A Case of Ovarian Hyperstimulation Syndrome with Massive Pleural Effusion (다량의 흉수를 동반한 난소 과자극 증후군 1례)

  • 1be ovarian hyperstimulation syndrome is a rare but serious complication of ovulation induction therapy with gonadotropin. The clinical manifestations are generalized edema, ascites with pleural effusion and may become life-threatening in severe cases. The pathophysiology is still unknown, therefore, the treatment should be symptomatic and conservative. We report a case of severe OHSS with massive right pleural effusion in excess of ten liters after human menopausal gonadotropin therapy because of secondary infertility. Fluid and electrolyte imbalances were corrected and albumin was administered. A right chest tube was placed for a total of sixteen days, draining eleven liters of pleural effusion totally, resulting a dramatic decrease of pleural effusion and improvement of symptoms.

Study on Clinical Diseases of Blood Stasis Pattern (어혈증(瘀血證)의 임상 질환 범위에 대한 고찰)

  • Objectives : This article is a study on to which categories of modern diseases blood stasis patterns are assigned and the meaning of blood stasis interpreted with perspectives of Korean Medicine and modern medicine. Methods : We reviewed "Neijing", "Shanghanlun", "Yilingaicuo", "Xuezhenglun" and other books and modern clinical papers related with blood stasis. Results : 1. Blood stasis patterns are related with disorders of hemorrheology, hemodynamics, platelet function, microcirculation, microelements and endothelial damage. 2. From the types of syndrome differentiation, diverse diseases classified in type of qi deficiency with blood stasis and type of blood stasis due to qi stagnation are reported, which reflects qi and blood are closely connected. And many diseases are classified in type of kidney deficiency with blood stasis, which has something in common with chronic diseases can achieve effect from treatment considering blood stasis. 3. Diseases related with kidney involve menopausal disorder, mazoplasia, prostatitis, erectile dysfunction, chronic nephritis, renal calculus, osteoporosis and bursitis. Diseases related with heart involve coronary artery disease, arrhythmia and cerebral thrombosis. Diseases related with spleen involve gastritis, colonitis and digestive organ ulcer. Diseases related with liver involve hepatitis, hyperthyroidism and stroke. Diseases related with lung involve neurodermatitis, bronchitis and paranasal sinusitis. Conclusions : Blood stasis pattern which is one of the areas to draw medicine's attention has broad clinical application.

  • Korean women are now living almost 1/3 of their life after menopause. Sex-steroid hormone deficiency adversely affect various fields of physical & mental activity and quality of life during this period. Therefore, replacement of deficient hormone is thought to be natural therapeutic modality. Postmenopausal syndrome is recently redefined as an endocrinopathy with both short-and long-term sequelae, as a result of cessation of ovarian function. Hormone replacement therapy taken at or near time of menopause alleviates shot-term acute menopausal symptoms such as vasomotor disturbances and psychological problems. HRT also beneficially affects some of intermediate symptoms such as urogenital atrophy and cutaneous problems. The major benefits of long-term use were reductions in risk of long-term sequelae, that is, total fracture by 50-60%, cardiovascular disease by 50% and cerebrovascular disease by 30-40%, respectively. In addition, HRT may also positively influence Alzheimer's disease, reduce the colorectal cancer risk and increase longevity of the life. In conclusion, all postmenopausal women should consider preventive HRT when there are no contraindications.

Induction of Ovulation by Intermittent Subcutaneous Injection of Pure Follicle-Stimulating Hormone in Polycystic Ovarian Syndrome (다낭성난소 증후군 환자의 Pure Follicle-Stimulating Hormone 간헐 피하주사법을 이용한 배란유도)

