Purpose: Although multi-directional reaching exercises are commonly used clinically, the effects of specific movement directions on the muscle systems of the lower extremities have not been explored. We therefore investigated lower extremity muscle activity during reaching exercises with different sagittal and horizontal plane movements. Methods: The surface electromyography responses of the bilateral rectus femoris, tibialis anterior, peroneus longus, and gastrocnemius muscles were measured during reaching exercises in three directions in the horizontal plane (neutral, $45^{\circ}$ horizontal shoulder adduction, and $45^{\circ}$ abduction) and three directions in the sagittal plane (neutral, $120^{\circ}$ flexion, and $60^{\circ}$ flexion). A total of 20 healthy, physically active participants completed six sets of reaching exercises. Two-way repeated ANOVA was performed: body side (ipsilateral and contralateral) was set as the intra-subject factor and direction of reach as the inter-subject factor. Results: Reaching at $45^{\circ}$ horizontal shoulder adduction significantly increased the activity of the contralateral rectus femoris and gastrocnemius muscles, while $45^{\circ}$ horizontal shoulder abduction activated the ipsilateral rectus femoris and gastrocnemius muscles. The rectus femoris activity was significantly higher with reaching at a $120^{\circ}$ shoulder flexion compared to the other conditions. The gastrocnemius activity decreased significantly as the shoulder elevation angle increased from $60^{\circ}$ to $120^{\circ}$. Conclusion: Our results suggest that multi-directional reaching stimulates the lower extremity muscles depending on the movement direction. The muscles acting on two different joints responded to the changes in reaching direction, whereas the muscles acting on one joint were not activated with changes in reaching direction.
Purpose: The purpose of this study was to compare the spatiotemporal and kinematic gait parameters and muscle activity of the lower extremities between forward walking on sand (FWS) and backward walking on sand (BWS) in normal adults. Methods: This study was conducted on 13 healthy adults. Subjects performed FWS and BWS and the spatiotemporal and kinematic gait parameters of stride time, stride length, velocity, cadence, step length, stance, swing, double support, and hip range of motion (ROM), knee ROM were measured by a wearable inertial measurement unit system. In addition, the muscle activity of the rectus femoris (RF), biceps femoris (BF), tibialis anterior (TA), and gastrocnemius (GA) was measured. Results: The stride length, stride velocity, cadence, and step length in the BWS were significantly lower than FWS (p<0.05), and stride time was significantly greater (p<0.05). However, there was no significant difference in the ratio of stance, swing, and double support between the two (p>0.05). The kinematic gait parameters, including hip and knee joint range of motion in BWS, were significantly lower than FWS (p<0.05). The muscle activity of the RF in BWS was significantly higher than FWS (p<0.05), but the muscle activity of the BF, TA, GA did not show any significant differences between the two movements (p>0.05). Conclusion: A strategy to increase stability by changing the gait parameters is used in BWS, and this study confirmed that BWS was a safe and effective movement to increase RF muscle activity without straining the joints. Therefore, BWS can be recommended for effective activation of the RF.
Purpose: To better understand falls in elderly women, we measured differences in muscle activities of 4 lower extremity muscles (tibialis anterior, soleus, rectus femoris, biceps femoris) based on angle plantar-flexion in elderly women. Methods: Subjects were 15 healthy elderly women. No subjects had musculoskeletal or nervous disorders. EMGs were used to check muscle activity of lower extremity muscles. For statistic analysis, data were expressed as a percentage of maximal voluntary isometric contraction (%MVIC). We measured %MVIC for 5 sec under 4 conditions and did not use data for the first and last second. Muscle activities of the muscles of the lower extremities were measured based on the degree of plantar-flexion of the ankle joint. The 4 conditions corresponded to different degrees of: 0, 10, 20 and 30 degrees. This was done using a 50 cm board for measuring the degrees. Results: The tibialis anterior, soleus, and bicpes femoris showed increasing muscle activity associated with increasing degree of plantar-flexion of the ankle joint. All muscles showed a significant increase in muscle activities in association with increasing degrees. Rectus femoris showed a significant increase in muscle activity for 0 degrees and for the other degrees, but there were no further changes when plantar flexion of the ankle joint was over 10 degrees. Conclusion: Activities of the muscles of the lower extremities except rectus femoris were higher with increasing degree of plantar flexion. High muscle activity may result in muscle fatigue. Thus, increasing degree of plantar flexion may be risk factors of falls in elderly women.
PURPOSE: This study was conducted to compare the muscle activity of the proximal muscles of the lower limb according to the distance between the front and rear foot during lunge and to determine the most effective foot position for activation of the proximal muscle in the limb. METHODS: A total of 49 young adults were enrolled in this study. All subjects performed lunge by positioning the big toe of the back foot and the heel of the front foot at intervals of 40%, 60%, and 80% of the subject's own leg length. Muscle activity of the vastus medialis oblique, rectus femoris (RF), vastus lateralis oblique (VLO), gluteus medius, biceps femoris, and semitendinosus (ST) was then measured during three intervals of lunge operation. Each operation was measured three times for 10 seconds each, after which the average value was calculated and analyzed. RESULTS: There were significant differences in muscle activities of RF, VLO, and ST among the three intervals of the foot (p<.05). Post hoc, comparisons revealed lunge at 40% intervals resulted in higher RF and VLO activity than at 60% and 80% intervals (p<.05). In the semitendinosus muscle, 80% leg length intervals showed higher muscle activity than 40% (p<.05). CONCLUSION: Strengthening of the proximal muscles of the lower extremities during lunge exercise is considered to be most effective when placing the fore- and rear foot at intervals corresponding to 40% of the leg length.

