Conventional versus Doxorubicin-Eluting Beads Transarterial Chemoembolization for Unresectable Hepatocellular Carcinoma: a Tertiary Medical Centre Experience in Malaysia

  • Background: Hepatocellular carcinoma (HCC) is a common cancer that is frequently diagnosed at an advanced stage. Transarterial chemoembolisation (TACE) is an effective palliative treatment for patients who are not eligible for curative treatment. The two main methods for performing TACE are conventional (c-TACE) or with drug eluting beads (DEB-TACE). We sought to compare survival rates and tumour response between patients undergoing c-TACE and DEB-TACE at our centre. Materials and Methods: A retrospective cohort study of patients undergoing either treatment was carried out from January 2009 to December 2014. Tumour response to the procedures was evaluated according to the modified Response Evaluation Criteria in Solid Tumors (mRECIST). Kaplan-Meier analysis was used to assess and compare the overall survival in the two groups. Results: A total of 79 patients were analysed (34 had c-TACE, 45 had DEB-TACE) with a median follow-up of 11.8 months. A total of 20 patients in the c-TACE group (80%) and 12 patients in the DEB-TACE group (44%) died during the follow up period. The median survival durations in the c-TACE and DEB-TACE groups were $4.9{\pm}3.2$ months and $8.3{\pm}2.0$ months respectively (p=0.008). There was no statistically significant difference noted among the two groups with respect to mRECIST criteria. Conclusions: DEB-TACE demonstrated a significant improvement in overall survival rates for patients with unresectable HCC when compared to c-TACE. It is a safe and promising approach and should potentially be considered as a standard of care in the management of unresectable HCC.

Identification of Cryptosporidium from Dairy Cattle in Pahang, Malaysia

  • Cryptosporidium, a protozoan parasite, can cause cryptosporidiosis which is a gastrointestinal disease that can infect humans and livestock. Cattle are the most common livestock that can be infected with this protozoan. This study was carried out to determine the prevalence of Cryptosporidium infection in cattle in Kuantan, Pahang, Malaysia and to find out the association between the occurrence of infection and 3 different ages of cattle (calves less than 1 year, yearling, and adult cattle). The samples were processed by using formol-ether concentration technique and stained by modified Ziehl Neelsen. The results showed that 15.9% (24/151) of cattle were positive for Cryptosporidium oocysts. The occurrence of Cryptosporidium in calves less than 1 year was the highest with the percentage of 20.0% (11/55) followed by yearling and adult cattle, with the percentage occurrence of 15.6 % (7/45) and 11.8% (6/51), respectively. There was no significant association between the occurrence and age of cattle and presence of diarrhea. Good management practices and proper hygiene management must be taken in order to reduce the infection. It is highly important to control the infection since infected cattle may serve as potential reservoirs of the infection to other animals and humans, especially animal handlers.

Stratifying Patients with Haematuria into High or Low Risk Groups for Bladder Cancer: a Novel Clinical Scoring System

  • Haematuria is a common presentation of bladder cancer and requires a full urologic evaluation. This study aimed to develop a scoring system capable of stratifying patients with haematuria into high or low risk groups for having bladder cancer to help clinicians decide which patients need more urgent assessment. This cross-sectional study included all adult patients referred for haematuria and subsequently undergoing full urological evaluation in the years 2001 to 2011. Risk factors with strong association with bladder cancer in the study population were used to design the scoring system. Accuracy was determined by the area under the receiver operating characteristic (ROC) curve. A total of 325 patients with haematuria were included, out of which 70 (21.5%) were diagnosed to have bladder cancer. Significant risk factors associated with bladder cancer were male gender, a history of cigarette smoking and the presence of gross haematuria. A scoring system using 4 clinical parameters as variables was created. The scores ranged between 6 to 14, and a score of 10 and above indicated high risk for having bladder cancer. It was found to have good accuracy with an area under the ROC curve of 80.4%, while the sensitivity and specificity were 90.0% and 55.7%, respectively. The scoring system designed in this study has the potential to help clinicians stratify patients who present with haematuria into high or low r isk for having bladder cancer. This will enable high-risk patients to undergo urologic assessment earlier.

Knowledge, Attitude and Behavior of Healthcare Providers towards Breast Cancer in Malaysia: a Systematic Review

  • Breast cancer is the most common cancer among women in Malaysia. Therefore, it is highly important for the public to be educated on breast cancer and to know the steps to detect it early on. Healthcare providers are in the prime position to provide such education to the public due to their high knowledge regarding health and their roles in healthcare. The present systematic review involved studies conducted in recent years to analyze the knowledge, attitudes and behavior of Malaysian healthcare providers regarding breast cancer, in attempts to obtain an overall picture of how well equipped our healthcare providers are to provide optimal breast cancer education, and to see their perceptions and actual involvement in said education. The systematic review was conducted via a primary search of various databases and journal websites, and a secondary search of references used by eligible studies. Criteria for eligibility included being published from the year 2008 till present, being conducted in Malaysia, and being written in the English language. A total of two studies were eligible for this review. Findings show that Malaysian future and current healthcare providers have moderate knowledge on breast cancer, have a positive towards involvement of breast cancer education, but have poor actual involvement.

