폐경기 여성 골다공증 환자에서 갱년기지수와 골밀도 간의 상관성 연구 (A Study on the Correlation between Menopausal Rating Scale and Bone Mineral Density for Menopausal Osteoporosis Patients)

  • Objectives : The purpose of this study is to evaluate the correlation between bone mineral density(BMD), age, weight, visual analog scale(VAS), menopausal rating scale(MRS), and to compare the difference between BMD, age, weight, VAS, MRS according to severity of the menopausal symptoms. Methods : The participants were 62 menopausal women diagnosed with osteoporosis who visited Cheonan Oriental Hospital, Daejeon University from December, 2011 to July, 2014. Age, weight, VAS, MRS were used as an evaluation tool. Results : 1. In all 62 cases of menopausal patients diagnosed with osteoporosis, the study found a statistically significant correlation between BMD and weight, while inverse correlation between BMD and MRS index. 2. Among menopausal patients with above 9 MRS index(group B), BMD and weight showed statistically significant correlation, while VAS and MRS index showed statistically significant inverse correlation. 3. Among group B, VAS were higher compared to control group(group A). Conclusions : This study showed that as patients with severer menopausal symptoms apt to show increased VAS and decreased BMD. Therefore, long-term follow-up in patients with menopausal syndrome is needed to improve the patients' quality of life.

폐경기 전.후 여성의 골다공증 영향요인 (Factors Influencing Osteoporosis)

  • Purpose: This study was carried out to identify factors influencing osteoporosis in women at pre- and post-menopausal state. Methods: The subjects of this study were 52 pre-menopausal and 125 post-menopausal women who were assessed of bone density in one general hospital. The data were collected through review of clinical records and telephone interviews using a questionnaire. Results: In the pre-menopausal women, the factors influencing osteoporosis were regular exercise (protective factor) and age (risk factor). Regression analysis showed that the factors attributable to osteoporosis included educational level, weight, age and number of pregnancy, accounted for 41.89% of the total variance. In the post-menopausal women. the factors influencing osteoporosis were age (risk factor), low educational level (risk factor), low economical state (risk factor), high parity (risk factor), and intake of coffee (protective factor). Regression analysis also showed that factors attributable to their osteoporosis included age, educational level, number of delivery, intake of the coffee, regular exercise, number of pregnancy and duration of oral pill intake, accounted for 37.41 % of the total variance. Conclusion: In pre-menopausal women, regular exercise was one of the most powerful determinant of their bone mass. Therefore, it is necessary to participate in a regular exercise program to maintain peak bone mass density prior to the onset of menopause. In post-menopausal women, increased age was the most influencing factor of their bone mass. Therefore, it is essential to establish early diagnosis and management of osteoporosis after menopause.

폐경단계별 근골격계 통증, 폐경증상 및 우울 (Musculoskeletal Pains, Menopausal Symptoms and Depression by Women's Menopausal Stage)

  • Purpose: The purpose of this study was to compare musculoskeletal pain, menopausal symptoms and depression by women's menopausal stage. Method: With a survey design, a convenience sample of 203 women between age of 40 to 64 was recruited through a community. Data were collected via a structured study questionnaire from May to July, 2008. Measures included with symptom severity list for menopausal symptoms, Aches and Pains scale, and Beck's Depression Inventory. Results: Thirty-seven percent of women belong to early peri-menopausal stage, 31% to late peri-menopausal stage, and 32% to postmenopausal stage. By group comparison, late peri-menopausal and post menopausal women reported higher scores of depression than early peri-menopausal women. Postmenopausal women had greater scores in two pain subscales (bodily pain and pain that interferes with normal work) and two menopausal symptoms subscales (sleep pattern and sexual behaviors) than early peri-menopausal women. There were moderate levels of positive relationships among study variables at each menopausal stage. Conclusion: Postmenopausal women seem to have greater musculoskeletal pain and menopausal symptoms, and have depression more than did early peri-menopausal women. Tailored nursing strategies are required to reduce these kinds of common symptoms for women with menopausal transition to improve their physical and psychological wellbeing.