Purpose: To evaluate changes in lower extremity muscle activity caused by high heeled shoe wearing during normal, brisk, and upslope walking. Methods: Twenty healthy young women (age, $23.9{\pm}2.47$) participated in this study. Muscle activities of the tibialis anterior, peroneus longus, gastrocnemius lateralis, gastrocnemius medialis, soleus, hamstring, vastus lateralis, and vastus medialis while walking normally, walking briskly, and walking up a slope. Results: When walking normally, the peroneus longus, gastrocnemius lateralis, soleus, and vastus lateralis evidenced higher activity when high-heeled shoes were worn (p<0.05). During brisk walking, the peroneus longus and gastrocnemius lateralis exhibited higher activity (p<0.05). Although the peroneus longus and vastus lateralis exhibited higher activity when walking up an incline with high-heeled shoes, the activity levels of the tibialis anterior and gastrocnemius medialis were lower (p<0.05). Conclusion: The results of this study demonstrate that increased heel height substantially reduces muscle effort when walking up a slope. From a therapeutic perspective, it is possible that using high heeled shoes over a short period might enhance muscle activity of ankle evertor, although it can cause mediolateral muscle imbalances in the lower extremities.
Background: The present study was conducted with 30 adult males in order to examine the muscle activity of the trunk and the lower extremities at diverse initial bending angles of the hip joint during bridge exercise on a stable surfaces and on an unstable surface that is widely performed for stabilization. Methods: The initial angles of the hip joint used were $0^{\circ}$, $45^{\circ}$ and $90^{\circ}$ and the subjects were divided into a matt experimental group and a balance training group. Results: In maximum values of muscle activity at different exercise methods and angles, the matt experimental group showed statistically significant differences in the muscle activity values of the rectus abdominis muscle, the erector spinae muscle, the rectus femoris muscle and the peroneus muscles between different angles while the balance training group showed significant differences only in the muscle activity values of the erector spinae muscle between different initial angles of the hip joint. The matt experimental group showed significant differences in muscle activity between initial angles $0^{\circ}$ and $90^{\circ}$, between $45^{\circ}$ and $90^{\circ}$ in the rectus abdominis muscle, between $0^{\circ}$ and $90^{\circ}$ in the erector spinae muscle, between $45^{\circ}$ and $90^{\circ}$ in the rectus femoris muscle and between $0^{\circ}$ and $90^{\circ}$ in the peroneus muscles while the balance training group showed significant differences between $0^{\circ}$ and $90^{\circ}$ in the erector spinae muscle. Conclusions: Therefore, it is thought that bridge exercises should be applied to patients using diverse methods.
Objective: The purpose of the study was to investigate the effect of shoe sole deformation on the muscle activity and balance of the lower extremities when standing on one foot. Design: Crossed-control group study Methods: A total of 18 healthy adults participated in this study. 9 participants with normal shoe and 9 participants with deformed in shoe out-soles (wear shoes) were included. Muscle activity of the tibialis anterior, fibularis longus and gluteus medius during one leg standing was measured using a electromyography. A balance board was used to evaluate balance ability when standing on one leg. The balance ability when standing on one leg was measured by the sway speed and distance of the center of gravity. Results: Muscle activity of the tibialis anterior, fibularis longus and gluteus medius was no significant difference between groups (P > 0.05). Balance ability when standing on one leg was significantly different from the group wearing normal shoes in all variables related to the sway distance and sway speed of the center of gravity. Conclusions: Through this study, the wear of the outer sole of the shoe indirectly confirmed the effect on the wearer's lower extremity muscle activity and balance ability when standing on one foot. These results of this study are expected to be used as basic data for future studies on shoe wear, lower extremity muscle activity and balance ability.
Purpose: This study compared the muscle activity of the lower limb according to the three types of fixed angles of the ankle joint during a lunge exercise. Methods: Twenty healthy subjects performed the lunge motion in a trial including the three types of fixed angle. The lunge motion with a neutral, 20° dorsiflexion, and 20° plantarflexion of the ankle joint were randomized and measured repeatedly. The muscle activity of the rectus femoris (RF), vastus medialis (VM), vastus lateralis (VL), biceps femoris (BF), and semitendinosus (ST) was measured by surface electromyography. Results: In the change in ankle joint angle, the RF, VL, BF, and ST muscle activity showed significant differences (p<0.05). In the 20° dorsiflexion position, the muscle activity of VL, BF, and ST showed a significant decrease compared to that in the neutral position (p<0.017). The muscle activity of RF and VL in the neutral position was greater than that in the 20° plantarflexion position (p<0.017). Only the muscle activity of the BF in the 20° plantarflexion position was significantly greater than the 20° dorsiflexion position (p<0.017). Conclusion: These results revealed a difference in the muscle activity of lower extremities in the proximal region according to the angle of the ankle joint during the lunge.
Purpose: This study examined the effects of a kettlebell and aqua bag on the gluteus maximus, gluteus medius, and vastus medialis lower extremity muscle strength during a one-leg deadlift, a top-down exercise. Methods: Twenty healthy adults were enrolled in the one-leg deadlift exercise under two conditions. The muscle activity of the gluteus maximus, gluteus medius, and vastus medialis was measured by surface electromyography (S-EMG). A paired t-test was used for statistical analysis, and the statistical significance criteria were set to p<0.05. Results: A comparison of the muscle activity according to the degree of contraction of the gluteus maximus revealed low muscle activity in AD exercise compared to KD exercise (p<0.05), and there was no significant difference in the gluteus medius and vastus medialis (p<0.05). Conclusion: These results suggest that when the one-leg deadlift exercise is performed according to the weight characteristics, the gluteus maximus is suitable for intervention using a kettlebell, and the gluteus medius and vastus medialis are more suitable for intervention by providing instability using an aqua bag.
