한의학 분야 연구개발 인력의 수급전망 및 정책제안 (Current and Future R&D Manpower Requirements and Policy Recommendations in the Korean Oriental Medicine Research Area)

  • To strengthen the R&D capability and the competitiveness of the Korean oriental medicine industry, an adequate supply of qualified R&D personnel including medical doctors of Korean oriental medicine is an important precondition. This study analyze current and future R&D manpower requirements including medical doctors in the Korean oriental medicine research area. Our analyses can be utilized for developing the government R&D manpower planning including the adequate supply of medical doctors for the Korean oriental medicine research. For the study, we conducted and analyzed a delphi survey of the experts, the principal investigators, with expertise in Korean oriental medicine research areas. The results of this study can be summarized as follows; First, in 2007 the Korean oriental medicine R&D personnel is currently under-supplied as many as 302 people including 111 medical doctors of Korean oriental medicine. The rate of under-supplied is 28.2%. Second, in 2017 the forecast shows that the R&D personnel in this area will be more severely under-supplied as many as 539 people including 185 medical doctor of Korean oriental medicine. The rate of under-supplied will be 32.6%. As a result, the confrontation of demand and supply forecasts shows that, in general, severe shortages of R&D manpower in the areas of Korean oriental medicine will result if there are not adequate manpower policy adjustment.

융복합시대에 의료기관 정보가 일당진료비와 삭감율에 미치는 영향에 관한 연구 (A study of the impacts of medical institution information on daily medical expenses and medical expense reduction rate in convergence age)

  • 본 연구는 일당진료비와 삭감율의 차이와 변동요인을 파악하고 이를 중심으로 요양급여비 관리대책 수립에 필요한 기초자료를 제공할 목적으로 수행되었다. 전국 종합병원 50개소의 감염성 장염(A09.0) 질환의 1년 동안 평균 일당진료비와 평균 삭감율을 입원과 외래로 구분 조사하였고 결과는 다음과 같다. 입원 일당진료비는 의료기관의 총인원, 외래 일당진료비는 병상수, 입원 삭감율은 병상수와 총인원에 상관이 있는 것으로 나타났다. 의료기관 정보가 일당진료비에 미치는 영향은 입원과 외래 동일하게 의료기관의 총인원에 정적(+) 영향을 미치는 것으로 나타났다. 이러한 삭감율과 일당진료비에 대한 연구는 의료기관의 효율적이고 합리적인 의사결정을 돕고, 요양급여비 정책의 우선순위를 결정하는데 중요한 근거가 될 것이다.

치과위생사 의료인화에 대한 현안과제와 직무확충 방안 (Problems to Solve and Job Enlargement on the Inclusion of Dental Hygienists in the Category of Medical Personnel)

