Purpose: This study aimed to investigate the current and future status of Parkinson's disease rehabilitation service in Busan. Methods: A literature search of domestic journals was conducted using the keywords "Parkinson's", "exercise", "rehabilitation", and "physical therapy". The chosen databases were Research Information Sharing Service (RISS), e-articles, and Korean studies Information Service System (KISS). International literature was searched in PubMed, Pedro, DOI, Publisher, CINAHL (EBSCOhost), and PsycINFO using the same combination of keywords. Results: The results of this study showed that 33 medical institutions provide Parkinson's disease rehabilitation service and five do not. Regarding the composition of Parkinson's disease rehabilitation teams, 15 medical institutions provide physical therapy, occupational therapy, and speech therapy as their rehabilitation program, 15 medical institutions provide physical therapy and occupational therapy, and three provide only physical therapy. The study found that muscle-strengthening, flexibility, endurance, and balance exercises were commonly provided in all 33 medical institutions for Parkinson's disease. Additional exercises were provided in only three medical institutions. The frequency was five times a week in 20 medical institutions. Conclusion: Medical institutions located in Busan provide a variety of Parkinson's disease rehabilitation services, not only in general hospitals but also in multiple medical institutions, although the composition of their Parkinson's disease rehabilitation teams and the frequency of treatment vary.
The purposes of this research were to investigate home care rehabilitation services in rural areas and to collect basic data about disabled persons necessary when for carrying out rehabilitation services. Respondents were selected from six of a total of eight townships (Myon) and one town (Eup) in the Wonju city area. Wonju is in Kangwon Province (Do). Of a total of 338 names provided by the Myon offices, 298 persons were located and included registered and non-registered persons. Conditions included stroke, spinal cord injury, and cerebral palsy in addition to disabilities classified as first, second or third degree, in the case of registered cases. Respondent demographic characteristics, medical characteristics, rehabilitation service needs, willingness to receive rehabilitation service and individual opinions regarding rehabilitation services were analysed by frequency and percentage. The results were as follows: 1) Rehabilitation services received by disabled persons living at home in the rural areas surrounding Wonju city were medical rehabilitation (41.7%), diagnosis (36.5%), rehabilitation assistive devices (7.6%), social assistance (7.1%), rehabilitation counseling (3.0%), vocational rehabilitation (1.8%), educational rehabilitation (1.6%) and housekeeping services (0.5%). The majority of rehabilitation services were medical rehabilitation provided at hospitals and oriental medicine hospitals. 2) Sixty point eight percent of respondents expressed their willingness to receive home care rehabilitation services. Needs expressed were highest for medical rehabilitation (27.0%), followed by social assistance (19.4%), medical examination (12.4%), physician-generated diagnosis in the home setting (11.6%), sociopsychological rehabilitation (9.3%), vocational rehabilitation (7.6%), rehabilitation engineering (6.0%), educational rehabilitation (3.3%), and housekeeping services (3.3%). 3) Rehabilitation service needs were analyzed by severity classification: 65.8% of first degree, 62.7% of second degree and 55.6% of third degree disability classification, and 62.7% of non-registered disabled individuals responded that rehabilitation service was necessary. 4) Rehabilitation service needs were also analyzed by diagnosis: 62.6% of stroke, 85.5% of amputation, 60.0% of spinal cord injury and 52.4% of traumatic brain injury respondents answered positively that they were willing to receive rehabilitation service if it were to be provided. Rehabilitation service utilization data of disabled individuals living at home in rural areas were investigated and their rehabilitation needs analyzed. This critical information can be used when community-based rehabilitation programs for disabled persons living at home are planned for provision out of a public health center or when community-based rehabilitation welfare policy is formulated.

This study was to find out the relationship between the rehabilitation service and the life satisfaction of industrially injured workers and how medical rehabilitation service, psycho-social rehabilitation service, and occupational rehabilitation service influence in this relation. For this, it analyzed 977 people who were using rehabilitation service with industrial disaster disability among 2,000 research subjects using the first year materials of industrial disaster insurance panel of Workers' Compensation & Welfare Service. For the relationship of variables on the suggested 3 models, the mediating effect of the occupational rehabilitation service in the relation of medical rehabilitation of the industrially injured workers and life satisfaction by applying SPSS Macro and Sobel method and did the regression analysis to find out the control effect of psycho-social rehabilitation service by the mean centering of the material. The result of the analysis showed; when the satisfaction on the medical rehabilitation service was increased the satisfaction on the voacational rehabilitation was increased, when the satisfaction on the occupational rehabilitation service was increased, the satisfaction on the life satisfaction was increased, and when the satisfaction on the psycho-social rehabilitation service was increased the medical rehabilitation service had a indirect effect on the daily life satisfaction.
