기억력 향상을 위한 작업치료 중재 연구 분석: 국내 단일대상연구 중심으로 (Analysis of Occupational Therapy Intervention Research for Improving Memory: Focus on Single-Subject Research Design in Korean Academic Journals)

  • 목적 : 본 연구의 목적은 기억력 장애를 가진 성인 환자를 대상으로 기억력 향상을 위한 작업치료 중재를 수행한 단일대상연구의 특성을 확인하고 질적 수준을 알아보는 것이다. 연구방법 : 본 연구는 2011년부터 2020년까지 기억력 향상 작업치료 중재 연구 중 단일대상연구 설계를 적용한 총 6편의 논문을 분석한 문헌연구로, 연구내용에 대한 일반적 특성 및 연구방법의 질적 수준을 분석하였다. 결과 : 분석대상의 질적 수준은 66.7%(4편)가 중간 수준이었고 33.3%(2편)는 높은 수준이었으며, 낮은 수준의 연구는 없었다. 단일대상연구 설계 유형은 모두 반전설계로 ABA가 가장 많았다. 대상자는 뇌졸중, 치매, 경도인지장애 환자였으며, 대상자 수는 1~3명이었다. 독립변수는 오차배제훈련, 어플리케이션 활용 중재, 전산화인지훈련, 시간차회상훈련이었다. 종속변수는 공통변수인 기억력과 함께 집중력, 뇌파변화, 수단적 일상생활활동 및 우울이었다. 중재시간은 30~40분, 중재회기는 6~15회기였으며, 총 연구기간은 3~8주로 다양하였다. 중재 결과 모든 연구에서 중재 후 종속변수가 향상된 것으로 보고되었다. 결론 : 기억력훈련에 대한 단일대상연구 적용 작업치료 중재 연구들은 모두 중간 이상의 질적 수준을 보여 임상에서 적용 시 근거기반자료로 의의가 있으며, 기억력 향상에 효과적인 연구들이라는 것을 확인하였다.

Scopolamine으로 유도된 기억력 손상 동물모델에서 비파엽 추출물의 학습 및 기억력 개선 효과 (Anti-amnesic Effect of Eriobotrya japonica Leaf Extract on Scopolamine-induced Memory Impairment in Rats)

  • 본 연구는 scopolamine으로 유도한 기억력 및 인지기능 감퇴 동물모델에서 비파엽 추출물의 기억력 및 인지기능 개선효과를 평가하기 위해 수행되어졌다. 6주령의 SD-rat(male)을 총 5개 군으로 분리하였으며, 양성대조군으로 donepezil(1 mg/kg b.w.)을 처리하였다. 15일간 24시간 간격으로 EJE를 50, 200 mg/kg b.w. 농도로 전처리(경구투여) 하였으며, 16일째에 농도별로 EJE 및 양성대조군으로서 donepezil을 투여하고 scopolamine을 복강투여(1 mg/kg b.w.) 하였다. Water maze test와 passive avoidance test를 통해 scopolamine으로 유도한 동물모델에서 EJE의 기억력 및 인지능력 개선 효과를 확인한 결과, scopolamine에 의해 감소된 기억력 및 학습능력이 EJE를 투여함으로써 유의적으로 회복 및 증가한 것을 확인할 수 있었다. Scopolamine에 의해 인지능력과 함께 감소된 운동능력의 평가를 위해 수행한 rotarod test 및 vertical pole test에서는 scopolamine의 투여로 감소된 균형감각, 협응력 그리고 grip strength와 같은 운동능력들이 EJE의 투여로 유의적으로 회복된 것을 확인하였다. 또한 scopolamine에 의해 증가한 AchE의 활성과 감소한 Ach의 함량이 EJE의 투여로 각각 개선되었다. 본 연구에서는 이와 같은 결과를 바탕으로 EJE의 scopolamine으로 유도한 동물모델에서 인지능력 개선과 감퇴한 운동능력의 향상 및 cholinergic neurotransmission system의 강화 효과를 확인할 수 있었으며, 이러한 연구 성과들로 비파엽 추출물이 퇴행성뇌질환인 AD 치료와 학습 및 기억력 개선에 있어 효과적인 대안으로서 보다 더 많은 분야에서 연구되기를 바라는 바이다.