  • 본 연구는 치과위생사 의료인화에 대한 요구와 논의가 있는 현 시대적 상황에서 치과의료인력을 대상으로 치과위생사를 위한 선행과제와 의료인이 되었을 때의 긍정적 부정적 효과, 확장 가능한 전문업무에 대해 파악하고자 하였다. 치과위생사 259명, 치과의사 128명을 대상으로 치과위생사 의료인화에 대한 인식과 선행 과제, 긍정 및 부정적 효과와 확장 가능한 전문업무 등에 대해 조사 분석하여 다음과 같은 결론을 얻었다. 치과위생사 의료인화에 대해 치과위생사의 94.2%, 치과의사의 46.9%가 인지하였고, 치과위생사는 협회매체(52.5%), 치과의사는 치과신문(23.4%)을 통해 인지하는 경우가 많았다. 찬성도는 치과위생사의 95.0%, 치과의사의 64.1%였으며, 당위성은 치과위생사가 84.9%, 치과의사 51.6%였다. 당위성의 근거로 치과위생사는 업무 전문성과 교육과정 부합성이 높았고 업무 부합성이 낮았으나 치과의사는 의료체계 유사성이 높았고 교육과정 부합성이 가장 낮았다. 의료인화를 위한 선행 과제 중 직업적 영역에서 치과위생사와 치과의사 모두 직업의식과 윤리의식이 높았다. 제도적 영역에서도 두 직종 모두 치위생교육 질 관리가 가장 높았으며, 학제 일원화가 가장 낮았다. 사회적 영역에서 치과위생사는 중앙정부설득과 국민공감대 형성이 높았고, 치과의사는 유관단체 협력과 중앙정부 설득이 높았다. 치과위생사 의료인화 시 긍정적 효과는 치과위생사 인식 확장이 치과위생사와 치과의사 모두 가장 높았고, 부정적 효과는 치과위생사의 경우 임금상승 심화와 위임진료 증가가 높았고, 치과의사는 임금상승 심화와 구인난 심화가 높았다. 의료인화 시 확장 가능한 전문업무로는 독자적 치주관리프로그램 운영으로 치과위생사(79.9%)와 치과의사(69.6%) 모두에게 가장 높게 나타났다. 치과위생사는 치주관리를 위한 진단(44.4%)과 전신질환자 구강건강관리(44.0%), 비외과적 치주처치(41.3%), 근육 정맥주사(27.4%) 순이었고, 치과의사는 근육 정맥주사(44.1%), 비외과적 치주처치(34.3%), 전신질환자 구강건강관리(29.4%), 치주관리를 위한 진단(13.7%) 순이었다. 이상의 결과를 통해 치과위생사 의료인화가 가지는 의미에 대한 충분한 검토와 논의가 필요할 것으로 생각되고, 의료인화와 관련하여 치과위생사의 업무가 합리적으로 조정되고 합법화하는 데 본 연구가 활용되기를 기대한다.

의료 서사와 의료 윤리 (Medical Narrative Texts and Medical Ethics)

  • 본 논문은 먼저 펠레그리노의 <의철학의 재탄생>에서 강조하는 현대적 의료 윤리의 가치 변화를 고찰하였다. 펠레그리노는 의료인의 윤리에 대한 인문학적 성찰을 강조해온 철학자이다. 그는 현대 사회의 변화에 따라 의료 윤리도 재정립되어야 한다고 주장한다. 이 논문은 펠레그리노의 주장을 바탕으로, 문학과 대중서사텍스트 속에서 문제적 상황을 주목하였다. 실제로 우리를 위해 의료 윤리가 무엇을 답변해야 하는지를 살펴보려 한 것이다. 의료인들은 철학적 딜레마를 겪거나 현실과 윤리 사이의 갈등을 겪어 왔다. 펠레그리노는 의료인은 질병과의 싸움에 앞서, 환자와의 상호작용을 통해 환자의 고통에 공감하고 그 요구에 응답해야 한다고 주장한다. 이것은 매우 당위적인 선언처럼 보이지만, 문학 작품과 대중서사텍스트 속의 의료인들은 이러한 윤리적 딜레마에 자주 노출된다. 이청준의 소설을 통해 의료인이 환자를 어떻게 '좋음'의 상태로 이끌어야하는 지를 성찰할 수 있으며, 메디컬 드라마들을 통해 대중들이 의료인에게 요구하는 윤리적 행위가 무엇인지를 포착할 수 있다. 코로나19로 전세계가 고통을 받고 있는 지금, 의료인들 또한 큰 고난에 처해 있지만, 동시에 대중의 존경을 받고 윤리적 존재로서의 가치를 높이고 있기도 하다. 의료가 일상의 서사가 된 지금, 우리에게 의료 윤리는 삶을 유지하기 위한 필요조건이 되고 있다. 본 논문을 통해, 의료 윤리의 중요성을 다시 인식하고, 의료 서사 속에서 구체화된 윤리적 문제를 성찰적으로 검토하는 의미를 얻을 수 있을 것이다.

