A Study of Protein Nutritional Status and Bone Metabolism of Postmenopausal Vegetarian Women (채식을 하는 폐경 후 여성의 단백질 영양상태와 골대사)

  • Recently, interests in the influences of vegetarian diet on bone mineral density after menopause have been rapidly increased. The purpose of this study was to compare the protein nutritional status and bone mineral density of postmenopausal vegetarian women with that of the omnivores. Vegetarian (n=38, seven day adventists) were chosen from the subjects in previous study, and the subjects were matched with omnivores counterparts with respect to age and BMI. Anthropometric measurements, dietary intakes, and bone mineral density (BMD) were taken. The bone metabolism related marker including urinary deoxypyridinoline and urinary pH, and serum protein and albumin concentrations were evaluated. The average age of vegetarians and omnivores were 60.7 yrs and 60.5 yrs, respectively md, there was no significant difference. The mean daily energy intake of vegetarians and omnivores were 1518.5 ㎉ (82.7% of RDA) and 1355.5 ㎉ (72.6% of RDA), respectively. The mean calcium intake of vegetarians (492.6 mg, 70.3% of RDA) was not significantly different from that of omnivores (436.6 mg, 62.3% of RDA). There was no significant difference in BMDs of spine and femoral neck between vegetarians and omnivores. Urinary deoxypyridinoline (DPD) level was not significantly different. In the vegetarians, the intakes of total protein (p<0.05) and plant protein (p<0.05) had significant negative correlations with urinary DPD. In the omnivores, serum albumin showed significant positive correlations with urinary DPD (p<0.05). In conclusion, we can not find the beneficial roles of vegetarian diet on bone mineral metabolism. For the postmenopausal vegetarian woman, protein intake would be an important factor to promote skeletal health.

The Effects of Ca and Mg Supplementation on Serum and Liver Lipid Parameters in Ovariectomized and Ca Deficiency Rats (난소절제 칼슘결핍 흰쥐에서 칼슘과 마그네슘의 보충이 혈액과 간의 지질함량에 미치는 영향)

  • This study was carried out to investigate the Ca and Mg supplementation on the serum and liver lipid parameters in ovariectomized and Ca deficiency rats. Total 50 Sprague Dawley female rats (6 weeks) were divided into 5 groups and bred for 12 weeks: sham operated control group (SNCa), OVX Ca deficiency poop (OLCa) with Ca deficiency diet (0.1% Ca modified AIN-93N diet), OVX Ca deficiency & Mg supplement group (OLCaMg), OVX adequate Ca group (OACa; 0.5% Ca AIN-93N diet) and OVX adequate Ca & Mg supplement group (OACaMg). There were no significant difference among the five groups in serum total cholesterol, triglyceride and HDL-cholesterol levels. LDL-cholesterol of OVX groups was significantly higher than that of SNCa group (p < 0.01). AI (Atherogenic index), TPH (Total cholesterol/HDL-C) and LPH (LDL-C/HDL-C) of OACa group were significantly lower than those of OLCa groups. OACaMg group had significantly lower levels LDL, AI and TPH than OLCa group. There was no significant difference in lever cholesterol level. However, liver total fat content of OACa was significantly lower than that of OLCa. From the above the results, it is concluded that the accumulation level of calcium shows how the supplement of magnesium affects hyperlipidemia. Therefore, in order to prevent women#s hyperlipidemia after menopause, and to keep healthy, it is encourage able to consider how the supplement of magnesium relates calcium intake.

