Cytologic Diagnosis of Malignant Fibrous Histiocytoma in Pleural Fluid - A Case Report (흉막강액에서 진단된 악성 섬유성 조직구종 - 1예 보고 -)

  • Malignant fibrous histiocytoma is the most common soft tissue sarcoma of late adult life, but it is rare in the pleura. We recently experienced a case of cytology of malignant fibrous histiocytoma in plural fluid. The smear revealed histiocytic malignant cells, spindle malignant cells and inflammatory cells. The histiocytic cells showed abundant, pale cytoplasm and ovoid, irregular eccentrically-placed nuclei. The spindle cells showed elongated nuclei. Intercellular collagen was also present.

Fine Needle Aspiration Cytology of Malignant Myoepithelioma of the Salivary Gland - A Case Report - (악성 근상피종의 세침흡인 세포학적 소견 -1 예 보고-)

  • Malignant myoepithelioma (myoepithelial carcinoma), is a very rare malignant epithelial accounting for less than 1% of all salivary gland tumors and has an intermediate malignant potential. We report a case of malignant myoepithelioma arising in the left parotid giand in a 54-year-old man, which was difficult to differentiate from pleomorphic adenoma and other malignant salivary gland neoplasms. Fine needle aspiration cytology of the parotid gland showed cellular smear, composed of overlapped sheets and clusters or individually scattered tumor cells without any acinic or ductal structures. The tumor cells were rather uniform, with distinct cell borders and moderate amount of cytoplasm. The eccentrically located nuclei were oval to round and pleomorphic and showed prominent nucleoli. A few clear cells were noted in the cellular aggregates Metachromatic matrix was seen between individual tumor cells in a lacelike fashion, resembling pleomorphic adenoma. According to the immunohistochemical staining, we recognized that the component cells are myoeplthelial in nature, showing reactivity for the S-100 protein, vimentin, and actin.

  • A case of malignant epithelial mesothelioma of the peritoneum diagnosed by fine needle aspiration cytology is described. The smear showed many Individually scattered or clustered large round malignant epithelial cells intermingled with relatively small nonneoplastic mesothelial and mesenchymal cells. Papillary configurations with thick fibrous core were also seen. The malignant cells were virtually reminiscent of reactive mesothelial cells but they were larger in size and had more prominent nucleoli and more frequent binucleated or multinucleated cell formations than reactive mesothelial cells. The characteristic features of malignant cell of mesothelioma compared with the metastatic adenocarcinoma were relatively uniform cellular size, prominent round nucleoli, large round vesicular nuclei with finely granular chromatin pattern, smooth nuclear membrane, abundant glassy cytoplasm rather than bubbly mucin-containing cytoplasm and fuzzy cell border.

Utility of Nuclear Morphometry in Effusion Cytology

  • Background: The cytological analysis of serous effusions is a common investigation and yields important diagnostic information. However, the distinction of reactive mesothelial cells from malignant cells can sometimes be difficult for the cytopathologist. Hence cost-effective ancillary methods are essential to enhance the accuracy of cytological diagnosis. The aim of this study was to examine the utility of nuclear morphometry in differentiating reactive mesothelial cells from malignant cells in effusion smears. Materials and Methods: Sixty effusion smears consisting of 30 effusions cytologically classified as malignant (adenocarcinomas) and 30 benign effusions showing reactive mesothelial cells were included in the study. ImageJ was used to measure the nuclear area, perimeter, maximal feret diameter, minimal feret diameter and the circularity. A total of ten representative cells were studied in each case. Results: Significant differences were found between benign and malignant effusions for the nuclear area, perimeter, maximal feret diameter and minimal feret diameter. No significant difference was found for circularity, a shape descriptor. Receiver operating characteristic (ROC) curve analysis revealed that nuclear area, perimeter, maximal feret diameter, and minimal feret diameter are helpful in discriminating benign and malignant effusions. Conclusions: Computerised nuclear morphometry is a helpful ancillary technique to distinguish benign and malignant effusions. ImageJ is an excellent cost effective tool with potential diagnostic utility in effusion cytology.

The Significance of AgNOR Count in Body Fluid - Differential between reactive mesothelial cells & malignant cells - (체액도말에서의 AgNOR수의 유의성 - 반응성 중피세포와 악성세포의 감별 -)

  • To distinguish reactive mesothelial cells from malignant cells in body fluid, we applied silver staining of nucleolar organizer regions(AgNORs) to ethanol fixed cytologic preparations. Fifty aspirated samples of benign(22 cases) and malignant(26 cases) body fluids were studied using the one step silver staining method. Two cytologically atypical samples were also included in the study. In malignant cases the mean AgNOR count was $3.56{\pm}0.81$, while in benign cases the mean AgNOR count was $2.02{\pm}0.33$. The difference of AgNOR counts between these two groups were statistically significant(p<0.001). The mean of atypical cases was 2.91. Both were diagnosed as malignant in follow-up cytology. In malignant effusions, there is statistically significant difference in AgNOR counts between cells forming complex papillae or clusters and singly scattered cells(p<0.05), $3.29{\pm}0.95\;and\;3.83{\pm}0.55$, respectively. We concluded that AgNOR count appears to be useful as a diagnostic tool especially when the cytologic differentiation is difficult.