공무원(公務員) 및 사립학교교직원(私立學校敎職員)의 건강진단(健康診斷)에서 나타난 결과(結果) (A Result on the Physical Checkup of Public Officials and School Personnel in Private Schools)

  • Korean Medical Insurance Cooperation executed the physical checkup intended for all the members of public officials, school personnel in private schools, and the insured as a national-wide event in 1980. This is the result of a part of Taegu district and its contiguous country this hospital took charge of. Physical checkup method was divided into the first health examination and tile second health examination. The second health examination was executed for those who needed reexamination according to the result of the first health examination. After that, we passed judgement on the result finally. The total number of the first health examination was 10,779; 4,606 in public officials, 2,327 in police constables, 3,976 in school personnel in private schools. The classification of physical checkup is as follows; A group: normal groups B group: those who do not require immediate medical care but require preventive measures or who are doubtful of disease or who had undetermined diagnosis (attention) C group: those who require immediate medical care but who are able to be on duty (simple recuperation) D group: those who require immediate medical treatment and recuperation (suspension from office and recuperation) Total B group to the in the first health examination was 4.73%, that of total C,D groups 2.21%. That of total C,D groups to the total in the first health examination by occupation was 2.30% in public officials, 2.19% in police constables, 2.04% in school personnel. Consequently there was no different among occupations. Total C,D groups of hypertension to the total in the first health examination was 1.68% and hypertension was 76.05% to all disease. These rates mentioned to above were higher than any other rate in disease. Subsequently, being low, the rate of diabetes was 15.54%. From the view point of age, the higher rate appeared in men and women over 35 years old of B group and over 45 years old of C,D groups in three occupations in comparison with other ages and the older men were, the higher men who took a disease were.

의료사고에서의 형사책임 -원내감염사고의 해결을 향하여- (The Criminal Liability of Physicians in the Case of Medical Accidents)

  • 예전부터 일본에서 의료사고가 형사사건으로 취급되는 경우는 드물었다. 하지만 요코하마시립대학병원과 같은 중대한 의료과오가 발생한 이후 검사가 의료과오를 기소하는 태도를 보이고 있다. 의료사고는 의료관계자간 역할분담의 형태가 과실 인정에서 중요한 하나의 요소가 된다. 요코하마시립병원사건에서는 환자의 동일성확인에 관하여 의료관계자 사이에 역할분담이 확립되지 않은 경우에는 관계자 전원이 환자의 동일성을 확인하여야 할 의무가 있다고 하였다. 또 사이타마 의과대학병원사건에서는 치료에 신중할 필요가 있는 중대한 증례에 관해서는 그 증례를 담당하는 주치의 뿐 만 아니라 대학병원의 과장도 치료방침에 관하여 확인할 의무가 있다고 하였다. 현립 오오노병원사건에서 무죄판결이 내려진 이후는 형사소추에 신중을 기할 필요가 있다는 목소리가 강해지고 있다. 이러한 상황 속에서 후생노동성은 의료사고방지를 위해 의료사고조사제도를 도입하여 의료사고에 제3자기관에 의한 검증의 강화를 도모하고 있다.

치과 의료 인력과 치과기공사의 업무협력에 대한 인식 - 치과기공사 중심으로 - (Perception of Dental Personnel and Dental Technicians on Interprofessional Collaboration - Focus on Dental Technicians -)

  • Purpose: This study has been carried out to help to enhance the measures of interprofessional collaboration between Dental personnel and dental technicians, and between dental technicians themselves through the analysis of recognition of interprofessional collaboration between dental technicians. Methods: The subjects in this study were 130 dental technicians who were self-written questionnaire. The results were analyzed by SPSS 12.0. The collected data was analyzed by frequency and Correlation. Results: As a result of examining the correlation analysis of each question to explore the factors that affect interprofessional collaboration recognition of dental technicians, the group which received criticism from dental personnel showed negative (-) correlation in the interprofessional collaboration recognition (p<0.05). The following groups showed positive (+) correlations in the recognition of interprofessional collaboration : the group who replied that they can utilize information on the patients associated with the prosthesis production (p<0.01); the group who thinks that they are sharing common goals (P<0.01); and the group who replied that Dental personnel are familiar with patients (p<0.01). The following groups also showed higher positive (+) correlations in the recognition of interprofessional collaboration : the group who replied that they frequently discuss patient's information with Dental personnel (p<0.01); the group who thinks they have mutual confidence in one another (p<0.01); the group who thinks that cooperation among their colleagues in dental technique laboratories are being done well (p<0.01); and the group that has high job satisfaction (p<0.01). Conclusion: Effective communication and team work between human resources for providing medical service of the facility are essential for safe and high-quality medical service to patients. Further, such elements are directly connected to efficiency of the entire team. The reliability between dental technician and Dental personnel for cooperation should not be neglected therefore continuous interests in inter-cooperative partnership is required.

