Gastrointestinal Tract Involvement of Gorham's Disease with Expression of D2-40 in Duodenum

  • We present a case of a 13-year-old boy with Gorham's disease involving the thoracic and lumbar spine, femur, and gastrointestinal (GI) tract, which was complicated by recurrent chylothorax and GI bleeding. The presenting symptoms were intermittent abdominal pain, back pain, and melena. Esophagogastroduodenoscopy and colonoscopy showed no abnormal lesions, but duodenal biopsy showed marked dilation of the lymphatics in the mucosa and submucosa, which revealed positive staining with a D2-40 immunohistochemical marker. In cases of GI bleeding with osteolysis, the expression of a D2-40 marker in the lymphatic endothelium of the GI tract may help to diagnose GI involvement in Gorham's disease. To the best of our knowledge, this is the first case report to pathologically demonstrate intestinal lymphatic malformation as a cause of GI bleeding in Gorham's disease.

hMSH2 and nm23 Expression in Sporadic Colorectal Cancer and its Clinical Significance

  • Objective: To study the expression of the mismatch repair proteins hMSH2 and nm23 in sporadic colorectal cancer, determine any inter-relationship, and further investigate any clinical significance. Methods: Expression of hMSH2 and nm23 proteins was assessed in 87 colorectal cancer tissues by SP immunohistochemistry, with analysis of survival using follow-up data. Results: In the sporadic colorectal cancer tissues, nm23 protein expression appeared independent of the histological type (P>0.05), but correlated with the invasion depth and lymphatic metastasis (P<0.05). In contrast, hMSH2 protein expression was not significantly correlated with these clinicopathologic features (P>0.05), although it positively correlated with that of nm23 protein in the sporadic colorectal cancers (rs=0.635, P<0.05). Combined expression of the two was found to be related with invasion depth, lymphatic metastasis and prognosis of sporadic colorectal cancer (P<0.05). Conclusion: nm23 protein level was related with the degree of malignancy, and could be used as an index to predict the invasion and metastasis potential. The expression of hMSH2 protein is positively correlated that of nm23 protein, and the combined expression of the two has certain guiding significance for the prognosis of sporadic colorectal cancer.

  • The Z-plasty has been known as a most common surgical treatment of congenital circumferential constriction band syndrome of the hand. There were thirteen patients of congenital circumferential constriction band syndrome of the hand. All patients underwent Z-plasty under microscope. we did microsurgical dissection to minimize vascular, neural and lymphatic injury, and then considering secondary correction and scar contracture, tried to preserve as much subcutaneous fat and skin flap as possible without any excision. There were less skin necrosis and lymphedema as a result of vascular compromise. Using microscope offers several advantages. First, preventing vascular, neural and lymphatic injury. Second, getting an accurate suture approximation. finally, preserving as much subcutaneous fat and skin flap as possible without any excision. The outcome of digit growth and contour can be excellent than we expected. As a results, we believe that correction of congenital circumferential constriction band syndrome of the hand under the microscope have better results.

  • 고열, 하복부 동통, 배뇨곤란을 주소로 내원한 35세 여자 환자에서 발생한 림프관종을 보고한다. 골반강에서 발생하여 방광을 침범한 감염된 낭종성 림프관종으로 그 발생부위가 매우 드문 예로서 수술 및 병리 조직학적으로 확진되었기에 문헌고찰과 함께 보고하는 바이다.

  • Lymphangioleiomyomatosis (LAM) is a rare, cystic lung disease that is associated with mutation in the tuberous sclerosis genes, renal angiomyolipomas, lymphatic spread and a remarkable female gender predilection. The pathology of LAM is represented by the proliferation of immature smooth muscle cells in the walls of airways, and venules and lymphatic vessels in the lung. The clinical course of LAM is characterized by progressive dyspnea on exertion, recurrent pneumothorax and collections of chylous fluid. The diagnosis of pulmonary LAM can be made on chest X-ray, a high-resolution CT scan and lung biopsy. We experienced a case of pulmonary lymphangioleiomyomatosis in a 28-years-old female patient who had suffered from progressive dyspnea on exertion, so we report on it along with a brief review of the relevant literature.

