신규 간호사의 역할적응에 관한 탐색적 연구 (An Exploratory Study of Role Adaptation of Newly Employed Nurses)

  • The purpose of this study was to investigate factors influencing role adaptation in newly employed nurses and the consistency between role expectations before employment and after one year of employment. The results of this study may contribute to strategy development towards positive role adaptation in newly employed nurses. The data used in this study were collected from 111 nurses, the sample was drawn from all newly employed professional nurses working at Y Medical Center in 1987. The data were collected longitudinally in the 2nd - 3rd weeks, the 3rd month and the 12th month after employment. The results of this study were as follows : 1. The degree of change the importance of factors influencing role adaptation and the length of time after employment were investigated. the result showed that the degree of change was the greatest within the first 3 months after employment. Important factors related to role adaptation were working conditions, the environment of the assigned ward and the work load. These factors always displayed high scores without any great change over time. New employees put more importance on practical factors which were obtained through experience in their jobs rather than on ideal factors Which they had considered more important while in school. 2. Consistency between role expectation before employment and after 12 month of employment was investigated. The highest consintency item was the expectation about the variety of Patients, and the highest inconsistency was the expectation about their own welfare. An average score of 69.07 points was achieved from a maximum of 125 points for the 25 items. showing that expectations for role development before employment were not fully satisfied. In conclusion the administraor should assess the initial expectation at the time of employment of new nurses and she / he should make clear to the new nurses that these expectations may not be realistic. In this way the administrator can provide more satisfactory conditions towards the expectations of the new nurses and help them towards positive role adaptation and reduction of role conflict. Newly employed nurses have high and unrealistic expectations about socialization to the profession from their nursing educational program. It is suggested that a transitional training program should be planned and carried out for newly employed nurses.

Successful High Flow Nasal Oxygen Therapy for Excessive Dynamic Airway Collapse: A Case Report

  • Excessive dynamic airway collapse (EDAC) is a disease entity of excessive reduction of the central airway diameter during exhalation, without cartilage collapse. An 80-year-old female presented with generalized edema and dyspnea at our hospital. The patient was in a state of acute decompensated heart failure due to pneumonia with respiratory failure. We accordingly managed the patient with renal replacement therapy, mechanical ventilation and antibiotics. Bronchoscopy confirmed the diagnosis of EDAC. We scheduled extubation after the improvement of pneumonia and heart condition. However, extubation failure occurred due to hypercapnic respiratory failure with poor expectoration. Her EDAC was improved in response to high flow nasal oxygen therapy (HFNOT). Subsequently, the patient was stabilized and transferred to the general ward. HFNOT, which generates physiologic positive end expiratory pressure (PEEP) effects, could be an alternative and effective management of EDAC. Further research and clinical trials are needed to demonstrate the therapeutic effect of HFNOT on EDAC.

간호사의 연명의료에 대한 태도, 역할 인식 및 간호 스트레스 (Attitude, Role perception and Nursing stress on Life Sustaining Treatment of Nurses)

  • 본 연구의 목적은 간호사의 연명의료에 대한 태도, 역할 인식 및 간호 스트레스에 대하여 조사하고 관련 요인을 파악하는 것이다. 연구 대상자는 P시에 소재한 G 병원에 근무하는 간호사 160명이며, 구조화된 설문지를 이용하여 자료를 수집하였다. 연명의료에 대한 태도는 5점 만점에 3.33점, 역할 인식은 3.94점 그리고 간호 스트레스는 3.78점이었다. 연명의료에 대한 태도는 근무 병동 그리고 간호 스트레스는 성별, 결혼 상태, 근무 병동에서 유의한 차이가 있었다. 간호 스트레스는 역할 인식과 양의 상관관계가 있었으며, 간호 스트레스에 영향을 미치는 요인은 성별, 내과 병동, 중환자실, 역할 인식이었다. 그러므로 간호사의 연명의료 간호 스트레스 감소를 위한 교육프로그램의 개발과 효과를 확인하는 연구가 필요하다.

