An experimental investigation was conducted to investigate the heat/mass transfer for impingement/effusion cooling system with inclined jet. Jets with inclined angle of 60 are applied to impingement/effusion cooling. At the jet Reynolds number of 10,000, the experiments were carried out for blowing ratios ranging from 0.0 to 1.5. The local heat/mass transfer coefficients on the effusion plate are measured using a naphthalene sublimation method. The result indicates that the inclined jet causes the non-uniform and low heat/mass transfer compared to the vertical jet. At stagnation region, the peak position is shifted from the geometrical center of injection hole due to Coanda effect and its level is higher than that of vertical jet due to increase in turbulence intensity by steep velocity gradient near the stagnation region. Further, the secondary peak region disappears because the interaction between adjacent wall jets weakens. When the initial crossflow occurs, the distorted heat/mass transfer pattern appears. As the blowing ratio (crossflow rate) increases, the heat/mass transfer distributions become similar to those of the vertical jet. This is because the effect of crossflow is dominant compared to that of inclined jet under high blowing ratio $(M{\geq}1.0)$. At low blowing ratio $(M{\leq}0.5)$, averaged Sh value is 10% lower than that of vertical jet, whereas its value at high blowing ratio $(M{\geq}1.0)$ is similar to that of vertical jet.

Objective : Trigeminal neuralgia (TN) is a severe neuropathic condition that affects several elderly patients. It is characterized by uncontrolled pain that significantly impacts the quality of life of patients. Therefore, the condition should be treated as an emergency. In the majority of patients, pain can be controlled with medication; however, other treatment modalities are being explored in those who become refractory to drug treatment. The use of the trigeminal nerve block with a local anesthetic serves as an excellent adjunct to drug treatment. This technique rapidly relieves the patient of pain while medications are being titrated to effective levels. We report the efficacy and safety of percutaneous trigeminal nerve block in elderly patients with TN at our outpatient clinic. Methods : Twenty-one patients older than 65 years with TN received percutaneous nerve block at our outpatient clinic. We used bupivacaine (1 mL/injection site) to block the supraorbital, infraorbital, superior alveolar, mental, and inferior alveolar nerves according to pain sites of patients. Results : All patients reported relief from pain, which decreased by approximately 78% after 2 weeks of nerve block. The effect lasted for more than 4 weeks in 12 patients and for 6 weeks in two patients. There were no complications. Conclusion : Percutaneous nerve block procedure performed at our outpatient clinic provided immediate relief from pain to elderly patients with TN. The procedure is simple, has no serious side effects, and is easy to apply.

This paper proposes the extraction of coronary arteries based on DSA(Digital Subtraction Angiography) through a texture analysis of background in the angiography. DSA is a well established modality for the visualization of coronary arteries. DSA involves the subtraction of a mask image - an image of the heart before injection of contrast medium - from live image. However, this technique is sensitive to the movement of background and can result to a wrong detection by the variance of background gray-level intensity between two images. Therefore, this paper solves a structural problem resulted from a background movement bV selecting an image which has the least difference of movement through an analysis of the similarity of background texture and proposes a method to extract only the blood vessel efficiently through local gray-level correction of the selected image. Using the coronary angiogram of 5 patients clinical data, we proved that the proposed method has the lower false-detection rate, approximately $2\%$, and the higher accuracy than the existing methods.
The present study has been performed to investigate the influences of rectangular fins on heat transfer in an impingement/effusion cooling system with crossflow. To simulate the impingement/effusion cooling system with initial crossflow, two perforated plates are placed in parallel and staggered arrangements with a gap distance of 2 times of the hole diameter. The crossflow passes between the plates, and various rectangular fins are installed on the plates. Reynolds number based on the hole diameter is fixed to 10,000 and the flow rate of crossflow is changed from 0.5 to 1.5 times of that of the impinging jet. A naphthalene sublimation method is used to obtain the heat/mass transfer coefficients on the effusion plate. Also to analyze the flow characteristics, a numerical calculation is performed. When rectangular fins are installed, the flow and heat transfer pattern is changed greatly from the case without fins. In the injection hole region, the jet impinges on effusion plate without deflection and wall jet spreads symmetrically. In the effusion region, the crossflow accelerates due to the decrease of cross-sectional area in the channel. Local heat/mass transfer coefficients are enhanced significantly compared to the case without fins. As the blowing ratio increases, the effect of rectangular fins against the crossflow becomes more significant and then the higher average heat/mass transfer coefficients are obtained than the case without fins. However, the increase of blockage effect gives more pressure loss in the channel.