신경정신질환의 컴퓨터모델 개발에 관한 연구 (A Study on Developing Computer Models of Neuropsychiatric Diseases)

  • 뇌공학 분야에서 개발된 연상기억의 능력을 가지는 Hopfield 신경망을 구성하고, 신경정신질환 중에서 시냅스의 손실과 관련이 되어서 생기는 치매와 일부 정신분열증의 모델로 변형시키기 위하여 인공신경망의 시냅스를 Hoffman의 시냅스 제거법칙과 무작위 방법에 따라 제거하면서, 그에 따른 기억능력의 변화를 관찰하였다. 구성된 컴퓨터모델에서 기억능력의 저하는 시냅스의 감소가 상당히 진행되어야 나타났으며, Hoffman의 시냅스 제거법칙에 따랐을 때는 80% 제거를 기점으로 급격하게 나타났으며, 무작위 제거시는 더 적은 시냅스 제거율인 40% 제거 때부터 점진적으로 나타나는 양상을 보였다. 컴퓨터 모델의 기억력저하 양상이 실제환자에서 관찰되는 기억력저하 양상을 설명할 수 있기 위해서는 증상이 발현되는 시점의 시냅스 소실의 정도는 얼마나 되는지, 또한 시냅스의 소실은 어떤 규칙에 의해 일어나는지 아니면 무작위로 일어나는지에 관한 생물학적 실험의 필요성이 부각되었다. 이와 같이 컴퓨터모델을 이용하여 모의실험을 하고 연구의 방향을 잡은 후에 생물학적 실험으로 검증해 나간다면, 매우 효율적인 이론과 실험의 공조체제를 이루어 신경정신질환의 이해를 도울 수 있을 것이다.

The Effect of Sub-chronic Whole-Body Exposure to a 1,950 MHz Electromagnetic Field on the Hippocampus in the Mouse Brain

  • The increasing use of mobile phones has raised public concern about the possible biological effects of radiofrequency electromagnetic field (RF-EMF) exposure on the human brain. To investigate the potential effect of RF-EMF exposure on the brain, we examined the behaviors and hippocampal morphology of C57BL/6 mice after sub-chronic exposure to RF-EMFs with a relatively high SAR level (5.0 W/kg). We applied a 2-hour daily exposure of WCDMA 1,950 MHz using a reverberation chamber that was designed for whole-body exposure for 60 days. In the behavioral tests, RF-EMF did not alter the physical activity or long-term memory of mice. Moreover, no alteration was found in the neuronal and glial cells in the hippocampus by RF-EMFs. In this study, we showed that sub-chronic whole body RF exposure did not produce memory impairment and hippocampal morphological alteration in C57BL/6 mice.

비전형적 치매양상을 보이는 후부대뇌피질위축 증례보고 (A case of Posterior Cortical Atrophy Presenting with Features of Atypical Dementia)

  • Posterior cortical atrophy(PCA) is a presenile dementia that presents primarily with signs and symptoms of cortical visual dysfunction, while memory is relatively preserved until the late stage of the disease. We report a patient with PCA, confirmed by brain magnetic resonance imaging (MRI) and $F^{18}$-fluorodeoxyglucose positron emission tomography(FDG PET). A 58-year-old right-handed woman presented initially with visual dimness and difficulty finding things around her. She had partial Balint's syndrome, partial Gerstmann syndrome, and idiomotor apraxia. She also had a mild memory disturbance, but preserved insight of her disease. Neuropsychological evaluation showed decreased parietal and left temporal functions bilaterally. Brain MRI and $F^{18}$-FDG PET revealed typical bilateral occipitoparietal atrophy and hypometabolism, which were slightly worse on the right side. Cholinesterase inhibitor administration for 6 months improved the memory impairment slightly, but not the cortical visual dysfunction. This is a typical case of PCA, confirmed by neurologic signs and imaging findings.

