Clinical Study of 45 Cases of Primary Carcinoma of the Lung (폐암의 임상적 고찰)

  • The results of the clinical observations on the bronchogenic carcinoma of forty-five cases treated in the Department of Thoracic and Cardiovascular Surgery, Chungnam National University Hospital, during the period of 6 years from June.1978 to Aug. 1984, are as follows: 1. Of 45 cases, 44 cases were over 40 years of age and the sex ratio was 4.6:1 in male`s favor. 3. Among 50 cases, 45 cases were confirmed histopathologically. Of these, 30 cases [66.7%] were squamous cell carcinoma, 8 cases [17.8%] adenocarcinoma, and 7 cases [15.6%] undifferentiated cell carcinoma. 4. The thoracotomy was performed in 31 cases [68.9%] of 45 cases and the resection of the lesion in 16 cases [51.6%]. The empyema and bronchopleural fistula developed in 2 cases.

Sleeve Resection of Lung Cancer - A report of 5 cases- (폐암 환자에서의 기관지 성형술 -5례 보고-)

  • Sleeve resection is safe, effective, and appropriate treatment for a wide range of endo-bronchial lesions including neoplasms of low grade malignant potential and selected cases of bronchogenic carcinoma. Five cases of bronchoplastic procedures were performed for primary bronchogenic carcinoma patients at Department of Thoracic and Cardiovascular Surgery, Chonbuk National University Hospital from Aug. 1983 to Oct. 1987. Of the 5 patients, four were male and one patient was female and ages ranged from 51 years to 66 years old. Histopathologically, 4 cases were squamous cell carcinoma and one case small cell carcinoma. Operative procedures of the 5 patients were as follows: Right upper sleeve lobectomy, 1 case; Left upper sleeve lobectomy, 1 case; left lower sleeve lobectomy, 1 case; Left lower lobe and lingular segment sleeve resection, 2 cases. The early and late postoperative complications of the above operations were pneumonia, atelectasis, bronchopleural fistula, empyema, brain metastasis, and local recurrence of primary tumor.

  • Sleeve pneumonectomy can be a method of treatment in a selected patient with bronchogenic carcinoma involving carina. A 64 years old male with a history of mitral valve replacement via midsternotomy 13 years ago and resection of papilloma of the vocal cord 2 years ago. The patient was admitted due to blood-tinged sputum. Bronchoscopy and computerized tomogram of the chest revealed 3.5 cm mass at lower margin of the trachea and totally obstructing the left main bronchus. A biopsy revealed squamous cell carcinoma. He underwent left sleeve pneumonectomy through sequential bilateral thoracotomy without cardiopulmonary bypass, and the pathologic stage was T4N0M0 stage IIIB. The patient is being followed through the outpatient clinic in good general condition.

  • The clinical observations on the bronchogenic cancer of one hundred sixty cases treated in the Department of Thoracic Surgery, Seoul National University Hospital are as follows: 1] Most of them,. 90 percent of the cases were over 40 years of age and male was predominated over female with sex ratio of 6 to 1. 2] The average duration of symptoms before the first examination was about 5 months arid the most common complaints were cough, chest pain, breathing difficulty and blood-tinged sputum. 3] In ninety-four cases[59%] thoracotomy was undertaken with resection of the lesion in 52 cases[33% ]. 4] Histopathological diagnoses made in 141 cases were squamous cell carcinoma in 47.5%, undifferentiated cell carcinoma in 33.3%, adenocarcinoma in 9.2,% and alveolar cell carcinoma in a single case.

Treatment of Squamous Cell Carcinoma in Extremity (사지에 발생한 편평세포 상피암의 치료)

  • Purpose: to know the treatment result of squamous cell carcinoma in extremity had poor prognosis with risk factor including burn scar and chronic osteomyelitis. Material and Methods: Between Octorber 1993 and September 2002, 20 patients with squamous cell carcinoma in extremity had no distant metastasis was got operation and followed over 36 months. Amputation was done when it was hard to get enough wide margin or neurovascular structure was involved instead of wide excision. Mean age of patients was 57.2 years old and male to female was 16 to 4. TMN staging and histologic grading were performed. Results: There were 6 metastasis (30%) in 20 cases for mean 48.3 months (36-84 months). 3 metastasis to local lymph node and 3 distant metastasis were happened at lung (in 3 cases) and thorasic vertebra (in 1 case). Survival was 18 cases at last look. 5-year survival rate was 50%. 3 local recurrence was developed at average 11 months (4-18 months). Complication was focal skin defect after wide excision and skin graft in 2 cases. The patients by wide excision got average 1.9 time operation and by amputation got average 1.3 time. Conclusion: Squamous cell carcinoma at extremity in Korea had high metastasis rate due to burn scar and chronic osteomyelitis, and it showed equal treatment result which treated by wide excision compared with amputation.

Surgical Treatment of Lung Cancer with Bronchoplastic Procedures (기관지 성형술을 이용한 폐암의 수술적 치료)

  • Thirteen patients with lung cancer have undergone bronchoplastic procedures from January 1991 to July 1994 in Kyung Hee univ. Hospital.Among them,twelve patients were male and one patient was female from 33 to 76 years of age.[ mean 56.6 11.5 years of age The pathologic diagnoses were squamous cell carcinoma in twelve patients and adenoid cystic carcinoma in one patient.The bronchoplastic procedures consisted of seven right upper sleeve lobectomies, one right middle sleeve lobectomy, one left lower sleeve lobectomy and four sleeve pneumonectomies. In these sleeve pneumonectomies, right sleeve pneumonectomy was done in three patients and the other was left sleeve pneumonectomy. Including the four sleeve pneumonectomies, carinoplasty was performed in seven patients. Postoperatively, we carried out bronchoscopic suction of intrabronchial secretions in necessary for the prevention of postoperative complication. All except one were discharged without any complication and have been in good condition. Postoperative mortality was in one patient who expired 25days after right middle sleeve lobectomy with respiratory failure.