Utility of Calretinin in Distinction between Benign Reactive Mesothelial and Carcinoma Cells in Serous Effusions (전액성 삼출액내 반응성 중피세포와 암종세포간의 감별진단에서 calretinin의 유용성)

  • The cytological distinction of carcinoma cells from reactive mesothelial cells in serous effusions nay be difficult or imposslble based on morphology alone, especially In specimens containing reactive mesothelial cells which form glandular or ball- or papillary-shaped conglomerates or which mimic malignant nuclear features. Calretinin is a newly reported immunocytochemical marker for mesothelial cells, which can potentially be utilized for facilitating this distinction. This study evaluated the usefulness of calretinin for the discrimination between reactive mesothelial and metastatic carcinoma cells in serous effusion. Immunocytochemical staining was undertaken on 33 benign reactive and 87 malignant serous effusion specimens with histologically confirmed diagnoses. The specimens including smears and cell blocks were stained with polyclonal antibody to calretinin by labelled streptavidin-biotin method. The positive expression of calretinin was noted In 32(97.0%) of 33 benign reactive effusions and 9(10.3%) of 87 malignant effusions. The sensitivity and specificity of the calretinin immunostaining for reactive mesothelial cells was 97.0% and 89.7%, respectively. In conclusion, calretinin is a useful marker for distinguishing between reactive mesothelial cells and carcinoma cells in serous effusions.

  • Malignant fibrous histiocytoma(MFH) is relatively rare in the oro-maxillofacial region, particularly in the oral cavity. MFH usually arise in the extremities, the thigh being the most common site. The incidence of malignant fibrous histiocytoma in bones is rather low compared with that in soft tissues. MFH is predominant in the 40s and 50s. Histologically, the lesion are said to show high cellularity with fibrous stroma, cellular and nuclear pleomorphism, an admixture of fibroblast-like spindle cells which tend to be arranged in whorls or cartwheel or storiform patterns, rounded mononuclear cells and multinucleated giant cells. The cells frequently have abundant eosinophilic cytoplasm which has a foamy or vesicular appearance. Treatment consists of varying combinations of radiation therapy, chemotherapy, and surgery. We have observed a case of malignant fibrous histiocytoma occured in the right maxilla of 32-year-old woman.

  • Primary malignant lymphoma of the thyroid gland is uncommon malignancies. Its fine needle aspiration cytology (FNAC) findings are rarely described in the literature. This article highlights the FNAC diagnosis of primary malignant lymphoma of the thyroid gland. A 70-year-old female presented with a rapidly enlarging thyroid mass of five months' duration. FNAC smears showed low cellularity consisting of predominantly atypical enlarged lymphoid cells admixed with a few small lymphocytes, plasma cells, and oncocytic cells. Some disrupted lymphoid cells were also present. The tumor cells infiltrated into the thyroid follicular epithelium forming lymphoepithelial lesion, The cytologic appearance showed a diffuse mixture of cell types with only a few small, mature lymphocytes and many enlaraed lymphoid cells. The enlarged lymphoid cells were atypical and pleomorphic with nuclear clefting and irregularities. Grossly, the left lobe of the thyroid was nearly replaced by a diffuse firm to soft solid mass with smooth tan fish-flesh homogeneous cut surface. Histological diagnosis was diffuse large B-cell lymphoma with areas of marginal zone B-cell lymphoma of MALT type.

Metastatic Malignant Mixed Tumor of Mammary Glands in an Irish Setter Dog : A Case Report (개의 악성유선혼합종의 전이 예)

  • An 11 years old Irish Setter bitch was euthanlzed and necropsied because of clinical findings such as severe purulent nasal discharge and formation of large tumor mass, 8 ${\times}$8cm in size, in the abdominal cavity. A complete unilateral mastectomy had been carried out twice 14 and 22 months before necropsy. The surgically removed masess of the mammary glands had been diagnosed as malignant mixed tumor in each time. Grossly, tumor masses were observed in nasal cavity, infralumbar lymph node, lung, abdominal cavityn and brain. Microscopic findings of the surgically removed masses consisted of tumor epithelial cells, tumor hyaline cartilage-like structures and abundant connective tissues. The mass of the lymph node had similar microscopic features to those of the original malignant mixed tumor of the mammary glands. The tumor osseous tissue and osteoid were observed in the abdominal cavity, lung, and brain. Myoepithelial cells were frequently found on association with the metastatic tumors. From the results, it was concluded that malignant mixed tumor of the mammary glands metastasized to the infralunbar lymph node, abdominal cavity, lung and brain. In addition, the observation in this study supported two theories at the same time that the bone in malignant mixed tumor arises by endochondral ossification of the cartilage formed by the myoepithelial cells and arises by intramembranous ossification of stromal connective tissue or transformed myeopithelial cells. Solid carcinoma of the nasal epielia and granulosa cell tumor were also diagnosed in a mass of the nasal cavity and of the ovaries respectively.

Glandular papilloma of the lung with malignant transformation

  • Glandular papilloma of the lung is one of three histologic types of solitary endobronchial papillomas. It is known as an uncommon benign neoplasm. No malignant glandular papillomas have been reported. Herein, the first case of granular papilloma with malignant transformation is reported. A 74-year-old man with huge right lung mass extended upper and lower lobe was admitted to the hospital complaining of progressive cough and dyspnea. An open lung biopsy was performed. Microscopically, the tumor showed papillary growth pattern with thick fibrovascular cores. The stroma of the fibrovascular cores shown the infiltration of lymphoplasmacytic cells and proliferation of capillaries. The epithelial cells surrounding the papillary fronds were cilliated columnar cells with focal cellar atypia, and frequent mitoses. Suspicious pleural invasion foci were identified. The Ki-67 labeling index was about 24.3% and p53 labeling index was about 31.7%. Glandular papilloma is known as a benign neoplasm, which is lack of atypia and mitosis. In present case, there were several indications of malignant transformation, such as cellular atypia, frequent mitosis, architectural distortion, and pleural invasion. Pathologists must be aware that glandular papilloma can have a changes of malignant transformation. Further studies about disease behavior and molecular characteristics are needed.