The Associated Factors with Whole Saliva Flow Rate and Xerostomia (전타액분비율과 구강건조증의 관련요인 분석)

  • The aim of this study was to analyze the factors related to whole saliva flow and the xerostomia for the ground use of the materials in developing oral health improvement program and life quality improvement program. So, this study performed a questionnaire survey, targeting at 160 adults, older than 40 years, living in Gyeonggi and Incheon provinces from January to February 2011 and measured unstimulated salivary flow rate and stimulated salivary flow rate. The results are as followings. The group of participants who took gums and candies to relieve oral dryness had low stimulated whole saliva flow. The group of participants who graduated from middle schools, who separated by death, who were not healthy with menopause, and who recognized periodontal disease symptoms and tongue burning sensation recognized more symptoms of dry mouth than others. In addition, the group of participants who were not satisfied with life so much, who couldn't feel the meaning of life, and who recognized negative feelings frequently recognized symptoms of dry mouth more. Women, the group of participants who graduated from middle schools, who suffered from depressive symptoms, and who recognized halitosis had low unstimulated whole saliva flow. The group of participants who graduated from middle schools, who were not satisfied with life so much, who couldn't feel the meaning of life, and who were not satisfied with themselves had low stimulated saliva flow.

Sanjoin-tang (Suanzaoren decoction) for Insomnia in Menopausal Syndromes: A Systematic Review and Meta-analysis (산조인탕치료가 여성갱년기 수면장애에 미치는 효과 : 체계적 문헌고찰 및 메타분석)

  • Objectives : The purpose of this systematic review was to investigate the efficacy and safety of Sanjoin-tang (Suanzaoren decoction, SZRD) for insomnia in menopausal syndromes. Method : We searched the following databases: PubMed, Cochrane Library, EMBASE, China National Knowledge Infrastructure (CNKI), Chinese Science and Technique Journals Database (VIP), Wanfang, Research Information Service System (RISS), OASIS, Korean studies Information Service System (KISS), and National Digital Science Library (NDSL) databases. Randomized controlled trials (RCTs) or quasi-RCTs that used SZRD for women in menopause were included. The methodological quality of each RCT was assessed using the risk of bias tool. Results : In total, nine RCTs were finally included. The included RCTs had a high risk of bias across their domains. Three RCTs examined the effects of SZRD compared with western medicine (WM) in insomnia. Three RCTs showed favorable effects of SZRD with insomnia. Infrastructure also showed that SZRD resulted in better clinical WM (RR 1.19, 95% CI 1.07 to 1.32, P=0.001, I2=0%). Four RCTs showed favorable effects of Modified SZRD with insomnia. Infrastructure also showed that Modified SZRD resulted in better clinical WM (RR 1.13, 95% CI 1.04 to 1.23, P=0.004, I2=0%). Among the 4 RCTs, three RCTs showed an equivalent effect on the total Pittsburh Sleep Quality Index (PSQI). The meta-analysis also showed that Modified SZRD had a superior effect on the total of PSQI (total of PSQI : MD -2.55, 95% CI -3.72 to -1.37, P<0.0001, I2=85%). Only 2 trials reported adverse events and none reported severe adverse events. Conclusion : SZRD appears to be safe, but there is insufficient evidence to make a definitive conclusion because only a few studies reported adverse events. Due to the poor methodological quality of the included studies and the small number of trials included, the evidence cannot be reproduced and assessed. Well-designed RCTs with a larger sample size are needed in the future.

Change in the Expression of Occludin, a Gene for Blood-Brain Barrier by Phytoestrogens in Hippocampus of Rat Model for Menopause (폐경기모델 백서 해마에서 식물성 에스트로젠에 의한 뇌-혈액장벽 유전자 occludin 발현의 변화)

  • To elucidate the effect of phytoestrogens on the prevention of neurodegenerative disease in postmenopausal women, the expression of occludin which build up the blood-brain barrier was examined in hippocampus following oral administration of estrogen (E2), genistein, diadzein or combination of genistein and diadzein in ovariectomized (OVX) female rats. E2 significantly increased occludin mRNA level in OVX rat hippocampus, suggesting that estrogen is a physiological regulator for structural integrity of the blood-brain barrier in hippocampus. Following isoflavone diet for 4 weeks, there was significant increase in occludin mRNA level in hippocampus, suggesting that isoflavone diet may be effective for protection of structural integrity of blood-brain barrier in hippocampus from degenerative changes in estrogen deficiency.

