Clinical and biological analysis in graftless maxillary sinus lift

  • Maxillary sinus lift for dental implant installation is a well-known and versatile technique; new techniques are presented based on the physiology of intrasinus bone repair. The aim of this review was to determine the status of graftless maxillary sinus lift and analyze its foundations and results. A search was conducted of the literature between 1995 and 2015 in the Medline, ScienceDirect, and SciELO databases using the keywords "maxillary sinus lift," "blood clot," "graftless maxillary sinus augmentation," and "dental implant placement." Ten articles were selected for our analysis of this technique and its results. Despite the limited information, cases that were followed for at least six months and up to four years had a 90% success rate. Published techniques included a lateral window, elevation of the sinus membrane, drilling and dental implant installation, descent of the membrane with variations in the installation of the lateral wall access and suturing. The physiology behind this new bone formation response and the results of the present research were also discussed. We concluded that this is a promising and viable technique under certain inclusion criteria.

Anatomical factors of the maxillary tuberosity that influence molar distalization

  • Objective: To examine the areas of the maxillary tuberosity (MT) (coronal, apical, width, and height) with respect to the presence or absence of the third molar to establish possible anatomical limitations for molar distalization. Methods: A total of 277 tuberosities were evaluated through sagittal computed tomography (CT) images, divided for measurement into coronal (free of bone), apical (area of influence of the maxillary sinus), and tuberosity (bony area) zones, and stratified by the presence or absence of the third molar, sex, and two age subgroups. Mann-Whitney U test was used to compare the groups considering the third molar. Results: The medians of the width and height of the tuberosity decreased significantly in the absence of the third molar (P < 0.001). The apical area also showed differences, with negative values in the absence of the third molar and positive values in the presence of the third molar (P < 0.001). However, no differences were observed for the coronal area (P > 0.05). Conclusions: In the absence of the third molar, the size of the MT, represented by its width and height, was smaller and negative values (decrease) were observed for the maxillary sinus. The sagittal CT provides useful information regarding the amount of bone tissue available for distalization and relationship of the second molar with respect to the maxillary sinus, which allows individualizing each case in relation to the amount and type of movement expected.

${\beta}$-TCP를 이용한 상악동 점막거상술 (SINUS AUGMENTATION WITH ${\beta}$-TCP)

  • Sinus floor augmentation has been proven an effective treatment procedure to increase bone volume in the posterior edentulous maxilla. Autogenous bone considered to be the best material for reconstructive bone surgery and has been successfully used as a graft material to augment posterior maxilla. However, the collection of autogenous bone required extra risks for morbidity and complaints. So, various bone graft materials included ${\beta}$-tricalcium phosphate(${\beta}$-TCP) has been introduced for replacing the autogenous bone. The objective of this clinical study was to determine the predictability of endosseous implant placed in a maxillary sinus with ${\beta}$-TCP grafting. We performed sinus elevation with ${\beta}$-TCP to install the implant in the 10 maxillary cases. The prosthetic procedure was performed 6-9 months after. The implant-prosthetics was checked about 1 year. We checked the implant and measured the maximum bite force to evaluate the function of the implant. There was not observed the specific problem and complication in dental implant and maxillary sinus in the grafted materials. The maximum bite force was 558N in case of natural tooth, 365N in implant without grafting, 318N in implant with ${\beta}$-TCP grafting. There was no significant difference between with and without sinus grafting on maximum bite force(p>0.05). As though the long term check-up is needed, the grafting of ${\beta}$-TCP as a osteoconductive materials can expand the volume and induce dense new bone formation in maxillary sinus. So, this short-term results support that ${\beta}$-TCP can be a suitable material for sinus augmentation.

생체요업재료와 차폐막의 복합사용후 골연하 결손부의 재생효과 (Effect Of Bioceramic Grafts With And Without eptfe Membrane In Periodontal Osseous Defects In Dogs)

