THE EFFECTS OF POSTERIOR RETRACTION ON THE DISPLACEMENT OF THE MAXILLA

  • Three-dimensional finite element model was made from adult skull to find desirable direction of retraction force to treat skeletal class II malocclusion. The retraction force of 400g was applied to the first molar. The direction of the force application was $23^{\circ}$ downward, parallel, $23^{\circ}$ upward and $45^{\circ}$ upward to the occlusal plane. The stress distribution and the displacement within the maxilla were analyzed by three-dimensional finite element method. The findings obtained were as follows: 1. Maxillary first molar was displaced posteriorly and inferiorly in $23^{\circ}$ downward, parallel, $23^{\circ}$ upward retraction but it was displaced posteriorly and superiorly in $45^{\circ}$ upward retraction. 2. ANS, A point and prosthion were moved posteriorly and inferiorly and pterygomaxillary fissure was moved posteriorly and superiorly. Clockwise rotation of maxilla occurred when retraction force was applied. 3. The degree of clockwise rotation of maxilla was greatest when the force was applied $23^{\circ}$ upward to the occlusal plane and was least when the force was applied $23^{\circ}$ downward to the occlusal plane. 4. Large tensile stress appeared in maxillary first molar and alveolar bone and the infraorbital region of maxilla when the force was applied $23^{\circ}$ downward to the occlusal plane. Tensile stress was smaller as the direction of force move upward. 5. Large compressive stress was appeared in maxillary first molar and infraorbital region in $45^{\circ}$ upward case and large compressive stress occurred in the posterior part of maxilla as the retraction force was upward.

Comparison of clinical and histological characteristics of orthodontic tooth movement into recent and healed extraction sites combined with corticotomy in rats

  • Objective: This study was performed to investigate the rate of tooth movement and histological characteristics of extraction sockets those were subjected to corticotomy. Methods: A split-mouth randomized controlled trial experiment was designed. Thirty-two adult, male Wistar rats were divided into 2 groups: healing extraction socket (H) and recent extraction socket (R); these groups were randomly classified into 4 subgroups (0/7/21/60 days). The first maxillary molar was extracted on 1 side and 2 months were allowed for complete bone healing; then, the corresponding molar was extracted on the other side and surgical intervention was performed at the mid-alveolar point of the first maxillary molar. Ten grams of continuous force was applied. The outcomes measured were rate of tooth movement, percentage of periodontal space and histological evaluation. The rate of tooth movement was calculated as the measured distance divided by the duration of molar movement. Histomorphometric evaluations were performed on the second and third maxillary molars. The Wilcoxon signed rank test was used to compare differences between the two groups. Results: There were no significant differences in the rates of tooth movement between H and R groups at any of the 4 time points. The histological appearance and percentage of periodontal space between the R and H groups also demonstrated no significant differences. Conclusions: The rates of orthodontic tooth movement into recent and healed socket sites did not differ between the groups. Histological analysis of tooth movement revealed regional acceleration during every time period.

Influence of zirconia and lithium disilicate tooth- or implant-supported crowns on wear of antagonistic and adjacent teeth

  • PURPOSE. To investigate the influence of crown material (lithium-disilicate, 3Y-TZP zirconia) and abutment type (rigid implant, resin tooth with artificial periodontium) on wear performance of their antagonist teeth and adjacent teeth. MATERIALS AND METHODS. A mandibular left first molar (#36) with adjacent human teeth (mandibular left second premolar: #35, mandibular left second molar: #37) and antagonistic human teeth (maxillary left second premolar: #25, maxillary left first molar: #26, maxillary left second molar: #27) was prepared simulating a section of the jaw. Samples were made with extracted human molars (Reference), crowned implants (Implant), or crowned resin tooth analogues (Tooth). Crowns (tooth #36; n = 16/material) were milled from lithium-disilicate (Li, IPS e.max CAD) or 3Y-TZP zirconia (Zr, IPS e.max ZirCAD, both Ivoclar Vivadent). Thermal cycling and mechanical loading (TCML) in the chewing simulator were applied simulating 15 years of clinical service. Wear traces were analyzed (frequency [n], depth [㎛]) and evaluated using scanning electron pictures. Wear results were compared by one-way-ANOVA and post-hoc-Bonferroni (α = 0.05). RESULTS. After TCML, no visible wear traces were found on Zr. Li showed more wear traces (n = 30-31) than the reference (n = 21). Antagonistic teeth #26 showed more wear traces in contact to both ceramics (n = 27-29) than to the reference (n = 21). Strong wear traces (> 350 ㎛) on antagonists and their adjacent teeth were found only in crowned groups. Abutment type influenced number and depth of wear facets on the antagonistic and adjacent teeth. CONCLUSION. The clinically relevant model with human antagonistic and adjacent teeth allowed for a limited comparison of the wear situation. The total number of wear traces and strong wear on crowns, antagonistic and adjacent teeth were influenced by crown material.