A Study on Health Promoting Behavior In Post-Mastectomy Patients (유방절제술을 받은 여성의 건강증진행위에 관한 연구)

  • The purpose of this study was to examine the relationship among perceived health status, self-esteem, self-efficacy and health promoting behavior, and to determine the predictors of health promoting behavior in post-mastectomy patients. The study, a descriptive correlational study, was done with structural questionnaires. A total of 51 post-mastectomy subjects from C university hospital in Kwang-ju, South Korea completed mail-in self-reporting questionnaires during a three month period from March to June, 1999. The data were collected using Lawstone's(1982) perceived health status scale, Rosenberg's(1965) self-esteem scale, the modified self-efficacy scale(Shere et al, 1982), and the modified health promoting lifestyle profile (Walker et al, 1987). The data obtained were analyzed according to percentage, mean and standard deviation, principal component analysis, varimax rotation, t-test, ANOVA, Pearson's correlation, and stepwise multiple regression. The results were as follows: 1. The health promoting behavior measurement resulted in six factors. Each factor was labelled as follows: self-actualization, nutrition, stress management, exercise, health responsibility and interpersonal support. The total percent of variance explained by the six factors was 58.4%. 2. The mean score of health promoting behavior was 85.92(range 58~117). The scores of six factor were nutrition 3.20, self-actualization 2.59, stress management 2.58, interpersonal support 2.58, health responsibility 2.49, and exercise 2.34 on a four point scale. 3. When the score of health promoting behavior factors were compared by general characteristics. Factor I: self-actualization, differed significantly by the frequency of pregnancy (F=3.06, p=.037). Factor II: nutrition differed significantly by drinking experience(t=-2.26, p=.028) and the pre- or post stage of menopause(F=2.69, p=078). FactorIII: stress management differed significantly depending on regularity of mensturation(t=-2.12, p= .042). FactorIV: exercise differed significantly by type of religion (F=2.49, p=.072), marital status(F=5.03, p=.010), and feeding type (F=2.64, p=.036). Factor V: health responsibility differed significantly by regularity of mensturation(t=2.18, p=.037). 4. The total health promoting behavior score was significantly related to self-esteem and perceived health status(r=.610, p.006; r= .378, p=.006). The score of selfactualization also corresponded with selfesteem and perceived health status(r=.556, p=.001; r=.343, p=.013). 5. The predictor to explain the score of health promoting behavior was self-esteem, which accounted for 37.1% of the total variance. The predictor to explain the score of self-actualization was self-esteem, which accounted for 30.9% of the total variance. The score of nutrition was primarily affected by both premenopause and drinking experience, which accounted for 13.1% and 9.5% respectively. Finally, the score of exercise was dictated by marriage, Buddhism, no experience of breast feeding, which accounted for 17%, 9.8%, & 5.2% respectively. In conclusion, self-esteem is the main predictor for health promoting behavior in post-mastectomy women. These findings suggest a need for nursing strategies which promote self-esteem in such patients.

The Role of Radiation Therapy in Adenocarcinoma of Endometrium (자궁 체부암의 방사선 치료)

  • From May, 1979 to October,1987, 38 patients with endometrial carcinoma were treated with radiotherapy at Seoul National University Hospital. Of these, 32 patients received radiotherapy Postoperatively, one received radiotherapy preoperatively, and five received radiotherapy only. Relative frequencies of obesity, nulliparity, late menopause, diabetes mellitus, and hypertension were all higher than those reported in normal women in Korea, and those of obesity, diabetes mellitus, and nulliparity were significantly high. The overall actuarial five-year survival rate was $75.6\%$. The overall actuarial five year survival rates of stage I (22 cases), stage II (six cases), and stage III (10 cases) were $90.0\%,\;80.0\%,\;and\;44.4\%$, respectively, and were significantly different from each other. Among various factors, stage only proved to be prognostic by multivariate analysis. There were two local failures, three local failures combined with distant metastasis, and three distant metastasis. Stages I and II could be adequately controlled by local modalities without severe complications, stage III endometrial carcinoma might need more aggressive treatment for better survival.

