산머루 관련 정보수집 및 데이터베이스의 구축 (Data Mining and Construction of Database Concerning Effects of Vitis Genus)

  • The database for the oriental medicine had been existed in documentation in past times and it has been developed to the database type for random accesses in the information society. However, the aspects of the database are not so diversified and the database for the bio herbal material exists in widened type dictionary style. It is a situation that the database which handles the in-depth raw herbal medicines is not sufficient in its quantity and quality. Korean wild grape is a deciduous plant categorized into the Vitaceae and it was found experimentally that it has various medical effects. It is one of the medical materials with higher potentiality of academic study and commercialization recently because it has a bigger possibility to be applied into diverse industrial fields including the medical product for health, food and beauty. We constituted the cooperative system among the Muju cluster business group for Korean mountain wild grapes, Physiology Laboratory in Kyung Hee University Oriental Medicine and Medical Classics Laboratory in Kyung Hee University Oriental Medicine with a view to focusing on such potentiality and a database for Korean wild grapes was made a touchstone for establishing the in-depth database for the single bio medical materials. First of all, the literatures based on the North East Asia in ancient times had been categorized into the classical literature (Korean literature published by government organization, Korean classical literature, Chinese classical literature and classical literature fro Korean and Chinese oriental medicine) and modern literature (Modern literature for oriental medicine, modern literature for domestic and foreign herbal medicine) to cover the eastern and western research records and writings related to Korean wild grapes and the text-mining work has been performed through the cooperation system with the Medical Classics Laboratory in Kyung Hee University Oriental Medicine. First of all, the data for the experiment and theory for Korean wild grape were collected for the Medline database controlled by the Parliament Library of USA to arrange the domestic and foreign theses with topic for Korean wild grapes and the network hyperlink function and down load function were mounted for self-thesis searching function and active view based on the collected data. The thesis searching function provides various auxiliary functions and the searching is available according to the diverse searching/queries such as the name of sub species of Korean wild grape, the logical intersection index for the active ingredients, efficacy and elements. It was constituted for the researchers who design the Korean wild grape study to design of easier experiment. In addition, the data related to the patents for Korean wild grape which were collected from European Patent Office in response to the commercialization possibility and the system available for searching and view was established in the same viewpoint. Perl was used for the query programming and MS-SQL for database establishment and management in the designing of this database. 생쥐 신경교세포 유래 신경영양인자 유도성 전사인자 (mGIF) 유전자의 유전체 구조 및 프로모터 특성 분석 (Genomic Organization and Promoter Characterization of the Murine Glial Cell-derived Neurotrophic Factor Inducible Transcription Factor (mGIF) Gene)

  • 생쥐 신경교세포 유래 신경영양인자 유도성 전사인자(mGIF)의 발현조절에 필요한 전사기작을 연구하기 위하여 mGIF cDNA를 탐침자로 이용하여 genomic clone을 분리하였다. 전체 유전자 13-kb 영역 중 전사개시점에서 4-kb 상류영역의 유전자 서열을 파악한 결과, 프로모터 영역에서 TATA box와 CAAT box는 발견할 수 없었으며 G+C content는 높은 것으로 나타났고 여러 개의 Sp1 전사인자 결합영역이 있었다. 또한 mGIF 유전자는 AP2 결합에 필요한 보존적 영역이 있었다. mGIF 유전자의 프로모터 영역의 단편들을 프로모터가 없는 pGL2-Basic 플라스미드의 luciferase 유전자의 상류에 연결하여 서로 다른 5종류의 결손 돌연변이체를 제조하고 NB41A3 세포주를 이용하여 전사활성을 측정하였다. Transient expression assays 결과, 모든 결손 돌연변이체에서 전사활성이 나타났으며 -213과 -129사이에 전사촉진 영역이 존재하며 -806과 -214사이에 전사억제 영역이 있는 것으로 나타났다. 신경세포주인 NB41A3과 신경교세포주인 C6 그리고 간세포주인 HepG2에서 mGIF 유전자 프로모터의 높은 활성이 관찰되었으며, 근육세포주인 C2C12에서는 낮은 활성이 관찰되었다. 따라서 mGIF 유전자는 조직특이적으로 발현하며 도파민 수용체 유전자와 구조적, 기능적 유사성이 있는 것을 알 수 있었다.