Clinical Significance of Atypical Squamous Cells of Undetermined Significance in Detecting Preinvasive Cervical Lesions in Post-Menopausal Turkish Women

  • Background: To evaluate the clinical significance of atypical squamous cells of undetermined significance (ASCUS) in PAP test in post-menopausal women and compare with reproductive age women. Materials and Methods: A total of 367 patients who referred to our gynecologic oncology clinic were included to the study between September 2012 and August 2013. Data for 164 post-menopausal (group 1) and 203 pre-menopausal (group 2) women with ASCUS cytology were evaluated retrospectively. Immediate colposcopy and endocervical curettage was performed for both groups and conization for all women with a result suggestive of CIN2-3. Histopathological results and demographic features of patients were compared between the two groups. Results: Mean age of the patients was $54.6{\pm}6.5$ years in group 1 and $38{\pm}6.6$ years in group 2. Some 14 (8.5%) of post-menopausal women and 36 (17.7%) of pre-menopausal women were current smokers (p=011). Totals of 38 (23.2%) post-menopausal and 64 (31.5%) pre-menopausal women were assessed for HPV-DNA. High risk HPV was detected in 7 (4.3%) and 21 (10.3%), respectively (p=0.029). Final histopathological results recorded were normal cervix, low grade cervical intra-epithelial neoplasia (CIN 1), and high grade cervical intra-epithelial neoplasia (CIN2-3). In group 1 results were 84.8%, 12.2% and 1.8%, respectively, and in group 2 were 71.9%, 23.2% and 4.9%. There were no cases of micro invasive or invasive cervical carcinoma in either group. Two cases were detected as endometrial carcinoma in the menopausal group (1.2%). Conclusions: In current study we found that preinvasive lesions were statistically significantly higher in pre-menopausal women than post-menopausal women with ASCUS. Cervicitis was more common in menopausal women. Therefore, we think that in case of ASCUS in a post-menopausal woman there is no need for radical management.

  • Very little is known about the relation between isoflavone intake and menopausal symptoms in Korean woman To find the effects and correlations between these factors, questionnaires(maternal factors, menopausal symptoms) anthropometric measurement, 24hr dietary recalls, and urinary isoflavones analysis were conducted in 100 postmenopausal women residing in the Kyunggi-do rural area. The average age hight and weight of the subject were 61.5 years, 153.6 cm and 56.2kg. The average age at menarche, menopause, and menstrual cycle were 16.7 years, 47.2 years, 29.8 years, respectively. Most of the postmenopausal women experienced mildly menopausal symptoms. The mean calorie, protein and calcium intake were 1417.6 kcal(74.7% of the Korean RDA) 53.2 g(88.6% of the Korean RDA) and 454.0mg(65.6% of the Korean RDA) respectively. The ration of energy from carbohydrate, protein, and fat was 68: 15: 17 The average isoflavone intake from soy foods was 27.27 mg. The major food source of the isoflavone were soybeans and soybean curd. The average urinary isoflavone excretion was 2. 78nmol/mg Cr and showed significant positive correlation with isoflavone intake. Isoflavone intake from soy foods had a negative correlation with the severity of menopausal symptoms. Therefore, soy foods which contain isoflavone may have a protective effect on menopausal symptoms of women in Korea.

Effects of Smoking on Menopausal Age: Results From the Korea National Health and Nutrition Examination Survey, 2007 to 2012

  • Objectives: Decreased fertility and impaired health owing to early menopause are significant health issues. Smoking is a modifiable health-related behavior that influences menopausal age. We investigated the effects of smoking-associated characteristics on menopausal age in Korean women. Methods: This study used data from the Korea National Health and Nutrition Examination Survey from 2007 to 2012. Menopausal age in relation to smoking was analyzed as a Kaplan-Meier survival curve for 11 510 women (aged 30 to 65 years). The risk of entering menopause and experiencing early menopause (before age 48) related to smoking were assessed using a Cox proportional hazards model. Results: The menopausal age among smokers was 0.75 years lower than that among non-smokers (p<0.001). The results of the Cox proportional hazards model showed pre-correction and post-correction risk ratios for entering menopause related to smoking of 1.26 (95% confidence interval [CI], 1.09 to 1.46) and 1.27 (95% CI, 1.10 to 1.47), respectively, and pre-correction and post-correction risk ratios for experiencing early menopause related to smoking of 1.36 (95% CI, 1.03 to 1.80) and 1.40 (95% CI, 1.05 to 1.85), respectively. Conclusions: Smokers reached menopause earlier than non-smokers, and their risk for experiencing early menopause was higher.