Incidence and Clinicopathologic Features of Primary Lung Cancer: A North-Eastern Anatolia Region Study in Turkey (2006-2012)

  • Background: Lung cancer is the most frequent cancer among men and second highest among women overall, including in Turkey. Cigarette smoking is the most important etiologic factor for the development of cancer in both men and women. Objective: To determine the lung cancer incidence in Northeastern Anatolia Region of Turkey with a focus on clinical properties, cancer subtypes, the relationships of tumors with cigarette smoking and radiological properties of the lesions. Materials and Methods: In a retrospective study design, 566 lung cancer cases diagnosed at the Pathology Department of Ataturk University in Erzurum over the last seven years extending from January 2006 to June 2012 were investigated. The results were compared with statistical analyses. Results: The most common histopathological subtype of primary bronchogenic carcinoma in our study was found to be the squamous cell carcinoma, 46.1% (261 out of 566), and the second was small cell lung carcinoma 15.7% (89 out of 566). Based on our data, an overall male predominance was noted with a male/female ratio of 6.1/1. While 296 (52.2%) of the patients were found to be smokers at the time of diagnosis, 125 (22.0%) were nonsmokers and 145 (25.6%) were ex-smokers. Smoking status was found to have a strong correlation with primary lung cancer (p<0.05), and there were significant differences between males and females (p<0.001). Conclusion: Although relative prominence of subtypes of lung cancers differ between Turkish and other populations, lung cancer overall remains as an important health problem in Turkey. Our findings stress the critical need for effective cancer prevention programs such as anti-smoking campaigns.

Epidemiology of Lung Cancer in Korea: Recent Trends

  • Lung cancer causes the most cancer deaths in Korea. Although the smoking rate has begun to decrease, the prevalence of lung cancer is still increasing. We reviewed the national lung cancer registry data and the data published about lung cancer in Korea. In 2012, the crude incidence rate of lung cancer was 43.9 per 100,000. The age-standardized mortality rate of lung cancer was 19.8 per 100,000. The 5-year relative survival rate for lung cancer was 11.3% from 1993 to 1995 and increased to 21.9% in the period from 2008 to 2012. Lung cancer occurring in never-smokers was estimated to increase in Korea. Adenocarcinoma is steadily increasing in both women and men and has replaced squamous cell carcinoma as the most common type of lung cancer in Korea. In patients with adenocarcinoma, the frequency of EGFR mutations was 43% (range, 20%-56%), while that of the EMK4-ALK gene was less than 5%.

Anticancer and Related Immunomodulatory and Anticachexic Effects of Insamyangyoung-tang Extracts on Non Small Cell Lung Carcinoma, NCI-H520, Xenograft Mice (인삼양영탕(人蔘養榮湯)이 비소세포 폐암세포 이식 마우스에서 항암, 면역활성 및 악액질 억제에 미치는 영향)

  • The object of this study was to observe anticancer and related immunomodulatory and anticachexic effects of Insamyangyoung-tang aqueous extracts (ISYYTe) on non-small cell lung carcinoma (squamous epithelial carcinoma), NCI-H520, xenograft Balb/c nu-nu nude mice. Changes on the tumor volume and weights, lymphatic organ(spleen and popliteal lymph node), serum interferon (IFN)-${\gamma}$ levels, splenocytes and peritoneal macrophage activities (NK cell activity), splenic tumor necrosis factor (TNF)-${\alpha}$, interleukin (IL)-$1{\beta}$ and IL-10 contents, splenic T-lymphocyte subsets (CD3+, CD4+ and CD8+) and TNF-${\alpha}+$ cells were observed with tumor mass and lymphatic organ histopathology to detect anticancer and immunomodulatory effects. In addition, changes on the body weights, epididymal fat weights and serum IL-6 levels were also detected with the thicknesses of deposited cervical brown adipose tissue and their mean diameters to monitor the tumor-related anticachexic effects. The results obtained in this study suggest that over 50 mg/kg of ISYYTe showed favorable anticancer effects on the NCI-H520 cell xenograft with immunomodulatory and anticachexic effects. However, detail mechanism studies should be conducted in future with the screening of the biological active compounds in this herb.

Loss of Heterozygosity at 3p in Korean Non-Small Cell Lung Cancer (한국인 비소세포폐암에서의 3p의 소실)

  • Purpose: Frequent deletion of 3p in numerous cancer including lung cancer suggests the presence of tumor suppressor gene. 3p has been analysed by RFLP and PCR-LOH of microsatellite locus. In this study, we observed the deletion of 3p in Korean NSCLC by PCR-LOH of 4 microsatellite loci and investigated the clinical significance. Method: 62 surgically resected NSCLC DNA and normal lung DNA have been analysed by PCR-LOH at three dinucleotide[D3S1228 (3p14.1-14.3), D3S1067 (3p14.3-21.1), D3S1029 (3p21.1-21.3)] and one tetranucleotide[D3S1537 (3p 22-24.2)] repeat microsatellite loci. Results: Among 59 informative cases, 3p deletion by PCR-LOH at four microsatellite loci was found in 31 patients(52.5%). 3p deletion were found in 55% of squamous cell lung cancer and 47% of adenocarcinoma patients. No significant difference has been found in clinical parameters such as staging, smoking and survival according to the status of 3p deletion. Conclusion: Deletions in 3p have played an important role in Korean NSCLC though no clinical significance was detected.