의약분업(醫藥分業) 실시(實施)에 따른 보건소(保健所)의 내부변화(內部變化)와 업무개선방안(業務改善方案) (Internal Changes and Countermeasure for Performance Improvement by Separation of Prescribing and Dispensing Practice in Health Center)

  • 보건소의 의약분업 시행에 따른 업무변화와 업무 개선방안에 대해 조사 분석하여 보건소의 기능 및 역할 재정립에 필요한 기초자료를 얻고자 2001년 4월과 5월에 경상북도내 25개 보건소와 대구광역시 6개 보건소의 소장 또는 과장에게 의약분업 실시 전후의 보건소 업무 및 진료실적변화 정도를 조사하였고, 이와 함께 보건소 공무원 221명에게 의약분업에 따른 보건소 업무개선방안에 대해 설문 조사하였다. 31개 대상 보건소 가운데 77.4%인 24개 보건소가 주민진료편의 조치를 취하였다고 하였다. 주민 진료편의 조치를 한 보건소의 조치내용으로 약국배치도마련(73.9%), 인테리어 개선(39.1%), 전자처방전달시스템 도입(34.8%) 순이었다. 의약분업 실시 후 의사는 대상 보건소의 3.2%에서 감소하였다. 의약분업에 따라 월평균 진료건수는 대상 보건소의 58.1%에서 감소하였다고 하였고, 조제건수는 96.4%, 총진료비는 80.6%, 본인부담금은 80.6%, 약품구입비는 96.7%의 보건소에서 감소하였다고 하였다. 의약분업 실시 이후 진료부문에 비해 보건사업 부문의 비중은 54.2%의 보건소에서 증가하였다고 하였다. 의약분업 전후이 분기별 진료실적을 분석한 결과 진료실인원은 의약분업이전과 비교하여 의약분업 이후에 감소하였고, 진료연인원은 군보건소와 보건의료원은 감소하였으며, 시화 구보건소는 감소했다가 점차 증가하고 있다. 조제건수 총진료비 본인부담금 약품구입비는 크게 감소하였다. 보건소 공무원들은 의약분업 실시 이후 진료부문의 기능에 대해서는 57.6%가 축소시켜야 한다고 하였고, 보건소에서 우선적으로 개선해야 할 부분으로는 보건사업내용 개발(62.4%), 인력재배치(51.6%), 사업우선순위 결정(48.4%), 조직개편(36.2%), 진료서비스의 질 향상(32.1%), 예산재배치(23.1%) 순으로 응답하였다. 보건소의 이미지를 개선하기 위해서는 지역주민건강정보관리 강화(60.7%)가 가장 시급하다고 하였으며 홍보를 통한 보건소의 이용 확대(15.8%), 보건소 공무원의 친절(15.3%), 건강상담요원 배치(8.2%) 순이었다. 의약분업 실시 이후 바람직한 보건소 역할 설정을 위하여 보건소 전체 업무 영역에 대해 의약분업 이전과 이후에 상대비중을 매기도록 한 결과 25개 세부영역 중 일반진료 및 응급진료 영역만 모두 상대비중이 높아졌다. 의약분업 이후 보건소가 중점을 두어야 할 우선 순위 5위까지의 업무영역은 순서대로 예방접종, 건강증진, 모자보건, 급만성전염병, 지역보건의료계획 이었다. 향후 보건소가 바람직한 공공보건의료조직으로 기능 및 역할을 재정립하기 위해서는 의약분업이라는 중대한 보건의료환경변화를 계기로 진료부문의 기능은 축소하되 노후시설 장비의 개선, 진료방식의 다양화, 건강정보관리 강화 등 진료서비스의 내용과 질에 있어서는 강화하는 방향으로 나아가야 할 것이다. 또한 인력재배치 및 조직개편과 함께 다양한 보건사업의 개발과 지역특성에 맞는 사업우선순위에 의해 예방접조, 건강증진, 모자보건, 급 만성전염병, 지역보건의료계획 수립, 구강보건, 만성퇴행성질환 등 지역주민의 건강증진 질병예방 기능을 강화하되 지역특성(대도시, 중소도시, 농어촌)에 맞게 예방위주의 건강 증진업무와 환자 진료업무의 비중을 차별화 시키는 방향으로 개선해 나가야 할 것이다.

  • The purpose of this study is proving the effects of the internal customer satisfaction management on worker's job satisfaction at general hospital. In the relationship between the management and the satisfaction, the study also finds the moderating effect of the organizational support awareness and the organizational justice. For this study, it was surveyed targeting 270 employees who work in seven general hospitals in Busan. As the result, 246 questionnaires returned. All of these questionnaires were used in the final analysis. The method of analysis is descriptive statistics using SPSS VER. 18.0, correlation analysis, regression analysis, factor analysis and reliability analysis. The main results of empirical analysis were as follows: First, in the effect of the internal customer satisfaction management to worker's job satisfaction, it has positive effect to internal communication and the commission of authority and it has positive effect to worker's job satisfaction. Second, the organizational support awareness & the organizational justice have moderating effect on the relationship between the internal customer satisfaction management and worker's job satisfaction. Third, the study proves that supporting management hierarchy shows positive effects for all socio-demographic characteristics in the organizational support awareness and the organizational justice.

