The purposes of the present study were: (1) to investigate aeromedical dissorders prevalent among ROK air force pilots and (2) to evaluate their detrimental effects on flight performance. The study examined the eight flight-induced disorders primarily responsible for permanent grounding and flying restrictions. According to the survey of 600 randomly selected pilots, 80.2% of the respondents have experienced low back pain, which was 20% higher than the lifetime prevalence in Swedish male workers. Bradyacusia ranked second (56.0%) resulting from continuous exposure to high aircraft noise. The cardiovascular disorders, the most common cause for permanent grounding of Canadian forces pilots, were found in 16.0% of the Korean pilots. Due to the high prevalence of the disorders, in 1993 the Korean pilots received medical treatments 10 times as many as factory workdes. Compared to the flight performance of Israeli and British pilots in wars, the flight performance of the Korean pilots was found to be inferior. Such poor performance can be attributable to the high incidence rates of disorders. Statistical analyses revealed that the aeromedical disorders were related to personal variables such as age, flight career- related factors, and fatigue. Cigarette smoking and drinking habit contributed to worsening back pain, barotitis media, and pulmonary disorders. More comprehensive research was recommended to identify health hazards, combined with intervention to minimize disorders that could lead to early termination of flight careers or excessive loss of flight time. This study further suggests that aeromedical institutions in Korean air force should be fostered more professionally and financially.
Purpose: The purpose of this study was to understand and describe the experiences of patients with Meniere disease. Methods: Data were collected from February 19, 2019, to February 5, 2020, through individual in-depth interviews with 13 Meniere patients. Verbatim transcripts were analyzed using Colaizzi's phenomenological analysis. Results: The experiences of patients with Meniere disease were clustered into the following four themes from 22 meaning units: 1) Complex unfamiliar symptoms that shatter both balance of the body and peace of the heart; 2) A disease that medical treatment and health professionals cannot cure; 3) Suffering hardships that cannot be understood by non-Meniere sufferers; and 4) Making daily efforts to become healthier. Symptoms of Meniere disease are life-shattering and depressing because they are neither visible nor easily curable. Over time, as they accepted the reality of living with the disease, the participants would shift their focus from complete symptomatic cure to leading a healthy and more balanced life. Conclusion: This study shows that Meniere disease has a pervasive impact on all aspects of the patients' lives. Patients are prone to experiencing restrictions in their social functioning and activities. They also experience psychosocial problems due to the unseen nature of their symptoms. This study elucidates the experiences of Meniere patients and the need for nursing intervention to help improve their quality of life and ability to self-manage. Lastly, this study shows the need for a coordinated interdisciplinary approach to raising public awareness of the disease.
Background : Chemotherapy-induced peripheral neuropathy (CIPN) is a common adverse effect in cancer patients who were exposed to chemotherapy. CIPN impacts on the quality of life and could delay chemotherapy. The aim of this review was to assess the therapeutic effectiveness of herbal medicine in CIPN patients. Methods : Randomized controlled trials (RCTs) were included in this review. We searched MEDLINE, Cochrane database, EMBASE, CNKI, Wanfang and four Korean databases without restrictions on time or language. The risk of bias was assessed using the Cochrane risk of bias tool. Results : Eleven RCTs involving 706 patients met the inclusion criteria. Eleven different herbal medicines were examined in the included trials. Almost RCTs showed insufficiency in the reporting randomization method and allocation concealment. One trial used allocation concealment and a double-blinding method. Five studies reported that participants dropped out of RCTs and conducted an 'as-treated analysis'. One trials reported adverse effects of herbal medicine. In ten of the eleven trials, the use of herbal medicine had shown significant differences in clinical symptoms or nerve conduction velocity. Conclusions : The use of herbal medicines for CIPN showed significant improvements in the management of CIPN. However, conclusions cannot be drawn because of the generally low quality of methodology and low quantity of data for each single herbal medicine. Further rigorous trials are needed.

The advent of personal healthcare record(PHR) technology has been changing the uses as well as the paradigm of internet services, and emphasizing the importance of services being personalization. But the problem of user's privacy infringement and leaking user's sensitive medical information is increasing with the fusion of PHR technology and healthcare. In this paper, we propose a security labeling scheme for privacy protection in PHR system. In the proposed scheme, PHR data can be labeled also manually based on patient's request or the security labelling rules. The proposed scheme can be used to control access, specify protective measures, and determine additional handling restrictions required by a communications security policy.

