International Myeloma Working Group의 최신 가이드 라인에 따른 다발성 골수종의 영상검사법 및 MY-RADS에 따른 전신 MRI에서의 영상 획득과 반응 평가 소개 (Imaging for Multiple Myeloma according to the Recent International Myeloma Working Group Guidelines: Analysis of Image Acquisition Techniques and Response Evaluation in Whole-Body MRI according to MY-RADS)

  • 다발성 골수종은 골수에서 클론 형질 세포의 증식으로 인한 악성 혈액 질환으로 최근 우리나라에서도 발병률이 증가하고 있다. 다발성 골수종의 치료법이 발전하면서 질환의 조기 진단과 조기 치료의 필요성이 대두되었고 International Myeloma Working Group (이하 IMWG) 중심으로 질병의 진단의학적, 영상의학적 진단 기준이 여러 차례 개정되고 있다. 또한 다발성 골수종 환자의 진단과 치료 반응 평가에 전신 자기공명영상(whole body MR; 이하 WBMR)의 사용량이 증가하면서 WBMR의 영상 획득 기법이나 영상 결과 해석, 반응평가법을 표준화하기 위해 Myeloma Response Assessment and Diagnosis System (이하 MYRADS)가 만들어졌다. 이 종설에서는 다발성 골수종의 정확한 진단을 위해 영상의학과 의사로서 알아야 할 최신 지견으로 IMWG의 최신 가이드라인에 따른 다발성 골수종의 영상검사법 및 MY-RADS에 따른 WBMR에서의 영상 획득 기법 및 반응 평가 기법에 대해 설명하였다.

Spinal Nerve Root Swelling Mimicking Intervertebral Disc Herniation in Magnetic Resonance Imaging -A Case Report-

  • A herniated intervertebral disc is the most common type of soft tissue mass lesion within the lumbar spinal canal. Magnetic resonance imaging (MRI) is a useful tool for the assessment of patients with lower back pain and radiating pain, especially intervertebral disc herniation. MRI findings of intervertebral disc herniation are typical. However, from time to time, despite an apparently classic history and typical MRI findings suggestive of disc herniation, surgical exploration fails to reveal any lesion of an intervertebral disc. Our patient underwent lumbar disc surgery with the preoperative diagnosis of lumbar disc herniation; however, nothing could be found during the surgical procedure, except a swollen nerve root.

Primary Eosinophilic Granuloma of Adult Cervical Spine Presenting as a Radiculomyelopathy

  • We report a case of 29-year-old man diagnosed as a primary eosinophilic granuloma (EG) lesion of the seventh cervical vertebra. He had paresthesia on both arms, and grasping weakness for 10 days. Cervical magnetic resonance image (MRI) showed an enhancing mass with ventral epidural bulging and cord compression on the seventh cervical vertebra. Additionally, we performed spine series MRI, bone scan and positive emission tomography for confirmation of other bone lesions. These studies showed no other pathological lesions. He underwent anterior cervical corpectomy of the seventh cervical vertebra and plate fixation with iliac bone graft. After surgical management, neurological symptoms were much improved. Histopathologic evaluation confirmed the diagnosis of EG. There was no evidence of tumor recurrence at 12 months postoperative cervical MRI follow-up. We reported symptomatic primary EG of cervical spine successfully treated with surgical resection.

A novel detection method of periodically moving region in radial MRI

  • The appropriate handling of motion artifacts is essential for clinical diagnosis in magnetic resonance imaging (MRI). In many cases, motion is an inherent part of MR images because it is difficult to control during MR imaging. As the motion in the human body occur in a deformable manner, they are difficult to deal with. This paper proposes a novel detection method for periodically moving regions to produce MR images with less motion artifacts. When the data is acquired by the radial trajectory, the proposed method can extract the deformable region easily using the difference in the modulated sinograms, which have different periodic phase terms. The simulation results applied to the various cases confirmed the good performance of the proposed method.

  • The clinical acceptance of magnetic resonance imaging(MRI) system has been more rapid than that of the other medical image diagnosis system(X-ray, CT, etc) with the advantage of nonhazardous nature, high resolution capability, potential for chemically specified imaging. MRI system is composed of super conducting magnet, gradient fields, rf transceiver, system controller and imaging software technology. In this paper, introducing the principle of magnetic resonance imaging, it proposes the implementation of PC-based MRI system.

신경계 질환에서 철 관련 자기공명영상 기법의 활용 (Application of Iron Related Magnetic Resonance Imaging in the Neurological Disorders)

  • Iron is an important element for brain oxygen transport, myelination, DNA synthesis and neurotransmission. However, excessive iron can generate reactive oxygen species and contribute neurotoxicity. Although brain iron deposition is the natural process with normal aging, excessive iron accumulation is also observed in various neurological disorders such as neurodegeneration with brain iron accumulation, Parkinson's disease, Alzheimer's disease, multiple sclerosis, Friedreich ataxia, and others. Magnetic resonance image (MRI) is a useful method for detecting iron deposits in the brain. It can be a powerful tool for diagnosis and monitoring, while furthering our understanding of the role of iron in the pathophysiology of a disease. In this review, we will introduce the mechanism of iron toxicity and the basics of several iron-related MRI techniques. Also, we will summarize the previous results concerning the clinical application of such MR imagings in various neurological disorders.