This study evaluated the community-based rehabilitation services provided by the Wonju Public Health Center from Jan. 2000 to Dec. 2002. Ninety-four persons with disabilities dwelling in the community participated and the surveys were completed in an interview during home visits. The respondents' demographic, socio-economic, and medical characteristics, rehabilitation service received, willingness to receive home-visit rehabilitation services, and satisfaction with the rehabilitation services were analyzed by frequency and percentage. A Likert scoring system consisting of five agreement-disagreement categories was applied to each item, consisting of Very Satisfied, Satisfied, So-So, Poorly Satisfied, and Very Poorly Satisfied. The major findings were as follows: 1) The rehabilitation services used included medical rehabilitation (26.9%), followed by social assistance (23.5%), diagnosis by a physician at home (17.3%), medical examination (12.3%), housekeeping services (6.2%), and vocational and educational rehabilitation (3.5%). 2) Of the medical services, the respondents desired physical therapy at home and free rental of rehabilitation equipment, such as wheelchairs, canes, walkers, the most, followed by home visit occupational therapy, nursing services, and oriental medicine service in descending order. 3) Some of the respondents expressed so-so satisfaction (50.0%) or dissatisfaction (16.9%) with the rehabilitation services provided by the Wonju Public Health Center. These findings should prove useful when planning or extending community-based rehabilitation programs for the homebound disabled in the community.
Objectives To obtain future research basis of Korean Medicine for total knee arthroplasty patient by analyzing medical and Korean Medical service utilization and treatment duration. Methods Data sampling was performed on Health Insurance Review and Assessment Service patient data of 2015 (Confidence level of 97%) to analyze patients' medical and Korean Medical service tendency. Sampling groups were divided into two groups; i) Patients who completed their treatment within 5 months of total knee arthroplasty, ii) Patients who continued their treatment after 5 months of total knee arthroplasty, to investigate patients' medical and Korean Medical service tendency and individual characteristics were carefully monitored. Results It was confirmed that total of 1,655 patients had gone through total knee arthroplasty out of 1,453,486 patients who were gathered for sampling. First sampling group (Patients who completed their treatment within 5 months of total knee arthroplasty) was 287 patients and second sampling group (Patients who continued their treatment after 5 months of total knee arthroplasty) was 385 patients. The proportion of patients who visited Korean Medical service in first sampling group was lower than that of second sampling group. Conclusions It was confirmed that medical and Korean Medical service and cost consumed by second group (Patients who continued their treatment after 5 months of total knee arthroplasty) was higher than that of first group (Patients who continued their treatment after 5 months of total knee arthroplasty). It is highly recommended to continue with further study for efficient medical and Korean Medical service and reduced cost.
Objectives : The aim of this study was to investigate the satisfaction determinants of rehabilitation care group inpatients in rehabilitation long-term care hospitals. Methods : A survey was conducted with 262 patients from 6 rehabilitation long-term care hospitals located in Busan Kyungnam. Data were collected from June 2016 to July 2016 with a structured questionnaire and analyzed with SPSS 23.0. Results : The significant factors affecting the overall satisfaction score were disease group and 5 quality indices of medical services(Medical service quality, Rehabilitation services quality, Hospital reputation, External physical environment, and Internal physical environment). Additionally, the significant factors affecting intention to revisit were age, disease group, length of hospital stay and the 5 quality indices of medical services. Conclusions : The results of this study indicate that rehabilitation long-term care hospitals should try to improve the overall satisfaction of inpatients, especially focusing on the internal physical environment and the quality of the rehabilitation services and medical services, which will lead to a high level of intention for patients to revisit.

