다엽콜리메이터의 선량학적엽간격에 따른 치료계획 정도관리시스템의 효용성 평가 (Assessment for the Utility of Treatment Plan QA System according to Dosimetric Leaf Gap in Multileaf Collimator)

  • 세기조절방사선치료(IMRT)는 복잡하고 정교한 방사선치료 기법을 이용한 환자 치료 시 치료계획의 정확성을 평가하기 위해 첫 치료 시작 전 품질관리를 권고하고 있다. 이 목적 실현을 위해 최근 상용화된 품질관리 소프트웨어를 이용하면 치료 전후의 정확한 방사선량의 전달을 확인할 수 있다. 이에 본 연구에서는 품질관리 시스템 내에 MLC의 선량학적엽간격(DLG)의 변화에 따른 IMRT 치료계획 경우별 계산 결과의 분석을 통해 그 효용성을 평가하고자 한다. MobiusFx에 입력할 MLC 다이나로그 파일(Dynamic MLC Log File)을 획득하기 위하여 HD120 MLC가 장착된 Novalis Tx를 이용하였다. IMRT 치료계획을 수립하기 위해 Eclipse 치료계획시스템을 사용하였으며, IBA사의 I'mRT 팬톰영상에 표적 및 장기 윤곽(다중표적, 전립선, 두경부, C-모양)을 묘사하였다. DLG에 따른 선량분포의 변화를 확인하기 위해서 MLC 다이나로그 파일을 MobiusFX 시스템에 입력하고 각 경우마다 0.5, 0.7, 1.0, 1.3, 1.6 mm로 DLG를 변화시켜 선량계산의 차이를 평가하였다. 선량평가는 허용범위가 3%/3 mm인 3차원 감마지표의 통과율과 DVH 그리고 CC13 전리함에 대한 점선량을 통해 분석하였다. MobiusFx에서 4가지의 경우에 대해 각 DLG 마다의 선량분포를 비교 분석한 결과, 다중표적과 두경부는 DLG가 0.5와 0.7 mm에서 3~5%, 1.0~1.6 mm에서 3% 미만의 선량차이를 보였다. 감마 지수 통과율에서도 0.7 mm 이하의 DLG에서 다중표적 경우의 중앙표적과 상위표적은 30% 미만, PTV와 척수는 81% 미만이었으며, 다른 DLG에서는 95% 이상의 통과율을 보였다. 하위표적과 귀밑샘은 모든 DLG에서 100.0%이었다. 전립선에서 점선량은 모든 DLG에서 3% 미만의 차이를 보였으나 PTV에서 DLG 0.7 mm 이하와 1.0 mm 이상에서는 각각 95% 미만과 98% 이상의 통과율을 보였다. 직장과 방광은 모든 DLG에서 100.0%로 나타났다. C-모양에서 점선량은 1.3 mm를 제외한 모든 DLG에서 3~9%의 차이를 보이며 PTV는 1.0과 1.3 mm의 DLG에서 각각 96.7, 93.0%의 통과율을 보였으나 다른 DLG에서는 86% 미만이었다. 코어에서는 모든 DLG에서 100.0%였다. 본 연구에서는 치료계획 품질관리 시스템에서 DLG 적용 값의 차이에 따라 품질관리의 정확성에 유의미한 영향을 미치는 것으로 확인되었다. 그러므로 IMRT, VMAT 등 MLC의 움직임을 이용한 방사선치료법의 경우 정확한 품질관리를 위해서는 사용하고 있는 치료장비 및 치료계획시스템에 맞는 적합한 DLG 값을 상호 일치시켜 사용해야 할 것으로 판단된다.

