Effect of Acupotomy with Selective Cryolysis for Localized Fat: Case Report (도침치료와 냉각요법을 병용한 부분 비만의 치료: 증례보고)

  • The aim of this study is to report clinical progresses on localized fat. We devised the method of treat localized fat using acupotomy with selective cryolysis. We selected 4 subjects among the patients who maintained their body weight (within 5%) to minimize effects of losing weight. We practiced acupotomy with selective cryolysis therapy on 4 lesion of abdominal fat, 2 lesion of upper arm fat, 5 times for 2 month. All subjects had loss of abdominal circumference, and 1 subject had significantly loss of upper arm circumference. And there was no adverse effects. It shows that method of using acupotomy with selective cryolysis may be effective on localized fat.

The Relationships between Type of Localized Fat and Traditional Diagnostic Method in Korean High-school girls (서울지역 여고생의 국소지방 침착의 양상과 변증과의 연관성)

  • Background : There are many korean women who have localized fat despite of they have normal or slightly over weight. The relationships of traditional diagnostic method with total body fat and body mass index had been reviewed in other study, but there is no study regarding the relationship of traditional diagnostic method and localized fat. So we designed this study to find out the relationships between localized fat and traditional diagnostic method among Korean high-school girls. Methods : This research was performed in 21 high-school girls with localized fat during April 2007. We measured body mass index, percent of body fat, waist circumference, hip circumference and means of both thigh circumference. Phlegm questionnaire, blood stasis questionnaire, cold-heat questionnaire, Korean eating attitude test-26 and questionnaire of Sasang constitution classification Ⅱ+ have been administered. Results : There were significant correlations between body mass index, percent of body fat and body size. Regardless of body mass index, they had high phlegm and heat pattern. Most of them are classified as Tae-um type person. Conclusions : High-school girls with localized fat had pattern of high phlegm, heat and characteristics of Tae-um person.

Effects of High Frequency Therapy on Localized Obesity (고주파요법이 부분비만치료에 미치는 영향)

  • Objectives : This study was performed to identify the effects of high frequency therapy on localized obesity. Methods : This trial was carried out in 12 volunteers. Volunteers were divided into 3 groups; upper arm group (n=4), thigh group (n=4) and abdomen group (n=4) according to local obesity type. Body weight and body fat were measured by Inbody 720 and CT (Computed Tomography) immediately before and following high frequency therapy. Diathermy was performed twice a week for 4 weeks for a total of 8 treatments. Results : In the upper arm group, body weight, body fat mass, fat area by CT scan and circumference were increased after treatment but not significantly (p>0.05). In the thigh group, body weight, body fat mass and circumference were decreased and fat area by CT scan was increased but both not significantly (p>0.05). In the abdomen group, significant differences were not found despite decreases in body weight, body fat mass, visceral fat and subcutaneous fat after diathermy (p>0.05). Conclusions : There was no significant effects of high frequency therapy on localized obesity.

A Case Report for the Effects of Pharmacopuncture Combined Thread Implantation Therapy to Improve Localized Obesity (부분비만 개선을 위한 매선과 약침의 병행 치료 임상 효과 증례 보고)

  • The aim of this study is to investigate the effect of pharmacopuncture combined thread implantation therapy for treating localized obesity. The study was conducted on 3 subjects with abdominal localized fat, 1 subject with thigh and 1 subject with calf localized fat. We practiced two kinds of pharmacopunture, which were cultivated wild ginseng and contacting Ephedra sinica pharmacopuncture. In addition, we practiced thread implantation to 20 points on muscle membrane layer. We performed these treatments once in 2 weeks, totally for 3 times. After treatments, abdominal circumference and waist-hip ratio were reduced by 4.6 % and 0.13, respectively. In addition, circumferences of thigh and calf were reduced by 5.8 % and 4%. We supposed that this combined therapy could be useful to deal with localized fat deposit. Long-term evaluation and further studies are required to identify effect size of this therapy.