  • 1. Objectives Sasang Constitutional Medicine is based on difference of Nature & Emotion(性情). The mind is held in high value to characteristics of each constitution's symptoms and prescriptions. This study is for psychotheraphy with Herbal medicines, Acupuncture and Moxibustion in Sasang Constitutional Medicine. 2. Methods Comparing with ${\ulcorner}$DongYi Soose Bowon Gabobon${\Ircorner}$ and ${\ulcorner}$DongYi Soose Bowon ShinChukbon${\Ircorner}$, we research the principle of human's organization which is connected with their talent and will. 3. Results and Conclusions (1) Sasang Constitutional Medicine consider human being more than disease in medical science. A Human being works up to take a reasonable in modern societies, as making friends, a practice of his daily life, mentality. (2) Psychotheraphy is as important as each constitution's symptoms and prescriptions on clinical medicine (3) Taeyangin should be moderate when expressing their sorrow & anger(哀怒) and take a step backward for a real purpose(恒心). (4) Soyangin should be moderate when expressing their sorrow & anger(哀怒) and watch the interior for a real purpose(恒心). (5) Taeumin should be moderate when expressing their happiness & pleasantness(喜樂) and watch the exterior for a real purpose(恒心). (6) Soeumin should be moderate when expressing their happiness & pleasantness(喜樂) and go one step forward for a real purpose(恒心).

1개 대학 한방병원에서 EMR을 통해 보고된 한약에 의한 약물유해반응의 현황 (Status of Herbal-drug-associated Adverse Drug Reactions Voluntarily Reported by EMR)

  • Objectives : The aim of this study was to systematically investigate herbal-drug-associated adverse drug reactions (herbal ADRs) reports submitted by a single oriental hospital and to analyze the general characteristics, causative agents, clinical manifestations, severity and types of herbal medicines which caused herbal ADRs. Methods : This study proceeded with IRB approval. The data on herbal ADR were collected prospectively from January 2008 to February 2012 by EMR of Dongguk University Ilsan Oriental Hospital. The World Health Organization (WHO)-Uppsala Monitoring Center (UMC) criteria was used to determinate causality for each herbal ADR. WHO-Adverse Reaction Terminology (WHO-ART) System Organ Class (SOC) code and WHO severity category were also used in this study. Results : A total of twenty eight cases were reported. Twenty two cases were assessed to have over possible relations with herbal medication. The gender ratio of these cases were 64.6 percent female and 36.4 percent male, demonstrating no statistical significance. Patients aged over 60 were 59.1%. Gastro-intestinal system was reported to be the most frequently affected organ (38.8%), and followed by psychiatric system (22.4%), and integumentary system (22.4%). The most common clinical symptom was headache (12.2%), followed by diarrhea (10.2%), and pruritus (10.2%). The severity of most cases was assessed to be mild (89.8%). The percentage of moderate ones was 10.2%, and there were no severe cases. Conclusions : Progressive study and further analysis on herbal ADRs are warranted for safety in the clinical use of herbal medicines.

단일 상급종합병원에서 마약성 진통제의 최근 13년간 사용 경향 조사 (Trends in the Consumption of Opioid Analgesics in a Tertiary Care Hospital from 2000 to 2012)

  • Background: World Health Organization considers opioid analgesic use as an important measure in the treatment of pain relief. However, there are limited data about the pattern of opioid analgesic use in tertiary care hospitals in Korea. The aim of this study was to describe the trends in the prescribed amount of the opioid for 13 years from 2000 to 2012 in a single tertiary care hospital. Methods: The data from the prescribed amount of opioid use in patients aged over 18 years were retrieved from medical charts and longitudinal pharmacy records of Seoul National University Hospital. Yearly prescribed amount of opioids were calculated using defined daily dose adjusted by hospital stay (DDD/1000${\bullet}$HS). Results: Over the 13 years of the study period, overall use of opioid has increased by 64.1%. Although, the opioid use by hospitalized patients comprised 98%~99% of total amount of opioid use, the proportions of opioid use by outpatient and by cancer patient increased from 1.1% to 2.2% and from 60.5% to 69.3%, respectively. The use of non-injectable opioids has increased by 47% and that of injectables has increased by 70%. While the amount of codeine and morphine use has decreased, the use of both transdermal and injection formulation of fentanyl has increased dramatically. Also, the use of oxycodone has increased, especially in outpatient setting. Conclusion: This longitudinal study showed that opioid analgesic use in tertiary hospital, especially in outpatient is continuously increasing. Improvement in pain management in tertiary care hospital can be cautiously inferred based on this results.