Prevalence of missing and impacted third molars in adults aged 25 years and above

  • Purpose: The purpose of this study was to determine the prevalence of missing and impacted third molars in people aged 25 years and above. Materials and Methods: The study sample of 3,799 patients was chosen randomly from patients who visited Pusan National University Dental Hospital and had panoramic radiographs taken. The data collected included presence and impaction state, angulation, and depth of impaction of third molars, and radiographically detected lesions of third molars and adjacent second molars. Results: A greater percentage of men than women retained at least one third molar. The incidence of third molars decreased with increasing age. The incidence of partially impacted third molars greatly declined after the age of 30. Vertically impacted maxillary third molars and horizontally impacted mandibular third molars were most frequent in all age groups. Among the maxillary third molars, those impacted below the cervical line of the second molar were most frequent in all age groups, and among the mandibular third molars, deeply impacted third molars were most frequent in those aged over 40. Dental caries was the most common radiographic lesion of the third molars. Mesioangularly impacted third molars showed radiographic lesions in 13 (9.5%) adjacent maxillary second molars and 117 (27.4%) mandibular second molars. Conclusion: The number of remaining third molars decreased and the percentage of Class C depth increased with age. Caries was the most frequent lesion in third molars. Partially impacted mesioangular third molars showed a high incidence of caries or periodontal bone loss of the adjacent second molar. Regular oral examination will be essential to keep asymptomatic third molars in good health.

제1유구치의 조기 상실로 인한 공간 변화에 대한 3차원적 분석 (THREE-DIMENSIONAL SPACE CHANGES AFTER PREMATURE LOSS OF THE PRIMARY FIRST MOLAR: A LONGITUDINAL STUDY)

  • 본 연구는 제1유구치의 조기 상실 시공간 변화에 대한 분석을 위하여 3-Dimensional Laser Scanner를 이용하여 그 유용성을 평가하고자 하는 예비논문이다. 6세-8세 사이의 제1대구치가 맹출하여 확실한 교합 관계를 이루는 혼합치 열기 어린이 중 편측 제1유구치의 조기 발치가 필요한 6명의 발치 전 후의 모형을 이용하여 다음과 같은 결과를 얻었다. 1. 유구치 공간변화는 상악은 실험군과 대조군 모두 현저한 감소를 보이지 않았고, 하악은 실험군에서 3명 중 2명에서 공간감소가 관찰되었다. 2. 치열궁 너비, 치열궁 둘레는 초기모형과 최종모형에서 비슷한 값을 보였다. 3. 협설측 경사도 변화는 유견치에서는 상하악 모두 특별한 경향을 보이지 않았으며, 제2유구치는 상악은 실험군과 대조군이 유사하였고, 하악에서는 3명중 2명에서 실험군이 더 큰 설측경사도 변화를 보였다. 제1대구치는 하악에서 실험군이 더 큰 설측경사도 변화가 관찰되었다. 4. 근원심 경사도 변화 측정에서 유견치는 상하악 모두 특별한 경향이 관찰되지 않았고 제2 유구치의 경우 하악에서 실험군이 더 큰 근심경사도 증가가 보였다. 제1대구치는 상악에서 실험군, 대조군 모두 원심 경사도가 증가되었다.

중증 치주염에 의해 발거된 치아의 수평부착상실에 대한 연구 (Horizontal attachment loss in extracted teeth due to severe periodontitis)

  • Purpose: The attachment level is strongly associated with tooth loss and provides useful information on patterns of destruction of the periodontium. The presence of horizontal attachment loss would not be detected in clinical measurement. Therefore, the purpose of the present study was to estimate the patterns of periodontal destruction based on the attachment area and horizontal attachment loss in extracted teeth due to severe periodontitis. Materials and Methods: 307 teeth satisfied the criteria for assessment. An indirect method, based on digital images obtained from a digital camera and an image analysis program, was used to calculate the area of root surface and attachment loss and the extent of horizontal attachment loss. The data were analysed using SPSS. Results: No statistically significant differences among root surfaces were observed in anterior teeth on the loss of attachment area. However, in posterior teeth statistically significant differences in palatal surfaces of maxillary and mandibular premolar and molar surfaces compared with buccal surfaces were observed. Horizontal attachment loss was observed in 21.5% of the teeth examined. Frequency of horizontal attachment loss was highest in the maxillary first premolar (34.8%), followed by the maxillary second premolar (27.3%) and maxillary canine (25%). The mean length of horizontal attachment loss was 1.5mm. Conclusion: More meticulous examination will be needed of the palatal surfaces of maxillary and mandibular premolar and molar teeth. The percentage of teeth with horizontal attachment loss greater than 2.1 mm was 5.2%. Considering the length of curette blades, about 5.2% of teeth were not properly debrided. Therefore, Additional supportive therapy such as local drug delivery has to be considered in treatment of the first maxillary, second premolar and canine due to the high prevalence of horizontal attachment loss.

