Risk Factors for the Treatment Failure of Antibiotic-Loaded Cement Spacer Insertion in Diabetic Foot Infection (당뇨병성 족부 감염에서 항생제 혼합 시멘트 충전물 사용의 치료 실패 위험 인자 분석)

  • Purpose: To evaluate the efficacy of antibiotic-loaded cement spacers (ALCSs) for the treatment of diabetic foot infections with osteomyelitis as a salvage procedure and to analyze the risk factors of treatment failure. Materials and Methods: This study reviewed retrospectively 39 cases of diabetic foot infections with osteomyelitis who underwent surgical treatment from 2009 to 2017. The mean age and follow-up period were $62{\pm}13years$ and $19.2{\pm}23.3months$, respectively. Wounds were graded using the Wagner and Strauss classification. X-ray, magnetic resonance imaging (or bone scan) and deep tissue cultures were taken preoperatively to diagnose osteomyelitis. The ankle-brachial index, toe-brachial index (TBI), and current perception threshold were checked. Lower extremity angiography was performed and if necessary, percutaneous transluminal angioplasty was conducted preoperatively. As a surgical treatment, meticulous debridement, bone curettage, and ALCS placement were employed in all cases. Between six and eight weeks after surgery, ALCS removal and autogenous iliac bone graft were performed. The treatment was considered successful if the wounds had healed completely within three months without signs of infection and no additional amputation within six months. Results: The treatment success rate was 82.1% (n=32); 12.8% (n=5) required additional amputation and 5.1% (n=2) showed delayed wound healing. Bacterial growth was confirmed in 82.1% (n=32) with methicillin-resistant Staphylococcus aureus being the most commonly identified strain (23.1%, n=9). The lesions were divided anatomically into four groups; the largest number was the toes: (1) toes (41.0%, n=16), (2) metatarsals (35.9%, n=14), (3) midfoot (5.1%, n=2), and (4) hindfoot (17.9%, n=7). A significant difference in the Strauss wound score and TBI was observed between the treatment success group and failure group. Conclusion: The insertion of ALCSs can be a useful treatment option in diabetic foot infections with osteomyelitis. Low scores in the Strauss classification and low TBI are risk factors of treatment failure.

Development of Low-Power IoT Sensor and Cloud-Based Data Fusion Displacement Estimation Method for Ambient Bridge Monitoring (상시 교량 모니터링을 위한 저전력 IoT 센서 및 클라우드 기반 데이터 융합 변위 측정 기법 개발)

  • It is important to develop a digital SOC (Social Overhead Capital) maintenance system for preemptive maintenance in response to the rapid aging of social infrastructures. Abnormal signals induced from structures can be detected quickly and optimal decisions can be made promptly using IoT sensors deployed on the structures. In this study, a digital SOC monitoring system incorporating a multimetric IoT sensor was developed for long-term monitoring, for use in cloud-computing server for automated and powerful data analysis, and for establishing databases to perform : (1) multimetric sensing, (2) long-term operation, and (3) LTE-based direct communication. The developed sensor had three axes of acceleration, and five axes of strain sensing channels for multimetric sensing, and had an event-driven power management system that activated the sensors only when vibration exceeded a predetermined limit, or the timer was triggered. The power management system could reduce power consumption, and an additional solar panel charging could enable long-term operation. Data from the sensors were transmitted to the server in real-time via low-power LTE-CAT M1 communication, which does not require an additional gateway device. Furthermore, the cloud server was developed to receive multi-variable data from the sensor, and perform a displacement fusion algorithm to obtain reference-free structural displacement for ambient structural assessment. The proposed digital SOC system was experimentally validated on a steel railroad and concrete girder bridge.

