A Study of Evaluation Index Development of Healthcare Rehabilitation Device Design (헬스케어 재활훈련기 디자인 평가 요소 개발에 관한 연구)

  • Due to the increase of the aged population and population of the disabled today, there is a growing demand for rehabilitation medical instruments. Furthermore, there is a growing demand for evaluation indices for services that should be provided for uses of the rehabilitation medical instruments. In order to evaluate rehabilitation medical instrument designs in this study, the basic index for design evaluations shall be identified to search for assessment plans. Through this, new evaluation indices will be deduced through discussions and analysis of rehabilitation medical experts, biomedical engineers, and designers. The results of this study are summarized as follows. First, the existing design evaluation indices were collected and analyzed to construct 10 rehabilitation medical instrument design evaluation indices and 44 sub-evaluation items. These will be important evaluation standards for designing rehabilitation medical instruments in the future. Second, the design evaluation indices that must be taken into consideration when developing health care rehabilitation medical instruments are the 10 design evaluation indices of usability, cognition, safety, learning, motility, durability, economic feasibility, space, aesthetics and environmental aspects. Third, design evaluation indices of environment, space, cognition, usability, economic feasibility and aesthetics are indices that must be taken into consideration for product design, while learning, safety, motility and durability are factors that must be given special consideration for rehabilitation medical instrument design evaluation indices. Fourth, if existing product design evaluation indices placed importance on environment, space, cognition, usability, economic feasibility and aesthetics of products for design evaluation indices, rehabilitation medical instrument design evaluation indices placed importance on learning, safety, motility and durability on top of usability and economic feasibility, which are the differences between the design evaluation indices of rehabilitation medical instrument and other product designs. The 10 rehabilitation medical device design evaluation indices and 44 sub-evaluation items were carried out in this study. This research is only on the overall rehabilitation medical device design evaluation indices. In future research, the evaluation indices will be applied in the actual rehabilitation medical design device through production of prototypes, while making revisions and supplementations where necessary.

Case Report about Postoperative Rehabilitation of Oriental Medicine for the Distal Clavicular Fracture (쇄골 골절 환자의 수술 후 한방 재활 치료에 대한 증례 보고)

  • The objective of this study is to report the effectiveness of active postoperative rehabilitation on clavicular fracture by oriental medical interventions and to suggest the importance of proper rehabilitation after operation. A female patient, 43 years old, who had operation for right distal clavicular fracture was treated by oriental medical rehabilitation. We evaluated the consequence by checking numeric rating scale(NRS), shoulder range of motion and shoulder pain and disability index(SPADI). The pain of the patient was decreased and the shoulder range of motion was advanced. SPADI score was decreased. Postoperative-oriental medical rehabilitation can be effectively used for a patient on postoperative state of clavicular fracture. Further studies are needed to set up a rehabilitation protocol with oriental medical interventions after operation.

The Domestic Trends of Korean Medical Treatments for Neck Pain Caused by Whiplash Injury (편타성 손상으로 인해 발생된 경부통의 한의학적 치료에 대한 국내 연구 동향)

  • Objectives We attempted to investigate the trends of Korean Medical treatments for neck pain caused by whiplash injury by analyzing domestic clinical studies. Methods We searched for studies listed on Korean electronic databases (KTKP, OASIS, RISS, KMbase, NDSL, KCI, and National Assembly Library). Clinical studies were chosen according to the inclusion criteria. Subsequently, we compared and analyzed the data. Results The search yielded 35 studies, of which there were 12 randomized controlled trials (RCTs), 15 non-randomized controlled trials (NRCTs), 6 case reports, and 2 before-after studies. Among these, 54.7% of participants were in the acute stage of the damage period. Pharmacopuncture was used the most in main treatment methods. Only 14.3% of studies mentioned approval by an institutional review board (IRB) and only 8.6% of studies considered side effects. Conclusions There is some evidence that Korean Medical treatment is effective for neck pain caused by whiplash injury. Since all studies, however, were exposed to the risk of bias. Therefore, future studies of a higher quality are required.

Effectiveness of Oriental Medical Therapy and Bongchuna on Anterior Cruciate Ligament Rupture of Knee, Two Case Reports (슬관절의 전방십자인대파열에 대한 한방치료와 봉추나의 치료효과 증례 2)

  • Anterior cruciate ligament tear is a common disease of knee injury. We report 1 case of anterior cruciate ligament complete tear diagnosed by MRI(magnetic resonance imaging) imaging and 1 case after reconstruction. Outcomes were measured by visual analogue scale(VAS) and Lysholm knee scoring scale. We applied acupuncture treatment, bee venom injection, bongchuna and prescribed herbal medication. Patients who treated by oriental medical treatments had a significant effect on the pain decrease and range of motion and knee function. Oriental medical treatments are very useful on pain reduction and prevention of muscle contraction, leading to satisfied rehabilitation, as wee as diminishing recurrence after operation.

