수지요법이 갱년기 여성의 폐경증상과 FSH, LH 및 Estradiol에 미치는 효과 (Effects of Koryo Hand Therapy on Menopausal Symptoms and FSH, LH, and Estradiol in Climacteric Women)

  • Purpose: The purpose of this study was to examine the effects of Koryo Hand Therapy (KHT) on menopausal symptoms and hormone levels (Follicle Stimulating Hormone [FSH], Luteinizing Hormone [LH], and estradiol [$E_2$]) in climacteric women. Methods: The research design was a nonequivalent control group pretest-posttest design. Data were collected from November 28, 2005 to February 28, 2006. The 45 participants were assigned to either the experimental group (23) or control group (22). KHT was applied three times a week, for a total of 8 weeks to the women in the experimental group. Results: There was a statistically significant decrease in menopausal symptoms (F=42.49, p=.000), FSH level (F=26.98, p=.000) and LH level (F=5.31, p=.026) between the experimental and control groups. There was an increase of the estradiol level in the experimental group but the difference between the two groups was not statistically significant. Conclusion: KHT can be applied as a supportive nursing intervention to climacteric women. KHT is expected to be a complementary alternative intervention for health management of the climacteric women.

  • Objectives: Hormone replacement therapy may relieve many symptoms of menopause; however, hormone replacement therapy may increase the risk of breast cancer, stroke, and dementia. This case study describes the effect of Hoechunboeum-tang on the menopausal disorder of hot flashes, which has been well documented as a cause of distress to patients undergoing menopause. Methods and Results: Two female patients suffering from severe hot flashes related to menopausal disorders were administered the herbal medicine, Hoechunboeum-tang. Measurements included hot flash frequency over a 24 hour period, hot flash severity (visual analogue scale), and hot flash-related daily interference and were recorded during the last week before herbal medicine and again after 7 and 15 days of herbal treatment. The patients were asked about their hot flash symptoms 6 months later and both reported that the symptoms had been successfully reduced after herbal treatment and were still reduced after 6 months. Conclusions: Hoechunboeum-tang could have a beneficial effect on menopausal disorder with hot flashes.

  • Menopausal women experience urogenitory and vasomotor symptoms with increased risk of osteoporosis and cardiovascular diseases, which can be reduced by hormone replacement therapy. However unopposed estrogen therapy has been associated with an increased risk of endometrial hypeiplasia or cancer. The objectives of this study were to compare effects of continuous vs. sequential hormone replacement therapy (HRT) on lipid profile, bone mineral density and menopausal symptoms of postmenopausal women and to assess how they perceive the menopause and HRT culturally. In this retrospective study, women in menopause longer than 6 months, normal in the mam-mogram and Papanicolaou smear, cholesterol level lower than 190 mg/dL or triglyceride level lower 4han 500 mg/dL were treated with Srogen (conjugated equine estrogen 0.625 mg tablet) and Provera (medroxyprogesterone acetate 2.5 mg tablet) for continuous treatment(CT) or Cycloprogynova (Estradiol valerate 2 mg and Norgestrel 0.5 mg complex tablet) for sequential treatment(ST). They were evaluated for lipid profile, bone mineral density, menopausal symptoms, side effects and their perception of menopause and HRT. As results, total sixty-seven patients out of ninety-four enrollees were included in final analysis (33 in continuous therapy, 34 in sequential therapy). There were significant decrease in total cholesterol ($15.04\pm3.17$, p=0.0001), LDL ($19.72\pm3.27$, p=0.0001), and increase in HDL ($5.89\pm1.63$, p=0.0001). Bone minora) density increased significantly with HRT ($0.02\pm0.11$, p=0.0001). But, there were no significant differences in change of lipid profile between continuous and sequential therapy: Total cholesterol, $13.12\pm4.7\;vs.\;16.91\pm4.3;\;LDL\;20.53\pm4.1\;vs.\;18.93\pm5.12:HDL\;7.15\pm2.3\;vs.\;4.67\pm2.2,\;p>0.05$. Incidences of flush reduced from $75\%\;(CT)\;to\;3.13\%\;and\;71.88\%\;(ST)\;to\;9.35\%$. The change of endometrium and breast were found 3 (CT) and 5 (ST) women, respectively. Most of women recognized that HRT is necessary $(70\%)$ for postmenopausal period but did not understand well the cardiovascular protective effect. In conclusion, hormone replacement therapy was effective in improving lipid profile, bone mineral density and menopausal symptoms in both continuous and sequential treatments with similar efficacy.