Scopolamine 유발 기억력손상 흰 쥐에서 영계감조탕가용골모려(笭桂甘棗湯加龍骨牡蠣)의 인지기능개선과 항산화 효과 (Anti-amnesic and Antioxidant Effect of Yeongkyekamjotanggayonggolmoryo Aqueous Extracts on Scopolamine-induced Memory Impairment in Mice)

  • Objectives: The purpose of this in vivo study is to observe anti-amnesic effects of Yeongkyekamjotanggayonggolmoryo (YGYM), a novel mixed herbal prescription, Ossis Mastodi and Ostreae Testa added Yeongkyekamjo-tang, on scopolamine induced amnesia in C57BL/6 mice through acetylcholine (ACh) and acetylcholinesterase (AChE) activity, Choline acetyltransferase (ChAT) mRNA expression, and antioxidant effects. Methods: Six groups, total 20 intact or 100 Sco treated mice were used in this study after one week of acclimatization period. Half the animals were used for passive avoidance task tests and hippocampus ACh content, AChE activity, and ChAT mRNA expression were measured. The other half was subjected to an underwater maze test and then the cerebral cortex antioxidant defense system was measured. Results: In the passive avoidance experiment, there was significant decrease in residence time in the bright room and in the underwater maze test, escape latency to escape from the esophagus significantly increased compared with the normal control group. At the final sacrifice, ACh content and ChAT mRNA expression decreased, AChE activity increased, and cerebral cortical MDA increased GSH content, SOD and CAT activity in Sco control mice, as compared to intact vehicle control mice. However, these Sco treatment-related memory loss through AChE activation destroyed the cerebral cortex antioxidant defense system, and was inhibited dose-dependently by 28 days consecutive oral pretreatments of YGYM extracts 500, 250, 125 mg/kg. Conclusions: In the above results, YGYM extract that oral administration of YGYM extracts alleviates the antioxidant defense system, through preservation of ACh mediated by upregulation of ChAT mRNA expression, and increase of AChE inhibition and brain antioxidant defense systems.

  • Objectives : This experiment was designed to investigate the effect of the ODTCMT hot water extract & ultra-fine Powder on Alzheimer's Disease Model Induced by 13A. Methods : The effects of the ODTCMT hot water extract on expression of IL-l${\beta}$, IL-6, TNF-${\alpha}$ in BV2 microglial cell line treated by lipopolysacchaide(LPS) were investigated. The effects of the ODTCMT hot water extract & ultra-fine powder on the behavior of the memory deficit mice induced by scopolamine were investigated. Results : 1. The ODTCMT hot water extract significantly suppressed the production of TNF-${\alpha}$, IL-6 and IL-l${\beta}$ in BV2 microglial cell line treated with LPS. 2. The ODTCMT hot water extract & ultra-fine powder groups showed significantly inhibitory effect on the scopolamine-induced impairment of memory in the experiment of Morris water maze. Conclusions : These results suggest that the ODTCMT hot water extract & ultra-fine powder may be effective for the prevention and treatment of Alzheimer's disease. Investigation into the clinical use of the ODTCMT hot water extract & ultra-fine powder for Alzheimer's disease is suggested for future research.

