편마비 환자의 계단과 경사로 보행 동안 하지의 운동학적 분석 (Kinematic Analysis of Lower Extremities during Staris and Ramp Walking with Hemiplegic Patients)

  • Purpose: This study was conducted in order to investigate the kinematic gait parameter of lower extremities with different gait conditions (level walking, stair, ramp) in hemiplegic patients. Methods: Ten hemiplegic patients participated in this study and kinematic data were measured using a 3D motion analysis system (LUKOtronic AS202, Lutz-kovacs-Electronics, Innsbruk, Austria). Statistical analysis was performed using one-way repeated measure of ANOVA in order to determine the difference of lower extremity angle at each gait phase with different gait conditions. Results: Affected degree of ankle joint in the heel strike phase showed significant difference between level walking and climbing stairs, and toe off phase showed significant difference between level walking and climbing stairs, ramps, and climbing stairs. Affected degree of knee joint showed no significant difference in all attempts. Affected degree of hip joint in the toe off phase showed significant difference between level walking, ramps and stairs, and climbing ramps. Swing phase showed significant difference between sides for level walking and stairs, climbing ramps. Affected ankle joint of heel strike and toe off, and affected hip joint of toe off and the maximum angle of swing phase in the angle was increased. Unaffected side of the ankle joint, knee joint, and hip joint showed a significant increase in walking phase. Conclusion: These findings indicate that compared with level walking, different results were obtained for joint angle of lower extremity when climbing stairs and ramps. In hemiplegia patient's climbing ramps, stairs, more movement was observed not only for the non-affected side but also the ankle joint of the affected side and hip joint. According to these findings of hemiplegic patients when climbing stairs or ramps, more joint motion was observed not only on the unaffected side but also on the affected side compared with flat walking.

스쿼트 운동 방법에 따른 하지 근활성도 연구 (The Study for Muscle Activating of Lower Extremities According Various Squatting)

  • Background : The purpose of this study was find out more effective method for improving muscle strengthening in lower extremities according squatting exercise method among knee joint angle 45, 60, 90 degree and between gym ball behind back and wall. Methods : Participants were 21 university students(males 10, females 11) who didn't have any problem with orthopedic surgery. We divided participants to two groups with gym ball group and wall group. Gym ball group were performance squatting exercise with gym ball behind back. Each group had performance squatting exercise according knee joint angle 45, 60, 90 degree. We collected data from E.M.G of Biceps femoris, Gastrocnemius, Vastus medialis and lateralis, Tibialis anterior in lower extremity according knee joint angle 45, 60, 90 degree of each groups. We analyzed data using by ANOVA and ANCOVA of SPSS PC ver. 12.0 in order to determine more effective method according various squatting exercise. Results : Each groups show significantly difference value of biceps femoris, vastus lateralis and medialis, tibialis anterior, gastrocnemius lateralis and medialis among knee joint angle 45, 60, 90 degree. But there was no significantly difference value between gym ball group and wall group according knee joint angle. Conclusions : We suggested that squatting exercise with gym ball was more effective method improving lower extremity muscle strengthening, and changing knee joint angle was more effective compared than continued knee joint angle.

Differences in the Gait Pattern and Muscle Activity of the Lower Extremities during Forward and Backward Walking on Sand

  • Purpose: The purpose of this study was to compare the spatiotemporal and kinematic gait parameters and muscle activity of the lower extremities between forward walking on sand (FWS) and backward walking on sand (BWS) in normal adults. Methods: This study was conducted on 13 healthy adults. Subjects performed FWS and BWS and the spatiotemporal and kinematic gait parameters of stride time, stride length, velocity, cadence, step length, stance, swing, double support, and hip range of motion (ROM), knee ROM were measured by a wearable inertial measurement unit system. In addition, the muscle activity of the rectus femoris (RF), biceps femoris (BF), tibialis anterior (TA), and gastrocnemius (GA) was measured. Results: The stride length, stride velocity, cadence, and step length in the BWS were significantly lower than FWS (p<0.05), and stride time was significantly greater (p<0.05). However, there was no significant difference in the ratio of stance, swing, and double support between the two (p>0.05). The kinematic gait parameters, including hip and knee joint range of motion in BWS, were significantly lower than FWS (p<0.05). The muscle activity of the RF in BWS was significantly higher than FWS (p<0.05), but the muscle activity of the BF, TA, GA did not show any significant differences between the two movements (p>0.05). Conclusion: A strategy to increase stability by changing the gait parameters is used in BWS, and this study confirmed that BWS was a safe and effective movement to increase RF muscle activity without straining the joints. Therefore, BWS can be recommended for effective activation of the RF.

