Diffusion-Weighted Magnetic Resonance Imaging of the Breast: Standardization of Image Acquisition and Interpretation

  • Diffusion-weighted (DW) magnetic resonance imaging (MRI) is a rapid, unenhanced imaging technique that measures the motion of water molecules within tissues and provides information regarding the cell density and tissue microstructure. DW MRI has demonstrated the potential to improve the specificity of breast MRI, facilitate the evaluation of tumor response to neoadjuvant chemotherapy and can be employed in unenhanced MRI screening. However, standardization of the acquisition and interpretation of DW MRI is challenging. Recently, the European Society of Breast Radiology issued a consensus statement, which described the acquisition parameters and interpretation of DW MRI. The current article describes the basic principles, standardized acquisition protocols and interpretation guidelines, and the clinical applications of DW MRI in breast imaging.

  • 1948년 Harvard 대학의 Purcell교수와 Stanford 대학의 Bloch교수가 핵자기 공명(Nuclear Magnetic Resonance : NMR) 현상을 발견한 이래로 NMR은 물질의 분자단위에서 화학적, 물리학적 성질을 밝혀내는 탁월한 방법으로 널리 이용되어 왔다. NMR 현상을 이용한 영상촬영법(Magnetic Resonance Imaging, MRI)은 1970년대초 Lauterber와 Damadian 교수가 처음 영상을 얻을 수 있다는 가능성을 제시한 이후 급속한 발전을 하여 1980년대 초에는 Moore와 Holland에 의해 의학분야에 응용 가능할 정도의 영상이 얻어졌다. 1980년대 중반부터 상용화 되었으며 최근 그 기법도 NMR현상과 연관된 파라미터인 $T_{1}$, $T_{2}$는 물론 혈류의 속도, 자화율, 확산(Diffusion), Perfusion의 영상기법을 비롯해 혈관조영술(MR Angiography), 뇌기능영상(Functional Imaging)등 과거에는 상상도 할 수 없었던 다양한 영상기법 개발되었다. 여기서는 먼저 MRI의 원리를 설명한 후 MRI의 여러 촬영기법들과 그 응용에 관해 설명하겠다.

Spontaneous Intracranial Vertebral Artery Dissection in a 2-Year-Old Child Diagnosed with High-Resolution MRI: a Case Report

  • Although many imaging modalities can play some roles in the diagnosis of vertebral artery dissection (VAD), digital subtraction angiography (DSA) remains the gold standard method, with the highest detection rate and ability to assist in planning for endovascular treatment. However, this tool is often avoided in children because its invasive nature and it exposes them to radiation. High resolution magnetic resonance imaging (HR-MRI) have been suggested to be a reliable and non-invasive alternative, but it has never been discussed in children in whom vertebral artery dissection is a rare condition. In this report, we evaluate a case of a 2-year-old child who initially presented with cerebellar symptoms, and was early diagnosed with vertebral artery dissection using HR- MRI and was successfully treated.

  • Stainless steel and titanium used for orthopedic prosthetic metal, magnetic resonance imaging was measured quantitatively the extent of distortion. Based on reading tests at varying metal magnetic resonance imaging (MRI) were investigated. To two image acquisition conditions for having three metal bodies mutually quantitative it compared analyzed the image of cross direction and the image of lengthwise. As for the distortion of the magnetic resonance image, as for the distortion of image it became small in the order of Clip, Strainless and Titanium. In addition, with T1WI and T2WI which are image acquisition condition T2WI the distortion of image became small. As for the distortion of the image by the metal, you could see the distortion of image in elliptical shape, the metal a bigger image distortion appeared in many parts. If clinical doctor has the condition of the same operation, use the Titanium material, it can, raising the diagnostic value of magnetic resonance image inspection after the operating from Stainless.

  • This paper is to suggest a concept design of the permanent magnet type magnetic resonance imaging (MRI) device based on the parameter optimization method. Pulse currents in the gradient coils will introduce the effect of eddy currents in the ferromagnetic material, which will worsen the quality of imaging. In order to equalize the magnetic flux in the MRI device for good imaging, the eddy current effect in the ferromagnetic material must be taken into account. This study attempts to use the design of experiment (DOE) and the response surface method (RSM) for equalizing the magnetic flux of the permanent magnet type MRI device using that the magnetic flux can be calculated directly using a commercial finite element analysis package. As a result, optimal shapes of the pole and the yoke of the PM type MRI device can be obtained. The commercial package, ANSYS, is used for analyzing the magnetic field problem and obtaining the resultant magnetic flux.