호르몬대체요법 환자의 복약실태분석 (Analysis of Drug Compliance Status for the Hormone Replacement Therapy (HRT) Treatment Patients)

  • This study is intended to provide information to menopausal women so they can control their symptoms. The basic data was gathered by interviewed 200 postmenopausal women on a HRT program. The women were questioned about their perception of menopause and compliance to the therapy with the fellowing result $71.7\%$ of the women experianced hot flushes, and $64.1\%$ experienced short term memory loss and some psychological symptoms. The women reported that these symptoms reduced their quality of life. $53\%$ of the women responded that menopause is a natural state and $59.1\%$ agreed to HRT based on their doctor's recommendation. $76.2\%$ of the women reported that HRT was effective, with $74.7\%$ reporting reduced hot flush symptoms $67.3\%$ of the women reported compliance with the medication schedule. and $43.9\%$ reported periodic non-compliance. Although HRT has been shown to be an effective treatment, the study found that less than $10\%$ of postmenopausal women are currently being treated and these treated women are generally negligent in continuing with HRT, The study recommends that a program be developed that explains the benefits and risks of HRT to be distributed to patients being treated by doctors and pharmacists. In conclusions, It is very important for hormone replacement therapy on menopausal women to take more intensive medication consultation to increase medication compliance and effectiveness of pharmacotherapy.

작물에 함유된 Phytoestrogen의 특성과 생리활성 (Biological Activities of Phytoestrogens in Plant and Foodstuff)

  • Phytoestrogens are oestrogenic compounds found in plants and consist of isoflavones, lignans, and coumestans. The structural similarity of phytoestrogens to endogenous oestrogens has promoted the hypothesis that phytoestrogens exert hormonal or anti-hormonal effects relevant to the risk of hormone-dependent disease and/or their suitability as a dietary alternative to hormone replacement therapy. Epidemiological studies suggest that food stuffs containing phytoestrogens may have a beneficial role in protecting against a number of chronic disease and conditions. It is thought that these estrogen-like compounds may protect against chronic diseases, such as hormone-dependent cancers, cardiovascular disease and osteoporosis. Furthermore, phytoestrogens are used as a natural alternative to hormone replacement therapy and to reduce menopausal symptoms. Phytoestrogens are considered good candidates for use in natural therapies and as chemopreventive agents in adults. However safe and efficacious levels have yet to be established.

  • Korean women are now living almost 1/3 of their life after menopause. Sex-steroid hormone deficiency adversely affect various fields of physical & mental activity and quality of life during this period. Therefore, replacement of deficient hormone is thought to be natural therapeutic modality. Postmenopausal syndrome is recently redefined as an endocrinopathy with both short-and long-term sequelae, as a result of cessation of ovarian function. Hormone replacement therapy taken at or near time of menopause alleviates shot-term acute menopausal symptoms such as vasomotor disturbances and psychological problems. HRT also beneficially affects some of intermediate symptoms such as urogenital atrophy and cutaneous problems. The major benefits of long-term use were reductions in risk of long-term sequelae, that is, total fracture by 50-60%, cardiovascular disease by 50% and cerebrovascular disease by 30-40%, respectively. In addition, HRT may also positively influence Alzheimer's disease, reduce the colorectal cancer risk and increase longevity of the life. In conclusion, all postmenopausal women should consider preventive HRT when there are no contraindications.

  • Isoflavones are reported to playa role in menopausal women as a phytoestrogen, which can replace estradiols in hormone replacement therapy (HRT). Recently; due to the risk of breast cancer by HRT, phytoestrogens (e.g. isoflavones) have been focused as an alternative therapy in menopause. In the study, we investigated whether isoflavones derived from Sophorae fructus (SISO) have more benefit than that of soybean isoflavones in estrogen deficient rats. We found that SISO effectively controled $H_2O_2$ comparing with the baseline (p<0.01 vs. post value of OVX-Cont), and the blood sugar and weight were also controlled with decreasing patterns. Additionally, in LDH assay for cytoprotective effect in Neuro-2a cell line, SISO protected cells from the damage by SNAP (p<0.05). In conclusion, SISO may have more beneficial effect in enhancing the menopausal signs than that of soybean isoflavones and the cytoprotective effect in neuron cells suggests that SISO can play a certain role in neuroprotection after menopause.

