Study on the Role and Adoption of Medical Laser Safety Officer (MLSO) (의료레이저안전관리자의 역할과 국내 적용 방안 연구)

  • According to the development of laser technology, applications of laser in medical area, and the resulting risks are increasing. To ensure the safe use of laser, it is necessary to assign a person who has the knowledge of laser safety as MLSO(medical laser safety officer). In many countries, the system of laser safety officer management is operating. MLSO has the responsibilities and duties of the operation of the laser safety program, training, and education. In this study, we analyzed the adoption of MLSO through the investigation of the MLSO management status in United States.

  • Background and Objectives Skin and soft tissue defects can be treated according to a range of strategies, such as local flap, skin graft, biological dressing, or free flap. On the other hand, free tissue transfer usually leaves a distinct scar with an inconsistency of color or hypertrophy. This problem is highlighted if the defect is located on the face, which could have devastating effects on a patient's psychosocial health. Materials and Methods The authors used an erbium : yttrium-aluminum-garnet (Er:YAG) laser to resurface the free flap skin and match the color with the surrounding facial skin. This study evaluated the effectiveness of laser skin resurfacing on the harmonious color matching of transferred flap. Patients who had undergone laser resurfacing on facial flap skin between January 2014 and December 2018 were reviewed retrospectively. An ablative 2,940-nm fractional Er:YAG laser treatment was delivered to the entire flap skin at 21 J/cm2 with the treatment end-point of pinpoint bleeding. Several months later, the clinical photographs were analyzed. The L*a*b* color co-ordinates of both the flap and surrounding normal skin were measured using Adobe Photoshop. The L*a*b* color difference (ΔE) for the scar and normal surrounding skin were calculated using the following equation: ${\Delta}E=\sqrt{({\Delta}L)^2+({\Delta}a)^2+({\Delta}b)^2}$ Results All five patients were satisfied with the more natural appearance of the flaps. The ΔE values decreased significantly from the pre-treatment mean value of 19.64 to the post-treatment mean value of 11.39 (Wilcoxon signed-rank test, p = 0.043). Conclusion Ablative laser resurfacing can improve the aesthetic outcome of free tissue transfer on the face.

  • Various laser systems have been widely used in almost all industrial technologies because they have high energy density, directivity and coherence. Recently the clinical application is becoming wider in medical parts such as incurable disease, diagnosis and so on. Generally, ruby laser beam has the greatest efficacy for removing tattoos, freckle and other skin problem. But current medical ruby laser system has the maximum repetition rate of 2Hz and optical output beam energy of 1J. Many medical doctors really want to have a high repetition ruby laser system because that can reduce the operation time. We investigated a new ruby laser system with high repetition rate of 4Hz using double cavities. Furthermore, we develop a new power supply system adopting zero voltage switching(ZVS) to minimize switching loss by LLC resonant converter designed as 2kW class.

Use of piezoelectric surgery and Er:YAG laser:which one is more effective during impacted third molar surgery?

  • Background: Reduction in postoperative complications is of vital considerations in impacted third molar teeth surgery. The aim of this study was to compare postoperative complications of impacted third molar surgeries for bone removal using laser, piezoelectric equipment, and conventional rotary instruments. Methods: To address the research purpose, the investigator designed the prospective double-blind clinical trial study. The sample size was determined 20 (40 teeth) by sampling formula in any kind of operation. The data of patients were obtained in the different periods in terms of pain, trismus, swelling, ecchymosis, and patient's satisfaction and then analyzed using SPSS 20 software via paired t test and Wilcoxon and McNemar's tests. Results: The pain immediately after surgery and 2 days and 7 days after surgery was higher in the laser group. The swelling immediately after surgery was more in the laser group but not significant. The amount of mouth opening immediately after surgery and 2 days and 7 days after surgery was significantly lower in the laser group than in the piezosurgery group. The total duration of surgery and duration of osteotomy were significantly longer in the laser group. The patient's satisfaction from surgery with piezosurgery was more than that with laser, but this difference was not significant. Conclusion: Due to the rising demand for impacted wisdom tooth surgery, the present study suggests that hard tissue laser surgery and piezosurgery can clear the future of impacted molar surgery, and these approaches are more efficient in reducing postoperative complications compared to the conventional surgeries.

  • Objectives : To get to know the effects of placental extract and laser theraphy for Temporomandibular Joint Disorder, we have tried a injection of placental extract and laser theraphy into 聽宮(SI19), 聽會(GB02) for patient with Temporomandibular Joint Disorder. Methods : In this case clinical assessment of temporomandibular joint dysfunction is assessed by the VAS score and the Korean Version of Research Diagnostic Criteria for Temporomandibular Disorders(RDC/TMD). Results : The VAS score was significantly decreased after Placental extract herbal acupuncture and laser theraphy Conclusion : Placental extract herbal acupuncture and laser theraphy effectively resolved pain for Temporomandibular Disorders.

