정신건강유형에 따른 미충족 의료 현황 분석 (Analysis of Unmet Medical Needs according to Mental Health)

  • Objectives : This study investigated the prevalence of unmet health care needs among Korean adults and related factors. Methods : The study participants were adults over the age of 20 mental health experience from the Korea Health Panel in 2012(n=4,730). Statistical analysis methods used in this study were the ${\chi}^2$-test, Logistic Regression Analysis and other basic statistics such frequency-and percentage using SPSS version 22.0. Results : (1)Significant variables of stress: Factors were age, economic activity, subjective health status, and activity limitation. (2)Significant variables of depression: Factors were age, income class(low) and activity limitation. (3)Significant variables of suicidal impulse: Factors were age, chronic diseases, income class, and activity limitation. Conclusions : Stress, depression, and suicidal impulse can be unmet medical factors; therefore improvement measures and mental health counseling programs in response to suicide impulses, should be developed. Thus there is a need for a health sciences approach.

Conjugal Violence and Mental Health of Korean Elderly Women

  • The purposes of this article is to assess the current status of conjugal violence among elderly women and to examine the relationship between conjugal violence and the mental health status of elderly women in Korea. The results of this study are; (1) the largest proportion (44.9%) of elderly women experienced verbal violence, followed by minor physical violence (35.5%), severe physical violence (13.1%), and sexual violence (5.6%); (2) the mental health status of elderly women who have been exposed to violence was worse than that of elderly women who have not. In conclusion, this article argues that interventions for better marital relationships among elderly couples are needed and comprehensive programs for improvement of mental health are also needed for elderly women in general.

  • The primary objective of this research is to utilize machine learning models to analyze factors tailored to each dataset for predicting mental health conditions. The study aims to develop appropriate models based on specific datasets, with the goal of accurately predicting mental health states through the analysis of distinct factors present in each dataset. This approach seeks to design more effective strategies for the prevention and intervention of depression, enhancing the quality of mental health services by providing personalized services tailored to individual circumstances. Overall, the research endeavors to advance the development of personalized mental health prediction models through data-driven factor analysis, contributing to the improvement of mental health services on an individualized basis.

학교기반 정신건강정책의 흐름 분석: Kingdon의 정책흐름모형을 중심으로 (Analysis of School-based Mental Health Policy Stream based on Kingdon's Policy Stream Model)

  • Purpose: This study aims to analyze the factors affecting the agenda-setting process and the formation process of school-based mental health policies by applying a policy stream model. Methods: For this purpose, Kingdon's policy stream model was used as the analytical framework. Results: First, when establishing a school-based mental health policy, the agenda was set going through unpredictable and nonlinear changes. Second, for the school-based mental health policy to be selected onto the agenda and to be developed and implemented as an actual policy, the role of policy makers was considered most important in the process. Third, the policy window for school-based mental health policy was closed around the year 2013. Finally, an analysis of the school-based mental health policy stream identified two key features. One is that the school-based mental health policy first emerged when school violence prevention policy expanded its scope into relevant neighboring policies. The other is that the school-based mental health policy has taken shape through a linear decision-making process (being put on the government's agenda, searching for an alternative, selection, and implementation) during the policy implementation period after it has been selected as an alternative policy. Conclusion: Conclusions can be summed up as follows. The school-based mental health policy needs continuous development and improvement in case the window for the policy may open in the coming future. The government's support is needed to draw policy makers' interest and participation who play the biggest role in establishing policies.

An Integrative Review on Augmented Reality/Virtual Reality Simulation Programs in the Mental Health Area for Health Professionals

  • This is an integrative review paper of Augmented Reality (AR)/Virtual Reality (VR) simulation programs in the mental health area including the analysis of the general characteristics, contents, and the impact of the interventions studies. The keywords used to search the studies were "AR/VR" and "medical/nursing students". The author and a postdoctoral research fellow searched four electronic databases: Web of Science, PubMed, EmBase, and CINHA, and as a result nine studies met the inclusion criteria. Among the selected studies AR/VR simulation programs in the mental health area for healthcare professionals were found to be effective in clinical skills as well as for the interpersonal relationship and the stigma of mentally ill patients. Providing an opportunity to experience a safe and effective tool is important when educating health professionals and AR/VR simulation programs are safe and effective. Thus, standardized AR/VR simulation programs are needed to be developed for health professionals.