Centrifugal Test on Behavior of the Dolphin Structure under Ship Collision (선박충돌 시 돌핀 구조물의 거동에 대한 원심모형실험)

  • The impact protection system consists of an arrangement of circular sheet pile cofferdams-denoted dolphin structuredeeply embedded in the seabed, filled with crushed rock and closed at the top with a robust concrete cap. Centrifuge model tests were performed to investigation the behaviors of dolphins in this study. Total 7 quasi-model tests and 11 dynamic model tests were performed. The main experimental results can be summarized as follows. Firstly, The experimental force-displacement results for quasi-static tests show a limited influence on the initial stiffness of the structure from the change in fill density and the related change in the stiffness of the fill. And by comparing the dissipation at the same dolphin displacement it was found that the denser fill increase the dissipation by 16% for the 20m dolphin and by 23% for the 30m dolphin. The larger sensitivity for the large dolphin is explained by a larger contribution to the dissipation from strain in the fill. In low level impacts the dynamic force-response is up to 26~58% larger than the quasi-static and the dissipation response is showed larger in small displacement. Hence, it is concluded conservative to use the quasi-static response characteristics in the approximation of the response, and it is further concluded that the dolphin resistance to low level impacts is demonstrated to be equivalent and even superior to the high level impacts.

  • Carotenoids are red, orange, and yellow fat-soluble pigments that exist in nature, and are known as physiologically active substances with various functions, such as provitamin A, antioxidant, anti-inflammatory, and anticancer. Because of their physiological activity and color availability, carotenoids are widely used in the food, cosmetics, and aquaculture industries. Currently, most carotenoids used industrially use chemical synthesis because of their low production cost, but natural carotenoids are in the spotlight because of their safety and physiologically active effects. However, the production of carotenoids in plants and animals is limited for economic reasons. Carotenoids produced by bacteria have a good advantage in replacing carotenoids produced by chemical synthesis. Since carotenoids produced from bacteria have limited industrial applications due to low productivity, studies are continuously being conducted to increase the production of carotenoids by bacteria. Studies conducted to increase carotenoid production from bacteria include the activity of enzymes in the bacterial carotenoid biosynthesis pathway, the development of mutant strains using physical and chemical mutagens, increasing carotenoid productivity in strain construction through genetic engineering, carotenoid accumulation through stress induction, fermentation medium composition, culture conditions, co-culture with other strains, etc. The aim of this article was to review studies conducted to increase the productivity of carotenoids from bacteria.

A Study on the Low Speed Impact Response and Frictional Characteristics of Shear Thickening Fluid Impregnated Kevlar Fabrics (전단농화유체를 함침한 케블라 직물의 저속충격 거동 및 마찰특성 연구)

  • In this study, shear thickening fluid (STF) filled with rigid nano silica particles was impregnated in plain woven Kevlar fabrics to improve the impact resistance performance. The nano silica particles with an average diameter of 100nm, 300nm, and 500nm were used to make shear thickening fluid to estimate the effect of particle size on the impact behavior of STF impregnated Kevlar fabrics. The yam pull-out and frictional tests were conducted to estimate the effect of impregnated STF on the frictional characteristics. The test results showed that the friction forces were dramatically increased at the STF onset shear strain rates that were measured in preliminary rheology tests. The low speed impact tests were performed using the drop test machine. The results showed that the impregnated STF improved the impact resistance performance of the Kevlar fabrics in terms of the impact energy absorption and the deformation. It has been shown through tests that the impregnated STF affects the interfacial friction which contributes to improve the energy absorption in the Kevlar fabrics. Especially, the impregnation of the STF with the smaller particle size into the Kevlar fabrics showed the better performance in impact energy absorption.

A Study on Antiulcer Effects of Opuntia dillenii Haw. on Stomach Ulcer Induced by Water-immersion Stress in Rats (랫드의 스트레스성 위궤양에 대한 Opuntia dillenii Haw.(선인장)의 항궤양작용에 관한 연구)