  • The purpose of this study was to observe the effect of $Biocoral^R$ graft and bioglass 45S5 graft in combination with ePTFE membrane in periodontal osseous defects for new bone formation. Nine healthy dogs were used. Under general anesthesia, 3-wall defects were created on the mesial and distal surfaces of the maxillary right canines, the mesials of the maxillary right second premolars, the distals of the mandibular right canines and the mesials of the mandibular right third premolars. To induce periodontitis, a silicone rubber, $Provil^R$ light body, was injected under pressure into the defects. Ninety days later, $Provil^R$was removed and followed by thorough root planing. The followings were then applied in the mesial and distal defects of the maxillary right canines, the mesials of the maxillary right second premolars, the distals of the mandibular right canines and the mesials of the mandibular right third premolars by random selections : 1) ePTFE membrane only application, 2) $Biocoral^R$ graft, 3) $Biocoral^R$ graft and ePTFE membrane application, 4)Bioglass 45S5 graft, 5) Bioglass 45S5 graft and ePTFE membrane application. The membranes were removed 1 month later. The dogs were sacrified at 1, 2 and 3 months following the graft, and block sections were made, demineralized, embedded, stained and examined by light microscope and transmission electron microscope. On the sections from teeth treated with ePTFE membrane only, the defect demonstrated extensive connnective tissue and alveolar bone regeneration. The $Biocoral^R$ graft group demonstrated extensive bone regeneration compared with ePTFE membrane only group. In the $Biocoral^R$ graft plus ePTFE membrane group, regeneration of new alveolus and crest occurred within the defect. As the experimental period lengthened, bone regeneration was increased and bone bridge was formed among the graft particles. The but bioglass 45S5 graft group demonstrated extensive bone regeneration but the amount of new bone was less than that of the $Biocoral^R$ graft group. For the bioglass 45S5 graft plus ePTFE membrane group, the amount of new bone was also increased. As the experimental period lengthened, bone regeneration was increased. Multinucleated giant cells, fibroblasts and macrophages were observed. As the bone formation was increased, the number of such cells was decreased. In conclusion, the $Biocoral^R$ was found better than the bioglass 45S5 for new bone formation, and the use of ePTFE membrane alone or with $Biocoral^R$/bioglass 45S5 can be supported as potential methods of promoting bone formation.

골 흡수가 심한 상악 구치부에서 치조정 접근법을 이용하여 상악동 골이식술 동시 임플란트 식립 (Implant placement simultaneously sinus augmentation using crestal approach in severely atrophic maxilla; minimally invasive approach)

  • 상악 구치부는 치조제 골 흡수와 상악동 함기화로 인해 가용골의 양은 제한되고 골질도 좋지 않은 경우가 대부분으로 상악동저의 거상 후 가용골의 증대와 취약한 골에서 임플란트의 견고한 초기 고정을 획득하는 것이 중요하다. 심하게 위축된 치조제의 경우 임플란트의 적절한 초기 고정 획득을 위해 측방접근법을 통한 상악동 골이식술이나 지연 임플란트 식립법이 추천되나 본 연구에서는 수직적 잔존골 높이가 3 mm 이하인 상악 구치부 세 증례를 통해 수직 골 소실이 심한 상악 구치부에서 전통적인 개념과는 달리 최소침습적 수술법으로 치조정 접근을 통한 상악동 골이식과 동시에 임플란트 식립을 시행하여 합병증 없이 충분한 길이의 임플란트 식립이 가능함을 제안한다.

급속상악확대술 시행 후 지대치와 지지골 표면에 나타나는 치근흡수의 불화나트륨 단독투여를 통한 예방에 대하여 (The effect of NaF on bone and tooth resorption around an anchor tooth during a rapid maxillary expansion procedure)

  • 본 연구는 급속상악확대술 시행 후 보정기간중에 지대치의 압박측 치근흡수에 불화나트륨의 경구 투여가 영향을 미치는지를 알아보기 위하여 시행되었다. 이차성징을 아직 나타내지 않은 10마리 소아기 암캐에 견치를 주지대치로 사용하는 주조형 상악 확대장치를 제작, 장착하였다 상악확대장치는 20일간에 걸쳐 5mm 확장시켰으며, 교합면 방사선사진을 이용하여 정중구개봉합의 이개를 확인하였다. 실험동물은 실험군 5 마리와 대조군 5 마리로 나뉘었으며, 보정기간 중 실험군에는 2.2 mg/kg/day의 불화나트륨 (불소로서 1.0mg/kg/day)을 캅셀제의 형태로 1 일 2 회로 나누어 단독 경구 투여하였다. 실험동물들을 보정 후 0 일, 15 일, 30 일, 45 일, 및 60 일째에 각 군당 한 마리씩 배정하여 충분한 마취로 희생시킨 후 상악골을 절취하였다. 이후 실험동물의 좌측 상악 견치의 압박측 치근표면과 압박측 견치 협면을 덮고 있던 골 조직의 치아측 표면을 주사전자현미경을 이용하여 관찰하였다. 본 실험에서 압박측 치근표면을 관찰한 결과 45일 보정군과 60일 보정군의 대조군에서 흡수소와가 발현되는 빈도가 높았다. 지대치 압박측 치근을 덮는 골면에 나타나는 흡수소와의 발현은 30일의 보정기간 이후부터 실험군에서 적게 나타남을 알 수 있었다. 차후 급속상악확대술을 시행하는 경우 효과적인 NaF의 투여시기와 용량에 대한 검증이 필요하리라 생각된다.

