관절염 환자의 운동행위 결정요인 (Determinant factors of Exercise behaviors in Patients with Arthritis)

  • The aims of this study were to understand and to predict the determinant factors affecting the exercise behaviors and physical fitness by testing the Ponder's health promotion model, and to help the patients with rheumatoid arthritis and osteoarthritis perform the continuous exercise program, and to help them maximize the physical effect such as muscle strength. endurance, and fuctional status and mental effects including self efficacy and quality of life, and improve the physical and mental wellbeing, and to provide a basis for the nursing intervention strategies. We analyzed the clinical records of 208 patients with rheumatoid arthritis and degenerative arthritis who visited the outpatient clinics at H university hospital in Seoul between October 5, 1999 and October 24, 1999. Data were composed of self reported questionnaire and good of fitness score which were obtained by pedalling the ergometer of bicycle for 9 minutes. SPSS Win 8.0 and Window LISREL 8.12a were used for statistical analysis. 24 Of 54 hypothetical paths were supported in modified model, which was considered as a proper model with improved fit index. The physical fitness was directly influenced by exercise participation behavior and education level, and indirectly by physical fitness, while fatigue, physical disability, pastexercise behavior, life-style, self-efficacy, which explained 20% of physical fitness. The exercise participation were directly influenced by perceived benefits and self-efficacy, and indirectly influenced by life-style, fatigue and physical disability, and directly and indirectly by past exercise behavior, which explained 53% of exercise participation. Exercise score were directly affected by perceived health status, perceived benefits, self efficacy, and past exercise behavior, and were indirectly affected by fatigue, physical disability, and life-style, which explained 50%. Perceived health status were directly influeced by level of education, depression, sleep disorder, and physical disability, which explained 34% of perceived health status. Perceived benefit was directly influenced by fatigue, sleep disorder, physical disability, and life-style, which explained 45%. Perceived barriers was directly influenced by fatigue, sleep disorder, and lifestyle, which explained 9%. Self- efficacy was directly influenced by fatigue, physical disability, past exercise behavior, and level of education, which explained 61%. In conclusion, important variables for physical fitness were exercise participation and level of education, and variables affecting exercise participation were perceived self-efficacy, benefits, and past exercise behavior. Perceived self-efficacy of exercise was a significant predictor of exercise participation. Life-style, fatigue, and physical disability showed direct effects on perceived benefit, perceived barriers, and self-efficacy, and indirect effects on exercise behavior. Therefore, disease related factor should be minimized for physical performance and well being in nursing intervention for patients with rheumatoid arthritis, and plans to promote and continue exercise should be soaked to reduce disability. In addition, Exercise program should be planned and performed by the exact evaluation of exercise according to the ability of the patients and the contents to improve the importance of exercise and self efficacy in self control program, dedicated educational program should be involved.

농촌지역 노인들의 우울 및 인지기능장애에 관한 연구 (A Study on the Depression and Cognitive Impairment in the Rural Elderly)

  • For the purpose of promotion of mental health in the rural elderly, the author surveyed 558 elderlies aged 60 years or more, and assessed the prevalence rates of depression and cognitive impairment by using self-rating depression scale of Zung (SDS) and the Korean version of mini-mental state examination (MMSEK). Also the association between depression or cognitive function and socio-environmental factors were investigated. The major findings were as follows ; 1. The prevalence rates of severe depression and cognitive impairment were 20.9% and 14.9% in all the elderly of both sexes, respectively. 2. The rates of depression and cognitive impairment increased with increasing age in both sex groups. The mean scores of SDS increased and the mean scores of MMSEK decreased significantly among them (p<0.01). 3. Those being female, widows or widowers, and those having low levels of physical activity, showed significantly high the mean scores of depression and had significantly low the mean scores of cognitive impairment (p<0.01). 4. The depression scores relating to decreased libido, confusion, psychomotor retardation, hopelessness and indecisiveness were relatively high in both sexes. 5. All the items of mini-mental state examination were significantly correlated with depression. 6. In stepwise multiple regression analysis on depression, MMSEK, level of physical activity, chronic disease, marital status and family income were selected as highly correlated variables, and the $R^2$-value for these variables was 33.7%. 7. In stepwise multiple regression analysis on cognitive function, level of physical activity, age, depression, sex and marital status were selected as highly correlated variables, and the $R^2$-value for these variables was 62.6%. The depression and cognitive impairment of the elderly were positively correlated with nearly all sociodemographic variables.