To evaluate the effectiveness of cupping and bloodletting therapy in the treatment of musculoskeletal diseases. Systematic searches were conducted on KSI, KISTI, DB Pia, KIOM Database, and Koreamed until January 2007 Hand-searches included conference proceedings and our own files. There were no restrictions regarding the language of journals published in Korea. Controlled trials of dry cupping, wet cupping, or blood letting for patients with musculoskeletal disease were considered for inclusion. Trials testing other forms of dry cupping therapy were included. Methodological quality was assessed by two doctors. 20 possibly relevant studies were identified and 5 studies were included. One trial tested wet cupping for ankle sprain and reported positive result. Two trials tested blood letting for low back pain, one was positive and the other one was neutral. One trial tested the types of dry cupping for low back pain, and Ki-gong cupping therapy was superior to other two types of cupping. One trial compared wet cupping with dry cupping for low back pain and the result was negative. The effectiveness of bloodletting plus acupuncture for treating patients with low back pain is superior to acupuncture in spite of low quality. One trial of wet cupping for ankle sprain had effects in reducing pain. However, I suggest that the rigorous RCTs of cupping and blood letting therapy will be conducted in well designed features.

Despite the well-proven, safe and effective therapies for hepatitis B infection, delivery of treatment remains a significant challenge in resource-poor settings. Geopolitical and economic restrictions present additional difficulties in providing care in North Korea. However, treatment of patients with chronic hepatitis B remains a top priority for both the North Korean Ministry of Public Health and international agencies working in North Korean hepatitis healthcare facilities. Working in partnership, a path was created to institute this much-needed program. A consortium of United States and Australian humanitarian non-governmental organizations along with generous individual and corporate donors working in concert with local and national health authorities have succeeded in establishing the first hepatitis B treatment program in North Korea. The essential elements of this program include renovation of existing hepatitis hospitals, access to antiviral medications, establishment of laboratory facilities, creation of medical documentation and record-keeping, training of local health care professionals, and quarterly visits by international volunteer physicians and laboratory experts. Management and treatment decisions are made bilaterally. To date, nearly 1,500 patients have been evaluated, and over 800 have been started on long-term antiviral therapy. It is envisioned that this program will eventually be managed and funded by the Democratic People's Republic of Korea Ministry of Public Health. This program's success demonstrates a potential model for delivery of antiviral therapy for patients suffering from hepatitis B in other developing countries.

Objective: The aim of this study was to provide a systematic review of the Korean literature on the influence of herbal medicine on liver function. Additionally, we tried to estimate the change in the data on liver function tests (LFTs) and the incidence of liver injury (LI) after taking herbal medicine. Methods: Systematic literature searches were performed on 6 major Korean databases (Research Information Sharing Service (RISS), Korea Institute of Science and Technology information (KISTI), National Assembly Electronic Library, Korean Medical database (KMBASE), Korean Traditional Portal, and Dongui University Electronic Library) from January 2010 to June 2020. No restrictions were placed on the types of publications, which included grey literature. Results: The data indicate that herbal medicine might be a minor cause of LI in Korea. However the results are not supported strongly enough to verify the safety issues because of the limitations of the original studies. Conclusions: Additional well-planned studies are still needed that can overcome the heterogeneity and the considerable methodological flaws in the analyzed studies.

When the COVID-19 crisis hit air transport, the whole aviation industry was affected. The change in the behavior of passengers following the COVID-19 crisis, travel restrictions and the ensuing economic crisis have resulted in a dramatic drop in demand for airline services. The COVID-19 pandemic, with all its associated consequences, has had a significant impact on the mental health and well-being of pilots, which could impact operational safety. As the number of international flights has decreased, pilots are exposed to high levels of psychological stress such as job instability, reduced income and increased risk of infection during flight work. Psychological stress lowers work performance and threatens aviation safety. The pilot needs mental health management during the COVID-19 pandemic, but psychological support is weak. For pilot mental health management, it is important to raise awareness about mental health and to continuously respond. To this end, it is necessary to foster a safe and trustworthy culture in which mental health issues are discussed freely and solved together without hiding them. They should also support the establishment of a peer support program that is easily accessible and that allows pilots to discuss their issues with confidence.
Objectives: ncluding stroke. The aim of this study was to systematically review the clinical evidence of CT for stroke. Methods: To identify randomized controlled trials (RCTs) reporting the effectiveness and/or safety of CT, seven databases including PubMed, EMBASE, and Cochrane Library were searched for articles published from January 2000 to February 2021 without language restrictions. Meta-analysis was performed using Review Manager 5.4 software and the results were presented as mean difference (MD) or standard mean difference (SMD) for continuous variables and odds ratio (OR) for diverse variables with 95% confidence intervals (CIs). Assessment of the methodological quality of the eligible trials was conducted using the Cochrane Collaboration tool for risk of bias in RCTs. Results: Twenty-two RCTs with 1653 participants were included in the final analysis. CT provided additional benefit in improving upper limb motor function (Fugl-Meyer assessment for upper limb motor function, MD 6.91, 95% CI 4.64 to 1.67, P<0.00001) and spasticity (response rate, OR 3.28, 95% CI 1.31 to 8.22, P=0.08) in stroke survivors receiving conventional medical treatment. These findings were supported with a moderate level of evidence. CT did not significantly increase the occurrence of adverse events. Conclusions: This study demonstrated the potential of CT to be beneficial in managing a variety of complications in stroke survivors. However, to compensate for the shortcomings of the existing evidence, rigorously designed large-scale RCTs are warranted in the future.