  • Objective : This study was designed to assess effects of He-Ne laser irradiation on high-level of Total Cholesterol and Triglyceride. Methods : The 76 patients who have been checked Total Cholesterol and Triglyceride high were divided into a study group of patient who were irradiated and into a control group of patients who did not receive irradiation.. Then, patients rechecked Total Cholesterol and Triglyceride after 1 month later. Results : In patients of study group, The level of Total Cholesterol and Triglyceride was decreased. But In comparison with control group, there was no significant effect of decrement of Total Cholesterol and Triglyceride. Conclusions : These results suggest that He-Ne laser irradiation may have no significant decrement effects on high-level of Total Cholesterol and Triglyceride.

  • Transmycardial laser revascularization has made its position as a sole therapy for patients with chronic angina nonamenable to maximal medical therapy, percutaneous transluminal coronary angioplasty, and coronary artery bypass grafting. We report three cases of transmyocardial laser revascularization as a sole therapy for patients with recurrent angina after CABG.

Live Blood Analysis on Interior Vascular Laser Irradiation Therapy and Exterior Vascular Laser Irradiation Therapy (혈관레이저 치료와 관련한 생혈액 검사의 진단 의미 고찰)

  • Objectives : The purpose of this study was to investigate the usability of live blood analysis on interior and exterior vascular laser irradiation therapy. Methods : We had analyzed the changing forms of the live blood sample with microscope before and after vascular laser irradiation therapy of blood. The live blood analysis was operated on Rouleau of red cell, erythrocyte aggregation, thrombocyte aggregation, uric acid crystals, red crystals, protoplasts. First, we analyzed all patients on each item, then did same thing classified two groups, Interior and exterior. Results : Rouleau of red cell, erythrocyte aggregation, thrombocyte aggregation, uric acid crystals, red crystals, protoplasts were decreased significantly, after interior and exterior aggregation, uric acid crystals. Interior vascular laser irradiation therapy was more effective than interior on Rouleau of red cell, erythrocyte aggregation, thrombocyte aggregation, uric acid crystals. Interior vascular laser irradiation therapy was more effective than exterior on red crystals, protoplasts. Conclusions : This study suggests that live blood analysis has the usability on vascular laser irradiation therapy. Then according to interior and exterior vascular laser irradiation therapy, the result has some different on each item. So it is better that choose the method, interior or exterior, for more effective therapy.

The efficacy of low-level diode laser versus laser acupuncture for the treatment of myofascial pain dysfunction syndrome (MPDS)

  • Background: Myofascial pain dysfunction syndrome (MPDS) is the most common type of temporomandibular disorder. This study compared the efficacies of low-level diode laser therapy (LLLT) and laser acupuncture therapy (LAT) in the treatment of MPDS. Methods: This double-blind randomized controlled clinical trial included 24 patients with MPDS who were randomly divided into two equally sized groups. Patients in the LLLT group received 12 sessions of low-level diode laser irradiation applied to the trigger points of the masticatory muscles during 1 month. The same protocol was also used in the LAT group according to the specific trigger points. We measured pain intensity and maximum mouth opening in both groups at baseline, during treatment, and 2 months after treatment completion. Results: The pain intensities decreased from 6.58±1.31 to 0.33±0.65 and from 7.08 ± 1.37 to 0 in the LLLT and LAT groups, respectively. The maximum mouth openings increased from 32.25 ± 8.78 mm to 42.58 ± 4.75 mm and from 33 ± 6.57 mm to 45.67 ± 3.86 mm in the LLLT and LAT groups, respectively. Pain intensity (P = 0.839) and level of maximum mouth opening (P = 0.790) did not differ significantly between the groups. Conclusion: Our results showed similar efficacy between LLLT and LAT in the treatment of MPDS signs and symptoms.

Effects of Propolis on 808 nm Laser Induced Wounds

  • Wound healing is a complex and highly organized biological response to injury that results in the loss of tissue integrity. Our particular interest was propolis, traditional used as an antimicrobial and an anti-inflammatory agent. The purpose of this study was to see the effects of propolis on healing of the laser induced wounds and the level of collagen formation. 808 nm laser (20 J) was irradiated on the back of rats. Irradiated wounds were divided into gel control and 6 experimental groups (3 and 5% of different three kinds of propolis). Gel type of propolis (supplied by Seoul Propolis Co., Daejeon, Korea) in water based were applied on the laser wound once daily for 14 days. One fourth of rats were sacrificed on $3^{rd}$, $7^{th}$, $10^{th}$, and $14^{th}$ day and each wound was evaluated for degree of wound healing and the level of collagen formation. Healing of wound was evaluated by measuring and comparing the width and depth of the wounds. The levels of collagen were increased in 3 and 5% gel groups comparing to control on $3^{rd}$ day and the collagen level were increased more on $7^{th}$ day. Wounds of the experimental groups showed better healing in width comparing to the healing of the control. The results of this study demonstrated that propolis accelerated healing of wounds induced by laser irradiation and good collagen formation.