  • Polycystic ovarian syndrome (PCOS) patients have a characteristic of high leuteinizing hormone (LH) to follicle -stimulating hormone (FSH) ratio. Usually, human menopausal gonadotropin (hMG) is used to induce ovulation in clomiphene citrate-resistant PCOS patients. However, HMG contains two components, namely, LH and FSH, with 50%, respectively. Therefore, FSH is theoretically recommended to stimulate follicular maturation. From the pituitary, LH is secreted by pulsatile pattern. So, we have been using intermittent subcutaneous injection of pure FSH for ovulation induction in 10 PCOS patients from March, 1990 to August, 1992. We obtained good results by intermittent subcutaneous injection of pure FSH. Ovulation is 100% per patient, and 88.2% per cycle. Pregnancy rate is 80% per patient, and 23.5% per cycle. Ovarian hyperstimulation syndrome (OHSS) is 50% per patient, 41.2% per all cycles, and 46.7% per all ovulated cycles. In comparison with HMG, pregnancy rate per cycle is relatively low. But, ovulation rate and pregnacy rate per person is higher than HMG. Because of the strict check of ovaries by the vaginal ultrasonography, OHSS rate is relatively high.

The Effects of Menopause on the Metabolic Syndrome in Korean Women (폐경이 대사증후군에 미치는 영향)

  • This study was the secondary analysis using the Korea National Health and Nutrition Examination Survey of 2012 and purpose was to investigate the effects of menopause on the metabolic syndrome(MetS) in Korean women. In raw data, total 2,746 women aged ${\geq}19$ years and provided the correct data about menstruation and menopause were analysed. The results were as follows: First, components of MetS were significantly different according to menopausal status. Second, the prevalence of MetS was 13.2% in premenopausal women and 42.7% in postmenopausal women. Third, the Odds ratio of postmenopausal women was 4.88(95% CI=3.888-6.126) compared with premenopausal women for MetS. As 5 years from 40 years increased, the Odds ratios of postmenopausal women were 3.15(95% Confidence Interval=1.862-5.331), 4.159(2.558-6.761), 5.971(3.955-9.016), 9.52(6.591-13.749) compared with premenopausal women for MetS, respectively. Based on the results, the intervention should be developed and provided for preventing and managing the MetS in postmenopausal women.

The Comparison of Kupperman's Index and MENQOL between Women with Surgical Menopause and Women with Natural Menopause (수술적 폐경을 경험한 여성과 자연 폐경 된 여성의 Kupperman's Index, MENQOL 비교)

  • Purpose: To investigate the change of the climacteric syndrome with women with induced menopause by hysterectomy or ovariectomy compared with women with natural menopause. Methods: Menopausal women(aged 45-55) were selected at two oriental medicine hospitals, Sam-se Oriental Medicine Hospital and Daegu Hannny Oriental Medicine Hospital. They were asked about Kupperman's Index, MENQOL and a few additional questions. They were divided into two groups: women with induced menopause by hysterectomy or ovariectomy and women with natural menopause. To analyse the Kupperman's index and MENQOL, SPSS for Windows 12.0 and Wilcoxon Ranked Sum Test were used. Results: The total sums of Kupperman's index and MENQOL were significantly correlated with women with induced menopause and women with natural menopause and women with induced menopause had higher total sums. Both the renal and urinary tract disorder and the sexual disorder were significantly correlated with women with induced menopause and women with natural menopause. The gastrointestinal and psychosocial disorder of Kupperman's index and MENQOL were not significantly correlated with women with induced menopause and women with natural menopause. Conclusions: Women with induced menopause by hysterectomy or ovariectomy experienced climacteric syndrome more severely compared with women with natural menopause, specifically the renal and urinary tract disorder and sexual disorder.

  • Burning mouth syndrome (BMS) is defined as the xerostomia, burning sensation and various discomfort of tongue and oral mucosa. BMS can occur in both men and women, but is more frequent in middle-aged menopausal women. Because exact cause can't be identified clearly and it is hard to make diagnosis in clinic, the purpose of the treatment have been to relieve symptoms. Etiology of BMS is divided into local, systemic, and psychological factors. ${\alpha}$-lipoic acid, clonazepam, supplemental therapy and cognitive behavior therapy can be prescribed for BMS. Nowdays, many experts focus attention on effect of combination therapy. It is necessary to solve the symptoms of the patients by combination of pharmacological approach and psychotherapy with cognitive behavior therapy considering the factors in various aspects.