Long-term Chemotherapy with Hydroxyurea in a Dog with Suspected Intracranial Meningioma

  • A 9-year-old female mixed breed dog was presented due to cluster seizure episode. A mass in the frontal lobe was noted on brain magnetic resonance imaging (MRI). The dural tail sign was identified on contrast MR images. Based on MRI findings, intracranial meningioma was suspected strongly. The patient's symptom was controlled well by a combination therapy of hydroxyurea and prednisolone, and survived for fourteen months after diagnosis. This case report demonstrated that the clinical findings, imaging characteristics of a dog with suspected intracranial meningioma and long-term survival after hydroxyurea plus prednisolone therapy.

  • Placenta accreta spectrum (PAS) is an abnormal placental adherence or invasion of the myometrium or extrauterine structures. As PAS is primarily staged and managed surgically, imaging can only guide and facilitate diagnosis. But, imaging can aid in preparations for surgical complexity in some cases of PAS. Ultrasound remains the imaging modality of choice; however, magnetic resonance imaging (MRI) is required for evaluation of areas difficult to visualize on ultrasound, and the assessment of the extent of placenta accreta. Numerous MRI features of PAS have been described, including dark intraplacental bands, placental bulge, and placental heterogeneity. Failure to diagnose PAS carries a risk of massive hemorrhage and surgical complications. This article describes a comprehensive, step-by-step approach to diagnostic imaging and its potential pitfalls.

대동맥판 역류질환 진단시 이면성심초음파 박출계수의 보정을 통한 오진율 개선에 관한 연구 (A study on improvement of misdiagnosis rate in aortic regurgitation disease by physically correcting EF in 2D echo cardiography)

  • 본 연구는 좌심실의 박출계수가 주된 예후인자인 대동맥판 역류환자를 대상으로 cine MRI와 이면성심초음파의 물리적인 시간해상도 차이를 분석하여 이면성심초음파의 박출계수에 보정함으로써 오진율을 감소시키고자 하였다. 연구대상은 2010년 2월부터 2011년 12월까지 이면성심초음파와 cine MRI를 병행한 환자 110명 중 대동맥판 역류환자 37명을 대상으로 하였다. 각 검사 장비별로 이완기말 면적, 수축기말 면적, 일회 심박출 면적을 측정하였으며, 측정된 값을 이용하여 박출계수를 구하고, 이면성심초음파와 cine MRI의 정상 수치 범위를 비교하여 보정 전 오진율을 평가하였다. 이면성심초음파와 cine MRI의 물리적인 시간해상도 차이는 두 측정치간의 상관관계를 알아본 후 선형회귀분석을 이용하여 회귀계수를 도출하고 도출된 회귀계수를 이용하여 이면성심초음파의 박출 계수에 보정하였다. 보정된 이면성심초음파 박출계수의 신뢰도를 평가하기 위해 Bland-Altman plot를 통해 측정치간 오차를 분석하였고, 이 수치를 cine MRI의 정상 수치 범위와 비교하여 보정 후 오진율을 재평가하였다. 연구결과, 이면성심초음파 박출계수의 물리적 시간해상도 보정 전 오진율은 32.4%(12명)로 높게 나왔고, 보정 후 오진율은 18.9%(7명)로 13.5% 감소하였다. 또한 Bland-Altman plot에서 확인한 보정된 이면성심초음파 박출계수는 cine MRI의 박출계수와의 일치도가 상당히 높다는 것을 알 수 있다. 결론적으로 대동맥판 역류질환 진단시 단순한 이면성심초음파의 박출계수로만 감별하는 것은 오진율을 높일 수 있으므로 오진율이 낮은 cine MRI장비의 물리적 특성을 고려한 시간해상도의 차이를 보정함이 중요하며, 본 연구를 통하여 오진율 감소를 확인할 수 있었으므로 임상 적용의 유용성이 높다고 사료된다.

Diagnosis of Residual Tumors after Unplanned Excision of Soft-Tissue Sarcomas: Conventional MRI Features and Added Value of Diffusion-Weighted Imaging

  • Purpose: To assess conventional MRI features associated with residual soft-tissue sarcomas following unplanned excision (UPE), and to compare the diagnostic performance of conventional MRI only with that of MRI including diffusion-weighted imaging (DWI) for residual tumors after UPE. Materials and Methods: We included 103 consecutive patients who had received UPE of a soft-tissue sarcoma with wide excision of the tumor bed between December 2013 and December 2019 and who also underwent conventional MRI and DWI in this retrospective study. The presence of focal enhancement, soft-tissue edema, fascial enhancement, fluid collections, and hematoma on MRI including DWI was reviewed by two musculoskeletal radiologists. We used classification and regression tree (CART) analysis to identify the most significant MRI features. We compared the diagnostic performances of conventional MRI and added DWI using the McNemar test. Results: Residual tumors were present in 69 (66.9%) of 103 patients, whereas no tumors were found in 34 (33.1%) patients. CART showed focal enhancement to be the most significant predictor of residual tumors and correctly predicted residual tumors in 81.6% (84/103) and 78.6% (81/103) of patients for Reader 1 and Reader 2, respectively. Compared with conventional MRI only, the addition of DWI for Reader 1 improved specificity (32.8% vs. 56%, 33.3% vs. 63.0%, P < 0.05), decreased sensitivity (96.8% vs. 84.1%, 98.7% vs. 76.7%, P < 0.05), without a difference in diagnostic accuracy (76.7% vs. 74.8%, 72.9% vs. 71.4%) in total and in subgroups. For Reader 2, diagnostic performance was not significantly different between the sets of MRI (P > 0.05). Conclusion: After UPE of a soft-tissue sarcoma, the presence or absence of a focal enhancement was the most significant MRI finding predicting residual tumors. MRI provided good diagnostic accuracy for detecting residual tumors, and the addition of DWI to conventional MRI may increase specificity.