  • This study was performed to investigate the antiulcer effects of Opuntia dillenii Haw. on the stomach ulcer induced by restraint and water-immersion stress in rats. For this experiment, 48 male Sprague-Dawley strain were used. The experimental groups were divided into four: a control (C) and 3 Opuntia dillenii Haw. treatment groups (E-1, E-2, E-3). Each dose of Opuntia dillenii Haw. was 30 mg/kg bw (E-1), 60 mgfKg bw (E-2) and 120 mg/kg bw (E-3). The rats were allocated to each group by 12 and observed for 4 weeks. The results were as following: 1. The stomach surface pH in each group showed no significant difference, but the values of Opuntia dillenii Haw. treatment groups were higher than the value of the control group. 2. The gastric wall mucus was increased in all Opuntia dillenii Haw. treatment groups compared with the control group. Especially in E-1 difference was higher (p<0.05) and in E-2 difference was significantly higher (p<0.01). 3. At shear rate 11.25, 45.0, 90.0, $225\;sec^{-1}$, whole blood viscosity and plasma viscosity were measured. Most of the values of Opuntia dillenii Haw. treatment groups were low compared with that of the control group. At shear rate 90.0, $225\;sec^{-1}$ the values of whole blood viscosity in E-1 were significantly low (p<0.05) and at shear rate 11.25, $45.0\;sec^{-1}$, more significant (p<0.01). At shear rate 11.25, 45.0, 90.0, $225\;sec^{-1}$ the values of whole blood viscosity in E-2 were significantly low (p<0.01). At shear rate $90.0\;sec^{-1}$ the value of plasma viscosity in E-1 was significantly low (p<0.05) and at shear rate 90.0, $225\;sec^{-1}$ the values of plasma viscosity in E-2 we resignificantly low (p<0.01). 4. Less severe ulcers were obsered in Opuntia dillenii Haw. treatment groups than in the control group. Especially E-1 groups tissues had only slight ulcers and necrosis of tissue was not observed in this group. From the results of this study, it can be concluded that the oral administratio-n of Opuntia dillenii Haw. results in protection of stomach ulcer by stimulating the secretion of gastric mucus and improving the gastric mucosal microcirculation.

Studies on the Preserving Method for Inhibiting Feed Degradation (사료변질 방지를 위한 보존방법에 관한 연구)

  • The experiment using Anake broiler strain chicks was conducted to study the effort fungistatic agents on microbial counts, Ins of nutrient, growth rate and feed efficiency of the broiler. Feed was adjusted to 12% and 15% moisture level during hot and high humidity season and sorbic acid at the level of 0.02%, 0.04% and Ca-propionic acid at the level 0.1% 0.2% was added. The results obtained were as follows. 1, The Addition of fungistatic agents could slightly reduce mold and yeast counts. The highest effect on inhibition of mold and yeast counts was observed for the addition of sorbic acid at 0.04% level to the fled contained 15% moisture. 2, Approximately 14% starch loss of ground corn was observed from the fred contained 15% moisture and the loss could be diminished to 3-7% by the addition of fungistatic agents with the superior effect of sorbic acid to Ca-propionic acid. 3. Approximately 15% fat loss was detected when high moisture fled was und and this was reduced to 7% by the addition of 0.04% sorbic acid to the feed. 4. Significantly higher growth rate (p<0.05) during starter period was observed for low moisture feed added by sorbic acid compared with that for high moisture diet without fungistatic agents or with Ca-propionate at the level of 0.1%. 5. Significantly lower feed efficiency (p<0.05) during starter period of high moisture feed without fungistatic agents was observed; hower no significant different response was detected by either moisturer level of kinds and levels of fungistatic agents used. 6. Significantly higher growth rate (p<0.05) during finisher period of lower moisture feed with 0.04% sorbic acid was observed compared with that of high moisture fled without fungistatic agents or the feed added by 0.1% Ca-propionate level. 7. Significantly higher feed efficiency (p<0.05) during finisher period of low moisture feed added by sorbic acid 0.02% or 0.04% level was found compared with that of high moisture feed without fungistatic agents and low moisture feed added by Ca-propionate at the level of 0.1% or 0.2%. 8. Mort bacteria, mold and yeast were observed in the ceca than in small intestine. The. moisture content of dict had no effect on intestinal microfloral counts. However, numbers of mold and yeast of intestine could slightly be reduced by fungistatic agents administration. 9. Nothing but encephalomalacia to chicks fed feed contained 15% moisture without addition of fungistatic agents was observed. In conclusion, addition of either sorbid acid at 0.04% level and Ca-propionate at 0.2% level to high moisture feed or reduced moisture level to 12% could be con-sidered more effective to enhance growth rate and fled efficiency of broilers during summer period.