잠김 금속판을 이용한 하지의 전이성 병적 골절에 대한 치료 (Metastatic Pathologic Fractures in Lower Extremities Treated with the Locking Plate)

  • 목적: 골격계는 전이성 암에 의해 흔히 영향을 받는 부위이다. 본 연구를 통해 하지에서 발생한 전이성 병적 골절에 대한 치료로서 잠김 금속판을 이용한 치료 결과에 대해 알아보고자 한다. 대상 및 방법: 2004년부터 2010년까지 하지에 발생한 전이성 암에 의한 병적 골절에 대하여 잠김 금속판을 이용하여 치료받은 12명(13예)의 환자를 대상으로 평가를 시행하였다. 평균 환자 나이는 62.2세(50-81세)였으며, 골절은 각각 근위 대퇴골 2예, 대퇴골 간부 3예, 원위 대퇴골 3예, 근위 경골 4예, 원위 대퇴골 1예에서 발생하였다. 치료 결과로서 휠체어 보행 가능 시기, 통증 완화 정도 및 합병증을 평가하였으며, 또한 수술 시간 및 술 후 실혈량에 대하여 평가하였다. 결과: 수술 후부터 휠체어 보행까지는 평균 3.2일(1-6일)이 소요되었다. 평균 시각 통증 척도는 수술 전 8.1점(7-9점)에서 술 후 1주일째 2.7점(2-4점)으로 호전되었으며, 수술과 연관된 조기 합병증은 발생하지 않았다. 평균 수술 시간은 88.4분(70-105분)이었으며, 술후 평균 실혈량은 246.5 ml (130-320 ml)이었다. 결론: 하지의 전이성 병적 골절에 대한 치료로서, 잠김 금속판을 이용한 내고정술은 심한 골 파괴나 골 결손을 보이는 장골의 골간단부 혹은 골간부 병변에 효과적인 치료 방법이며, 또한 조기 보행을 가능하게 하고, 통증 및 술후 합병증 감소에 도움을 줄 수 있을 것이다.

인공 진피(알로덤®)을 이용한 하지의 골이 노출된 연부 조직 결손의 재건 (Reconstruction of the Bone Exposed Soft Tissue Defects in Lower Extremities using Artificial dermis(AlloDerm®))

  • Purpose: In extensive deep burn of the lower limb, due to less amount of soft tissue, bone is easily exposed. When it happens, natural healing or reconstruction with skin graft only is not easy. Local flap is difficult to success, because adjacent skins are burnt or skin grafted tissues. Muscle flap or free flap are also limited and has high failure rate due to deep tissue damage. The authors acquired good outcome by performing one - stage operation on bone exposed soft tissue defect with AlloDerm$^{(R)}$(LifeCell, USA), an acellular dermal matrix producted from cadaveric skin. Methods: We studied 14 bone exposed soft tissue defect patients from March 2002 to March 2009. Average age, sex, cause of burn, location of wound, duration of admission period, and postoperative complications were studied. We removed bony cortex with burring, until conforming pinpoint bone bleeding. Then rehydrated AlloDerm$^{(R)}$(25 / 1000 inches, meshed type) was applicated on wound, and thin split thickness(6 ~ 8 / 1000 inches) skin graft was done at the immediately same operative time. Results: Average age of patients was 53.6 years(25 years ~ 80 years, SD = 16.8), and 13 patients were male(male : female = 13 : 1). Flame burn was the largest number. (Flame burn 6, electric burn 3, contact burn 4, and scalding burn 1). Tibia(8) was the most affected site. (tibia 8, toe 4, malleolus 1, and metatarsal bone 1). Thin STSC with AlloDerm$^{(R)}$ took without additional surgery in 12 of 14 patients. Partial graft loss was shown on four cases. Two cases were small in size under $1{\times}1cm$, easily healed with simple dressing, and other two cases needed additional surgery. But in case of additional surgery, granulation tissue has easily formed, and simple patch graft on AlloDerm$^{(R)}$ was enough. Average duration of admission period of patients without additional surgery was 15 days(13 ~ 19 days). Conclusion: AlloDerm$^{(R)}$ and thin split thickness skin graft give us an advantage in short surgery time and less limitations in donor site than flap surgery. Postoperative scar is less than in conventional skin graft because of more firm restoration of dermal structure with AlloDerm$^{(R)}$. We propose that AlloDerm$^{(R)}$ and thin split thickness skin graft could be a solution to bone exposured soft tissue defects in extensive deep burned patients on lower extremities, especially when adjacent tissue cannot be used for flap due to extensive burn.