Mindfulness-based Practices in Workers to Address Mental Health Conditions: A Systematic Review

  • The effectiveness of mindfulness techniques in addressing mental health conditions in workers is uncertain. However, it could represent a therapeutic tool for workers presenting with such conditions. Our objective was to assess the effects of mindfulness-based practices for workers diagnosed with mental health conditions. We conducted a systematic review of randomized controlled trials. Participants included were workers with a mental health condition. Interventions included any mindfulness technique, compared to any nonmindfulness interventions. Outcomes were scores on validated psychiatric rating scales. A total of 4,407 records were screened; 202 were included for full-text analysis; 2 studies were included. The first study (Finnes et al., 2017) used Acceptance and Commitment Therapy (ACT) associated or not with Workplace Dialogue Intervention (WDI), compared to treatment as usual. At 9 months follow-up, for the ACT group, depression scores improved marginally (standardized mean difference [SMD]: -0.06, p = 0.021), but anxiety scores were worse (SMD: 0.15, p = 0.036). Changes in mental health outcomes were not statistically significant for the ACT + WDI group. In the second study (Grensman et al., 2018), no statistically significant change in mental health scales has been observed after completion of mindfulness-based cognitive therapy compared to cognitive behavioral therapy. Substantial heterogeneity precluded meta-analysis. This systematic review did not find evidence that mindfulness-based practices provide a durable and substantial improvement of mental health outcomes in workers diagnosed with mental health conditions.

한의 난임 치료의 심신 기능 개선 평가를 위한 표준 평가도구 개발에 대한 임상 한의사 대상 기초 설문조사 (A Basic Survey of Korean Medicine Doctors to Develop Standard Evaluation Tools for Evaluating of Mental and Physical Function Improvement following Korean Medicine Infertility Treatment)

  • Objectives: The purpose of this study is to investigate the perception of Korean Medicine (KM) doctors toward infertility treatment evaluation tools and develop a standard evaluation tool for mental and physical function improvement following Korean medicine treatment. Methods: An online survey (Moaform) was conducted, and responses were analyzed for members of the Society of Korean Medicine Obstetrics and Gynecology and KM doctors who participated in the 2021-2022 traditional Korean medicine treatment infertility support program in Gyeonggi-do. Results: In fertility treatment, the most effective indicators that KM doctors wanted to evaluate other than pregnancy and childbirth were reproductive health indicators, overall health improvement, and stress improvement (33.1%, 21.2%, 18.9%). Furthermore, 45.1% of the respondents had experience using the presented infertility stress evaluation tools. For each evaluation tool, 52.1% and 54.6% of the respondents answered that it 'does not reflect the changing circumstances of the times' and that there were 'too many questions'. As for the developing a standard evaluation tool for infertility treatment in KM, KM doctors expected a simple and clear tool focusing on the pregnancy rate. They also wanted the tool to include a reproductive health index, overall function improvement and stress improvements. Conclusions: The survey confirmed the need to evaluate indicators for reproductive health, mental health, and infertility stress in addition to pregnancy rate to assess the effectiveness of KM infertility treatment. It also confirms the necessity of developing an infertility stress evaluation tool with simplified questions that reflects the changing time.