  • The tarsus in dogs has a complex structure that makes its evaluation relatively challenging. Because an accurate diagnosis of the tarsus is difficult through basic examinations alone, imaging tests are essential. Previous studies have explored the anatomical and radiological features of the canine tarsus using several imaging modalities. However, the imaging utility of the tarsus across different modalities has not been thoroughly evaluated. This study aimed to visualize the tarsal structures using magnetic resonance imaging (MRI) and ultrasonography, compare their utility, and propose suitable imaging modalities and conditions for evaluating specific tarsal structures. Magnetic resonance imaging and ultrasound scans of the tarsus of four healthy dogs were performed, and two observers rated the utility of each image on a five-point scale. Although MRI is more beneficial for assessing the tarsal structures than ultrasound, ultrasound also appears clinically useful for evaluating the cranial tibialis muscle, deep digital flexor tendon, subcutaneous fat, joint space, and superficial digital flexor tendon. In addition, each structure of interest can be evaluated for optimal visibility using specific ultrasound sections, MRI sequences, and planes. In veterinary clinical practice, an initial assessment using ultrasound imaging with optimal visibility is required and if further evaluation is necessary, MRI examinations with optimal MRI sequences and planes can be performed.

MRI Findings of an Ampulla of Vater Neuroendocrine Tumor with Liver and Lymph Node Metastasis: a Case Report

  • An ampulla of Vater neuroendocrine tumor (AOV-NET) is a rare subset of gastroenteropancreatic neuroendocrine tumors (GEP-NETs). Very few studies have been undertaken regarding MRI findings of an AOV-NET. We report on a case of a 59-year-old woman diagnosed with an AOV-NET with liver and lymph node metastasis, with an emphasis on the MRI findings. This case shows rare and precious typical MRI findings of an AOV-NET. The MRI visualized the AOV-NET very well and is helpful for the differentiation of an AOV-NET from other tumors in the ampullary area as well as with treatment planning.

  • The equipment and techniques associated with magnetic resonance imaging (MRI) have rapidly evolved. The development of 3.0 Tesla MRI has enabled high-resolution imaging of the intracranial vessel wall. High-resolution MRI (HRMRI) can yield excellent visualization of both the arterial wall and lumen, thus facilitating the detection of the primary and secondary features of intracranial arterial dissection. In the present report, we describe the manner in which HRMRI affected our endovascular treatment planning strategy in 2 cases with unruptured intracranial vertebral artery dissection aneurysm. HRMRI provides further information about the vessel wall and the lumen of the unruptured intracranial vertebral artery dissecting aneurysm, which was treated by an endovascular approach in the 2 current cases.

Are Magnetic Resonance Imaging Findings of Ankle Instability Always Correlated with Operative and Physical Examination Findings? (외측 인대 손상의 자기공명영상 소견들이 수술 소견 및 신체검사와 항상 일치할까?)

  • Physical examination and surgical findings and symptoms are often inconsistent with magnetic resonance imaging (MRI) findings when diagnosing chronic ankle lateral ligament instability, and confirmed surgical findings are used as the gold standard in most clinical studies. Anterior drawer testing is considered unreliable because normal findings are highly variable, and its accuracy ranges from 50% to 100%. Furthermore, radiographic stress imaging, such as in anterior drawer stress view, is performed under manual stress or using a stress device, and its findings also vary widely and confuse when interpreting stress views. The average accuracy of MRI findings is around 85% (range, 66%~91.7%), and thus, cannot be used as a primary indicator for surgery. For patients with suspected lateral ankle ligament instability, based on symptoms and physical examination findings, MRI may be useful for identifying lesions in ankle joints and for differentiating them from other conditions.

Magnetic Resonance Enhancement Pattern as a Predictor of Cement Volume in Vertebroplasty Procedures for Osteoporotic Fractures

  • Purpose: To identify the differences between injected cement volumes during vertebroplasty procedures according to the enhancement pattern of pre-procedure magnetic resonance imaging (MRI) findings. Materials and Methods: Thirty-two patients who underwent 52 vertebroplasty procedures as well as pre-procedure contrast-enhanced spine MRI in the authors' institution were reviewed retrospectively. The 52 procedures were divided into two groups according to different enhancement patterns shown by pre-procedure MR imaging [E(+) and E(-)]. The volumes of the enhancing/non-enhancing portions of the fractured vertebral body shown by pre-procedural MR imaging were calculated and compared to the volumes of the injected cement during the vertebroplasty procedures. Results: The 52 injections included 28 (56%) in Group E(+) and 24 (44%) in Group E(-). The actual volume ratio of the injected cement to the volume of the non-enhanced or enhanced region calculated based on the contrast-enhanced MRI was $0.22{\pm}0.11(cc/cm^3)$ in the E(+) group and $0.93{\pm}0.62(cc/cm^3)$ in the E(-) group. The average amount of injected cement was significantly different between Group E(+) and Group E(-) (P < 0.001). In addition, the ratio of the injected cement amount to the volume of the enhanced or non-enhanced portion based on the contrast-enhanced MRI in Group E(-) was significantly higher than that of Group E(+) (P < 0.001). Conclusion: Different enhancement patterns shown by pre-procedure MRI can predictors of the injected cement volume during vertebroplasty procedures for osteoporotic fractures.