Water extracts of Eucommia ulmoides improve lipid, glucose, and antioxidant metabolism in ovariectomized rats (두충 열수추출물 급여에 의한 갱년기 유도 모델 흰쥐의 지질, 당질 및 항산화 대사 개선에 미치는 영향)

  • In postmenopausal women, estrogen deficiency can be associated with metabolic diseases, such as obesity, diabetes, and cardiovascular diseases. The purpose of this study was to investigate factors related to lipid, glucose and antioxidnat metabolism. Sprague-Dawley female rats were subjected to either bilateral ovariectomy or sham operation and randomly divided into 3 dietary groups, (n=8) sham-operated fed with normal diet (SHAM), ovariectomized fed with normal diet (OVX), and ovariectomized fed with normal diet supplemented with Eucommia ulmoides extract (OVX-EU). The OVX-EU group showed significantly lower body weight, triacylglycerol, and total cholesterol than that showed by the OVX group. In addition, the OVX-EU group showed improved lipogenesis, glucose-regulating enzyme activities, adipokine and antioxidant enzyme activities. These findings demonstrate that extracts from E. ulmoides extract can be used as a functional food.

Current Medical Therapies for Osteoporosis and Its Alternative Treatments Using Natural Products (골다공증 치료법과 천연물을 이용한 대체요법)

  • Osteoporosis is a major bone disorder defined as having bone mineral density (BMD) of 2.5 standard deviations or more below the peak bone mass. Osteoporosis will increasingly be a major disorder that faces the aging mankind. It is the result of an imbalance in the bone remodeling system, where bone constantly undergoes a cycle of resorption by osteoclasts and formation by osteoblasts. Estrogen deficiency in women following menopause is identified as the predominant reason that causes disparity in this system. Current medical treatments for osteoporosis include hormone replacement therapy (HRT), biphosphonates, and teriparatide, but have various side effects that raise questions concerning their medical safety and practicality. Alternative treatments involving natural product sources are under study to find a safer therapy. Many natural sources including lactoferrin and isoflavones and numerous traditional herbal medicines exhibit anti-resorptive or anabolic effects on bone and thus show promises to provide therapeutic agents in treating osteoporosis. Unfortunately, the majority of natural product treatments are still in its preliminary stages to prove their efficacy even though the development pace of treatment for osteoporosis is astounding in the past few decades. Further progress in pre-clinical studies and the subsequent clinical studies will someday lead to a breakthrough that takes us another step forward in science.

Analysis change in Bone Mineral Density before and after Kidney Transplant in Renal Failure Patient (신부전환자의 신장이식 전후 골밀도변화 분석)

  • Disease, such as osteopenia, osteoporosis, etc caused by reduced bone density are common to women after menopause and as the social medical cost increases due to osteoporosis fractures the medical interest in bone density reduction has increased. The bone density reduction is observed even for renal failure patients, due to their decreased ability to synthesize vitamin D which leads to bone fibrosis because of deficiency in calcium absorption. Thus renal failure patients not only suffer from kidney dysfunction, but also are exposed to complications, such as osteoporosis, due to reduced bone density. This research observed the change in bone density of patients receiving renal failure treatment and analyzed the change in bone density before and after kidney transplantations. Subjects were 214 renal failure patients at the department of nephrology Busan B General Hospital. The change in bone density was studied for subjects with and without kidney transplantation according to their age and sex. The research showed improvement or maintenance of bone density for subjects that received kidney transplantation, but showed a tendency of consistent decrease in bone density for subjects without kidney transplantation. Kidney transplantation can be considered as the best cure for renal failure patients, and this researched confirmed that bone density can be improved through kidney transplantation. Thus, this study can also be used as data for preventing complications due to renal failures.