Objective: We evaluated the efficacy of a newly-developed spinal orthoses (V-LSO) by comparing the stabilizing effect, abdominal pressure, and comfort of 3 different semirigid LSOs (classic LSO, V-LSO, and Cybertech®) during various body movements. Method: Thirty healthy volunteers (23~47 years, 24 males, 6 females) were selected. A dual inclinometer measured the range of motion (ROM) while the participants performed flexion/extension and lateral flexion of the lumbar spine with 3 LSOs. The LSO's pressure on the abdominal surface was measured using 9 pressure sensors while lying, sitting, standing, flexion/extension, lateral flexion, axial rotation, and lifting a box. Comfort and subjective immobilization were analyzed by a questionnaire. Results: V-LSO had a statistically significant effect on flexion over Cybertech®. No significant differences were noted during extension and lateral flexion between the 3 LSOs. The abdominal pressure showed no significant differences while supine. While sitting, standing, and lifting a box, the mean abdominal pressure for V-LSO were significantly higher than those for Cybertech®. During lumbar flexion, the mean abdominal pressures for classic LSO and V-LSO were significantly higher than that of Cybertech®. For extension, lateral flexion and axial rotation, the abdominal pressure for V-LSO was significantly higher than those of classic LSO and Cybertech®. In the subjective analysis, V-LSO and Cybertech® scored best for comfort. Conclusion: The V-LSO and Cybertech® were more comfortable than the classic LSO, and hence, may have improved compliance with decreased discomfort. V-LSO may be superior to the other LSOs in restricting lumbar movement and increasing intraabdominal pressure.
The objective of this study is to suggest measures to support the community-based rehabilitation sports service for health of people with disabilities, which can be applicable to South Korea, by analyzing the rehabilitation sports service support system of Germany and Japan. Through literature review and direct visiting to Japan, the researcher explored such factors as, background of initiating the rehabilitation sports service, concept, range, and subject of theservice, service delivery procedure, and institutions, and finance of the service. The institutional implications are as follows: First, the concept and range of rehabilitation sports are gradually expanding. Initially, the rehabilitation sports started targeting for veterans with disabilities for the purpose of medical rehabilitation in Germany and Japan. As time passes, however, the importance of rehabilitation sports has emphasized, and the subjects have gradually expanded for the purpose of preventing disability or secondary diseases caused by disability. Second, in order to perform rehabilitation sports service, it is essential to have manpower with expertise in medical treatment, welfare, disability, and exercise prescription, likewise Germany and Japan do. Third, Germany and Japan emphasize the assignment of rehabilitation sports instructors in institutions, rather designating specific institutions, which is moving toward the manpower-based rehabilitation sports service, rather place-based service. Fourth, rather having simple medical programs or sports-oriented programs, it would be necessary to have the systematized disease-oriented programs for each type of disability. Lastly, the clients-oriented service delivery process should be established.

The incidence of heterotopic ossification (HO) was reported to be higher in combat-injured patients than in civilian trauma patients. HO is often considered a possible cause of residual limbs pain in amputee. Here, we report the case of a 21-year-old male, who underwent a traumatic right transfemoral and left transtibial amputation with two segments of painful HO around his left amputation site. We report the effect of extracorporeal shock wave therapy (ESWT) on size and pain associated with HO. After ESWT, the visual analog scale score decreased from 5~6 to 0~1 and the size of two masses decreased from 13.1 × 6.7 mm and 12.5 mm to 11.9 × 4.7 mm and 12.2 mm, respectively. To the best of our knowledge, this is the first case that has reported on the treatment of HO using ESWT for a traumatic transtibial amputation patient. The case suggests that ESWT could serve as a complementary treatment for HO in traumatic amputation patient.

Background: The effect of the Valsalva maneuver applied in resistance exercise has shown contradictory results. This study aims to investigate the effect of the Valsalva maneuver on resistance exercise by examining the changes that occur by applying the Valsalva maneuver during trunk flexion and extension resistance exercise in healthy adults. Design: Randomized controlled trial. Methods: 34 healthy adults were conveniently recruited. According to the with or without Valsalva maneuver, the flexion and extension of the trunk were measured and evaluated. Results: Paired t test showed that there was a statistically significant difference in maximum torque of trunk flexion in the resistance exercise with Valsalva maneuver. However, there was no statistically significant difference in trunk extension (p<.05), in the total and average work of trunk flexion and extension (p<.05). Conclusion: The results of this study in which the difference in the amount of exercise according to the application of the Valsalva maneuver was not significant, show that the practice of the Valsalva maneuver, which has a risk of injury, should be avoided in rehabilitation exercises.