Historical Study of "Wuqinxi" as a Medical Exercise (화타오금희(華陀五禽戱)의 의사학적 고찰 및 현대적 활용가능성)

  • Objectives The purpose of this study is to describe the origin of Wuqinxi and transition to various forms. Also evaluating the feasibility of Wuqinxi as a modern medical exercise. Methods For collecting data, various documents came from China, Taiwan, and Korea. Also we searched Pubmed, KISS, NDSL and National assembly library using "Wuqinxi (오금희)" for clinical trials. Results In this study, Chinese Wuqinxi can be divided into 10 forms, 15 forms, 18 forms, 20 forms, 30 forms and 40 forms while Taiwanese Wuqinxi can be divided into classic form (125 forms) and short form (84 forms). Also we found some evidence that Wuqinxi played a certain role for health promotion. Conclusions These days Wuqinxi has a 8 different major forms and some of the exercises can be used as a preventive medical exercise to promote health for people.

  • Objectives: We want to understand body composition and states of obese patients and the clinical studies of obesity attribute to Dept. of Oriental Rehabilitation Medicine. Methods : The clinical studies were done on 42 cases of obese patients who were treated by obesity clinic in Dept. of Oriental Rehabilitation Medicine Dae-Jeon Oriental medical hospital in Dae-Jeon University from May 2000 to September 2000. We checked body composition using Inbody3.0. Results : Sexual ratio on this report is 1:4.98(male: female). Abnormal eating is Major cause of obesity that has caused orthopedic disease. Increasing of BMI, fluid, protein and mineral mass decrease.

  • Radiation recall is an uncommon phenomenon in which administration of a chemotherapy or another systemic agent induces an acute inflammatory reaction in previously irradiated tissues, often weeks to years after completion of radiotherapy. Gemcitabine can induce an inflammatory reaction within an area of prior radiation. Radiation recall is known to medical oncologists, however only few cases have been reported in Korean journals, therefore physiatrist who diagnose and treat the treatment-related physical impairments of cancer patients must know about it. We emphasize the importance of knowledge of this phenomenon when considering the differential diagnosis of painful limb edema in a patient who has received cancer treatment.

  • In cases of hyponatremia induced by brain damage, it is important to distinguish between the syndrome of inappropriate anti-diuretic hormone secretion (SIADH) and cerebral salt wasting syndrome. A ventriculoperitoneal (VP) shunt is the standard treatment for hydrocephalus, and external lumbar drainage (ELD) is an option to evaluate the effect of a VP shunt. However, ELD has potential complications, such as subarachnoid hemorrhage, meningitis, and rarely hyponatremia. Therefore, we report a case of a patient with cerebral salt-wasting syndrome resulting from ELD to treat normal-pressure hydrocephalus during the rehabilitation of acute ischemic stroke.

A Case Report of Lumbar Spinal Stenosis Improved with Diarrhea-Inducing Treatment by Gamsui-mal and Korean Medicine Treatment (감수말 공하법을 병행한 한방치료로 호전된 요추 척추관 협착증 환자의 증례보고)

  • The purpose of this study is to report the effectiveness of diarrhea-inducing treatment by Gamsui-mal for lumbar spinal stenosis. During hospitalization four patients with lumbar spinal stenosis were treated with diarrhea-inducing treatment by Gamsui-mal and acupuncture, herbal medicine, moxibustion, cupping treatment, Bee-venom pharmacopuncture. Before treatment, we explained in detail about diarrhea-inducing treatment by Gamsui-mal and patients agreed with treatment. The symptoms appeared in patients of diarrhea, abdominal pain, and vomiting in the treatment process. We evaluated the degree of improvement by visual analogue scale (VAS), oswestry disability index (ODI). After treatment, we found that pain was reduced and patient's symptoms were improved by visual analogue scale (VAS), oswestry disability index (ODI) score. The results show that diarrhea-inducing treatment by Gamsui-mal was rapidly improving symptoms. Further studies are needed to clarify the effect of diarrhea-inducing treatment by Gamsui-mal.

The Clinical Effects of Complex Korean Medicine Treatment in Patient with Avulsion Fracture of the Proximal 5th Metatarsal (제5 중족골 근위부 견열골절 환자의 복합 한의치료 효과)

  • The purpose of this study is to report the effect in treating a patient with a case of avulsion fracture of the proximal 5th metatarsal with a combination of Korean medicine therapy. We treated the patient with acupuncture, pharmacopuncture, moxibustion, cupping, and herbal medicine The numeric rating scale, X-ray were applied as outcome measures. After complex Korean medicine treatment, this patient's avulsion fracture of the proximal 5th metatarsal was improved and pain was reduced. This result shows that Korean medical therapy may be an effective option for avulsion fracture of the proximal 5th metatarsal.