Effect of Polygonatum sibiricum Rehd. pharmacopuncture on localized adiposity in obese mice (황정 약침의 비만 쥐에서의 국소 지방분해 효능 연구)

  • Objectives: Excessive accumulation of fat on specific region, regarded as localized fat, is one of the serious problems and well-known risk factors of health. Recently, an interest in health and aesthetics is growing by treating lipolytic injection. Polygonatum sibiricum Rehd (PS) has been known to have anti-oxidant, -aging and -atherosclerotic effects. In this study, we investigated the lipolytic effects of PS pharmacopuncture in obese mice. Methods: Male C57BL/6J mice was fed with high fat diet to induce obesity for 12 weeks. PS pharmacopuncture was dissolved in saline by adjusting pH 7. 100 μL of PS pharmacopunture was injected subcutaneously into the left side inguinal fat pad, while saline was injected into the right side inguinal fat pad in mice as self-control. Samples were treated 3 times per weeks for 2 weeks. Results: PS pharmacopunture significantly decreased the inguinal fat weight compared to left side inguinal fat pad. Decrease rate of PS pharmacopuncture was about 21%. In addition, the diameter of adipocyte in inguinal fat tissues was significantly reduced by 17% compared to saline-injected side. There was no sign of toxicity through whole experiments. Conclusion: The present study indiates that PS pharmacopunture could be a material derived from natural herb as a lipolytic injection for decreasing localized fat.

The Efficacy and Safety of Ephedra and Green Tea Mesotherapy on Localized Fat (피하지방 감량에 있어 경피침주요법의 유효성 및 안전성 평가)

  • Objectives : This study was performed to identify the efficacy and safety of ephedra and green tea mesotherapy on localized fat. Methods : Twenty-five healthy $20{\sim}35$ year overweight women (body mass $index{\geq}23kg/m^2$) were recruited and randomly assigned to two groups : experimental(mahuang and green tea) group (n=13), placebo(Normal saline) group (n=12). Total 8 times of herbarl mesotherapy treatment were performed once per week, 4 times were performed on right thigh (1st period), after wash-out for 1 week, the other 4 times were performed on left thigh (2nd period) without any modification of diet or exercise. Body composition, thigh circumferences were measured at each period before and after the intervention. Subcutaneous fat was measured by CT scan at before and after the 1st period of intervention and free fatty acid(FFA) was measured immediately before 1st intervention, 1 hour later, 24 hours later, and 7 days later. Satisfaction score and adverse events were evaluated. Results : In the subcutaneous fat, there were no significances, but in the circumferences. Experimental group had significant change when compared with placebo group (p<0.05). In FFA, experimental group has significant elevation at after 1 hour (p<0.05) evaluation. There were no serious adverse events in both groups. Conclusions : Experimental group had some significant findings in circumference, FFA, and had no serious adverse events comparing with placebo group, and no serious adverse events have been reported in both group. Mesotherapy with herb have a possibility of treating localized fat

  • Localized fat and cellulate have not only esthetic but health problem, they are treated by surgical or non-surgical method. There are elecro-acupuncture, ultrasound, radio frequency, endermologie, mesotherapy, and carbon dioxide therapy in prevalent non-surgical treatments. We reviewed the studies on them to analyze their seicntific evidence. The studies are not many. They reported good results of circumference loss or appearance improvement of histologic change but most of them have poor study design. Before marketing or using these modalities, it is ontcal to evaluate to their effectiveness through strict and scientific clinical trials.

  • The aim of this study is to report clinical progresses on localized fat in three Japanese with Korean medicine. We conducted electro-acupuncture on thigh and/or Gamitaeyeumjowi-tang, all subjects had loss of thigh circumference and 2 subjects who took Gamitaeyeumjowi-tang had body weight loss. It shows that Korean medical treatments may be effective on localized obesity and can also apply to Japanese.

A surgical approach to linear scleroderma using Medpor and dermal fat graft

  • Linear scleroderma en coup de sabre (LScs) is a variant of localized scleroderma. This disease typically occurs in patients in their 20s or younger individuals and predominantly occurs in the forehead area. A 26-year-old man with linear scleroderma was surgically treated at our center with Medpor (porous polyethylene) and dermal fat graft for the forehead lesion. After 26 months of postoperative follow-up, the depressed lesion that appeared scarred as well as the margins improved significantly. The surgical treatment of LScs using Medpor and dermal fat graft is an effective treatment modality that can increase patient satisfaction.