Factors Related to Depression among Older People Living in Homes for the Aged of the Western Part of Thailand

  • Introduction: Geriatric depression is one of the significant mental health issues for older people. This descriptive study aimed to investigate the correlation between selected factors and depression among older people, living in homes for the aged in the western part of Thailand. Methods: 107 older people, living in two homes for the aged of the western part of Thailand were recruited to the study, using cluster sampling. Three questionnaires, including the Barthel Index Activity of Daily living (BADL), the Thai Geriatric Depression Scale (TGDS), and personal characteristics, perceived health status, chronic diseases, family relationship, and recent grief and loss experience were used. The quality of all questionnaires, especially the reliability of BADL, TGDS, and selected factors (e.g., health factors) were 0.95, 0.93, and 0.72 respectively. After finishing data collection, percentages, medians, and Spearman's rank correlation and Mann-Whitney U were used to analyze the data. Results and discussion: The results revealed that nearly half (41.1%) of older people suffered from depression. Furthermore, income, perceived health status, and Barthel ADL index were significantly negatively associated with depression at p<0.01; whereas, the number of chronic diseases was significantly positively associated with depression at p<0.05. Conclusion: results on the correlation may guide the practice in the future. For example, the officers or health care providers in homes for the aged should encourage older people to be independent, improve their perceived health status, increase health promoting behaviors to prevent or delay chronic illnesses, or increase older people incomes by promoting the appropriate occupation.

기공 수련이 대학생의 육체적, 정신적 건강 및 자존감에 미치는 영향 (Effect of Qigong Training in a University Student's Physical, Mental Health and Self-esteem)

  • Background: Qigong is one of energy-healing intervention used to prevent and cure ailments and to improve health through regular practice. Although Qigong-neither itself nor its postulated mechanism of action-is within the paradigm of modern Western medical science, effects on the human body could be possible. Objectives: This study aims to know effect of Qigong training in a University student's physical, mental health and self-esteem. Method: There are 120 students who take a Yangsaengkigong(養生氣功) course in D University college of oriental medicine during four weeks beginning in April 10th 2008. I researched 41 of them trained about effect of Qigong training. Result: Qigong training made significant change in self-esteem measurement and SCL-90-R. And there wasn't significant change in KHP and happiness index. Conclusions: The depth study for the each Qigong is needed. Specifically, I think it should be a clinical studies and qualitative research methods for evaluation are needed.

치위생과 신입생의 신체·정신건강과 대학 생활 스트레스와의 관계 (The relationship between physical·mental health and life stress in dental hygiene freshman)

  • Objectives : The purpose of this study was to investigate the relationship between psychosocial stress and depression symptoms in dental hygiene freshman. Methods : A self-reported questionnaire of 45 questions was filled out by 207 dental hygiene students in Gyeongnam from December 9 to 11, 2013. Data were analyzed using SPSS WIN 18.0 program for descriptive statistics, t-test, ANOVA. Results : Religion tended to influence on positive effects on academic stress because those having religion had less academic stress. Economic condition also influenced on physical and mental stress in the students($8.52{\pm}5.63$)(p=0.000). Low economic status influenced on academic problems(p=0.022), family problems(p=0.014), rational relationship problems(p=0.002), and interpersonal problems(p=0.007). There was no significant difference between future problems(p=0.125) and self-efficacy(p=0.076). The physical symptoms included digestive disorders($16.73{\pm}5.37$), cardiac symptoms($14.61{\pm}5.51$), headache($9.46{\pm}3.59$), insomnia($8.32{\pm}3.67$) (p=0.000). The mental symptoms included negative emotions($15.07{\pm}3.94$)(p=0.000). Conclusions : In order to reduce the psychosocial stress symptoms in dental hygiene freshman, it is important to develop the stress debriefing program in the future.

종합병원 간호사의 교대근무와 건강상태에 관한 연구 (A Study of Health Condion and Shift Service of the Nurse in)