  • One of the most important factors for successful endodontic therapy is an accurate length determination of physiological root apex. Some methods suggested for the measurement of root canal length, include digital-tactile sense and roentgenographic technique with measuring wire, scale and grid. But these methods do not derermine an accurate working length to physiological root apex. Recently electronic measuring devices are used to locate the physiological root apex in root canal length determination and these devices are accepted as an effective apparatus. The 89 patients (116 teeth, 144 canals) among the out-patients of Yonsei University Dental Infirmary, who had had an endodontic treatment in the Department of Operative Dentistry, were measured by the Root-Canal Meter$^{(R)}$ as an electronic device, and radiographs to determine the distribution and location of physiological root apex, then the following results were made: (1) Range of ${\pm}$1mm from the radiographic root apex were present in 88.88% (128 canals) of the subjects. (2) Physiological root apex and radiographic root apex were coincided in 31.94% (46 canals) of the subjects. 유구치(乳臼齒) 인접면(隣接面) 우식에 관(關)한 연구(硏究) (A STUDY ON THE PREVALENCE OF PROXIMAL CARIES OF DECIDUOUS MOLARS)

  • The author studied the prevalence of proximal dental caries of deciduous molar by observing intraoral radiographies of 478 children aging from 2 to 5 who visited the Dep. of Pedodontics, Seoul National University Hospital. The following results were obtained; 1. The prevalence of proximal caries of deciduous molar did not show significant difference between male and female, right and left side. 2. Mandibular deciduous molar had higher prevalence than maxillary deciduous molar. 3. The prevalence was increased with age. 4. The prevalence was highest in the distal surface of lower 1st deciduous molar and lowest in the distal surface of upper 2nd deciduous molar.

상악 제2유구치의 근심구개측 부가치근과 상악 제2소구치 사이의 연관성 (The Relationship between Additional Mesiopalatal Roots of Maxillary Primary Second Molars and Premolars)

  • 상악 제2유구치는 일반적으로 3개의 치근을 갖는다. 하지만 드물게 근심구개측에 부가치근이 존재할 수 있다. 이 연구의 목적은 상악 제2유구치의 근심구개측 치근과 계승 영구치인 상악 제2유구치의 비정상적인 맹출 양상과의 연관성에 대해서 알아보는 것이다. 2010년 1월부터 2018년 12월까지 경희대학교 치과병원에 내원하여 전산화 단층 방사선 사진을 촬영한 3세부터 12세 어린이 916명을 대상으로 하였으며 744명의 어린이가 최종대상자로 선정되었다. 상악 제2유구치의 근심구개측 치근 발생률은 3.2%였으며 상악 제2소구치가 비정상적인 맹출 양상을 보이는 발생률은 19.2%였다. 상악 제2유구치의 근심구개측 치근이 존재할 때 상악 제2소구치가 비정상적인 맹출 양상을 보일 위험성이 상악 제2유구치가 근심구개측 치근이 없는 경우보다 약 13배 더 높았다. 상악 제2유구치에 근심구개측 치근이 존재한다면, 계승 영구치의 맹출 양상을 주의 깊게 관찰해야 하며 필요 시 치료가 개입되어야 한다.

상악궁 확장술을 통한 치료전, 후 및 보정후 상악 구치부 변화에 관한 연구 (A study on the changes of the posterior segments between before, after treatment and postretention period through maxillary expansion therapy)

  • 치열궁의 크기와 폭경 변화에 대한 연구가 사용된 장치 형태, 사용시기에따라, 그리고 사용 목적에 따라 다양한 연구가 활발히 이루어져 왔다. 또한 Quad-helix도 선학들에게 관심의 대상이었으며 Quad-helix의 안정성과 장치 사용 전, 후에 대한 효과도 연구가 이루어졌다. 그러나 장치 사용에 대한 장기적인 효과에 대한 연구가 미흡하여 본 논문은 이 장치를 사용하여 치료한 환자의 치료전,후 및 보정후의 치열궁 변화에 대한 연구를 시행하여 Quad-helix사용 전, 후 구치부의 적응 양상을 연구함으로써 임상에서 Quad-helix를 통한 치료를 시행할 때 이 장치사용에 따른 효과 및 예후에 대하여 효과를 이해함으로써 교정치료에 도움을 주고자 연구를 시행하여 다음과 같은 결과를 얻었다. 1. Quadhelix를 사용한 교정치료시 상악 제 1 소구치 와 제 1 대구치는 치료전과 치료후 및 보정후에서 교두간 폭경의 증가를 보였고 제 2 소구치에서는 치료후와 보정후간에 유의성있는 증가를 보였다. 2. 상악 제 1 소구치와 제 1 대구치의 치축은 치료전과 치료후 그리고 보정후에서 협측 경사도가 유의하게 증가를 보였으며, 상악 제 2 소구치는 치료전과 치료후 및 치료전과 보정후 비교에서 설측 경사도가 유의하게 증가를 보였다. 3. 최대 풍융부 항목에서 상악 제 1, 2소구치는 치료전과 치료후의 비교에서 유의한 증가를, 제 1 대구치에서는 치료전,후, 및 보정후에 유의한 증가를 보였다. 4. 상악 제 1 대구치의 설면구 폭경은 치료전,후 및 보정기간을 통하여 유의한 증가를 보였고,구개 높이는 치료전과 치료후 비교시 유의한 증가를 보였다. 5. 확장시킨 상악 치열궁은 회귀현상을 보이지 않고 안정적으로 유지되었다.