건강한 폐경이행 구조모형 (Structural Equation Modeling on Healthy Menopausal Transition)

  • Purpose: This study was designed to construct and test structural equation modeling on healthy menopausal transition in middle-aged women in order to identify variables affecting healthy menopausal transition. Methods: Participants, 276 women, 45 to 60 years of age, with menopausal symptom score higher than 5 on the Korean version of Menopause Rating Scale, were recruited in three cities and one county of Gyeongnam Province. Research data were collected via questionnaires and analysed using SPSS version 18.0 and AMOS version 20.0. Results: After confirmatory factor analysis, one of the observed variables was excluded due to relatively low factor loading. The model fit indices for the hypothetical model were suitable for the recommended level: GFI=.93, CFI=.92, RMSEA=.05. Self-efficacy, self-differentiation, and menopausal symptoms explained 67.7% of variance in menopausal transition, and self-differentiation was the most influential factor for menopausal transition. Self efficacy and menopausal symptoms explained 9.6% of variance in menopausal management, although "menopausal symptoms" was not significant. Conclusion: These results suggest that nursing interventions to improve self-differentiation, self efficacy, menopausal management and decrease menopausal symptoms are critical for healthy menopausal transition in middle-aged women. Continued development of a variety of community-based nursing interventions to facilitate healthy menopausal transition is suggested.

  • Purpose: The aim of this study is to examine the effects of physical activities on mental health of menopausal women. Method: The data was collected from 5th National Health and Nutrition Examination Survey. 810 samples of 40 to 64 year age group and 660 samples of 65 and over age group were used. To achieve the results estimated without bias stratification variables and colonies, descriptive statistics, Chi-square test, multiple logistic regression were used. Result: Flexibility exercises showed that it had an effect on stress and suicide intension of menopausal women. On the other hand, vigorous physical activity might be a stress factor of menopausal women. Conclusion: Menopausal women in 65 year and over appeared that they had more beneficial to low-intensity exercise to manage stress or suicide intension. This result will be used to develop the mental health promotion programs for menopausal women.

Comparison of temporomandibular disorders between menopausal and non-menopausal women

  • Objectives: Hormonal changes during menopause alter a woman's susceptibility to some disorders. Information regarding the prevalence of temporomandibular disorder (TMD) in menopausal women is limited in the literature. In this study, the prevalence and severity of TMDs were compared between menopausal and non-menopausal women. Materials and Methods: The study included 140 women (69 premenopausal and 71 postmenopausal) 45 to 55 years of age that were examined in Shiraz Dental School, Shiraz in Iran. The Helkimo clinical dysfunction index (Di) was used to evaluate temporomandibular joint (TMJ) dysfunction. The data were analyzed using chi-square and Fisher's exact tests. Results: Occurrence of TMD was significantly higher in menopausal than non-menopausal women (P<0.001). All the TMD criteria based on Helkimo Di except range of mandibular movement were significantly more common in menopausal women. The range of mandibular movement was not significantly different between menopausal and non-menopausal women (P=0.178). Conclusion: The results from this study show that TMD can be considered more common and severe in menopausal than non-menopausal women. This finding indicates that, similar to other conditions in menopausal women such as arthritis and osteoporosis, TMD should be taken into consideration by dental and medical professionals.

중년여성의 갱년기 증상과 영적 안녕에 관한 연구 (On the Relationship Between Menopausal Symptoms and Spiritual Well-being)

  • The purpose of this study is to investigate the relationship between menopausal symptoms and spiritual well-being. In order to collect the research data, 350 questionaries were sent to the middle aged women who were 40-59 years old. During the collection period, March 20, 1977-April 20, 1977, 264 questionaries were collected. The questionnaire was prepared by using other researchers' scale, such as menopausal symptom scale by Neugarton and spiritual well-being scale by Paloutzian & Ellison. The research data were analyzed by various testable methods, such as frequency, t-test or ANOVA, and Pearson's correlation. The main results of this study were as follows ; Among the general characteristic variables, some variables have statistical significancy in explaining the difference of the menopausal symptoms. Such variables include age(F=5.17, p=0.002), years get married(F=4.23, p=0.002), number of children(F=3.08, p=0.028), income level of family(F=3.07, p=0.017), life style at leisure time(F=2.47, p=0.045). Some variables related to health condition could explain the differences of the menopausal symptoms among middle aged women. current menstruation stage(F=9.82, p=0.000), current health condition(F=9.82, p=0.000), and experience being operated in the past(F=9.82, p=0.000) are related to the menopausal symptoms. Serious menopausal symptoms which could be found in this study were 'back pain and joint pains(2.6)', nervousness(2.4), and psychosomatic symptom(1.97). Spiritual well-being inquiries could be classified into two subgroups, existential and religious. The mean score of existential well-being(3.04) was higher than that of religious well-being(2.76). Relatively higher existential well-being(3.10) was found in the women who have stable menstruation cycle. The age when the menopause began was related to spiritual well-being(F=3.29, p=0.046). The correlation between menopausal symptoms and spiritual well-being was statistically significant(r=-0.133, p=0.031). Based on the above results, nursing intervention program of menopausal symptoms is recommended in order to promote the health of middle-aged women.