갱년기 장애의 심한 근골격계 통증을 생활사건 스트레스와 연관지어 고찰한 치험1례 (A Therapeutic Case Report of Serious Musculoskeletal Systemic Pains of the Menopausal Disorder Studied in Connection with the Life Event Stress)

  • Purpose: This paper is aim to report that menopausal disorder can be effectively diagnosed and treated by referring to the life event stress of patient. Methods: Researchers got a grip on the life event stress of patient by using medical examinations by interview and compared the conditions of patient before and after treatment by using Kupperman's Index. The prescription of the Gamidangguisu-san hap samhap-tang( Jiaweidangguixu-san he sanhetang) and the Gamisoyo-san (Jiaweixiaoyao-san) and the acupuncture therapy and the moxibustion therapy were used for the treatment. Results: The serious musculoskeletal systemic pains, which includes back pain, lumbago, and myalgia, and other menopausal troubles were improved remarkably. Conclusion: The menopausal disorder could be caused by not only the change of hormone but also complex factors of environmental matters which includes social cultural surroundings, mentality etc. Therefore the life event stress could be one of critical factors of menopausal disorder. The diagnosis and treatment considering the life event stress could be effective method to relieve patient's menopausal disorder.

A Possible Role of Ginseng Root for Maintaining a Quality of Life in Womens

  • Chilliness is a complaint among menopausal women in Japan, but increasing evidence indicates that young women also suffer from chilliness. However, neither diagnostic criteria nor drug therapies exist for treating chilliness. We noticed it and recently elucidated methodological issues related to identifying women with chills. Using a discriminant analysis, we demonstrated that four parameters were important determinants of chilliness. On the other hand, after repeated reports of serious adverse effects with hormone replacement therapy, much attention has been given to the development of new remedies to alleviate menopausal depressive state in women, but methods for their preclinical evaluation have not been clarified. We previously developed a procedure to predict the effect of candidate substance on the menopausal depressive-like state in female mice. Since either chilliness or mood disorder is known to disturb a quality of life in women, in this symposium, the efficacy of Korean red ginseng on chilliness in women and depressive-like state will be discussed.

대체의학에서 갱년기 증후군에 사용되는 약물에 대한 고찰 (Review of Herbs Used as Treatment for Menopausal Patients in Alternative Medicine)

  • Purpose: Menopausal syndrome is caused by urgent changes of sex hormone in women. The hormonal therapy can soothe the problems of menopausal patients, but it can increase the probability of another disease, such as breast cancer, heart attack, etc. So the demand of alternative herb therapy is increasing in Western countries. In Korea, herbal medicine is imported and used by Western doctors. To know and use the alternative herbal medicine, Korean Medical doctors should know their efficacy and safety. Methods: We researched the Medline(Pubmed) with the key word such as 'CAM', 'herb' and 'menopause'. We selected the herbs known as treating the menopausal syndrome and researched the similar or same herbs that are used in Korean medicine by the scientific name and efficient ingredients. To evaluate the efficacy of the herbs, we compared the herbs that were differently defined in alternative and Korean medicine Results: Balm, black cohosh, chaste berry, dong quai. evening primrose oil. ginkgo, St. John's wort, valerian root, and sarsaparilla were chosen as prevalent herbs used in alternative medicine. The pharmacologic effect of Black cohosh, licorice root, St. John's wort were recognized. But these three herbs are not usually prescripted for menopausal patients in Korean medicine. So we could apply these three herbs as one of Korean medical treatments. The efficacy of other herbs cannot be surely prescripted for menopausal patients. Conclusion: The efficacy and safety of alternative medicine is not proved yet. But to diversify the treatmental methods of Korean medicine, we need to prove and use these medicine as one of Korean medical treatment.