New Insights into 4-Amino-2-tri-fluoromethyl-phenyl Ester Inhibition of Cell Growth and Migration in the A549 Lung Adenocarcinoma Cell Line

  • Objective: The present study was designed to investigate the probable mechanisms of synthetic retinoid 4-amino-2-tri-fluoromethyl-phenyl ester (ATPR) inhibition of the proliferation and migration of A549 human lung carcinoma cells. Materials and Methods: After the A549 cells were treated with different concentrations of ATPR or all-trans retinoic acid (ATRA) for 72 h, scratch-wound assays were performed to assess migration. Immunofluorescence was used to determine the distribution of CAV1 and $RXR{\alpha}$, while expression of CAV1, MLCK, MLC, P38, and phosphorylation of MLC and P38 were detected by Western blotting. Results: ATPR could block the migration of A549 cells. The relative migration rate of ML-7 group had significantly decreased compared with control group. In addition, ATPR decreased the expression of a migration related proteins, MLCK, and phosphorylation of MLC and P38. ATPR could also influence the expression of RARs or RXRs. At the same time, CAV1 accumulated at cell membranes, and $RXR{\alpha}$ relocated to the nucleus after ATPR treatment. Conclusions: Caveolae may be implicate in the transport of ATPR to the nucleus. Change in the expression and distribution of $RXR{\alpha}$ may be implicated in ATPR inhibition of A549 cell proliferation. The mechanisms of ATPR reduction in A549 cell migration may be associated with expression of MLCK and phosphorylation of MLC and P38.

MLC 플래시 메모리에서의 셀간 간섭 제거 알고리즘 (Cell to Cell Interference Cancellation Algorithms in Multi level cell Flash memeory)

  • NAND Multi-level cell Flash memory는 한 셀에 여러 bit의 정보를 저장하는 방법으로, 용량 집적도를 더욱 높일수 있는 기술로 각광 받고 있다. 하지만 한 셀당 레벨 수를 올릴 경우, 셀간 간섭 등 여러 물리적 이유들로 인해 오류가 발생하며, 이 주된 오류 방향은 unidirectional 함이 알려져 있다. 기존에는 오류 정정 부호(ECC)등을 이용하여 이를 해결하려 했지만, 우리는 셀간 간섭으로 인한 오류에 포커스를 맞추어, 이 영향을 예측하고 줄여서 오류를 보정하는 새로운 알고리즘들을 제안한다. 이 알고리즘은 기존 오류정정부호 기법들과 별도의 단계로 동시에 적용할 수 있기에 에러 정정능력 향상에 효과적이다. 제안된 알고리즘들을 시뮬레이션을 통하여 성능을 비교하고 효율적인 알고리즘이 무엇인지 알아본다.

Methods for quantitative measurement of tooth wear using the area and volume of virtual model cusps

  • Purpose: Clinicians must examine tooth wear to make a proper diagnosis. However, qualitative methosds of measuring tooth wear have many disadvantages. Therefore, this study aimed to develop and evaluate quantitative parameters using the cusp area and volume of virtual dental models. Methods: The subjects of this study were the same virtual models that were used in our former study. The same age group classification and new tooth wear index (NTWI) scoring system were also reused. A virtual occlusal plane was generated with the highest cusp points and lowered vertically from 0.2 to 0.8 mm to create offset planes. The area and volume of each cusp was then measured and added together. In addition to the former analysis, the differential features of each cusp were analyzed. Results: The scores of the new parameters differentiated the age and NTWI groups better than those analyzed in the former study. The Spearman ${\rho}$ coefficients between the total area and the area of each cusp also showed higher scores at the levels of 0.6 mm (0.6A) and 0.8A. The mesiolingual cusp (MLC) showed a statistically significant difference (P<0.01) from the other cusps in the paired t-test. Additionally, the MLC exhibited the highest percentage of change at 0.6A in some age and NTWI groups. Regarding the age groups, the MLC showed the highest score in groups 1 and 2. For the NTWI groups, the MLC was not significantly different in groups 3 and 4. These results support the proposal that the lingual cusp exhibits rapid wear because it serves as a functional cusp. Conclusions: Although this study has limitations due to its cross-sectional nature, it suggests better quantitative parameters and analytical tools for the characteristics of cusp wear.