  • The chemical investigation on the cell suspension culture of Morinda elliptica L. yielded eight anthraquinones, two of which, anthragallol-1,2-dimethyl ether (3) and purpurin-1-methyl ether (4), have not been isolated from the original plant. Other compounds isolated include nordamnacanthal (1), alizarin-1-methyl ether (2), rubiadin (5), soranjidiol (6), $lucidin-{\omega}-methyl$ ether (7), and morindone (8). The structures of anthraquinones were established based on spectral studies.

Predicting required licensed spectrum for the future considering big data growth

  • This paper proposes a new spectrum forecasting (SF) model to estimate the spectrum demands for future mobile broadband (MBB) services. The model requires five main input metrics, that is, the current available spectrum, site number growth, mobile data traffic growth, average network utilization, and spectrum efficiency growth. Using the proposed SF model, the future MBB spectrum demand for Malaysia in 2020 is forecasted based on the input market data of four major mobile telecommunication operators represented by A-D, which account for approximately 95% of the local mobile market share. Statistical data to generate the five input metrics were obtained from prominent agencies, such as the Malaysian Communications and Multimedia Commission, OpenSignal, Analysys Mason, GSMA, and Huawei. Our forecasting results indicate that by 2020, Malaysia would require approximately 307 MHz of additional spectrum to fulfill the enormous increase in mobile broadband data demands.

Malaysia's Flawed Democracy: A Stumbling Block Towards Becoming a First World Developed Nation

  • In 1991, Malaysia, under the leadership of then-Prime Minister Mahathir Mohamad, launched an ambitious 30-year national development program known as Vision 2020. The goal of this program was to transform Malaysia into a First World developed nation by the year 2020. One of the aspirations of the program was to create a psychologically liberated, secure, ethical, and mature democratic society. Vision 2020 is a failure and Malaysia is still not a mature democracy. This article identifies four main areas that make up a flawed democracy practiced in Malaysia, and shows how they work against the country's aspirations to become a developed nation. The electoral system is rigged to help the incumbent remain in power. The widespread practices of money politics have become a curse to the country. The press and media organizations are restricted. Civil society activities are suppressed. As a result of these issues, Malaysia will not be able to achieve the status of a developed nation, lacking democratic accountability and inclusive institutions.

말레이시아의 이민정책과 한국인 은퇴이주의 조우(遭遇) (A Study of the Cultural Characteristics and Meanings of the Encounter between Malaysian Migration Policy and Korean Retirement Migration in Malaysia)

  • 본 연구는 현대 말레이시아를 대상으로 말레이시아의 이민정책(migration policy)과 한국인들의 말레이시아로의 은퇴이주(retire migration)의 조우(遭遇, encounter)의 문화적 특징과 의미, 즉 말레이시아 이민정책의 특징과 한국인들의 말레이시아로의 은퇴이주가 어떻게 상호 조우하고, 또한 그 과정이 어떻게 진행되며, 그 의미는 무엇인지에 관해 이를 말레이시아 이민정책의 사회문화적 맥락 내에서 검토하는데 목적이 있다. 본 연구에서는 문헌조사와 현지조사가 병행되었는데, 특히 현지조사에서는 주요 제보자들(key informants)에 대한 인터뷰(interview)와 관찰(observation) 방법을 주로 활용하였다. 구체적으로 먼저 말레이아 이민정책의 특징을 살펴보고, 이를 한국인들의 말레이시아로의 은퇴이주와 연관시켜 접근함으로써, 말레이시아의 이민정책과 한국인들의 말레이시아로의 은퇴이주와는 어떠한 상호관계가 있으며, 그 문화적 특징과 의미는 무엇인지를 고찰하였다. 본 연구의 결과는 다음과 같다. 말레이시아 은퇴이주는 MM2H(Malaysia My 2nd Home) 제도 및 은퇴이주에 대한 잘못된 이해로 인해 자녀교육을 위한 도구로 변용되어 활용되거나 혹은 은퇴이주 자체를 취미활동이나 여가생활의 일부로 간주하는 경향이 나타났다. 이런 이유로 타국으로의 이주임에도 불구하고 은퇴 및 이주에 대한 체계적인 준비 없이 임하거나 혹은 낭만적인 기대로 임함으로써, 결국 대부분 별다른 성과 없이 귀국하는 사례들이 증가하였다. 본 연구는 말레이시아의 이민정책과 한국인들의 말레이시아로의 은퇴이주를 대상으로 그 특징과 의미를 사회문화적 맥락 내에서 고찰함으로써, 한국과 말레이시아 사이의 인적 교류와 이주를 통한 한국과 말레이시아 사이의 교류와 협력의 문화적 특징과 의미를 이해하고, 나아가 한국과 말레이시아 사이의 교류와 협력을 증진시킬 수 있는 학술적, 실천적 방안을 마련하는 데에도 기여할 수 있을 것으로 기대된다.

The First Report of Penicillium georgiense in Malaysia

  • Penicillium georgiense was isolated from sandy beach soil from Batu Ferringhi beach, Penang Island, Malaysia. The identification was based on morphological characteristics and phylogenetic analysis of internal transcribed spacer regions and ${\beta}$-tubulin sequences. To the best of our knowledge, this is the first report of P. georgiense in Malaysia.

  • This paper aims to describe the importance of data, data collection methods, parameters to estimate the potential of wave energy and environmental impacts. The technical and economical status in wave energy conversion is outlined. Power and energy efficiency relationships are discussed. Many different types of wave-energy converters have been detailed. The progress in wave energy conversion in Malaysia is reviewed.