  • Continuing shift service of clinical nurses can be not only the cause of occupational dissatisfaction by being connected with the change of circadian rhythm and the burden of duties to be applicable to such changes. But also of inviting the lowering of nursing quality by being affected to the resignation of the nurses as the threat to the health of the nurses. This study has selected 500 nurses at random under non-probability sampling who have been serving by shift in 7 general hospitals which have over 400 sickbeds for the purpose of cross-sectional survey design from Sep. 7 through 20, 1996. Standardized modification of the CMI has been used which was designed for Koreans with Cornell Medical Index developed by Broadman and his fellow workers as the study device. The structure of the device was composed of 35 questions on physical appeal(Chronbach's ${\alpha}=8507$) and 22 questions of mental appeal(Cronbach's ${\alpha}=.8166$ totalling 57 questions. The collected data has been computerrized by using SPSS. General character, present symptom, perceived symptom and others are sought by practical number and percentage, and the health condition comparison followed by general characters was conducted by t-test and ANOVA. The post test was by Duncan's test by the level of p<.05. 1) The items of the answer that they have the physical symptom presently by 50% or over of the nurses were as "Do you often have spells of severe dizziness", "Are your eyes often red or inflamed", "Does press or pain in the head often make like miserable", "Are your ankles often badly swollen", "Do pains in the back make it hard for you to keep up with your work". 2) The items of the answer by over 50% of the nurses as the mental symptom at present were "do you fell bad when criticized?", "Do you get angry when everything is against your will?", "do you get angry when ordered to do this and that?", "do you feel uneasy by such a trifle thing?", "do you tremble or are you freightened by sudden sound?". The mental and physical symptoms which have appeared presently in connection with the shift service have been agreed with each other. But the physical condition has been worse than the mental one. 3) In the physical health conditions followed by demosociological character, there were the significant differences by sex, religion and place of residence(p<.05), and in the mental health conditions, there were the significant differences by age, marital status, residence place and the required time for attending hospital(p<.05). 4) There was significant difference by the degree of satisfaction about the duty in both the physical and mental health conditions. In short, the higher the degree of duty satisfaction, the better the health conditions. 5) There were the significant difference according to the times of night duty and whether they take the drug or not or the kinds of the drugs in the physical health conditions related with the characters of night shift. Mental health conditions in the night shift case showed significant differences according to their taking drug or not or the kinds of the drugs(p<.05). I can confirm that the nurses have been affected continuously by the shift service mentally and physically. The maintenance of the physical and mental health of the nurses and its promotion are very important problem to guarantee the quality nursing in the performance of the nursing service continuously and effectively, so the hospital should make every effort to improve the duty conditions by finding out the causes affecting to their health. In the nursing management viewpoint, I think that elevating the satisfaction degree about the duty would be a great help to the promotion of physical and mental health conditions. But what is most important is that the nurses themselves should take care of themselves in maintaining the good conditions in their service in the hospital.

정신장애인 자살생각에 영향을 미치는 요인에 관한 연구 (A Study on the Factors Affecting Suicidal Ideation in People with Mental Disabilities)

  • 본 연구의 목적은 정신장애인의 자살생각에 영향을 미치는 요인을 탐색하는 것이다. 이를 위해 독립변수를 개인심리적요인, 사회적요인, 인구사회학적 특성으로 구분하여 분석을 실시하였다. 분석자료는 2017년 보건복지부 연구, '지역사회 정신장애인 현황조사'에 참여한 정신장애인 969명이다. 분석방법은 SPSS 23.0을 이용하여 빈도분석, 기술통계분석, 위계적 다중회귀분석을 실시하였다. 연구결과, 개인심리적 요인에서는 우울이 높을수록, 정신건강자신감이 낮을수록 자살생각이 증가하는 것으로 나타났다. 사회적 요인에서는 미등록장애 집단일수록, 폭력 및 차별 경험에 많이 노출될수록, 대도시에 거주할수록 자살생각이 높아지는 것으로 나타났으며, 이들 변수의 자살생각에 대한 설명력은 26.9%로 나타났다. 이러한 결과를 근거로 도출한 정신장애인의 자살예방에 관한 실천적·정책적 함의는 다음과 같다. 첫째, 정신장애인의 자살생각을 감소시키는 정책과 개입 프로그램을 강구하는데 있어 개인심리적요소에만 초점을 두는 것이 아니라, 사회적요소를 같이 평가하고 고려해야 한다. 둘째, 정신장애인의 자살예방 정책을 강구하는데 있어, 폭력과 차별 경험을 감소시킬 수 있는 권익옹호서비스가 마련되어야 한다. 셋째, 정신장애인의 자살생각을 감소시키기 위해 정신장애등록 제도를 개편하여 정신적 어려움을 경험하고 있는 당사자들이 제도 안으로 포섭될 수 있는 정책적 대안을 강구되어야 한다.