문헌에 수록된 전약(煎藥)에 관한 연구 (Study on Jeonyak in the Bibliography)

  • This study was aimed at research on Jeonyak (煎藥) appearing in the bibliography. It was during the Goryeo Dynasty that Jeonyak was first mentioned in literature. At this time, Jeonyak was served for consumption during Palgwanhoe. The ingredients and recipes of Jeonyak were listed for the first time in Suunjapbang, a book written by Kim Yu during the Joseon Dynasty. Since then, they have been found in various books and materials. During the Joseon Dynasty, Jeonyak was made in Neuiwon, a medical administrative organization in the palace, and administered as a seasonal food on Dongji Day in the winter. The king gave various to his guests or subjects as special gifts. As a result, Jeonyak became well known to many people and even to those in foreign countries. Jeonyak is a Korean traditional medicated diet food made from decocted beef-feet, bone stock, and other spices, including jujube paste, honey, ginger, pepper, clove, and cinnamon. Jeonyak has a long 800-year history, and its ingredients and recipes have changed only gradually. Milk was a major ingredient of Jeonyak during the Goryeo Dynasty, but glue and gelatin were added in the Joseon Dynasty. Since then, recipes have mainly used gelatin made from beef-feet, skin, beef-bone, and so on. In conclusion, Jeonyak has an 800-year history as a special medicated diet food (藥膳) served seasonally on Dongji Day in the winter.

Advantages of Distal Subtotal Gastrectomy over Total Gastrectomy in the Quality of Life of Long-Term Gastric Cancer Survivors

  • Purpose: This study evaluated differences and shifting patterns in the health-related quality of life (HRQoL) of 5-year gastric cancer survivors after either a distal subtotal gastrectomy (DSG) or total gastrectomy (TG). Materials and Methods: We analyzed the prospectively collected HRQoL data of 528 patients who survived 5 years without recurrence using the European Organization for the Research and Treatment of Cancer (EORTC) Core Quality of Life Questionnaire and the EORTC Quality of Life Questionnaire-Stomach module according to the type of surgery. The purpose was to identify the proportion of patients with deteriorating HRQoL and to assess the clinical significance of these changes. Results: Deteriorating HRQoL was prevalent in both groups, including a large proportion of the DSG group. Decreased overall health status and scores on several function scales were less in the DSG group, while increases on the symptom scales were higher in the TG group. For most of the scales, gaps in HRQoL during the early postoperative period did not merge within the 5 years. Scores on the diarrhea and body image scales revealed "moderate changes" in both groups. Conclusions: During the 5-year period after surgery, the TG group suffered from inferior HRQoL compared to the DSG group. However, a large proportion of the DSG group also suffered HRQoL deterioration. In general, the TG group experienced more HRQoL decline, with diarrhea and body image being the major concerns for both groups. To improve HRQoL after gastrectomy, patients must be better informed about post-gastrectomy symptoms. These symptoms must be vigorously investigated, and medical interventions should be available parallel to nutritional support. Favorable evidence of function-preserving gastrectomy should be established and disseminated to improve the HRQoL of early gastric cancer patients.

간호사의 근무부서 이동이 직무동기, 조직몰입에 미치는 영향 (A study on the relationships among inter-department rotation. job motivation and organizational commitment of nurses)

  • This study was conducted to offer a basic material for effective human resource management of nurses by studying correlations among inter-department rotation, job motivation and organizational commitment. For this study 159 nurses in a medical college hospital were surveyed. Organization Assessment Instrument(Van de Ven. 1980) and Organizational Commitment Questionnaire (Mowday, 1979) were used. SPSS was used to analyze the data. The results of this study are as follows : 1. Nurses who had been rotated to other departments showed higher motivation scores than those who had not. 2. Nurses who had been rotated also indicated higher organizational commitment scores than those who had not. 3. There was significant positive correlations among nurse's age, job motivation and organizational commitment of nurses. 4. There was significant positive correlations among nurse's experience, job motivation and organizational commitment of nurses. 5. Inter-department rotation times were significant positive correlation with job motivation. 6. "The sense of achievement perceived form performing one's duty" was the most important criteria that nurses consider in measuring their job motivation. As to the criteria for inter-department rotation, some respondents stated that it is needed when "current assignments are not suited for particular individuals" and such rotations would job motivation and elevate the morale of nurses involved." Many opposed using the rotations as a "means of reprimanding individuals." In conclusion, inter-department rotation has a positve effect on the nurse's job motivation and organizational commitment, so regular inter-department rotation have to be recommended to nurses who want rotation.