  • Objectives: Postmenopausal symptoms are subject to many factors. Recently, obesity has been suggested as a factor of hot flush. So this study aims to report the relationship between body composition and Menopause Rating Scale (MRS), The Menopause-specific Quality of Life Questionnaire (MENQOL) of postmenopausal women. Methods: We studied 42 climacteric women who had visited Kyung Hee University Hospital at Gangdong from April 2011 to July 2014. 42 women had filled out MRS, MENQOL, and they had taken body composition tests. Pearson correlation tests were conducted. (Correlation was statistically significant at the 0.05 level) Results: Correlation between psychological subscale of MENQOL and Waist-Hip Ratio (WHR) was statistically significant (pearson correlation coefficient=0.385; p-value=0.012). But other subscales of MRS and MENQOL were not statistically related with body composition. Conclusions: WHR could be an important factor of psychological health of menopausal women. So, abdominal fat reduction must be considered when curing menopausal symptoms. In connection with climacteric depression, well-designed studies would be necessary.

Associations between Vasomotor Symptoms and Depression, Stress and Quality of Life in Midlife Women (중년여성에서의 혈관운동증상과 우울, 스트레스, 삶의 질과의 상관성)

  • Objectives : The aim of this study was to evaluate the associations between vasomotor symptoms and factors such as sociodemographics, health behaviors, medical condition, depression, stress, anxiety, attitude toward menopause, and quality of life. Methods : We conducted a cross-sectional study in peri- and post-menopausal women enrolled by the Korean Association of Health Promotion. Subjects submitted self-report questionnaires about vasomotor symptoms and other clinical symptoms. Associations between vasomotor symptoms and clinical variables were analyzed using stepwise multiple regression analyses. Results : 1951 women completed self-report questionnaires and 1022 women were enrolled in the study. The prevalence of vasomotor symptoms in peri- and post-menopausal women was 63.9%. Variables showing significant differences between subjects with vasomotor symptoms and subjects without them were score of Beck Depression Inventory, Brief Encounter Psychosocial Instrument-Korean Version, proportions of clinically significant depression(Beck Depression Inventory ${\geq}16$), Menopausal rating scale, attitude towards menopause, the 4 subscales of World Health Organization Quality of Life-BREF(Physical health, psychological, social relationships, environment), and a History of Premenstrual syndrome/Premenstrual dysphoric disorder. Stepwise multiple regression analyses indicated that Beck Depression Inventory, Brief Encounter Psychosocial Instrument-Korean Version, Menopausal Rating Scale, and the Psychological subscale of World Health Organization Quality of Life -BREF show associations with vasomotor symptoms. Conclusions : Menopausal vasomotor symptoms are associated with various psychological factors, especially with depression. Midlife women suffering vasomotor symptoms should therefore be screened for depression. Future prospective studies where clinical subjects are diagnosed using structured interviews, focusing on the causal relationship between depression and vasomotor symptoms are necessary.

Superhongmi bran extract improves lipid profile and menopause symptoms: a randomized, placebo-controlled clinical trial (슈퍼홍미 미강 추출물의 폐경 후 여성의 혈중 지질 농도 및 대사성 질환 개선 효과)

  • Women who undergo natural menopause transition have increased numbers of risk factors relating to metabolic syndrome due to estrogen deficiency. This study was conducted to investigate the effect of Superhongmi bran extract on metabolic syndrome improvement in menopausal women. Thirty women, who participated in a randomized, double-blind, placebo-controlled trial, were assigned to placebo-control (n=15) or Superhongmi bran extract (n=15) groups and were asked to consume two tablets (350 mg per extract per tablet) per day. After 12 weeks, weight, body mass index (BMI), plasma triglyceride (TG) levels, and total cholesterol (TC) levels were significantly decreased, whereas HDL-cholesterol (HDL-C), apolipoprotein A1 (ApoA1), adiponectin, superoxide dismutase 1 (SOD1), and GSH (glutathione) concentrations were significantly increased in the Superhongmi bran extract group. Moreover, $17{\beta}-estradiol$, and progesterone levels in the Superhongmi group were significantly higher than those in the placebo-control group. These results suggest that Superhongmi bran extract alleviates metabolic symptom in menopausal women.