병원의 직종별 임금수준에 관한 연구 (An analysis of direct financial compensation of hospital personnel in Korea)

  • compensation is a major function of human resources management. The hospital industry is characterized by its remarkable labor intensity and human resource input by unit. That is why the hospital industry has a higher level of wage/cost ration. The issues of how much the hospital personnel's direct financial compensation amounts to and how the organizational and other factors generate compensation differentiation, are central problems for research in hospitals. But there have been few approaches to study staff compensation in hospitals, its magnitude and inter-hospital relative compensation amounts for the same personnel. A worker who moves from low-wage to a high-wage employer can usually increase his or her pay without change in job description. This means in the cases of same jobs, relative importance is different for each hospitals. The purpse of this study were to find the compensation levels of hospital personnel and to determine the factors affecting compensation levels of hospital personnel. The unit of analysis is the hospital and 145 hospitals were studied for nurse(RN), medical technoloist(MT), managerial personnel(MP) and 100 hospitals for medical doctor(MD). In this study the definitions of direct financial compensation are before tax, excluding employer's contriution and total annual remuneration received by the employee. Main findings of the research can be summarized as follows. 1. Direct financial compensation of hospital personnel are MD 45,056,000 won, RN 9,222,000 won, MT 9,513,000 won and MP 9,185,000 won in the starting year's employment in hospital. 2. According to determinants of hospital personnel compensations, there are no statistical significant variables to determine the level of MD's compensation. Wlith RN and MT's compensation level, the greater the patient revenue per 100 hospital beds, the higher the RN compensation and the tertiary hospital's compensation is much more than other types of hospitals. The location of hospital is another determinant factor for the MT's compensation level. Hospitals that are in the uban area have lower compensation level than rural area. There are the same results in MP with MT. Conclusions can be drawn from the results of the study. First, the wage differentiation of MD and other health personnel still remains and the differentiation existed in inter and intra job personnel of hospitals. Second, determinants of hospital personnel personnel compensation level are patient revenue, location, and type of hospital level.

병원 전 응급의료종사자의 감염관리 실태 연구 - 전북지역 중심으로 - (A Study on the Actual Profile of Emergency Medical Personnel's Pre-admission Infection Control - Based on Jeollabuk-do provincial area -)

  • Purpose: By identifying the actual profile of emergency medical personnel's pre-admission infection control practices, this study intended to provide a basic reference material for the improvement and reorientation of pre-admission infection control measures, and thereby help establish an effective plan for pre-admission infection control activities. Methods: Total 119 EMT's working for Jeollabuk-do Provincial Fire Defense HQ were asked to join a structured questionnaire survey from June to August 2006. Results: 1. It was found that 56.1% respondents answered no guideline available on the prevention of infection. Out of our rescue brigade members who knew about relevant guideline available, 34.2% respondents answered that their department conducted quality control program for the guideline. 2. For protective outfit in emergency practice, it was found that most respondents put on gloves or nothing at all(38%), which was followed by sterile gloves(29.2%), disposable mask(26.9%), gown(4.3%) and protective goggle(1.6%). And it was noted that all respondents(100%) washed out any clothing contaminated with somatic secretion on their own. 3. For a question about any experience in emergency activities exposed to infectious diseases, it was found that most of all respondents(77.9%) answered 'No', which was followed by 'Don't Know'(18.6%) and Yes(3.9 %). 4. For a question about any experience in inquiring of patients about infectious diseases, it was found that most respondents(49.4%) answered 'Yes' and 'Sometimes'(9.1%). It was noted that 20.2% respondents had extra medical examination in medical institution in terms of whether they were exposed to infectious diseases, apart from regular medical examination. Conclusions: In order to protect 119 EMT's from infectious diseases, it will be necessary to acquire emergency medical staffs specializing in infection control and organize corresponding personnel training units to keep providing reorientation and evaluation. In addition, it will be also necessary to supply them with a full set personal protection apparatuses and other equipments required for disinfection and sterilization.