  • Objectives Low back pain (LBP) is one of the most common reason for people in Korea to visit Korean medical institutions. To assess actual amounts of use in the treatment of LBP in Korean medicine and to provide objective base line data for policy decision making, research regarding the current state of LBP patients' treatment in Korean medical institutions are in need. Methods The current study was designed as a retrospective chart review to investigate descriptive characteristics of LBP patients. The clinical records of 304 patients who were hospitalized for the treatment of LBP in Korean rehabilitation medicine inpatient clinics of five different Korean medicine hospitals were analyzed. The percentage of patient characteristics such as sex, age, average duration of admission, insurance type, diagnosed LBP related disease code, and rates of interventions applied were assessed. Results 1. The female sex was significantly predominant among patients with LBP : 105 patients (34.5%) were male and 199 patients (65.5%) were female. Percentage of the patients' age appeared as followed : 76 people (25.0%) were in their 50s, 64 people (21.1%) were in their 40s, 51 people (16.8%) were in their 30s, 37 people (12.2%) were in their 60s, and 33 people (10.9%) were in their 70s. Approximately half of the total LBP patients investigated were older than 50. 2. The average duration of admission was 16.2 days. Approximately one third (30.3%) of the patients were hospitalized for 8 to 14 days. 3. Female patients tended to stay admitted in hospitals slightly longer than male patients. Elderly (age 60~79) patients stayed in hospitals longer (17.8 days) compared to younger (age 20~39) patients (13.5 days). 4. More than half of the patients (171 cases, 56.3%) had their hospital bills covered with automobile insurance. 40.1% (122 cases) of the patients had medical insurance to cover their hospital bills. The average duration of admission of patients who had automobile insurance was 14.2 days, while that of the patients who had medical insurance was 18.4 days. 5. "Sprain and strain of the lumbar spine and pelvis" was the most commonly used (195 cases, 64.1%) disease code in patients with LBP. Patients diagnosed as "lumbar and other intervertebral disc disorders with radiculopathy" required the longest admission duration (22.1 days). 6. Herbal medication was applied to all of the patients during admission. Acupuncture was applied to all of the patients except one case diagnosed as spinal stenosis. Physical therapy, cupping therapy, moxibustion therapy, chuna therapy, and pharmacopuncture therapy were applied to 94.7, 92.8, 85.2, 83.9, and 49.7% of the patients, respectively. 7. There were certain differences among Korean medicine hospitals in terms of the LBP patients' duration of admission, type of insurance, frequency of the disease code use, type of intervention applied. Conclusions It is thought that the current study can be used as reference data in assessing the current state of LBP treatment in Korean rehabilitation medicine and a basis for future research. Provided improvements of certain limitations of the current study in future researches, such data would act as better base line data in policy decision making.

A Study of well-being in Caregivers Caring for Chronically Ill Family Members (만성 질환자 가족의 부담감에 관한 연구)

