을 위한 학교정신보건사업 모형 - 광주광역시 남구효사랑 청소년정신보건센터 - (Model of School Mental Health Program for Middle School Students in a City : Center for the Hyosarang Adolescent Mental Health, Namgu, Gwangju City)

  • Purpose: The goal of this paper is to offer practical suggestions for developing, implementing, and maintaining a successful school mental health program based on the model of a school mental health program for middle school students at the Namgu Hyosarang adolescent Mental Health Center. The model will be divided into six areas and the challenges and future direction of this program: creative approach to funding, creative staffing, education, training, needs assessment & resource mapping, collaboration and partnerships in a school mental health program, and developing an outcome evaluation research. Method: This is a descriptive study of the school mental health program model for middle school students of Namgu Hyosarang Adolescent Mental Health Center implemented over a four year period. Conclusion: Within this paper, a blueprint that can guide the development and implementation of school mental health programs has been offered. The actual application of this model will vary depending on the structure and goals of individual programs and schools. This model of our center has been identified as an effective school mental health program and the actual application program in regular learning times to middle school students. Establishing guidelines about the types of activities necessary for the successful implementation and sustainability of a school mental health program constitutes the first step in standardizing this process, and the school mental health movement continues to receive national recognition as a viable services delivery model for adolescents in need of mental health services. The recommendations outlined indicate that a school mental health program is more effective and necessary than the clinical service of a psychiatric hospital for adolescents' emotional/behavioral problems.