여성노인의 피부두겹 두께와 우울의 관련성 연구 (Study on the Relationship between Skinfold Thickness and Geriatric Depression in Older Adult Woman)

  • Purpose: This research was aimed at evaluating the differences in depression by skinfold thickness, and the relationship between two variables. Methods: Research design was a non-experimental descriptive survey. Using a caliper, we measured skinfold thickness on the triceps, and then on the suprailiac, and the mid-thigh. Depression was evaluated using the Geriatric Depression Scale-Short Form (GDS-SF). Data were collected for one month from Sept 1st to 30th, 2011. There were 52 subjects, 25 of which were from elderly welfare centers and 27 were hospitalized. Results: Subjects were an average age of $76.4{\pm}4.45$, and the incidence rate of depression (${\geq}6$) was 36.5% and those who had a sum of 3 skinfolds over 62 mm were 22.7%. The GDS-SF was significantly different in only the suprailiac skinfold thickness (F=7.25, p<.05). Conclusion: Findings indicate that depression is different based on suprailiac thickness in older adult women at elderly welfare centers and those who were hospitalized in an medical ward. It suggests that the suprailiac skinfold may be an indicator of abdominal obesity when considering depression in elderly women. Further study is needed to evaluate a cutoff score of skinfold thickness in obesity for older adult women.

신경계 병동에서 억제대를 적용 중인 환자가족의 억제대 적용에 대한 인식과 태도 (Families' Perception and Attitude toward Applied Physical Restraints in General Neurological Wards)

  • 본 연구는 신경계병동에서 억제대를 적용 중인 환자의 가족을 대상으로 억제대 적용에 대한 인식과 태도를 파악할 목적으로 시행된 서술적 조사연구이다. 2014년 6월 1일부터 2014년 9월 30일까지 부산시 소재 일개상급종합병원의 신경계병동에서 억제대를 적용 중인 환자의 가족 70명을 대상으로 설문조사했고 환자자료는 의무기록으로 조사했으며 수집된 자료는 서술통계, t-test, ANOVA, $Scheff\acute{e}$ tests로 분석했다. 환자가족은 여자가 68.6%, 평균 47.9세, 환자의 배우자나 자녀가 70% 이상이었고 일일 평균 15.4시간 동안 환자를 돌보고 있었다. 억제대 적용에 대한 인식의 평균점수는 4.2/5점이었고, 치료장치 유지와 관련된 인식점수가 높았고(평균 4.6-4.9/5점) 억제대 적용부위에 따라 양손이 한손보다 더 중요하다고 인식했다. 억제대 적용태도에 있어서는 환자와 가족에게 적용과 제거에 대한 결정권이 있다와 관련된 문항에 동의하는 가족이 많았다. 본 연구의 결과를 토대로 환자가족에게 억제대 관련 정보제공과 지속적인 교육을 통한 지원이 요구된다.

Clinical Characteristics of Celiac Disease and Dietary Adherence to Gluten-Free Diet among Saudi Children

  • Purpose: To describe the clinical characteristics of celiac disease (CD) among Saudi children and to determine the adherence rate to gluten free diet (GFD) and its determinant factors among them. Methods: A cross-sectional study was conducted, in which all the families registered in the Saudi Celiac Patients Support Group were sent an online survey. Only families with children 18 years of age and younger with biopsy-confirmed CD were included. Results: The median age of the 113 included children was 9.9 years, the median age at symptom onset was 5.5 years and the median age at diagnosis was 7 years, the median time between the presentation and the final diagnosis was 1 year. Sixty two of the involved children were females. Ninety two percent of the patients were symptomatic at the diagnosis while eight percent were asymptomatic. The commonest presenting symptoms included: chronic abdominal pain (59.3%), poor weight gain (54%), abdominal distention, gases, bloating (46.1%) and chronic diarrhea (41.6%). Sixty percent of the involved children were reported to be strictly adherent to GFD. Younger age at diagnosis and shorter duration since the diagnosis were associated with a better adherence rate. Conclusion: CD has similar clinical presentations among Saudi children compared to other parts of the ward; however, the adherence to GFD is relatively poor. 고위험 임산부의 기질과 성격이 임산부의 우울, 불안, 모-태아 애착에 미치는 영향 (Effects of the Temperament and Character on Depression, Anxiety, and Maternal-Fetal Attachment in High-Risk Pregnant Women)

  • Objective : High-risk pregnant women are exposed to the physical and emotional crisis associate with the condition of high-risk pregnancy, making these women emotionally unstable, exhibiting such conditions as depression and anxiety. The aim of this study was to examine what effects the temperament and character of high-risk pregnant women had on their depression, anxiety, and maternal-fetal attachment. Methods : A hundred and five high-risk pregnant women were recruited in the maternity ward of the Keimyung University medical center. All subjects were administered a sociodemographic and obstetric questionnaire, Temperament and Character Inventory, Beck Depression Inventory-II, State Trait Anxiety Inventory-State, and Maternal-Fetal Attachment Scale. Results : First, higher scores on the harm-avoidance scale and lower scores on the self-directedness scale were associated with higher depression scores. Second, higher scores on the harm-avoidance and reward-dependence scale were associated with higher anxiety scores. Lastly, higher scores on the cooperativeness scale were associated with higher maternal-fetal attachment scores. Conclusion : Temperament and character in high-risk pregnant women were associated with the degree of depression, anxiety, maternal-fetal attachment. Based on these findings, it is suggested that examining temperament and character in advance and screening for vulnerability in pregnant women can help to prevent emotional problems for high-risk pregnant women.