Midfacial degloving approach for management of the maxillary fibrous dysplasia: a case report

  • Background: Fibrous dysplasia (FD) is a benign bone lesion characterized by the progressive replacement of normal bone with fibro-osseous connective tissue. The maxilla is the most commonly affected area of facial bone, resulting in facial asymmetry and functional disorders. Surgery is an effective management option and involves removing the diseased bone via an intraoral approach: conservative bone shaving or radical excision and reconstruction. Case presentation: This case report describes a monostotic fibrous dysplasia in which the patient's right midface had a prominent appearance. The asymmetric maxillary area was surgically recontoured via the midfacial degloving approach under general anesthesia. Follow-up photography and radiographic imaging after surgery showed the structures were in a stable state without recurrence of the FD lesion. Furthermore, there were no visible scars or functional disability, and the patient reported no postoperative discomfort. Conclusions: In conclusion, the midfacial degloving approach for treatment of maxillary fibrous dysplasia is a reliable and successful treatment option. Without visible scars and virtually free of postoperative functional disability, this approach offers good exposure of the middle third of the face for treatment of maxillary fibrous dysplasia with excellent cosmetic outcomes.

Bone thickness of the infrazygomatic crest area in skeletal Class III growing patients: A computed tomographic study

  • Purpose: This study was performed to investigate the bone thickness of the infrazygomatic crest area by computed tomography (CT) for placement of a miniplate as skeletal anchorage for maxillary protraction in skeletal Class III children. Materials and Methods: CT images of skeletal Class III children (7 boys, 9 girls, mean age: 11.4 years) were taken parallel to the Frankfurt horizontal plane. The bone thickness of the infrazygomatic crest area was measured at 35 locations on the right and left sides, perpendicular to the bone surface. Results: The bone was thickest (5.0 mm) in the upper zygomatic bone and thinnest (1.1 mm) in the anterior wall of the maxillary sinus. Generally, there was a tendency for the bone to be thicker at the superior and lateral area of the zygomatic process of the maxilla. There was no clinically significant difference in bone thickness between the right and left sides; however, it was thicker in male than in female subjects. Conclusion: In the infrazygomatic crest area, the superior and lateral area of the zygomatic process of the maxilla had the most appropriate thickness for placement of a miniplate in growing skeletal Class III children with a retruded maxilla.

치조골 파열환자의 자가망상골 이식을 이용한 치험례 (CASES REPORT OF CLEFT ALVEOLUS REPAIR WITH PMCB GRAFT)

  • 치조골 파열은 전체 구개파열 환자의 약 75% 정도 발생하며 발생원인은 유전적 요인이 강하며 환경적 요인으로 모체의 영양장애, 방사선조사, 스테로이드투여, 저산소증, 양수변화 바이러스에 의한 질환등을 들 수 있다. 치조골 파열시 상악골의 전후방관계의 전방골결손 및 수평관계의 후방골결손, 인접치아에 대한 골지지력 결여, 측절치의 조기결손, 그리고 안모의 외형에도 많은 변화를 줄 수 있다. 치조골 파열에 대한 골이식은 치조골을 안정시키고, 치조골의 연속성 도모 및 인접치아에 대한 골지지와 치아상실 예방 및 비익저부를 제공하는데 그 목적이 있다. 골이식은 시기에 따라 2세미만에 실시하는 일차 골이식술, 2세에서 5세사이에 시술하는 조기 이차 골 이식술, 5세에서 16세 사이에 실시하는 이차 골 이식술, 그리고 16세 이후에 실시하는 말기 이차골 이식술 등이 있으며 이중 혼합치열기인 5세에서 16세 사이에 실시하는 이차골 이식술이 가장 성공률이 높은 것으로 알려져 있다. 본 교실에서는 치조골 파열환자에 자가망상골을 이용한 골이식술을 시행하여 비교적 양호한 결과를 얻었기에 이에 보고하는 바이다.

Comparison of anterior maxillary and mandibular alveolar parameters in African American and Caucasian women: A retrospective pilot study

  • Purpose: The primary objective of this study was to analyze the thickness and height of alveolar bone around the maxillary and mandibular incisors. Additionally, this study aimed to compare bone parameters between Caucasian (CC) and African American (AA) female patients. Materials and Methods: In this retrospective pilot study, 50 female subjects(25 CC and 25 AA) were included. The inclusion criteria were AA or CC women between the ages of 18 and 50 with a normo-divergent facial pattern and Angle's class I, end-on class II, or mild class III malocclusion. The distance from the cementoenamel junction (CEJ) to the buccal and lingual alveolar crest; the alveolar ridge thickness at the mid-root and apex; and the buccal and lingual bone thickness at 3, 6, and 9mm from the CEJ were measured. Results: No significant difference was found (P>0.05) in the cortical bone thickness at 3mm, 6mm, or 9mm from the alveolar crest between CC and AA populations for most measurements. A significant difference in bone thickness was found (P<0.05) for the lingual surface of the central incisor, with maxillary bone thickness found to be higher than mandibular bone thickness. The measurements of lingual thickness were larger than those of buccal thickness for both races. Conclusion: There were no differences in maxillomandibular anterior alveolar bone measurements between normo-divergent adult AA and CC women, except for a few parameters at varying locations. However, future studies can be planned based the current pilot study data, which may provide valuable information.
