공공병원과 민간병원의 생산성에 관한 차이 분석 (Public hospitals and Private hospitals analysis of productivity differences)

  • 본 연구는 공공병원과 민간병원 간의 생산성을 자본생산성, 노동생산성, 부가가치 생산성으로 구분하여 비교분석하였으며, 연구결과를 토대로 공공병원이 양질의 의료서비스 제공하기 위해 공공성과 수익성을 안정적으로 확보할 수 있는 생산성 제고 방안을 제시하고자 한다. 조사기간은 2013년 회계연도 말을 기준으로 하였으며, 공공병원의 대상은 39개 민간병원은 108개 총 147개 병원으로 조사대상을 선정하였다. 분석방법은 ANOVA분석, 위계적 회귀분석을 실시하였다. 결론을 보면 생산성 중 노동생산성 변수들이 공공병원의 수익성에 가장 중요한 요인들로 나타났으며, 향후 공공병원의 생산성과 효율성에 큰 영향을 미치기 때문에 효과적인 인적관리체계의 수립이 요구된다. 또한 생산성과 수익성간의 연계를 통하여 분석하여야만 병원의 효율적인 관리에 대한 명확한 평가가 가능해야 할 것이다.

의료기관 종사자의 손 위생 수행 분석 (Analysis of Hand Hygiene Practices of Health Care Personnels)

  • 의료기관 종사자의 손 위생 수행은 의료관련 감염의 전파를 예방하는 가장 효과적인 방법이다. 본 연구는 직종별 손 위생 수행 측정과 분석을 위해 세계보건기구(WHO)의 손 위생 수행 평가도구를 이용한 직접관찰방법으로 2010년 10월 25일부터 12월 31일까지 수행되었으며, SPSS 21.0으로 기술분석, 교차분석, 다중로지스틱분석을 하였다. 총 8,644 건의 손 위생 상황이 관찰되었고, 손 위생 수행률은 94.1%였으며 의사가 가장 낮았다(83.3%; OR:0.209, 95%CI:0.174-0.252). 직종별 수행률(p<0.001), WHO에서 권고하는 5개 상황(WHO's 5Moments)별 수행률은(p<0.001) 유의한 차이를 보였으며, 간호사(p=0.003)와 의료기사직(p<0.001)이었다. 본 연구결과 손 위생 수행률 향상을 위한 직종별 특성화된 손 위생 전략과 WHO's 5Moments에 따라 차별화된 전략 개발이 필요할 것으로 사료된다.

소아 안면열상 환자의 치료에 있어서 유용한 봉합술 (Convenient Suture Technique for Pediatric Facial Lacerations)

  • Purpose: Lacerations requiring formal wound closure compose a significant number of all childhood injuries presenting to the emergency department. The problem with conventional suture technique are that suture removal is quite cumbersome, especially in children. Unwanted soft tissue damage can result in the process of suture removal, which calls for sedation, stressful for both medical personnel and child. The purpose of this study is to introduce the convenient suture technique for pediatric facial lacerations. Methods: Children under the age of four, presenting to the emergency department with facial lacerations were enrolled in the study. From March 2008 to June 2009, 63 patients (41 males and 22 females) with an average age of 1.4 years were treated with our convenient suture technique using utilized a loop suspended above a double, flat tie. Clean, tension free wounds were treated with our technique, wounds with significant skin defect and concomitant fractures were excluded. Results: The Patients were followed-up in 1, 3 and 5 days postoperatively. On the third hospital visit, suture removal was done by simply cutting the loop suspended above the wound margin and gently pulling the thread with forceps. There were no significant differences in the rates of infection and dehiscence compared with conventional suture technique. Conclusion: The use of our technique was to be simple with similar operative time compared with conventional suture technique. Removal of suture materials were easy without unwanted injuries to the surrounding tissue which resulted in less discomfort for the patient and greater parental satisfaction, minimized the complications. It can be considered as a viable alternative in the repair of pediatric facial lacerations.

공공의료기관에 근무하는 간호사가 경험하는 근무지 약자 괴롭힘과 조직몰입, 이직의도간의 관계 (The Relationship among Workplace Bullying, Organizational Commitment and Turnover Intention of the Nurses Working in Public Medical Institutions)

  • Purpose: The purpose of this study was to explore the relationships among workplace bullying, organizational commitment, and turnover intention of nurses. Methods: A cross-sectional study was conducted. Data were collected using a structured questionnaire from nurses at five public hospitals (N=190). Results: The experience of bullying was 31.29 points (range 16-64) on average: Work-related bullying was the highest, followed by verbal/non verbal bullying and external threats. The consequences of bullying were 30.54 points (range 13-52) on average: The physical/psychological withdrawal was the highest, followed by increasing distrust and the poor quality of patient care. The experiences of bullying had positive correlations with the consequences of bullying (r=.55, p<.001) and the turnover intention (r=.27, p<.001), and had a negative correlation with organizational commitment (r=-.28, p<.001). The consequences of bullying had a positive correlation with the turnover intention (r=.52, p<.001), and had a negative correlation with organizational commitment (r=-.49, p<.001). Organizational commitment showed a negative correlation with the turnover intention (r=.63, p<.001). Conclusion: The findings of this study suggests the needs of a regular monitoring system and organizational management for workplace bullying, which helps nurses to commit to their organization and to be retained in a workplace.