  • Today, more chronically ill and handicapped people are being cared for at home by a family member caregiver. The task of caring for a family momber may mean that the caregiver has less time and money and more work which may result in increased fatigue and symptoms of illness. This study was done to examine the well-being of family caregivers. Fifty three family caregivers were interviewed. Concepts were measured using existing tools and included : Burden(25 item 5 point scale), Social sup-port (21 item 7 point scale), Health status defined by a symptom checklist(48 item S point scale), and Well -being defined by a quality of life scale (14 item 7 point scale) and caregiving activities. Data collection was done by interview and Q-sort. Social support and well - being were positively correlated as were symptoms and burden. Symptoms and burden were negatively correlated with social support and well-being. Items on the quality of life scale had a mean score range from 3.09 to 4.96. Quality of life related to income was lowest (3.09) but the desire to use more money for the patient was rated 2.90 on the burden scale where the item means ranged from 0.73 to 3.55. The high mean of 3.55 was for obligation to give care and the low 0.73 was (or not feeling that this was helping the patient. Mean scores for symptoms ranged from 0.26 to 2.15 with the 2.15 being for “worry about all the things that have to be done.” Over half of the patients were dependent for help with some activities of daily living. The caregivers reported doing an average of 3.40 out of five patient care activities including bathing (77.4%), shampooing (67.9%), and washing face and hands (49.1%), and 3.74 out of seven home maintenance activities including laundry (98.1%), cooking (83.0%), and arranging bed-ding(75.5%). The caregivers reported their spouse as one of the main sources of social support, including in times of loneliness and anger The mean score for loneliness as burden was 2.15 and ranked fourth and 31 (58.5%) of the sample reported being lonely recently and not being satisfied with the support received. Similarly anger caused by the patient was given a mean score of 2.13, and anger was reported to have been present recently by 38 (71.7%) of the sample and satis-faction with the support given was low. Having someone to help deal with anger ranked twelfth out of 21 items on the social support scale and had a mean score of 3.98 (range 3.49 to 5.98). Spouses were reported as a major source of social support but the fact that 50% of the caregivers were caring for a spouse, may account for the quality of this source of social support having been affected. These caregivers faced the same problems as others at the same stage of life. but because of the situation, there was a strain on their resources, particularly financial and social. In conclusion it was found that burden is correlated negatively to quality of life and positively to symptoms, but in this sample, symptoms and bur-den were scored relatively low. Does this indicate that the caregivers accept caregiving as part of their destiny and accept the quality of their lives with burden and symptoms just being a part of caregiving\ulcorner Does the correlation between the bur-den and symptoms indicate they are a measure of the same phenomenon or that the sample was of a more mobile, less burdened group of caregivers\ulcorner Quality of life was the one variable that was significant in explaining the varience on burden. Further study is needed to validate the conclusions found in this study but they indicate a need for nurses to ap-proach these caregivers with a plan tailored to each individual situation and to give consideration to interventions directed at improving quality of life and expanding social support networks for those caring for spouses.

Effect of Sodium Intake on Responses of Blood Pressure, Renin-Aldosterone and Renal Excretions to Atrial Natriuretic Peptide in Spontaneously Hypertensive Rats (소금 섭취량을 달리한 정상 및 고혈압쥐에서 Atrial Natriuretic Peptide가 혈압, Renin-Aldosterone 및 신배설에 미치는 영향)

  • Effects of atrial natriuretic peptide (ANP) on blood pressure, plasma lenin activity, aldosterone and renal excretion were compared in conscious spontaneously hypertensive rats (SHR) and normotensive Wistar rats fed low, medium or high sodium diet (2, 10, 25 mmol NaCl/100g diet) for 6 weeks. ANP infusion (380 ng/kg/min for 20 min) produced reductions in blood pressure, plasma renin activity, and aldosterone level, but marked increases in hematocrit, urine flow, and excretions of sodium and potassium. The low sodium group showed a significantly enhanced aldosterone lowering effect of ANP than the high sodium group. However, three salt groups showed no difference in effects of ANP on blood pressure, plasma renin activity, hematocrit and diuresis. Natriuretic response to ANP was significantly greater in the high salt-than in the low sait-SHR, but was not different between the Wistar salt groups. There were strain differences in effects of ANP: SHR showed greater responses of blood pressure and natriuresis than Wistar rats. Above results indicate that aldosterone-lowering and natriuretic effects of ANP were modifed by different dietary sodium intakes. However, blood pressure- and renin-lowering, or diuretic effects of ANP were not affected by dietary sodium intakes. The mechanisms whereby dietary sodium intakes alter the effects of ANP in the pathogenesis of hypertension are not clear.
