A Tuberculoma in the Left Lower Lobe of Lung That Was Erroneously Diagnosed as Ectopic Liver (이소성 간으로 오진된 좌폐 하엽의 결핵종)

  • Tuberculoma of the lungs is not an uncommon finding, but an ectopic liver in the lung is extremely rare. Pulmonary tuberculosis presenting as tuberculoma can be diagnosed radiologically, but its definite diagnosis is established by confirmation of the acid-fast bacillus or the unique histology. We report here on a case of tuberculoma of the left lower lobe that was erroneously diagnosed as ectopic liver by ultrasono-guided fine needle aspiration biopsy. An understanding of the normal variants of the liver can prevent a patient from undergoing an unnecessary invasive procedure.

  • Hamartoma is one of the most common benign lung tumors. Most of them are located in the lung parenchyme, but very rarely it can originate endobronchially. Endobronchial hamartoma may cause irreversible lung damage due to the bronchial obstruction if it is not diagnosed and treated properly. The most frequent clinical features are hemoptysis and obstructive pneumonia. Transbronchial endoscopic surgery is a good therapeutic choice and is recommended first for patients who have resectable small endobronchial hamartoma. Here, we report a case of endobronchial hamartoma treated with rigid bronchoscopy and various microforceps with a review of the literature.

  • This research aims is to study physiolgy and pathology of lung on Suenbal-Sukkang action occuring with the exchang of Metal and Fire of nature. The exchange of Metal and Fire(金火交易) represents differience of temperature inside and outside of the atmosphere in according to Sun-heat on earth descended in passing Hazi(夏至). On the above fact, the Suenbal-Sukkang( action representing the Exchang of Metal and Fire happens in lung of body with properties of metal. too. These phenomena are physiological phenomenas of Pulmonary series(肺系). Accordingly, the action of Suenbal-Sukkang radiates heat and converge water with the properties of Autumn-Metals in change actions of nature. In order To interpret physiology action of Pulmonary energy(肺氣), I have studied Metal-glass theory(金鏡說) of Muk Za(墨子), and made a comparative study of a climatic change passing from summer to autumn in four season caused by the revolution of earth Thus, to interpret the action of Suenbal-Sukkang can be inferred by the phenomena of physiology and pathology of Pulmonary series. From the result of this study, I know that functions representing of the Suenbal-Sukkang take place in processing the exchange of Metal become phenomena in physiology of Pulmonary series(大腸, 皮膚, 鼻, …), and abnormal functions functions representing the Suenbal-Sukkang become pathological phenomenas of Pulmonary series.

Study on the Anti-angiogenic Activity of KMKKTE (가미계격탕의 혈관형성 저해작용에 관한 연구)

  • Cancer is an intractable disease for humans to overcome. Recently natural products or Oriental prescriptions have been on the spotlight to develop anticancer agents with little side-effects and good efficacy. KamikeKyuktang has been used for the treatment of cancer in Oriental medicine. However, its anti-cancer mechanism still remains unclear. KMKKTE is an ethanol extract of KamikeKyuktang composed of 12 medicinal herbs. Anti-proliferative effects of KMKKT was investigated on Lewis lung carcinoma cell (LLC) and A549 (human lung cancer cells). Half-maximal inhibition of the LLC and A549 cell proliferation by KMKKTE was found approximately 125㎍/㎖ and 250㎍/㎖, respectively. It also effectively inhibited the proliferation of HUVEC cells treated by bFGF and VEGF up to 30% of control at 125㎍/㎖ and the cell migration to 80% at 25 ㎍/㎖ in concentration dependent manner. Tube formation of HUVEC cells on matrigel also was significantly suppressed from 25㎍/㎖ of KMKKTE. Taken together, these results demonstrate that KMKKTE has antiangiogenic activity and be applied to angiogenesis dependent cancers.

Metastatic Tumors in Supraclavicular Lymph Node - Pathologic Analysis of 125 Cases - (쇄골상부 림프절의 전이성 종양 - 125예에 대한 병리조직학적 분석 -)

  • Supraclavicular lymph nodes are unique in that they can attract metastases from almost anywhere in the body and most common sites of metastasis from an unknown primary source. 125 cases which had been diagnosed as metastatic supraclavicular lymph node during the period between May 1983 and August 1991, were analysed pathologically, and the following distinctive characteristics could be outlined : 1) The most frequent sites of metastasis from primary lesions are lunge(43%), stomach(23%), lymphoreticular(6%), biliary(5%), esophagus(2%), and pancreas(2%). 2) Histologic examination of metastatic supraclavicular lymph node revealed adenocarcinoma(57%), squamous cell carcinoma(12%), undifferentiated carcinoma(9%), small cell carcinoma(7%), malignant lymphoma(6%), malignant melanoma(1%) and undetermined carcinoma(8%). 3) In cases that histologic type was adenocarcinoma, the most frequent primary sites were stomach(38%), lung(27%) and biliary(8%). 4) In cases that histologic types were squamous cell caricinoma, undifferentiated carcinoma, small cell carcinoma and undetermined carcinoma, the most frequent primary site was lung.