투석방법에 따른 자가관리 지식, 자가관리, 생리적 지표와 증상경험 (Self-Management Knowledge, Self-Management, Physiological Indexes, and Symptoms Experience according to Dialysis Methods)

  • Purpose: This study was a cross-sectional study comparing differences in self-management knowledge, self-management, physiologic indexes, and symptoms experience for two methods of dialysis. Methods: Participants were 90 patients on hemodialysis and 91 on peritoneal dialysis at A hospital. Results: There was no significant difference between the two groups for knowledge of self-management. A comparison of the categories in the evaluation of self-management showed that patients in the peritoneal dialysis group took better care of their dialysis access route (F=17.61, p<.001) and dialysis schedule (F=4.30, p=.040). The physiologic indexes between the two dialysis groups showed that hemoglobin levels were higher in the hemodialysis group (F=5.28, p=.023). The product of serum calcium and phosphate was higher in the peritoneal dialysis group (F=11.42, p=.001). Serum sodium level was also higher in the peritoneal dialysis group (t=5.36, p<.001) while serum albumin level (t=-3.36, p=.001) and mean arterial blood pressure (t=-2.50, p=.013) were higher in hemodialysis patients. There were no significant differences in the proportion of uncomfortable experiences between the two groups. Conclusion: Medical personnel should consider differences in self-management knowledge/self-management, physiologic indexes, and symptoms experience for hemodialysis and peritoneal dialysis populations, and should provide adequate education accordingly and promote behavioral change to improve physiologic indexes and reduce symptoms.

계층분석과정(AHP)을 이용한 지자체 감염병 취약지표의 가중치 평가 (Relative Weight Evaluation for the Vulnerability Indicators of Infectious Disease Using Analytic Hierarchy Process in Local Governments)

  • 본 연구의 목적은 지역사회에서 감염병 발생 및 확산에 영향을 미치는 감염병 취약지표의 상대적 가중치를 평가해보는 것이다. 이를 위해 선행연구 및 사례 고찰을 기반으로 감염병 취약지표를 확산취약시설, 감염취약 계층, 사회적 취약여건, 대응능력 4개의 부문으로 분류한 후, 각 부문별로 총 24개의 세부지표를 선정하였다. 그 후 22명의 전문가를 대상으로 계층분석과정(AHP)을 이용하여 각 지표들 간의 상대적 가중치를 산정하였다. 분석결과, 부문별 가중치는 확산취약시설이 가장 높게 나타났으며, 부문별 지표와 세부지표를 모두 고려한 종합 가중치는 병상확보율(1순위), 종교시설 밀도(2순위), 의료인력 비율(3순위), 고령자 비율(4순위), 유흥업소 밀도(5순위) 순으로 나타났다. 이러한 결과는 지자체의 감염병 대응정책 수립에 필요한 근거자료를 마련하는 데에 활용 가능할 것이다.

선택적 기능 동작 평가의 연구 동향 분석 (Investigation of Research Trends in the Selective Functional Movement Assessment (SFMA))

  • Purpose : Selective functional movement assessment(SFMA) is a movement-based assessment tool designed for clinicians to diagnose and treat pain and functional physical problems. This concept is used for assistance in occupations related to movement, including clinical medical personnel, such as physical therapists, physical education instructors, and athlete coaches. Although this concept is widely used by clinicians, research and scientific proofs are relatively insufficient. This study aimed to review the literature on the understanding, reliability and effectiveness of SFMA, and to consider its role in the future of rehabilitation. Methods : In this review, a total of nine articles were selected according to the eligibility criteria of three major thesis topics. The main topics covered in the literature are reliability and effectiveness of clinical use of SFMA. To gather research articles, we searched official term 'Selective Functional Movement Assessment'. Among the searched 60 studies, nine were mentioned in this study that contained overlapping information and matched our desired topic. We reviewed four reliability analyses, four case reports, and one experimental study. Results : Reliability has an intermediate degree between high raters and within raters. The validity of the SFMA system is influenced by a combination of experience and logic; hence, further improvement is needed. Therefore, if the intervention was effective based on the diagnosis result, then biomechanical evidence is necessary to further support the claim. Conclusion : In future research, to use SFMA as a diagnostic tool with high accuracy, it is necessary to improve the reliability of the main problem through breakout, support for guidelines and validity and efficiency.