  • Objectives: This research investigates the effect of the DHJHT extract on Alzheimer's disease. Specifically, the effects of the DHJHT extract on (1) the behavior (2) the infarction area of the hippocampus, and brain tissue injury in AD mice induced with ${\beta}A$ were investigated. Methods: The effects of the DHJHT extract on the proinflammation cytokines mRNA expression and production of BACE, APP and ${\beta}A$ in in BV2 microglial cell line treated by lipopolysacchaide(LPS) plus ${\beta}A$ were investigated. The effects of the DHJHT extract on the behavior of the memory deficit mice induced by scopolamine were investigated. Results: 1. The DHJHT extract suppressed the expression of IL-$1{\beta}$, IL-6, TNF-$\alpha$, COX-2, and NOS-II, BACE and APP mRNA in BV2 microglial cell line treated by LPS plus ${\beta}A$. 2. The DHJHT extract suppressed the expression of ${\beta}A$ production in BV2 microglial cell line treated with LPS plus ${\beta}A$. 3. The DHJHT extract showed significantly inhibitory effect on the scopolamine-induced impairment of memory in the experiment of Morris water maze. 4. The DHJHT group suppressed the expression of IL-$1{\beta}$, TNF-$\alpha$, MDA, and CD68+/CD11b+ in the brain tissue of the mice with AD induced by ${\beta}A$. 5. The DHJHT group reduced the infarction area of hippocampus, and controlled the injury of the brain tissue in the mice with AD induced by ${\beta}A$. 6. The DHJHT group reduced tau protein, and GFAP in the brain tissue of the mice with AD induced by ${\beta}A$. Conclusions: These results suggest that the DHJHT group may be effective for the treatment of AD. Thus, DHJHT could be considered among the future therapeutic drugs indicated for the treatment of AD.

장뇌삼 잎 추출물의 분획물이 Scopolamine으로 유도된 Mouse의 기억력 개선에 미치는 영향 (Effects of Korean Mountain Ginseng Leaves and Its Active Constituents on Scopolamine-induced Amnesia in Mouse)

  • Objectives: The purpose of this study was to characterize the effect of the fraction of Korean mountain ginseng folium (FKG) on the learning and memory impairments induced by scopolamine. Methods: The memory ameliorating effect of FKG was investigated using a passive avoidance test, the Y-maze test, and the Morris water maze test in mice. Drug-induced amnesia was induced by treating animals with scopolamine(1mg/kg, i.p.). Results: FKG (2 or 4mg/kg, p.o.) administration significantly reversed scopolamine-induced cognitive impairments in mice by the passive avoidance test and the Y-maze test(P<0.05), and also improved escape latency in the Morris water maze test at 2 or 4mg/kg(P<0.05). Although FKG has little inhibitory activity for AChE (IC50 value; 1847 ${\mu}g/ml$) in an invitro study, phosphorylated extracellular signal-regulated kinase(pERK) was increased by the administration of FKG inhippocampus on immunohistochemistry. Conclusions: These results suggest that FKG may be a useful cognitive impairment treatment, and its beneficial effects are mediated, in part, via activation of ERK pathway.

치매동물모델 SAMP8에 있어서 기억. 학습장해에 미치는 알로에의 영향 III. SAMP8의 신경전달물질 및 그 대사산물에 미치는 알로에의 투여효과 (Effect of Aloe on Learming and Memory lmpaiments in Dementia Animal Model SAMP8)

  • Aloe(Aloe arborescens M$_{ILL}$) has been used as a home medicine for the past several thousand in the world, and has been studied on anti-bacterial and anti-fungal activities, hypotension, atherosclerosis, myocardiac infartion, apoplexy, diabetes as a chronic digenerative disease, tumors, gastrointestinal tract, liver and pancreas' diseases, and genitourinary tract etc. SAMP8 as a learing and memory impairment animal model were fed basic and/or experimental diets with 1.0% freezing dried(FD)-aloe for 8 months. The passive avoidance tests such as acqusition trial and retention test were significantly higher in aloe group than in control group. Grading score of senescence resulted in a marked decreases in aloe group compared with control group. Acetylcholinesterase(AChE) activity was remarkably increased in aloe group compared with control group. Neurotransmitters such as dopamine(DA) and serotonin(5-HT) almost did not change by the feeding of aloe-added diet, but their metabolites such as homovanillic acid(HVA) and 5-hydroxy-indole acetic acid(5-HIAA) in aloe group were significantly increased compared with control group. Therefore, the ratios of HVA/DA and 5-HIAA/5-HT as a ratio of metabolite on neurotransmitter were significantly increased by the feeding of aloe-added diet. These results suggest that aloe vara may be activated acetylcholinesterase, the metabolite of neurotransmitter, and ratios of metabolite on neurotransmitter, resulting ina greater prevention of learning and memory impairments such as Alzheimertype dementia.