오르막보행 시 타이거스텝 하지 움직임에 미치는 영향 (Effect of Tiger Step on Lower Extremities during Uphill Walking)

  • Objective: The aim of this study was to investigate the effect Tiger-step walking on the movement of the lower extremities during walking. Method: Twenty healthy male adults who had no experience of musculoskeletal injuries on lower extremities in the last six months (age: 26.85 ± 3.28 yrs, height: 174.6 ± 3.72 cm, weight: 73.65 ± 7.48 kg) participated in this study. In this study, 7-segments whole-body model (pelvis, both side of thigh, shank and foot) was used and 29 reflective markers and cluster were attached to the body to identify the segments during the gait. A 3-dimensional motion analysis with 8 infrared cameras and 7 channeled EMG was performed to find the effect of tigerstep on uphill walking. To verify the tigerstep effect, a one-way ANOVA with a repeated measure was used and the statistical significance level was set at α=.05. Results: Firstly, Both Tiger-steps showed a significant increase in stance time and stride length compared with normal walking (p<.05), while both Tiger-steps shown significantly reduced cadence compared to normal walking (p<.05). Secondly, both Tiger-steps revealed significantly increased in hip and ankle joint range of motion compared with normal walking at all planes (p<.05). On the other hand, both Tiger-steps showed significantly increased knee joint range of motion compared with normal walking at the frontal and transverse planes (p<.05). Lastly, Gluteus maximus, biceps femoris, medial gastrocnemius, tibialis anterior of both tiger-step revealed significantly increased muscle activation compared with normal walking in gait cycle and stance phase (p<.05). On the other hand, in swing phase, the muscle activity of the vastus medialis, biceps femoris, tibialis anterior of both tiger-step significantly increased compared with those of normal walking (p <.05). Conclusion: As a result of this study, Tiger step revealed increased in 3d range of motion of lower extremity joints as well as the muscle activities associated with range of motion. These findings were evaluated as an increase in stride length, which is essential for efficient walking. Therefore, the finding of this study prove the effectiveness of the tiger step when walking uphill, and it is thought that it will help develop a more efficient tiger step in the future, which has not been scientifically proven.

Effects of Lower Extremity Resistance Exercise Using Elastic Bands on Balance in Elderly People

  • The purpose of this study was to assess the effects of lower extremity resistance training using elastic bands on balance in elderly people. Eight elderly persons each were randomly assigned to a test group(resistance exercise group, REG) or a control group(CG). FRT and TUG test were used to compare balance before and after exercise. Exercise programs were implemented three times a week for 40 minutes for nine weeks. They did warm-up exercise for 5 minutes and then lower extremity resistance exercise using elastic bands for 30 minutes. And then they did cool-down exercise for 5 minutes. TUG and FRT significantly decreased but in the control group it did not significantly decrease. In comparison between the groups, TUG and FRT significantly reduced in the resistance exercise group compared to the control group. Lower extremity resistance training using elastic bands performed by elderly persons are considered to be effective in improving balance.

사지동맥의 색전제거술 -26례의 분석- (Embolectomy of Arteries of Extremities -Clinical analysis of 26 cases)