교대근무를 하는 일반병동 간호사의 간호업무 수행실태, 수행주체와 직무만족에 대한 융복합적 조사연구 (A Convergence Investigation on Nursing Task Performance, Appropriate Performer, and Job Satisfaction of Nurses with Shift Work in General Ward)

  • 본 연구는 일반병동에 근무하는 교대근무 간호사의 간호업무 수행실태와 직무만족도를 확인하기 위한 2차 자료 분석 연구이다. 연구 결과, 전체 간호 업무 중 대상자가 수행한 직접간호는 45%, 간접간호는 55%를 차지하였다. 교대 근무별로 주간에 수행한 업무량은 전체 업무량의 37.2%였고 인계 후 수행한 업무는 '직접간호(34.5%)'와 '기록과 보고(25.8%)'였다. 대상자들은 본인이 수행한 업무 중 일부분에 대해 간호보조 인력에게 위임 가능하거나, 타 의료인과 경계가 모호한 업무가 있다고 응답하였다. 야간근무의 직무이행도와 직무만족도 간의 유의한 상관관계(rs=.43, p=.01)가있었다. 이러한 결과는 교대근무별 직무분석에 근거한 업무효율성 제고 및 EMR적용, 스마트 기기 등을 활용한 인계시간 감소 전략, 업무 수행주체를 명확히 하는 방안이 필요함을 시사한다.

FGI를 통한 노인장기요양시설 운영 개선 방향에 관한 연구 (Improvement of Management of Long-Term Care Facilities Through FGI)

  • 우리나라 인구의 빠른 고령화와 일상생활이 어려운 노인들이 증가하여 사회적 연대를 위한 노인장기요양 제도가 실시되었다. 제도 도입 후 나타난 장기요양제도의 구조적 문제 해결을 요구하고 있으며 기관 및 급여유형별로 통합된 요양서비스 제공체계, 노인의료-요양의 연속성 부족에 따른 요양병원-시설 간 기능정립 문제 등도 제기되고 있다. 이 연구에서는 문제점을 개선하기 위하여 입소정원, 종사인력, 제공서비스와 관련한 연구문제를 설정하였으며 FGI를 실시하였다. 연구결과 지역 내 노인 인구와 인정자 수, 노인성 질환자수 등 장기요양 수요를 반영, 지역별 적정 기관 및 인력 수급 정책 방향 제시하고, 지자체별 목표와 수급계획을 수립하여, 장기요양기관 지정요건 절차 강화 및 지정 갱신제 등 인증제도 도입이 검토되어야 하며, 남성요양보호사 육성 및 수가 인상 등을 통한 종사자 처우개선을 통하여 원활한 인력수급이 필요하다고 보았다. 급식비의 보험적용과 유관기관과의 연계를 통한 프로그램 확대제공, 원활한 입소관리를 위한 의료외 사고 중재기구가 필요하다고 나타났다.

훈련병에게 처음 발생한 내향성발톱에서 부분 발톱 절제술의 단기 추시 결과: 기질 절제술이 꼭 필요한가? (Short-Term Surgical Outcome of the Partial Nail Extraction in Ingrown Nail of Military Trainee: Is Matrixectomy Necessary?)

  • Purpose: An ingrown nail is common in military trainees who are exposed to highly demanding activities. Although the matrixectomy procedure has been the main treatment modality, several drawbacks may follow after the procedure, such as infection, periostitis, and continued pain that causes a delayed return to duty. This study examined the outcomes of a simple partial nail extraction with the hypothesis that this procedure may bring an earlier return to duty, lower the perioperative complications, and produce a comparable recurrence rate. Materials and Methods: The medical records of patients who had surgical treatment for an ingrown nail in the authors' institution between April 2016 and December 2017 were reviewed retrospectively. Under the inclusion and exclusion criteria, 28 patients with a simple partial nail extraction (group A) and 29 patients with a partial nail extraction with matrixectomy (group B) were investigated. As the clinical outcome, the visual analogue scale (VAS) and satisfaction score, time to return to duty, complications, and recurrence rate were checked and compared between the groups. Results: The VAS scores of group B were significantly higher during the first (p<0.001) and second (p=0.026) follow-up week than group A. The time to return to duty was shorter in group A (7.8 days) than group B (10.1 days), and this difference was significant (p<0.001). Group B had five patients with complications, whereas group A had none (p=0.028). No differences in the recurrence rate (p=0.197) and patient satisfaction (p=0.764) were found between the groups. Conclusion: In this study, simple partial nail extraction in military trainees resulted in lower postoperative pain, lower complication rates, and earlier return to duty than the procedure with matrixectomy. Military trainees are temporarily exposed to highly demanding activities. Thus, a satisfactory outcome would be expected with simple partial nail extraction without performing a radical procedure, such as a matrixectomy.