다엽 콜리메이터의 임상적용을 위한 엽측간 투과선량 및 엽종단간 투과선량 연구 (A Study on Inter-leaf Transmission and Cross-leaf Transmission for Clinical Applications of the Multileaf Collimator)

  • Multileaf collimator (MLC) is now rapidly replacing the lead ahoy block to shape the radiation treatment field. In addition to its defect of rectangular field shaping and increased penumbra width, it has another possible problem, and that is of radiation transmission between leaves, which needs to be maintained at as low a level as is permissible. The authors measured and analyzed the inter-leaf and cross-leaf transmissions of MLC by Varian Associates Inc, before its clinical application. The inter-leaf and cross-leaf transmissions were calculated by comparing the measured point doses in the polystyrene phantom in the open field and in a total closure of MLC. The beam profile of the inter-leaf and cross-leaf transmissions were depicted by using a water phantom. A photon beam of 6 MV was used in the measurement. The inter-leaf transmission was 1.63∼1.67%, indicating that the shielding effect of MLC is excellent. However, the cross-leaf transmission in the central area was 18.4∼18.7% and this is well over the clinically acceptable limitation of 5%. The beam profile of cross-leaf transmission displayed 80∼90% transmission near the field edge, so that the cross-leaf transmission was 14∼17% in this area. The multileaf collimator has an excellent shi디ding effect and the inter-leaf transmission is negligible so that it can be used in clinic as a good replacement of the conventional lead alloy block. However, care must be taken to avoid the cross-leaf transmission in the radiation field.

Spinal Cord Partial Block Technique Using Dynamic MLC

  • The spinal cord dose is the one of the limiting factor for the radiation treatment of the head & neck (H&N) or the thorax region. Due to the fact that the cord is the elongated shaped structure, it is not an easy task to maintain the cord dose within the clinically acceptable dose range. To overcome this problem, the spinal cord partial block technique (PBT) with the dynamic Multi-Leaf Collimator (dMLC) has been developed. Three dimension (3D) conformal beam directions, which minimize the coverage of the normal organs such as the lung and the parotid gland, were chosen. The PBT field shape for each field was designed to shield the spinal cord with the dMLC. The transmission factors were determined by the forward calculation method. The plan comparisons between the conventional 3D conformal therapy plan and the PTB plan were performed to evaluate the validity of this technique. The conformity index (CI) and the dose volume histogram (DVH) were used as the plan comparison indices. A series of quality assurance (QA) was performed to guarantee the reliable treatment. The QA consisted of the film dosimetry for the verification of the dose distribution and the point measurements. The PBT plan always generated better results than the conventional 3D conformal plan. The PBT was proved to be useful for the H&N and thorax region.

Plan-Class Specific Reference Quality Assurance for Volumetric Modulated Arc Therapy

  • Background: There have been much efforts to develop the proper and realistic machine Quality Assurance (QA) reflecting on real Volumetric Modulated Arc Therapy (VMAT) plan. In this work we propose and test a special VMAT plan of plan-class specific (pcsr) QA, as a machine QA so that it might be a good solution to supplement weak point of present machine QA to make it more realistic for VMAT treatment. Materials and Methods: We divided human body into 5 treatment sites: brain, head and neck, chest, abdomen, and pelvis. One plan for each treatment site was selected from real VMAT cases and contours were mapped into the computational human phantom where the same plan as real VMAT plan was created and called plan-class specific reference (pcsr) QA plan. We delivered this pcsr QA plan on a daily basis over the full research period and tracked how much MLC movement and dosimetric error occurred in regular delivery. Several real patients under treatments were also tracked to test the usefulness of pcsr QA through comparisons between them. We used dynalog file viewer (DFV) and Dynalog file to analyze position and speed of individual MLC leaf. The gamma pass rate from portal dosimetry for different gamma criteria was analyzed to evaluate analyze dosimetric accuracy. Results and Discussion: The maxRMS of MLC position error for all plans were all within the tolerance limit of < 0.35 cm and the positional variation of maxPEs for both pcsr and real plans were observed very stable over the research session. Daily variations of maxRMS of MLC speed error and gamma pass rate for real VMAT plans were observed very comparable to those in their pcsr plans in good acceptable fluctuation. Conclusion: We believe that the newly proposed pcsr QA would be useful and helpful to predict the mid-term quality of real VMAT treatment delivery.