제주지역의 돼지 써코바이러스 2형 관련 장염에 대한 병리학적 특징 (Pathological characteristics on porcine enteritis associated with porcine circovirus type 2 in Jeju)

  • From 2006 to 2009, 50 pigs suspected of enteritis associated with porcine circovirus type 2 (PCV-2) (EAPC) were collected. Gross and histopathologic examinations and immunohistochemistry (IHC) were performed on the small intestine of 50 pigs. The pigs with EAPC were concentrated in grower pigs (68%), and diarrhea (84%) was the most common clinical sign. Grossly, the walls of the small intestine were thickened, and mesenteric lymph nodes were enlarged. The histopathologic features in the small intestine with EAPC were characterized by lymphoid depletion, histiocytic infiltration, and formation of basophilic grape-like inclusion bodies and multinucleated giant cells in gut-associated lymphatic tissue (GALT) and lamina propria. According to IHC, PCV-2 antigens were more intense and distributed widely in GALT (98%) than lamina propria (82%) of the small intestine. Co-infection with other enteric pathogens was detected in 26 pigs (52%). Twenty-four pigs (48%) were confirmed to be a PCV-2 single infection. Collectively, infected PCV-2 in the small intestine may induce an immunosuppressive status of individuals and then allow secondary co-infections in the digestive system. This study shows that PCV-2 can induce diarrhea and enteric lesions in pigs without a co-infection of other enteric pathogens.

  • 13세의 miniature Poodle이 복부 피부의 자반을 주증으로 내원하였다. 자반은 전체 유선에 걸쳐 관찰되었으나 특히 복부유선과 서혜부 유선에서 심하게 관찰되었다. 유선을 덮고 있는 피부는 자줏빛으로 변해있었으며 비후되었고 황갈색의 분비물이 전흉부유선을 제외한 모든 유선에서 분비되었다. 조직학적 검사에서는 피부측 림프관들이 Cytokeratin에 양성반응을 보이는 종양세포 색전들로 차있었다. 본 증례는 임상검사와 조직학적 검사를 통해 inflammatory mammary carcinoma로 진단되었다.

안면부 혈관기형 환자의 수술적 처치 (Surgical Treatment of Facial Vascular Malformations)

  • Vascular malformations (VMs) in the head and neck region are present at birth and grow commensurately with the child, they can result in significant cosmetic problems for the patient, and some may lead to even serious life threatening hemorrhage. Although the molecular mechanisms underlying the formation of these VMs remain unclear, lesions are known to result from abnormal development and morphogenesis. Histologically, there are no evidence of cellular proliferation, but rather progressive dilatation of abnormal channels, which VMs are designated to their prominent channel types such as capillary, venous, lymphatic, arterial, and combined malformations. VMs with an arterial component are rheologically fast-flow, whereas capillary, lymphatic, and venous components are slow-flow. In this article, we review the clinical presentations, diagnosis, and management of VMs of facial regions with author's embolization and surgical treatment cases.

(+)-Catechin is a Potent Inhibitor of Intestinal Absorption of Cholesterol in Rats

  • Catechins exhibit a hypocholesterolemic effect in cholesterol-fed animals. The present study was conducted to examine whether (+)-catechin influences the absorption of cholesterol in rats. Male Sprague-Dawley rats were fed ad libitum an AIN-93G diet containing soybean oil for 5 wk. Rats with lymph cannulae were infused at 3.0 mL/h for 8 h via a duodenal catheter with a lipid emulsion containing radiolabeled cholesterol with or without (+)-catechin. Lymph was collected hourly for 8 h. The enteral infusion of (+)-catechin significantly lowered the lymphatic absorption of $^{14}$ C-cholesterol (21.1 $\pm$ 3.6% dose/B h) compared with controls infused with the lipid emulsion devoid of (+)-catechin (38.2 $\pm$ 1.2% dose/8 h). The intestinal absorption of $\alpha$-tocopherol (24.2$\pm$3.0% dose/8 h) also was significantly decreased by (+)-catechin infusion, relative to controls (32.2$\pm$2.2% dose/8 h). However, the lymphatic outputs of oleic acid and phospholipid were not affected by enteral (+)-catechin infusion. The results indicate that (+)-catechin has a profound inhibitory effect on the intestinal absorption of cholesterol and $\alpha$-tocopherol without affecting the absorption of fat.

Cavernous Lymphangioma Arising in the Chest Wall 19 Years after Excision of a Cystic Hygroma

  • Lymphangioma is a congenital malformation of the lymphatic system and is thought to result from the failure of the lymphatic system to connect with the venous system. Lymphangioma of the chest wall is a very rare disease entity, and only a few cases have ever been documented in the literature. To the best of our knowledge, there have been few cases of recurrent cavernous lymphangioma after surgical excision of a cystic hygroma on the same side of the chest wall. Here, we report a case of a cavernous lymphangioma of the chest wall in a patient who had undergone surgical excision of a cystic hygroma 19 years earlier.