Adult hippocampal dentate granule neurons are generated from neural stem cells (NSCs) in the mammalian brain, and the fate specification of adult NSCs is precisely controlled by the local niches and environment, such as the subventricular zone (SVZ), dentate gyrus (DG), and Toll-like receptors (TLRs). Epigallocatechin-3-gallate (EGCG) is the main polyphenolic flavonoid in green tea that has neuroprotective activities, but there is no clear understanding of the role of EGCG in adult neurogenesis in the DG after neuroinflammation. Here, we investigate the effect and the mechanism of EGCG on adult neurogenesis impaired by lipopolysaccharides (LPS). LPS-induced neuroinflammation inhibited adult neurogenesis by suppressing the proliferation and differentiation of neural stem cells in the DG, which was indicated by the decreased number of Bromodeoxyuridine (BrdU)-, Doublecortin (DCX)- and Neuronal Nuclei (NeuN)-positive cells. In addition, microglia were recruited with activating TLR4-NF-${\kappa}B$ signaling in the adult hippocampus by LPS injection. Treating LPS-injured mice with EGCG restored the proliferation and differentiation of NSCs in the DG, which were decreased by LPS, and EGCG treatment also ameliorated the apoptosis of NSCs. Moreover, pro-inflammatory cytokine production induced by LPS was attenuated by EGCG treatment through modulating the TLR4-NF-${\kappa}B$ pathway. These results illustrate that EGCG has a beneficial effect on impaired adult neurogenesis caused by LPS-induced neuroinflammation, and it may be applicable as a therapeutic agent against neurodegenerative disorders caused by inflammation.
Magnetic resonance electrical impedance tomography (MREIT) enables us to perform high-resolution conductivity imaging of an electrically conducting object. Injecting low-frequency current through a pair of surface electrodes, we measure an induced magnetic flux density using an MRI scanner and this requires a sophisticated MR phase imaging method. Applying a conductivity image reconstruction algorithm to measured magnetic flux density data subject to multiple injection currents, we can produce multi-slice cross-sectional conductivity images. When there exists a local region of fat, the well-known chemical shift phenomenon produces misalignments of pixels in MR images. This may result in artifacts in magnetic flux density image and consequently in conductivity image. In this paper, we investigate chemical shift artifact correction in MREIT based on the well-known three-point Dixon technique. The major difference is in the fact that we must focus on the phase image in MREIT. Using three Dixon data sets, we explain how to calculate a magnetic flux density image without chemical shift artifact. We test the correction method through imaging experiments of a cheese phantom and postmortem canine head. Experimental results clearly show that the method effectively eliminates artifacts related with the chemical shift phenomenon in a reconstructed conductivity image.
Purpose: Epidermoid cyst may be congenital or acquired. Acquired cysts are most commonly of traumatic origin and result from an implantation or downward displacement of an epidermal fragment. Traumatic epidermoid cysts are rare tumors occurring on the nasal tip, especially resulting aesthetic procedure. So, we report a rare case of an iatrogenic epidermoid cyst in the nasal tip following rhinoplasty. Methods: A 44 - years old man had undergone rhinoplasty for several times. First time, the previous augmentation rhinoplasty and wedge osteotomy were performed nineteen months ago, lastly implant removal and unknown filler injection were performed one year ago at another local clinic. He had induration and tenderness on nasal tip and dorsum continued for 3 months. We thought that it caused by foreign body reaction with residual alloderm in nose. For removal of residual alloplastic material, open approach using transcolumellar incision was done. But, incidentally we found cystic mass on the nasal tip. Results: The findings were of an $0.8{\times}0.5{\times}0.5cm$ sized round cystic mass containing cream coloured material with a thick cheese - like consistency. The mass was completely excised and submitted for histology. This confirmed the diagnosis of an epidermoid cyst lined by keratinizing squamous epithelium. There was no induration, tenderness and sign of recurrence after excision of the mass. Conclusion: Epidermal cyst of the nasal tip region represents an unusual clinical lesion and it presents as foreign body reaction. And then, our case demonstrates that meticulous surgical approach and suture technique are the keys to prevention against iatrogenic nasal epidermoid cyst, especially in secondary rhinoplasty.

