Evaluation of the Usefulness of Ancillary Devices for Patients with Acute Lumbar Pain During Magnetic Resonance Imaging (자기공명영상검사 시 급성 요추 통증 환자를 위한 보조기구의 유용성 평가)

  • The purpose of this study was to evaluate the convenience and image quality of patients with acute lumbar pain patients at a general hospital in Daejeon using ancillary devices for postural changes and correction. The results of the study are summarized as follows. First, the Turbo S pin Echo technique(TS E) using ancillary equipment has the highest image evaluation rating with an average score of 4.440, which is highly valuable on a diagnosis. Second, the average score for patient the questionnaire 'When using ancillary equipment, I feel that my body is calibrated to side without bias.' was shown as 4.440, which is very useful for the correction of the patient's body when using ancillary equipment. Finally, Breath Hold technique(BH) is very effective in shortening test time of acute lumbar pain patients, because it can reduce test time 86.4% faster than Turbo Spin Echo technique(TSE). The results of the study showed that the use of ancillary equipment to perform the test through the side lying postures helped to reduce the pain and control the patient's breathing, and the diagnostic value of the image was high.

  • Ancient schwannoma is a rare variant of schwannoma and a slow growing benign tumor associated with degeneration that may be diagnosed as a malignant tumor, because it presents with a large size and an inhomogeneous signal intensity. The main differential diagnosis of plexiform soft tissue tumor includes plexiform neurofibroma, malignant peripheral nerve sheath tumor (MPNST). In this case, we describe the MRI findings in a case of ancient schwannoma involving left thigh of a 63-year-old woman mimicking a plexiform MPNST. The tumor appeared as an inhomogeneous signal intensity and multinodular appearance, causing misdiagnosis as a plexiform MPNST.

Snapping Triceps Syndrome with Dislocation of the Ulnar Nerve - Usefulness of Dynamic Ultrasonography - (척골 신경 탈구와 동반된 발음성 삼두근 증후군 - 동적 초음파 검사의 유용성 -)

  • Snapping triceps syndrome is a rare condition in which a distal portion of triceps and ulnar nerve dislocate over the medial epicondyle as the elbow is flexed or extended from flexed position. Because it is frequently misdiagnosed as other elbow pathologies, accurate diagnosis is essential and imaging study is often needed to confirm the abnormal movement of ulnar nerve and triceps. Ultraonography is a convenient and effective method which is able to allow continual visualization of soft tissue movement compared to the other imaging modality including MRI. We reported one patient of snapping triceps syndrome who was diagnosed with the use of dynamic ultrasonography and treated with ulna nerve anterior transposition and repositioning of medial head of triceps. And we also provide the usefulness of musculoskeletal ultrasonography for the diagnosis of snapping triceps syndrome.

  • The occurrence of PVNS in the TMJ is very rare. We report a case of PVNS which was misdiagnosed as a parotid tumor at first. CT and conventional radiograph revealed a well defined mass demonstrating higher attenuation than adjacent soft tissue. Erosion and expansion of the cortical plate of the mandibular condyle and sclerotic change beneath the margin of the lesion were also shown. MRI demonstrated well defined mass of very low signal intensity on both Tl and T2-weighted images due to ferromagnetic effect of the high concentrated hemosiderin and clearly delineated the extent of the lesion. Histopathologic findings and characteristic appearance of the various imaging methods were described and the usefulness of these images for diagnosis and pretreatment evaluation of PVNS were also discussed.

Cystoperitoneal Shunting after Fenestration of an Enlarging Arachnoid Cyst

  • A two-month-old girl with a history of an incidental arachnoid cyst in the prenatal period (38 weeks) presented with persistent irritability. A follow-up computed tomographic (CT) scan revealed an enlarged arachnoid cyst with hydrocephalus. We performed craniotomy and fenestration, but the cyst size did not decrease, and hydrocephalus had worsened on a follow-up CT scan performed 13 months after fenestration. The patient was treated with cystoperitoneal shunting. Follow-up magnetic resonance imaging (MRI) performed 5 years later revealed that the arachnoid cyst had decreased in size and that the hydrocephalus had resolved. Enlarging arachnoid cysts are not common, and optimal surgical treatment is uncertain. Based on the features of this case, we believe cystoperitoneal shunting is an advisable surgical intervention for patients with enlarging arachnoid cysts presenting with hydrocephalus.

  • Although the lateral discoid meniscus has been observed frequently, the medial discoid meniscus is an extremely rare anomaly. The incidence of lateral discoid menisci ranges from 1.4% to 15.5% and the incidence of medial discoid menisci ranges from 0.06% to 0.3%. We present 1 patient of bilateral medial discoid menisci with tear on left side and 1 patient of unilateral medial discoid meniscus with tear, that were diagnosed by magnetic resonance imaging (MRI) and arthroscopy. We performed partial meniscectomy on 2 cases of symptomatic medial discoid menisci under arthroscopy and meniscus-related symptoms were relieved. We report 3 cases of 2 patients with medial discoid meniscus.

  • Purpose: To verify the sinificance of the hyperechoic cluster formation in medial meniscal tear on the sonography. Materials and Methods: The formation of hyperechoic cluster of the medial meniscus on the sonography were compared with the MRI and arthroscopic findings in 28 knees. Results: The cluster formation area coincided with the overlapped torn fragment of the meniscus in all cases. Conclusion: The hyperechoic cluster on the sonography reveals the position of the overlapped torn fragment.

  • Skip lesion is not uncommon feature in osteosarcoma and considered to be importantly associated with poor prognosis factor, and thus, should be excised with the main mass. The accurate pre-operative evaluation of the intramedullary extent of osteosarcoma is essential, because it determines the level of bone resection. Among the reliable detection methods, bone scan has a drawback of high rate of false negative results and regional MRI has a difficulty to cover the whole involved lesions without clinical suspicion. The authors report a case of osteosarcoma of the distal femur with a proximal skip lesion that was not detected by either regional MR imaging or by bone scan, but which was visualized by FDG-PET/CT.

  • Eddy current in MRI systems degrades gradient field linearity and distorts gradient waveform. When the waveform distortion is spatially variant, it is very difficult to perform special imaging techniques such as the echo planar imaging technique or the fast spin echo imaging technique. In this study, we have developed a new technique to estimate the distorted gradient waveforms at any points inside the imaging region using the finite element method. After obtaining the eddy-current-effect transfer function, which represents magnitude and phase characteristics of the gradient field at a particular point, we have used the transfer function to estimate the actual gradient waveforms at the point. To verify the proposed technique, we have compared the estimated gradient waveforms with the measured ones.

  • The aim of this study is to investigate the correlation cerebrovascular disease factors in police officers, to provide basic data for a systematic and effective resource management of police officers in the future. This study was conducted on police officers who visited the national police hospital and who underwent MRI for the diagnosis of cerebrovascular diseases. In study result, the probability of cerebrovascular disease was 1.63 times higher in patients with a hypertension. And, the probability of cerebrovascular disease was 1.95 times higher in patients with a Cardiovascular disease. Beside, the probability of cerebrovascular disease was 0.54 times higher in patients with a drinking. And, the probability of cerebrovascular disease was 0.32 times higher in patients with a high homocysteine level.