Study on Epidemic Warm Diseases with dampness of "OnByeongJoByeon" ("온병조변(溫病條辨)" 습류온병(濕類溫病)에 대한 고찰)

  • Oriental Medicine always attach great importance to the damp diseases. Dampness is related with many organs and many clinical diseases. The cause and the location of the damp disease, nature of the symptoms, combination with other pathogenic factors are very diverse. This article analyzed the concept, cause of disease, pathogenesis, characteristic of symptoms, treatment method and prescriptions of Epidemic Warm Diseases of dampness syndrome and cases of dampness-heat diseases based on the theories of Epidemic Warm Diseases and found that theories of Epidemic Warm Diseases have very wide area of application. Dampness is classified into cold-dampness and dampness-heat by combination of heat or cold. The dampness syndrome is related with organs such as lung, spleen, kidney, triple energizers and bladder, and affects liver and heart. The basic treatment methods are dispelling dampness turbidity and diffusing qi movement. The detail treatment methods are spreading lung qi with lightness and resolving dampness and excreting turbidity in upper energizer, opening and dipping down with pungent-bitter and diffusing qi movement and strengthening the spleen and stomach in the middle energizer, draining dampness with bland in the lower energizer. Warming Yang is the main method of treatment for cold-dampness and clearing heat is for dampness-heat with the assistant methods such as resolving dampness and promoting the flow of qi. 5. Acute fever, virus diseases, epidemic diseases among modern diseases are much related with the dampness-heat syndrome.

Features and Interpretation of Olfactory and Gustatory Disorders in the Corona Virus Disease-19 (코로나바이러스감염증-19에서 나타나는 후미각손상의 특성과 한의학적 분석)

  • Besides respiratory infection, COVID-19 has many neurological symptoms not only loss of smell and taste but also fatigue and brain fog. But it is a challenge to treat the neurological symptoms especially of anosmia and ageusia. In order to search for the therapeutic methods, the geographical diversity and pathological mechanisms of the COVID-19 and two symptoms were investigated from the latest clinical studies. Because the environmental conditions of the monsoon climate zone of East Asia and the Mediterranean and Oceanic climate zone of Italy, Britain, United States and tropical Brazil are different, each of diverse etiology and internal milieu should be considered differently in the treatment. SARS-CoV-2 exhibits the dampness-like characteristics and the olfactory and gustatory disorders are particularly more common than other flu or cold. and it tends to show features of damaging the lung qi of olfaction and heart-spleen qi of gustation. The mechanisms of olfactory and gustatory loss are various according to precursory, inflammatory, non-inflammatory and sequelar forms, so the therapeutic method should be designed for each period and pathology. If the process of inflammation arises from nasal and respiratory, olfactory epithelium to the central nervous structure by way of blood brain barrier, the treatment should be corresponded with the stage and depth of pathogen place. And if the olfactory loss is asymptomatic or in the initial stage, it can be applied intranasal topical scent therapy to relieve temporary locking of qi movement, but maybe also used in parallel together with herbs of relieving dampness toxin latent in the lung parenchyma.

Role of Oxidative Stress in the Radiation-Induced Lung Pathogenesis in Mice

  • In pre-transplant total-body irradiation (TBI), the lung is a critical dose-limiting organ. Also, the possible role of oxidative stress was suggested in the development of TBI-induced lung damage. This study explores the association between TBI-induced oxidative stress and the induction of lung pathogenesis by investigating TBI-induced oxidative stress in the lungs of male C57BL/6 mice after a single dose of 10 Gy TBI. We showed significant increases of reactive oxygen species (ROS) formation and lipid peroxidation, and also a depletion and oxidation of glutathione after TBI. There is evidence that pretreatment with 1,10-phenanthroline (o-phen) significantly reduces oxidative stress in the lung. This indicates that the TBI-induced ROS generation involves a metal-catalyzed Fenton-type reaction. A pretreatment of buthionine sulfoximine (BSO) augmented the glutathione depletion and oxidation, but had no effect on the ROS formation and lipid peroxidation up to 6 h after TBI. Histopathological features that are consistent with pneumonitis were observed in the BSO pretreated-mice 1 week after irradiation. The results suggest that TBI-induced oxidative stress in the lung involves a generation of ROS through a Fenton-type reaction. Also, glutathione plays an important inhibitory role in the radiation-induced lung pathogenesis by participating in the self-amplifying cascade subsequent to the ROS generation by irradiation.

  • Cancer is the leading cause of death in the Republic of Korea and cancer death accounts for 27.8% of the total deaths, which is not only a social issue but also a concern for the public. Among the cancer death rates, lung cancer mortality account for 34 deaths per 100,000 populations, making it the number one cancer death rate. In a preliminary report on cancer death in 2012, the lung cancer mortality ratio showed the regional variation indicating that there were differences in the qualitative level and the structure among the medical care benefit agency and in the assessment of the treatment process. Therefore, the Health Insurance Review and Assessment Service (HIRA) had begun evaluation of the assessment of lung cancer treatment since 2014 to improve the quality of lung cancer care through evaluation and feeds back the results of lung cancer care process. In this report, authors described the current Indicators for the lung cancer adequacy assessment proposed by HIRA and results of the evaluation reported in 2017.

A Case of Epithelioid Hemangioendothelioma Involving the Lung and Liver (폐와 간을 침범한 유상피 세포성 혈관내피종 1예)

  • Epithelioid Hemangioendothelioma(EH) is a rare vascular tumor, originating from endothelial cells. The principal locations are lung, soft tissue, bone and liver. This tumor is of borderline malignancy, relatively benign course. In the lung, the tumor is often multifocal, bilateral and frequently lead to the mistaken diagnosis of metastatic carcinoma. Although EH of the lung is relatively slow growing tumor, extensive pulmonary involvement, systemic metastasis, mainly to the liver have been documented. A 26-year-old man with EH involving the lung and liver was reported. Chest X-ray and chest CT showed multiple nodules in both lung fields and Abdominal CT multiple round low densities in liver. Transbronchial lung biopsy was performed. The patient diagnosed as EH by light microscopic finding and immunohistochemical study for Factor VIII-related antigen.