교대근무를 하는 일반병동 간호사의 간호업무 수행실태, 수행주체와 직무만족에 대한 융복합적 조사연구 (A Convergence Investigation on Nursing Task Performance, Appropriate Performer, and Job Satisfaction of Nurses with Shift Work in General Ward)

  • 본 연구는 일반병동에 근무하는 교대근무 간호사의 간호업무 수행실태와 직무만족도를 확인하기 위한 2차 자료 분석 연구이다. 연구 결과, 전체 간호 업무 중 대상자가 수행한 직접간호는 45%, 간접간호는 55%를 차지하였다. 교대 근무별로 주간에 수행한 업무량은 전체 업무량의 37.2%였고 인계 후 수행한 업무는 '직접간호(34.5%)'와 '기록과 보고(25.8%)'였다. 대상자들은 본인이 수행한 업무 중 일부분에 대해 간호보조 인력에게 위임 가능하거나, 타 의료인과 경계가 모호한 업무가 있다고 응답하였다. 야간근무의 직무이행도와 직무만족도 간의 유의한 상관관계(rs=.43, p=.01)가있었다. 이러한 결과는 교대근무별 직무분석에 근거한 업무효율성 제고 및 EMR적용, 스마트 기기 등을 활용한 인계시간 감소 전략, 업무 수행주체를 명확히 하는 방안이 필요함을 시사한다.

A Peripherally Inserted Central Catheter is a Safe and Reliable Alternative to Short-Term Central Venous Catheter for the Treatment of Trauma Patients

  • Purpose: To determine whether a peripherally inserted central catheter (PICC) meets the goals of a low infection rate and long-term use in trauma patients. Methods: From January 2016 to June 2018, the medical records of patients who underwent central venous catheterization at a level I trauma center were retrospectively reviewed. Data collected included age, sex, injury severity score, site of catheterization, place of catheterization (intensive care unit [ICU], emergency department, or general ward), type of catheter, length of hospital stay during catheterization, types of cultured bacteria, time to development of central line-associated bloodstream infection (CLABSI), and complications. Results: During the study period, 333 central vein catheters (CVC) were inserted with a total of 2,626 catheter-days and 97 PICCs were placed with a total of 2,227 catheter-days. The CLABSI rate was significantly lower in the PICC group when the analysis was limited to patients for whom the catheter was changed for the first time in the ICU after CVC insertion in the ER with similar indication and catheter insertion times (18.6 vs. 10.3/1,000 catheter-days, respectively, p<0.05). The median duration of catheter use was significantly longer in the PICC group than in the CVC group (16 vs. 6 days, respectively, p<0.05). Conclusions: The study results showed that the duration of catheter use was longer and the infection rate were lower in the PICC group than in the CVC group, suggesting that PICC is a safe and reliable alternative to conventional CVC.

상급종합병원과 여성전문병원 간호사의 산후 간호중재 조사 (Tertiary Hospitals' and Women's Special Hospitals' Postpartum Nursing Intervention Survey)

  • Purpose: This study was done to assess development and postnatal care interventions in postnatal care intervention records for maternity ward nurses in tertiary hospitals and women's hospitals in South Korea. Methods: This mixed-method research was a Time-Motion (TM) study. Data were collected through external observation of 12 nurses in 4 wards over 24 hours. Mann-Whitney U test and independent t-test were employed for the analysis of frequency and provision time of direct/indirect care activity. $x^2$ (Fisher's exact test) was utilized to determine the difference in frequency between two groups. IBM SPSS 22.0 statistical program was employed for calculation. All statistical significance levels were at ${\alpha}=.05$. Results: According to the KPCS-1 (Korean Patient Classification System-1), women's hospitals are group 3 and tertiary hospitals, group 4. With respect to time difference in direct care, tertiary hospitals showed 791 minutes and women's hospitals, 399 a difference of 392 minutes. For time difference in indirect care, women's hospitals had 2,415 minutes while tertiary hospitals, 2,080, a difference of 335 minutes for women's hospitals. No difference was found in the average total care workload between the two institutions. Individual time also showed no difference (p>.05). Conclusion: High-risk maternal care strength in tertiary hospitals and breast-feeding strength in women's hospitals need to be benchmarked with each other.