  • 인제대학교 서울 백병원에서 실시한 말초동맥 색전제거술을 후향적으로 분석하였다. 1987년 3월부터 1996년 2월까지 26명의 환자가 색전제거술을 시술받았고 남자는 18명 여자는 8명이였으며 평균연령은 56.8세 였다. 24명의 환자에서 휴식시 동통이 주소였고 2명의 환자에서 급성 증상의 회복후 장기간의 간헐적 파행을 호소하였으며 10명의 환자에서만 신경/근육 증상을 보였다. 대부분의 색전은 심인성으로 빈도가 높은 원인질환으로8명의 환자는 허혈성 심질환, 11명의 환자는 심장판막질환이 있었다. 동맥색전 부위는 상지동맥이 6명, 안장색전증이 2명, 하지동맥 18명이였고 대퇴동맥이 11명으로 가장 많았다. 술전 동맥조영술은13명의 환자에서 진단과 수술계획을 위해 실시하였으나 나머지 환자에서는 실시하지 않았다. 2명만이 6시간이내 색전제거술을 실시하였으나 나머지 환자는 모두 6시간 이후에 수술하였 다. 모든 환자에서 우회로 조성술 엄이 포가티 색전제거 카테터를 사용하여 색전제거술을 하였는데, 상지동맥 색전증은 상완동맥 절개로 수술하였고, 안장색전증의 경우 양측 서혜부 절개로 접근하였으며, 하지동맥 색전증은 경대퇴동맥 접근을 하 느립\ulcorner3명의 원위부 슬와-경골동맥 색전증 환자에서 재수술시 경술와 동맥 색전제거술을 시행하였다. 수술 전후로 18명의 환자가 헤파린이나 푸락시파린으로 항응 고제 요법을 받았고 17명의 환자에서 퇴원시 와파린을 투여하였으며 항응고제 투여의 적응증은 심판막 질환, 심방세동, 말초동맥 죽상경화증, 재발색전 등이었다. 색전제거술후 14명의 환자에서 좋은 결과가 있었고, 5명에서 재수술후 증상 호전되었으며, 심한 하지동맥 죽상경화증을 보인 환자 1명에서 슬관절 이하 하지절단을 시행하였다. 색전재발은 1명, 사망 환자는 2명이 였는데, 사인은 각각 급성 신부전과 뇌동맥 혈전증이였다. 색전제거술 합병증으로 재관류 손상, 가성동맥류, 내막박리 등이 각각 1례 발생하였다. 결론적으로 사지동맥 색전증치 진단이 지연되고 있고, 허혈성 심질환을가진 고령환자가 증가되고 있는 경향이었다. 술전 동맥조영술은 항상 진단과 수술을 위해 필요하지 않으며 항응고제는 적응증에 따라 선택적으로 투여하여 색전재발을 방지할 수 있다. 원위부 슬와-경골동맥 색전증의 경우, 경대퇴동 맥 접근으로 선택적인 경골동맥 색전제거에 어려움이 있었다.

청심온담탕가미방과 침구치료를 포함한 복합한의치료로 호전된 심인성 떨림 환자 1례 - 증례보고 (A Case of Psychogenic Tremor Improved by Complex Korean Medicine Treatment Including Cheongshimondam-tang-gamibang and Acupuncture Treatment - A Case Report)

  • Objectives: A case study about the effectiveness of integrative Korean medicine treatment in a psychogenic tremor patient. Case presentation: A 36-year-old male diagnosed with psychogenic tremor with low back pain underwent 29 days of inpatient treatment and 4 weeks of outpatient treatment. His pattern identification was a psychogenic tremor of the weakness of the heart and gall bladder (SimDamHeoGup) type. The treatments were herbal medicine and acupuncture. He took Cheongshimondam-tang for 29 days and Chunwangbosim-dan for 6 weeks when he had symptoms. He also received acupuncture for 20 minutes twice a day at GB20 (Pungji), L14 (Hapgok), HT4 (Yeongdo), PC6 (Naegwan), HT7 (Sinmun), GB34 (Yangleungcheon), ST36 (Joksamni), SP6 (Sameumgyo), and LR3 (Taechung). After 8 weeks of treatment, the Fahn Tolosa Marin rating (FTM) scale of his tremor, at rest, dropped from Grade 4 on both upper extremities, trunk, and both lower extremities at the time of admission to Grade 2 in both upper extremities and trunk, and Grade 0 in both lower extremities at the time of discharge. His Beck Anxiety Inventory (BAI) score decreased from 38 to 7, and his numerical rating scale (NRS) decreased from 8 to 1 for low back pain and from 9 to 2 for tremor. A follow-up visit to the hospital 2 months after the end of treatment confirmed continued symptom improvements and no significant side effects. Conclusions: This study suggests the possibility of treating psychogenic tremor using only Korean medicine treatments. Further studies with control groups and long-term follow-up are needed.

A Fast Lower Extremity Vessel Segmentation Method for Large CT Data Sets Using 3-Dimensional Seeded Region Growing and Branch Classification

  • Segmenting vessels in lower extremity CT images is very difficult because of gray level variation, connection to bones, and their small sizes. Instead of segmenting vessels, we propose an approach that segments bones and subtracts them from the original CT images. The subtracted images can contain not only connected vessel structures but also isolated vessels, which are very difficult to detect using conventional vessel segmentation methods. The proposed method initially grows a 3-dimensional (3D) volume with a seeded region growing (SRG) using an adaptive threshold and then detects junctions and forked branches. The forked branches are classified into either bone branches or vessel branches based on appearance, shape, size change, and moving velocity of the branch. The final volume is re-grown by collecting connected bone branches. The algorithm has produced promising results for segmenting bone structures in several tens of vessel-enhanced CT image data sets of lower extremities.