일부 노인의 구강내 상태와 자가인지 구강건강상태와의 관련성 (A Study on the Relationships of Oral Status and Self-perceived Oral Health Status in Elderly)

  • Purpose: The recent proliferation of the aging population, oral health care is closely related to the mental health of the elderly is very important being recognized. Provide basic data for improving the quality of life for the elderly, to determine whether the oral health of the elderly whether oral self-perception of health and any related wish. Methods: Surveys to May 20 from April 15, 2011, subjects were elderly and living in Daejeon and Chungnam 277 people were examined. All the statistics using SPSS 18.0 for the determination of statistical significance, and the significance level was 0.05. Results: 1. Recognized, the most common oral health, self-aware bad the state (46.2%) there was no significant difference in the general characteristics according to self-Oral health status, remaining natural dimensions or the intra-oral prosthetic, there was no significant difference. More teeth, residual phase, in accordance with intra-oral prosthetic mandibular Not all dentures, partial dentures, full dentures net self-perception of oral health status better recognized. 2. According to the type of partial dentures, oral health status self-phase, mixed mandibular removable, fixed, fixed, removable partial dentures in order to recognize that good oral health, self-aware state, showed between them, there was no significant difference. 3. Elderly people of any relevant oral health and the oral cavity by the state to determine whether to provide the basic data for oral health education was more residual value, intra-oral prosthetics there is no more self-aware, the oral health status is good to recognize that appeared. Strengthen the oral health education to improve the quality of life of the elderly, and is thought to be necessary to increase the number of residual.

정신보건사회복지사의 영성과 스트레스 (Spirituality and Stress in Mental Health Social Workers)

  • 목적: 이 연구의 목적은 정신보건사회복지사들의 영성과 스트레스의 수준을 파악하고, 이들 사이의 관계를 조사하여, 호스피스 완화의료 및 정신보건영역 등 휴먼서비스 전문가의 영성증진과 효율적인 스트레스 관리방안을 모색하기 위한 기초자료를 제공하는 데 있다. 방법: 2011년 11월 1일부터 11월 30일까지 국내 정신보건분야에 근무하는 정신보건사회복지사 154명을 대상으로, 한국형 영성척도(Spirituality Scale)와 심리사회적스트레스 척도(Psychosocial Well-being Index Short Form, PWI-SF)를 사용하여 측정하였다. 자료분석은 SPSS WIN 17.0 프로그램을 사용하여 처리하였다. 결과: 대상자의 영성수준은 평균 3.63점, 심리사회적 스트레스 수준은 18.78점이었고, 영성수준은 연령, 교육, 종교유무, 결혼상태에 따라, 심리사회적 스트레스는 결혼상태에 따라 각 집단간 유의한 차이를 보였다. 영성과 심리사회적 스트레스는 부적 상관관계(r=-0.548, P<0.001)가 있었고, 영성의 하위영역인 삶의 의미와 목적, 자비심, 내적 자원, 자각, 연결성, 초월성 모두 심리사회적 스트레스와 각각 부적 상관관계(P<0.01)를 보였다. 심리사회적 스트레스 수준은 정상군에서 영성수준이 가장 높았고, 고위험군에서 가장 낮았으며, 영성은 심리사회적 스트레스의 30.0%를 설명해 주었다. 결론: 본 연구의 결과는 영성과 심리사회적 스트레스는 부적 상관관계가 있으며, 영성의 수준이 높을수록 심리사회적 스트레스가 낮음을 시사해 주었다. 심리사회적 스트레스를 감소시키고, 영성의 수준을 증진시킬 수 있는 영적 감수성 지향의 개입전략이 요구된다.

청소년의 간접흡연이 정신건강에 미치는 영향 (Effects of Secondhand Smoke on Mental Health in Adolescents)

  • Purpose: The purpose of this study was to investigate the effects of secondhand smoke on depression and suicidal ideation in adolescents. Methods: The study used the raw data of the Korea Youth Risk Behavior Web-based Survey (2018). Age, academic achievement, living with family, economic status, and stress were adjusted and the relationship between secondhand smoke and depression and suicidal thoughts was analyzed by gender. Results: The results showed that secondhand smoke had a significant effect on mental health of adolescents. Male students who experienced secondhand smoke at school and at public places had 1.41 times and 1.37 times higher depression rates, respectively. Female students who experienced secondhand smoke at home, school, and public places had 1.27 times, 1.35 times, and 1.39 times higher depression rates, respectively. Suicidal thoughts were 1.17 times and 1.22 times higher among male students who experienced secondhand smoke at school and at public places. Female students who experienced secondhand smoke at home, school, and public places had 1.12 times, 1.08 times, and 1.21 times more suicidal thoughts. Conclusion: Secondhand smoke has a significant effect on adolescent depression and suicidal thoughts. Therefore, efforts should be made to have institutional changes and increased awareness across the country, such as homes, schools, and communities, to prevent exposure to secondhand smoke.