One hundred and forty-eight cases of tennis elbow were treated by conservative managements firstly such as rest, medication, immobilization, physical therapy and local steroid injection, from Jan. 1985 to Jun. 1994 at Department of Orthopaedic Surgery, National Medical Center. Among the 148 cases, 16 cases who failed conservative managements, were treated with Nirschl & Pettrone operation. The results were summarized as follows, 1. Among the 148 cases, 110 cases$(74\%)$ were female, and 61 cases$(41.2\%)$ were in the age group 41 to 50, and mean age was 42.3 2. Ninety-nine cases were housewives and only 15 cases were related to sports. 3. The results 116cases$(78\%)$, of conservative treatment were excellent and good 10 fair and failure in 32 cases$(22\%)$, by Nirschl and Peettrone assessment. 4. Among the 16 cases who were treated with Nirschl and Pettrone operation, excellent result was obtained 10 7cases, good in 3 cases, fair in 4cases and failure in 2 cases. 5. Recurrence was developed in 18 cases$(12.2\%)$, and we obtained excellent & good result in 17 cases with both conservative and operative treatment. Above results suggest that the term, tennis elbow is a misnomer because it occurs more commonly in non-athletes such as housewives than in tennis players. So it seems to be a occupational disease rather than sports injury. Now, we propose eliminate of the term, elbow and substitute with terminology that described the true understanding and therapeutic orientation of it.
A female Green iguana (Iguana iguana) was brought to the small animal clinic in Hannover Veterinary school because of anorexia and convulsion. Two months ago, the owner bought two iguanas, one female and one male and had to take a female to the one of local small animal clinic because she, born in 19931 didn't eat well and showed the tremor of the legs. The veterinarian suspected it to have a calcium deficiency and gave an injection of calcium and vitamin D3. But still during the last two months, the female iguana did only eat some lettuce while the other male ate lettuce, fruits and commercial cat-food ($Whiskas^{\circledR}$, Effem, Verden). It was not possible to evaluate the rations of these different "food-components". They are kept in the terrarium with water container. By palpation of the abdomen, hard thing like stone was palpated in the mid-region. The blood test wasn't carried out because of the difficulties of getting blood and because of her bad condition. It was firmly diagnosed as cystic calculus through radiography. Since the animal is in shock state already, the prognosis after surgery was bad. It was assumed to be too late to expect its surviving with the surgery carried out or without it. Cystotomy was performed in the cranial ventral paramedian incision in order to preserve the large ventral abdominal vein in the linen alba region. After two stay sutures ("holding suture" ) with 5-0 polyglactin 910 ($Vicryl^{\circledR}, Ethicon$) on the bladder, cystotomy was carried out and the stone inside was removed. Then, the bladder was closed in one-layer sutures. The abdomen was closed in simple interrupted suture with same material after the instillation of 10 ml saline solution into the abdominal cavity. The cystic calculus was in a size of $5.8{\times} 5.O{\times} 3.5 cm$. The analysis of the stone was carried out in two ways which are the infrared spectroscopy of the stone and quick analysis by $Merckognost^{\circledR}$(Diagnostica Merck, Merck). In the result of the infrared spectroscopy, the stone wee composed of 10% ammonium urate and 90% dihydrated uric acid. This iguana wasn't able to survive.n't able to survive.
We performed bromodeoxyuridine (BrdU) staining to observe the proliferation pattern of epithelial cells on the biliaJy mucosa in Clonorchis sinensis infection. Albino rats were infected with 100 metacercariae each and their livers were processed for histopathological observation after BrdU injection. Five to six sites in the liver of a rat were selected for paraffin section, and stained immunohistochemically to visualize BrdU incorporating cells. The flukes were mainly in the common bile duct and right or left hepatic bile ducts. The proportion of stained epithelial cells in the infected bile ducts where the worms were found on the section was 2.9-10.2% at 1 week after infection. 7.3-12.8% at 2 weeks, 7.3-13.4% at 5 weeks, and 8.4-14.8% at 15 weeks while in the non-infected ducts o to 2.7% cells were stained. The stained cells were mainly at the base of the mucosal layer. It is suggested that mucosal epithelial cells of the bile ducts infected with C. sinensis become hyperplastic mainly by direct and local stimulation of the worms.