Examples of Holistic Good Practices in Promoting and Protecting Mental Health in the Workplace: Current and Future Challenges

  • Background: While attention has been paid to physical risks in the work environment and the promotion of individual employee health, mental health protection and promotion have received much less focus. Psychosocial risk management has not yet been fully incorporated in such efforts. This paper presents good practices in promoting mental health in the workplace in line with World Health Organization (WHO) guidance by identifying barriers, opportunities, and the way forward in this area. Methods: Semistructured interviews were conducted with 17 experts who were selected on the basis of their knowledge and expertise in relation to good practice identified tools. Interviewees were asked to evaluate the approaches on the basis of the WHO model for healthy workplaces. Results: The examples of good practice for Workplace Mental Health Promotion (WMHP) are in line with the principles and the five keys of the WHO model. They support the third objective of the WHO comprehensive mental health action plan 2013-2020 for multisectoral implementation of WMHP strategies. Examples of good practice include the engagement of all stakeholders and representatives, science-driven practice, dissemination of good practice, continual improvement, and evaluation. Actions to inform policies/legislation, promote education on psychosocial risks, and provide better evidence were suggested for higher WMHP success. Conclusion: The study identified commonalities in good practice approaches in different countries and stressed the importance of a strong policy and enforcement framework as well as organizational responsibility for WMHP. For progress to be achieved in this area, a holistic and multidisciplinary approach was unanimously suggested as a way to successful implementation.

웨어러블 기기를 이용한 군 장병 정신건강 관리 향상 방안 (A Study on Improvement of Soldier Mental Health Care Using Wearable Devices)

  • 우리나라는 출산율 감소로 인한 인구 절벽 현상으로 병역자원이 계속 줄어들고 있다. 군 병력의 감소에도 불구하고 장병들의 정신건강 악화로 인해 발생하는 사고는 오히려 증가하고 있다. 장병들의 정신건강은 개인의 문제이기도 하지만, 부대에도 큰 영향을 미칠 수 있다. 개인의 정신건강 악화로 인해 자살, 이혼, 군무이탈 등의 사고는 부대의 전투력 발휘 제한과 사기 저하로 이어지기 때문이다. 현재 국내·외 학계에서는 웨어러블 기기를 이용한 정신건강 관리 방안이 연구되고 있다. 우리 군 역시 이를 활용하여 장병의 정신건강 상태를 객관적으로 측정하고, 축적된 빅데이터를 활용하여 다양한 상황을 예측, 조치할 수 있는 장병 정신건강 관리의 보조도구로 활용할 수 있다. 이에 본 연구에서는 웨어러블 기기를 활용한 군 장병의 정신건강 관리 방안을 제안하고 후속 연구가 원활하게 이루어져 우리 장병들의 정신건강이 향상되길 기대한다.

지역사회 공공보건자원과 지역사회 정신건강의 관계에서 공공보건서비스 이용률의 조절효과 (Moderating Effects of Public Health Service Utilization Rate between Public Health Resources and Community Mental Health)

  • 본 연구는 공공보건자원이 지역사회의 정신건강수준에 영향을 미치는 과정에서 공공보건서비스 이용률이 조절 역할을 하는 지를 확인하고자 수행되었다. 연구대상은 144개의 지역사회이며, 자료는 제6기 지역보건의료계획과 2015년 지역사회 건강조사에서 확보하였다. 연구변수 중 공공보건자원은 공공보건예산, 공공정신보건예산 및 공공정신보건인력으로 구성하였으며, 정신건강수준은 스트레스 인지율, 우울감 경험률 및 자살률로 하였다. 조절효과 분석에 사용된 방법은 위계적 회귀분석이었다. 연구결과는 첫째, 공공보건서비스 이용률은 공공정신보건예산이 우울감 경험률에 영향을 미치는 과정에서 조절변수로 작용하고 있었다. 둘째, 공공정신보건인력이 우울감 경험률에 영향을 미치는 과정에서 공공보건서비스 이용률은 조절효과를 나타냈다. 셋째, 공공정신보건인력이 자살률에 영향을 미치는 과정에서도 공공보건서비스 이용률은 조절효과를 나타냈다. 넷째, 공공보건자원의 정신건강수준에 대한 영향이 공공보건서비스 이용률이 높은 집단과 낮은 집단에 따라 다르게 나타났다. 결론적으로 공공보건자원 강화가 지역사회 정신건강수준의 향상으로 이어지도록 하기 위해 지역사회 정신건강증진 방안에 공공보건서비스 이용률 개선을 위한 내용을 포함시킬 필요가 있을 것이다.
